Public responses during health crises are shaped by interacting risk perceptions, affect, trust, information needs, overload, misinformation, and protective behavior. Existing applications of the Risk Information Seeking and Processing (RISP) model are largely static and therefore cannot represent stochastic multichannel exposure, delayed correction, or policy feedback. We develop the Stochastic Probabilistic Dynamic RISP (SP-D-RISP) model, which recasts RISP as a bounded stochastic state-space system. Its symmetry structure is explicit: the channel-allocation mechanism is equivariant under simultaneous relabeling of channels and their parameter blocks, while the multi-agent dynamics are invariant to agent relabeling under exchangeable sampling and a label-independent policy. Channel-specific effects, heterogeneous traits, rumor shocks, and interventions generate symmetry breaking. The model combines softmax–multinomial channel competition, discounted Bayesian trust updating, and policy-coupled state transitions. Projection guarantees feasible states by construction, whereas stronger stochastic stability is conditional on a coefficient-level small-gain criterion. For the stationary bounded-memory specification, this criterion is sufficient for Wasserstein contraction, uniqueness of the invariant distribution, and geometric forgetting of initial conditions. The criterion is formulated at the coefficient level and is kept distinct from finite-horizon simulation diagnostics. For the fully disclosed semi-synthetic coefficient vector, the scenario-specific gain matrices have spectral radii between 0.852765 and 0.857123; the worst-case column-sum norm is 0.983948. Thus, the fixed-policy kernels satisfy the stated contraction certificate. For deterministic time-varying paths, the calculation is used only as a common-path one-step certificate, and for the threshold-adaptive rule, it is used only mode by mode rather than as a stationary invariant-law claim. While concentration bounds and Monte Carlo inference quantify population and replication uncertainty, a semi-synthetic experiment with 2500 heterogeneous agents over 90 days examines trust and literacy heterogeneity, clarification delays, communication volume, and intervention portfolios. Within the calibrated SP-D-RISP scenarios, the simulations suggest that higher communication volume may reduce modeled protective behavior when overload effects dominate knowledge gains, delayed clarification may increase transient misinformation, and an integrated portfolio can yield a more favorable simulated outcome profile than the evaluated single-lever strategies. Background/Objectives: Low vaccination coverage and the persistence of zero-dose children remain the principal challenges for immunization efforts in Madagascar. To address these barriers, a socio-anthropological study was conducted to identify the determinants of both vaccination and non-vaccination in eight districts of the country. Methods: District selection was based primarily on immunization performance—specifically the proportion of zero-dose children—along with criteria of geographic and cultural diversity. A qualitative approach was employed, comprising 162 semi-structured individual interviews and 41 focus group discussions with key informants, including political–administrative, religious, and traditional authorities, healthcare workers, community health workers, and parents. Results: Overall, the benefits of vaccination were widely acknowledged by the population. Anti-vaccine rumors were found to be sporadic and, due to their provisional nature, potentially reversible even among those who relay them. Beyond conventional barriers such as scheduling constraints and limited accessibility, fluctuating motivation among community health workers and structural challenges affecting their work emerged as notable findings. Conversely, factors promoting vaccine acceptance were associated with trust in the vaccinators themselves and with a good understanding of vaccination-related issues, fostered through increased and context-specific sensitization efforts. Conclusions: In conclusion, no evidence was found to associate contexts such as rural settings or low-performing vaccination areas with lower vaccine acceptance. Similarly, anti-vaccine rumors were not confined to any particular category or group. Ultimately, the main obstacles are the prioritization of economic risk and concerns about potential side effects. The primary recommendation concerns strengthening awareness-raising efforts, while strengthening trust and improving the working conditions of community health workers. Full article

Introduction: COVID-19 and influenza are viruses that have been major causes of morbidity and mortality worldwide. While a combination vaccine for these two viruses is currently in development, little is known about public perceptions and attitudes towards such a vaccine. Therefore, we have aimed to conduct a scoping review to evaluate the attitudes and reasons for the acceptance/rejection of a combination COVID-19 and influenza vaccination. Material and Methods: Our review has followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews (PRISMA-ScR) guidelines. Search terms included those on COVID-19, influenza, and combination vaccines. Searches were conducted in a total of 10 different databases, as follows: Embase, Global Health, Google Scholar, CINAHL, Medline, Scopus, ScienceDirect, PubMed, PsycINFO, and Web of Science. Results: Searches across all databases produced a total of 1763 results, of which five studies were deemed to be eligible for this review. Data from included studies encompassed a total of 20,581 participants across 14 countries. Generally, participants had favorable views towards a combination vaccine. However, attitudes varied by region, demographics, and prior vaccine exposure. Reasons provided for accepting a combination vaccine include low cost, reasonable effectiveness, fewer injections, and higher safety. However, reasons for rejection of a potential combination vaccine include a potential lack of evidence and studies regarding its effectiveness and a fear of possible side effects. Conclusions: There are generally positive attitudes and perceptions towards a combination vaccination for COVID-19 and influenza, and there are also specific segments of populations that may show hesitancy towards such a vaccine. Vaccination delivery programs should clearly demonstrate that these vaccines are safe and must concurrently address false rumors and misinformation. Ensuring that as many people as possible can receive the vaccination without barriers via equity in access should be a public health priority. It is also important to recognize the limitations of this work, especially that the review was limited to five studies, the majority of which were cross-sectional and relied on self-reporting. In the digital era, social media proliferation accelerates rumor dissemination. During public health emergencies, such misinformation intensifies social harm. Studying the influencing factors of online rumor refutation behavior thus becomes crucial. This study uses the stimulus–organism–response (SOR) theory as an analysis framework, based on the perspective of information sources and integrating group norms as a moderating factor, to explore the psychological processes affecting Chinese university students’ online rumor refuting in public health emergencies. Against the backdrop of the COVID-19 pandemic, a questionnaire survey was conducted on 1017 respondents, and the collected data were analyzed using the structural equation modeling research method. The results indicate that both online and offline information seeking positively influence university students’ fear of contracting the COVID-19 virus. University students’ fear positively influences their engagement in online rumor refuting. Notably, fear mediates the link between online and offline information seeking and online rumor refuting. Additionally, group norms help strengthen the connection between university students’ fear and their involvement in online refuting rumors. These results provide theoretical explanations and practical guidance for university students to refute rumors online. Full article

Background/Objectives: To explore how Public Health Nurses (PHNs) in Fukushima perceived their roles and activities as necessary or inadequate from the immediate aftermath through the long-term recovery of the nuclear disaster. Methods: We conducted a qualitative study using a self-administered questionnaire with open-ended questions to capture the perceptions of PHNs across three disaster phases: peacetime; nuclear emergency; and recovery. Responses were analyzed through qualitative content analysis. Results: PHNs’ needs and perceived inadequacies varied across the disaster phases. In peacetime, the emphasis was on education for disaster preparedness for both nurses and residents. During the nuclear emergency, the focus shifted to the need for PHN deployment and radiation screening systems, highlighting a significant gap in radiation knowledge. In the recovery phase, the importance of ongoing resident support, rumor management, and trust-building was emphasized, alongside an increased need for radiation education. Conclusions: This study highlights the critical need for phase-specific support systems and educational programs to enhance PHNs’ disaster response capabilities. It underscores the importance of preparedness plans and continuous training to improve PHNs’ effectiveness in addressing public health challenges during nuclear disasters. Purpose: The post-pandemic management of COVID-19 infections and any emergent outbreaks is because this endemic disease remains a public health concern. Vaccine hesitancy may continue to hamper efforts to respond to any new disease outbreaks and future epidemics. This qualitative study aimed to explore the factors influencing COVID-19 vaccine acceptance and hesitancy in Kenya to gain deeper insights into this issue. Methods: This study was implemented in western Kenya using key informant interviews. Fourteen (14) key informants were purposively selected for this study. All interviews were transcribed and analyzed using thematic analysis. The interpretation of findings was conducted within the framework of the Health Belief Model. Key findings: Knowledge was a critical factor in combatting misinformation and fostering vaccine acceptance among participants in this study. Misinformation included rumors that the vaccine lowers immunity and was intended for population control. Cues to action included influence from political and opinion leaders and observing the loss of life among unvaccinated individuals. Perceived barriers to vaccine uptake included fear of vaccine safety, side effects, long waiting times at the time of our study, fear of contracting COVID-19 at vaccination sites, family/spousal influence on vaccine uptake and fear of the unknown with the vaccine. Conclusions: The findings from this study provide insight into areas for targeted strategies for managing COVID-19 vaccinations and future pandemics. Within the framework of the Health Belief Model, this study identified salient barriers and facilitators of COVID-19 vaccine hesitancy that may be helpful to inform future pandemic responses. Livestock management is challenging for resource-poor (R-P) farmers due to unavailability of quality feed, limited professional advice, and rumor-spreading about animal health condition in a herd. This research seeks to improve animal health in southern Africa by promoting sericea lespedeza (Lespedeza cuneata), a nutraceutical forage legume. An automated geospatial model for precision agriculture (PA) can identify suitable locations for its cultivation. Additionally, a novel approach of radio-frequency identifier (RFID) supported telemetry technology can track animal movement, and the analyses of data using artificial intelligence can determine sickness of small ruminants. This RFID-based system is being connected to a smartphone app (under construction) to alert farmers of potential livestock health issues in real time so they can take immediate corrective measures. An accompanying Decision Support System (DSS) site is being developed for R-P farmers to obtain all possible support on livestock production, including the designed PA and RFID-based DSS. Background: The WHO recommended the use of the RTS,S/AS01 malaria vaccine (RTS,S) based on a pilot evaluation in routine use in Ghana, Kenya, and Malawi. A longitudinal qualitative study was conducted to examine facilitators and barriers to uptake of a 4-dose RTS,S schedule. Methods: A cohort of 198 caregivers of RTS,S-eligible children from communities where RTS,S was provided through the pilot were interviewed three times over a ≈22-month, 4-dose schedule. The interviews examined caregiver perceptions and behaviors. Children’s vaccination history was obtained to determine dose uptake. Results: 162 caregivers remained at round 3 (R3); vaccination history was available for 152/162 children. Despite early rumors/fears, the uptake of initial doses was high, driven by vaccine trust. Fears dissipated by R2, replaced with an enthusiasm for RTS,S as caregivers perceived its safety and less frequent and severe malaria. By R3, 98/152 children had received four doses; 34 three doses; 9 one or two doses; and 11 zero doses. The health system and information barriers were important across all under-dose cases. Fears about AEFIs/safety were important in zero-, one-, and two-dose cases. Competing life/livelihood demands and complacency were found in three-dose cases. Regardless of the doses received, caregivers had positive attitudes towards RTS,S by R3. Conclusions: Findings from our study will help countries newly introducing the vaccine to anticipate and preempt reasons for delayed acceptance and missed RTS,S doses. Full article

COVID-19 vaccines are an effective public health intervention to reduce COVID-19-related morbidity and mortality. Given that pregnant and lactating women have a higher risk of severe COVID-19 complications, it is paramount to understand the factors that inform vaccine decision-making among this population. In this study, we sought to identify facilitators and barriers to COVID-19 vaccine acceptance and vaccine promotion in pregnant and lactating women in Bangladesh. We conducted 40 in-depth interviews with 12 pregnant women, 12 lactating women, and 16 health workers from one urban and four rural communities in Bangladesh. We used a grounded theory approach to identify emerging themes. Our results suggest that health workers and religious leaders played key roles in promoting COVID-19 vaccines in this population. Further, we found that the culture of trust in public health authorities and the existing vaccine infrastructure facilitated vaccine promotion. However, changes in vaccine eligibility and myths and rumors acted as both facilitators and barriers to vaccine promotion within our study. It is crucial that maternal immunization vaccine promotion efforts push pregnant and lactating women toward vaccine acceptance to protect the health of mothers and their babies. Additionally, as new maternal vaccines are developed and licensed, understanding how to best promote vaccines within this group is paramount. Full article

The nuclear accident that accompanied the Great East Japan Earthquake of 11 March, 2011, was also an information disaster. A serious problem that arose after the accident and persisted for a long time was the damage caused by harmful rumors (DCBHR). In 2016, a cross-sectional questionnaire survey on health and information was conducted in Fukushima. The eligible population of this survey was 2000 Fukushima residents, which included those in the evacuated areas. We received 861 responses. Data were analyzed using the responses to the question about perceived DCBHR as the objective variable and the sources of information residents trusted and the media they used as explanatory variables. A multiple logistic regression analysis revealed that those who trusted government ministries and local commercial TV were significantly associated with no effect. In contrast, those who used Internet sites and blogs were significantly associated with a negative effect. This study underlines the pivotal importance of media and information, literacy, and education and discusses how these should be improved to avoid DCBHR in the future. Furthermore, accurate information should be made available to all sections of the population to diminish DCBHR. Full article

In the digital media era, new media platforms have become the main channels for transmitting medical and health information in China. However, anti-intellectualism limits the effectiveness of disseminating health information. Therefore, in China, the government and health departments have made efforts to determine how to control anti-intellectualism to effectively disseminate medical and health information, given the situation of a global pandemic and its counter-measures. Against this backdrop, this study applied textual analysis to explore the manifestations of anti-intellectualism in network platforms. The key findings indicate that the irrational behavior of anti-intellectuals is manifested in emotional dominance, abusive behavior, overconfidence and trusting rumors. Based on these results, the authors propose some measures to balance the relationship between anti-intellectualism and health communication. The findings of the study have significant implications for improving the effectiveness of health communication in China. Full article

(1) Background: Rumors are a special type of information. Based on the classic theory of the communication of information, the “5W” communication model, this article aims to build a new model and thus explains the generation and communication of Internet health rumors. (2) Methods: The authors selected 50 Internet health rumors, which were widely spread in widely used websites and social media in China, then grounded theory is used to perform the qualitative analysis of the Internet health rumors. (3) Results: Three Core Concepts are abstracted after qualitative analysis. An internal dynamic mutual assistance mechanism of the communication of rumors is built and illustrated. Based on Lasswell’s “5W” communication model, the authors develop an eight-element communication model for Internet health rumors to illustrate the generation and communication of Internet health rumors. (4) Conclusions: By removing one or several elements of this new model, the chain of the communication of Internet health rumors could be cut off, which is valuable information for the government or websites to manage communication of Internet health rumors. Vaccine hesitancy (VH) is the seventh among the WHO’s top 10 threats to global public health, which has continued to perpetuate the transmission of vaccine preventable diseases (VPDs) in Africa. Consequently, this paper systematically reviewed COVID-19 vaccine acceptance rates (VARs)—including the vaccine uptake and vaccination intention—in Africa from 2020 to 2022, compared the rates within the five African regions and determined the context-specific causes of VH in Africa. Generally, COVID-19 VARs ranged from 21.0% to 97.9% and 8.2% to 92.0% with mean rates of 59.8 ± 3.8% and 58.0 ± 2.4% in 2021 and 2022, respectively. Southern and eastern African regions had the top two VARs of 83.5 ± 6.3% and 68.9 ± 6.6% in 2021, and 64.2 ± 4.6% and 61.2 ± 5.1% in 2022, respectively. Based on population types, healthcare workers had a marginal increase in their mean COVID-19 VARs from 55.5 ± 5.6% in 2021 to 60.8 ± 5.3% in 2022. In other populations, the mean VARs decreased from 62.7 ± 5.2% in 2021 to 54.5 ± 4% in 2022. As of 25 October 2022, Africa lags behind the world with only 24% full COVID-19 vaccinations compared to 84%, 79% and 63% reported, respectively, in the Australian continent, upper-middle-income countries and globally. Apart from the problems of confidence, complacency, convenience, communications and context, the context-specific factors driving COVID-19 VH in Africa are global COVID-19 vaccine inequality, lack of vaccine production/maintenance facilities, insecurity, high illiteracy level, endemic corruption, mistrust in some political leaders, the spreading of unconfirmed anti-vaccination rumors and political instability. With an overall mean COVID-19 acceptance rate of 58%, VH still subsists in Africa. The low VARs in Africa have detrimental global public health implications, as it could facilitate the emergence of immune invading SARS-CoV-2 variants of concern, which may spread globally. Consequently, there is a need to confront these challenges frontally and engage traditional and religious leaders in the fight against VH in Africa, to restore public trust in the safety and efficacy of vaccines generally. As the availability of COVID-19 vaccines improves, the vaccination of pets and zoo-animals from which reverse zoonotic transmission of SARS-CoV-2 have been reported is recommended, to limit the evolution and spread of new variants of concern and avert possible SARS-CoV-2 epizootic or panzootic diseases in susceptible animal species. Widely spread health-related rumors may mislead the public, escalate social panic, compromise government credibility, and threaten public health. Social collaboration models that maximize the functions and advantages of various agents of socialization can be a promising way to control health-related rumors. Existing research on health-related rumors, however, is limited in studying how various agents collaborate with each other to debunk rumors. This study utilizes content analysis to code the text data of health-related rumor cases in China during the COVID-19 pandemic. The study found that socialized rumor-debunking models could be divided into the following five categories: the government-led model, the media-led model, the scientific community-led model, the rumor-debunking platform-led model, and the multi-agent collaborative model. In addition, since rumors in public health crises often involve different objects, rumor refutation requires various information sources; therefore, different rumor-debunking models apply. This study verifies the value of socialized collaborative rumor debunking, advocates and encourages the participation of multiple agents of socialization and provides guidance for establishing a collaborative rumor-debunking model, thereby promoting efficient rumor-debunking methods and improving the healthcare of society. Low-income countries, such as Haiti, are facing challenges in fighting the COVID-19 pandemic due to resource shortages and fragile healthcare systems. This study assessed the functional capacity and preparedness of the Haitian healthcare system regarding the COVID-19 pandemic. It employed a narrative review approach to analyze secondary data and used the Donabedian model and the global health security index as the theoretical frameworks to evaluate preparedness. The findings reveal that Haiti faces challenges in tackling the COVID-19 pandemic due to a lack of biosafety and biosecurity regulations, inadequate laboratory systems for COVID-19 testing, and shortages of human resources and personal protective equipment. Moreover, poverty remains widespread, and people lack access to clean water and sanitation services, resulting in a high risk of COVID-19 infection. Furthermore, a lack of communication, rumors, the circulation of fake news regarding COVID-19, and stigmatization cause distrust and reduce the number of people seeking healthcare services. Haiti faces challenges with respect to tackling the pandemic. The Haitian government can strengthen and improve the capacity of the healthcare system to fight against the COVID-19 pandemic and infectious diseases emerging in the future. Full article
