Material wear significantly impacts the clinical success and longevity of dental ceramic restorations. This in vivo study aimed to assess the wear behavior of IPS Empress® glass-ceramic inlays and onlays over 14 years, considering the influence of different antagonist materials. Fifty-four indirect restorations of 21 patients were available for comprehensive wear analysis, with complete follow-up data for up to 14 years. Three-dimensional measurements relied on digitized epoxy resin models produced immediately post-insertion (baseline) and subsequently at 2, 4, and 14 years. The occlusal region on the baseline model was delineated for comparative analysis. Three-dimensional superimpositions with models from subsequent time points were executed to assess wear in terms of average linear wear and volumetric loss. Statistical analyses were conducted in R (version 4.4.1), employing Mann–Whitney U tests (material comparisons) and Wilcoxon signed rank tests (time point comparisons), with a significance threshold of p ≤ 0.05. During the entire study period, an increase in wear was observed at each assessment interval, gradually stabilizing over time. Significant differences in substance loss were found between the follow-up time points, both for mean (−0.536 ± 0.249 mm after 14a) and integrated distance (−18,935 ± 11,711 mm3 after 14a). In addition, significantly higher wear was observed after 14 years with gold as antagonist compared to other materials (p ≤ 0.03). The wear behavior of IPS Empress® ceramics demonstrates clinically acceptable long-term outcomes, with abrasion characteristics exhibiting stabilization over time. Full article

Three-dimensional (3D) printing has emerged as a transformative technology in dental splint fabrication, offering significant advancements in customization, production speed, material efficiency, and patient comfort. This comprehensive review synthesizes the current literature on the clinical use, benefits, limitations, and future directions of 3D-printed dental splints across various disciplines, including prosthodontics, orthodontics, oral surgery, and restorative dentistry. Key 3D printing technologies such as stereolithography (SLA), digital light processing (DLP), and material jetting are discussed, along with the properties of contemporary photopolymer resins used in splint fabrication. Evidence indicates that while 3D-printed splints generally meet ISO standards for flexural strength and wear resistance, their mechanical properties are often 15–30% lower than those of heat-cured PMMA in head-to-head tests (flexural strength range 50–100 MPa vs. PMMA 100–130 MPa), and study-to-study variability is high. Some reports even show significantly reduced hardness and fatigue resistance in certain resins, underscoring material-specific heterogeneity. Clinical applications reviewed include occlusal stabilization for bruxism and temporomandibular disorders, surgical wafers for orthognathic procedures, orthodontic retainers, and endodontic guides. While current limitations include material aging, post-processing complexity, and variability in long-term outcomes, ongoing innovations—such as flexible resins, multi-material printing, and AI-driven design—hold promise for broader adoption. The review concludes with evidence-based clinical recommendations and identifies critical research gaps, particularly regarding long-term durability, pediatric applications, and quality control standards. This review supports the growing role of 3D printing as an efficient and versatile tool for delivering high-quality splint therapy in modern dental practice. Full article

Pedestrian detection systems are widely used in safety-critical domains such as autonomous driving, where deep neural networks accurately perceive individuals and distinguish them from other objects. However, their vulnerability to backdoor attacks remains understudied. Existing backdoor attacks, relying on unnatural digital perturbations or explicit patches, are difficult to deploy stealthily in the physical world. In this paper, we propose a novel backdoor attack method that leverages real-world occlusions (e.g., backpacks) as natural triggers for the first time. We design a dynamically optimized heuristic-based strategy to adaptively adjust the trigger’s position and size for diverse occlusion scenarios, and develop three model-independent trigger embedding mechanisms for attack implementation. We conduct extensive experiments on two different pedestrian detection models using publicly available datasets. The results demonstrate that while maintaining baseline performance, the backdoored models achieve average attack success rates of 75.1% on KITTI and 97.1% on CityPersons datasets, respectively. Physical tests verify that pedestrians wearing backpack triggers could successfully evade detection under varying shooting distances of iPhone cameras, though the attack failed when pedestrians rotated by 90°, confirming the practical feasibility of our method. Through ablation studies, we further investigate the impact of key parameters such as trigger patterns and poisoning rates on attack effectiveness. Finally, we evaluate the defense resistance capability of our proposed method. This study reveals that common occlusion phenomena can serve as backdoor carriers, providing critical insights for designing physically robust pedestrian detection systems. Full article

The natural features of the face exhibit significant symmetry. In practical applications, faces may be partially occluded due to factors like wearing masks or glasses, or the presence of other objects. Occluded-face restoration has broad application prospects in fields such as augmented reality, virtual reality, healthcare, security, etc. It is also of significant practical importance in enhancing public safety and providing efficient services. This research establishes an improved occluded-face restoration network based on facial feature points and Generative Adversarial Networks. A facial landmark prediction network is constructed based on an improved MobileNetV3-small network. On the foundation of U-Net, dilated convolutions and residual blocks are introduced to form an enhanced generator network. Additionally, an improved discriminator network is built based on Patch-GAN. Compared to the Contextual Attention network, under various occlusions, the improved face restoration network shows a maximum increase in the Peak Signal-to-Noise Ratio of 24.47%, and in the Structural Similarity Index of 24.39%, and a decrease in the Fréchet Inception Distance of 81.1%. Compared to the Edge Connect network, under various occlusions, the improved network shows a maximum increase in the Peak Signal-to-Noise Ratio of 7.89% and in the Structural Similarity Index of 10.34%, and a decrease in the Fréchet Inception Distance of 27.2%. Compared to the LaFIn network, under various occlusions, the improved network shows a maximum increase in the Peak Signal-to-Noise Ratio of 3.4% and in the Structural Similarity Index of 3.31%, and a decrease in the Fréchet Inception Distance of 9.19%. These experiments show that the improved face restoration network yields better restoration results. Full article

Background: Mandibular movement is inherently complex, involving both lateral and vertical components. In older adults, this complexity is compounded by diminished neuromuscular coordination and reduced proprioceptive feedback, often impairing effective mastication. Objectives: This study aimed to review dental literature on the influence of masticatory parameters in the formation of wear facets on acrylic resin denture teeth. Methods: A targeted literature review was conducted, analyzing peer-reviewed studies from 1991 to 2024 through PubMed and Scopus. The review focused on masticatory parameters, age-related neuromuscular adaptations, chewing patterns, and preferred-side denture tooth wear, affecting older adults using complete removable dentures. Results: Mandibular movement characteristics—particularly velocity, duration, and cycle frequency—were found to significantly influence denture tooth wear, often resulting in reduced bite force and masticatory efficiency. Contemporary denture materials demonstrated improved wear resistance, aiding in the regulation of chewing frequency and velocity. These factors are increasingly important as age-related changes impact the stomatognathic system. Notably, wear facets were most prominent on the maxillary first molars, regardless of the occlusal scheme. Conclusions: This review provided a comprehensive overview of existing research, analyzed current evidence, and offered insights into the role of masticatory function and wear characteristics in older complete denture wearers. Industrial exoskeletons (a.k.a. wearable robots) have been developed to reduce musculoskeletal fatigue and work injuries. Human joint kinematics and human–robot alignment are important measurements in understanding the effects of industrial exoskeletons. Recently, markerless pose estimation systems based on monocular color (red, green, blue—RGB) and depth cameras are being used to estimate human joint positions. This study analyzes the performance of monocular markerless pose estimation systems on human skeletal joint estimation while wearing exoskeletons. Two pose estimation systems producing RGB and depth images from ten viewpoints are evaluated for one subject in 14 industrial poses. The experiment was repeated for three different types of exoskeletons on the same subject. An optical tracking system (OTS) was used as a reference system. The image acceptance rate was 56% for the RGB, 22% for the depth, and 78% for the OTS pose estimation system. The key sources of pose estimation error were the occlusions from the exoskeletons, industrial poses, and viewpoints. The reference system showed decreased performance when the optical markers were occluded by the exoskeleton or when the markers’ position shifted with the exoskeleton. This study performs a systematic comparison of two types of monocular markerless pose estimation systems and an optical tracking system, as well as a proposed metric, based on a tracking quality ratio, to assess whether a skeletal joint estimation would be acceptable for human kinematics analysis in exoskeleton studies. Background: In addition to behavioral and biochemical abnormalities, a parafunction associated with temporomandibular joint disorders (TMDs) resulted in stress-induced depression in rats. Exploring how chronic stress influences molar wear in rodents provides insights into the understanding of depression, TMD, and oral health. This study aimed to conduct a three-dimensional (3D) analysis of first molar wear in an animal model of depression by comparing molar attrition and cusp variation between stressed male rats and control groups. Methods: After obtaining a validated model of depression in male rats, we obtained 3D scans of lower molars to analyze wear patterns. The 3D analysis was applied to quantify cusps’ volume and the difference in first molar cusp morphological structure. The data were then compared to identify significant morphological differences between groups side by side. Results: The analysis revealed the reduction of cusps’ volume in the depression groups. Rats exposed to depression exhibited significantly greater occlusal table wear than their control counterparts (p < 0.05). Conclusions: As dentistry moves towards greater digital imaging, understanding the impact of psychological factors on TMD becomes increasingly necessary. This study shows that stress-induced depression in rats can result in significant tooth wear, as investigated using a 3D dental scanner. Full article

Background: Mucostatic impressions have been always indicated in thin, sharp, or flabby ridges, and have been addressed for their beneficial effect on long-term residual ridge stability. Nonetheless, a purely mucostatic impression was not possible until intraoral scans became available. This provides an option for digital removable denture which is biologically sensible but might reduce retention in comparison with a mucocompressive impression with border molding. On the other hand, pressure applied to the mucosa may have harmful effects on the long-term residual ridge stability, causing higher resorption and ultimately reduced denture retention. Hence, the possibility to merge mucostatics and mucocompressive philosophies would be a clinically and biologically sensible option for oral rehabilitation in aging populations where patients will potentially wear dentures for longer periods. This possibility is demonstrated in this technical report with a cast-free digital workflow. Technique: Baseplates for occlusion rims, closely adapted to the mucosa, were designed on intraoral scans of edentulous arches and, once 3D-printed, used to register maxillomandibular relations and information for tooth arrangement, as well as to perform border molding. Occlusion rims were then scanned and, within the 3Shape Dental System 2024 software program, the intaglio surfaces of their baseplates were segmented and inverted to obtain the digital master casts which incorporated the precise reproduction of the molded borders. Then, denture design was performed and manufactured; no limitations regarding manufacturing options are applicable to the presented technique. Conclusions: The potential benefits (i.e., improved retention in the initial period after denture delivery and the preservation of tissues) of the presented digital cast-free workflow, based on merging mucostatic and mucocompressive philosophies to obtain dentures with a mucostatic intaglio surface and functional borders, are sensible clinical outcomes which recommend the clinical application of the technique, although further validation, especially in the long term, is required. Helmet-wearing detection for electric vehicle riders is essential for traffic safety, yet existing detection models often suffer from high target occlusion and low detection accuracy in complex road environments. To address these issues, this paper proposes YOLO-CBF, an improved YOLOv7-based detection network. The proposed model integrates coordinate convolution to enhance spatial information perception, optimizes the Focal EIOU loss function, and incorporates the BiFormer dynamic sparse attention mechanism to achieve more efficient computation and dynamic content perception. These enhancements enable the model to extract key features more effectively, improving detection precision. Experimental results show that YOLO-CBF achieves an average mAP of 95.6% for helmet-wearing detection in various scenarios, outperforming the original YOLOv7 by 4%. Additionally, YOLO-CBF demonstrates superior performance compared to other mainstream object detection models, achieving accurate and reliable helmet detection for electric vehicle riders. Background/Objectives: Excessive occlusal forces manifest in bruxism and have consequences on teeth and jaws. The aim of this study was to determine the association of bruxism with clinical and radiological signs of excessive occlusal forces, such as tooth wear, fatigue dental fissures and fractures, abfraction, masseter muscle hypertrophy, and bone apposition at the mandibular angle. Methods: This cross-sectional clinical study included 181 patients presented for treatment in a general dentistry clinic. For each patient, data were extracted from the dental chart, as follows: demographic data (sex, age, and smoking), clinical data (number of teeth present, Eichner edentulous score, TWI wear score, number of fractured teeth, number of teeth with abfraction, presence of masseter hypertrophy, presence of hypersensitivity), and radiological data (bone apposition at the mandibular angle). The patients were divided into two groups according to the presence or absence of bruxism. A binomial logistic regression model was run to determine the association between bruxism and clinical and radiological signs of excessive occlusal forces. The data were statistically processed in SPSS. Results: In total, 99 women and 82 men with mean age 44.87 ± 12.67 were included in the study. Compared to the group without bruxism, the group of patients with bruxism (39.78%) showed statistically significant higher tooth wear index (TWI) (p < 0.0005), a higher number of fractured teeth (p = 0.037), a higher number of teeth with abfraction lesions (p = 0.001), and a significantly higher bone apposition score (p < 0.0005). The binomial logistic regression model showed a high prediction bruxism score for masseter muscle hypertrophy (15 times, p < 0.0005), for tooth wear index (almost 7 times, p = 0.010), and for bone apposition score (almost 3 times, p = 0.044). Conclusions: Patients with bruxism showed masseter muscle hypertrophy, higher attrition-type tooth wear index, and more teeth with fatigue fractures and abfractions than those without bruxism. Bruxism clinical signs were positively correlated with a higher bone apposition score. Full article

Background/Objectives: Dental agenesis is the congenital absence of at least one tooth and has been associated with several other developmental dental conditions, such as morphological dental alterations and Carabelli trait. This study sought to investigate whether third molar agenesis is associated with Carabelli traits in permanent molars. Methods: This was a cross-sectional study that used a convenience sample obtained from the orthodontic records of German patients. Patients with syndromes, oral clefts, congenital alterations including dental agenesis (except agenesis of third molars), and severe cases of bruxism with a loss of tooth tissue were excluded. Teeth with cavitated lesions of dental caries, occlusal wear, restorations, and evident dental deformities were not included in the evaluation. The Carabelli trait was evaluated in the permanent maxillary molars. The Carabelli trait was classified according to its expression for each tooth as either absent, negative, and positive expressions. Third molar agenesis was evaluated only in patients older than 10 years old (based on when initial tooth formation should be visible in the panoramic radiographs). The two-sided Chi-squared test was used to investigate the association between the conditions, using an alpha of 5% (p < 0.05). Results: A total of 155 patients (74 females and 81 males) were investigated; 39 had third molar agenesis and 75 had the Carabelli trait. There was no statistical significance difference between patients with third molar agenesis and those with Carabelli traits in relation to sex (p > 0.05). The Carabelli trait was more common in first molars than in second molars. There was no statistical significant association between third molar agenesis and Carabelli traits (p > 0.05). Conclusions: Third molar agenesis was not associated with the Carabelli trait in the permanent molars in this population. Full article

Abstract
Background/Objectives: Bruxism is a masticatory muscle activity, phasic or tonic, with/without teeth contact, that appears in sleep or an awake state. Background/Objectives: Bruxism is a masticatory muscle activity, phasic or tonic, with/without teeth contact, that appears in sleep or an awake state. An instrumental technique used to measure the surface electromyographic (sEMG) activity of the masseter muscle is used to diagnose bruxism activity during sleep and while awake. The objective of this study was to compare the variation in bruxism (sleep and awake) indices and masseter activity indices in low sleep bruxism and moderate sleep bruxism before and after wearing an occlusal appliance (OA) for 3 months each night. Methods: A clinical interventional study was designed in which subjects diagnosed with sleep bruxism were randomly selected to be included in the study. After the first sEMG recording, two groups were formed: a low sleep-bruxism group (number of sleep-bruxism events/h between 2 and 4) and a moderate sleep-bruxism group (number of sleep-bruxism events/h equal or higher than 4). All subjects received treatment with a 3D-printed occlusal appliance and wore it each night for 3 months, at which point the second sEMG recording was performed. For each participant of this study, a chart was created that included anamnestic data, clinical data, and sEMG data. The data were statistically analyzed with SPSS, using the Mann–Whitney U and Wilcoxon signed-rank tests. Results: A total of 21 participants were included in the final analysis, 18 women and 3 men, with a mean age of 24.5 ± 2.7 years. The OA lowered all bruxism indices in the whole group, but clusters analysis showed a significant reduction in sleep-bruxism indices in the moderate sleep-bruxism group, while in the low-bruxism group, the sleep and awake indices varied insignificantly, and the number of sleep-bruxism events/h remained constant. Conclusions: The 3D-printed occlusal appliances significantly lowered the sleep-bruxism indices and sleep masseter activity indices recorded with a portable sEMG device in the moderate sleep-bruxism group. The OA lowered the awake-bruxism indices and awake masseter activity indices in the moderate sleep-bruxism group. Full article

A safety helmet is indispensable personal protective equipment in high-risk working environments. Factors such as dense personnel, varying lighting conditions, occlusions, and different head postures can reduce the precision of traditional methods for detecting safety helmets. This paper proposes an improved YOLOv8n safety helmet detection model, YOLOv8-ADSC, to enhance the performance of safety helmet detection in complex working environments. In this model, firstly, Adaptive Spatial Feature Fusion (ASFF) and Deformable Convolutional Network version 2 (DCNv2) are used to enhance the detection head, enabling the network to more effectively capture multi-scale information of the target; secondly, a new detection layer for small targets is incorporated to enhance sensitivity to smaller targets; and finally, the Upsample module is replaced with the lightweight up-sampling module Content-Aware ReAssembly of Features (CARAFE), which increases the perception range, reduces information loss caused by up-sampling, and improves the precision and robustness of target detection. The experimental results on the public Safety-Helmet-Wearing-Dataset (SHWD) demonstrate that, in comparison to the original YOLOv8n model, the mAP@0.5 of YOLOv8-ADSC has increased by 2% for all classes, reaching 94.2%, and the mAP@0.5:0.95 has increased by 2.3%, reaching 62.4%. YOLOv8-ADSC can be better suited to safety helmet detection in complex working environments. Full article

Mandibular Advancement Devices in OSA Patients: Impact on Occlusal Dynamics and Tooth Alignment Modifications—A Pilot Prospective and Retrospective Study

Background: The widespread prevalence of obstructive sleep apnea (OSA) underscores the necessity for effective therapies. Mandibular advancement devices (MADs) have emerged as valid treatment for mild to moderate cases, despite the associated dental side effects. Methods: This study evaluates the nature, onset, and long-term manifestation of these side effects. In the prospective group (n = 12), dental impressions were taken pre-MAD-insertion and at intervals of three, six, nine, and twelve months post-insertion to monitor occlusal alterations. In the retrospective group, participants (n = 8) wearing MADs for 7 years at average underwent lateral cephalogram assessments to compare with pre-treatment X-rays. All participants completed a specific questionnaire. Statistical analysis was performed via t-test and with p < 0.05 as the significance level. Results: The majority of participants in both groups consistently used MADs and reported significant sleep quality improvements, rating common symptoms like jaw tension as negligible. In both the prospective group and the retrospective group, significant reductions in overjet were observed at multiple time points, with the prospective group showing reductions at six months (p = 0.001), nine months (p > 0.001), and twelve months (p = 0.019), while the retrospective group indicated a notable decrease between baseline and follow-up assessments after a mean of seven years of device wear (p = 0.004). A slight overbite increase of 0.2 mm was prospectively observed after one year, whereas a trend towards a minimal decrease over the long term was observed in the retrospective sample (p = 0.003). Noteworthy changes in angle class or lower incisor inclination were absent. Cephalograms revealed significant IOK-NL angle alterations with a mean of 98.2° before and 95.2° upon long-term treatment (p = 0.020). Conclusions: These findings suggest that MADs are effective in treating OSA with minor adverse effects. This study advocates for moderate mandibular protrusion to balance therapeutic efficacy with dental health considerations, crucial for optimizing treatment outcomes. Nonetheless, the limited sample size warrants caution when generalizing these results to the broader population. Full article

(1) Objectives: This study investigated and compared the wear and flexural strength of two highly filled (injectable), one flowable and one paste composite. (2) Methods: Two highly filled flowable composites (G-aenial Universal Injectable and Beautifil Plus F00), a paste composite (Empress Direct) and a conventional flowable (Tetric EvoFlow) were tested. A two-body wear test was carried out using 10 disc-shaped samples from each group, which were subjected to 200,000 wear machine cycles to simulate wear, followed by Scanning Electron Microscope analysis. Flexural strength was tested using a three-point bend test using 15 beam samples for each of the four groups. Values were statistically compared using one-way analysis of variance (ANOVA) for flexural strength and a Kruskal–Wallis test for wear. (3) Results: The median volume loss for G-aenial Universal Injectable and Beautifil Plus F00 was statistically lower than that of both Empress Direct and Tetric EvoFlow. For flexural strength the two highly filled flowable composites both exhibited statistically higher mean flexural strength values compared to Empress Direct (p < 0.004) and Tetric Evoflow (p < 0.001). There were no statistically significant differences in the values of wear and flexural strength between the two highly filled flowable composites. (4) Conclusions/significance: Highly filled flowable composite resins with nano filler particles outperformed a conventional flowable and a paste composite resin in terms of wear resistance and flexural strength, and may be suitable to use in occlusal, load-bearing areas.
