Background and Objectives: Gingival retraction is a critical pre-impression procedure in fixed prosthodontics, crucial for exposing tooth margins and ensuring accurate impressions for restorations like crowns and bridges. This study aimed to evaluate the absorptive capacity of different gingival retraction cords. Materials and Methods: Ninety samples each of Ultrapak (Ultradent, South Jordan, UT, USA) #00, braided cord, coreless thread, and monofilament thread (totaling 270 samples) were immersed in 0.9% NaCl, 10% aluminum chloride, and 12.7% ferrous sulfate solutions for 120, 300, and 1200 s. The liquid absorption capacity was measured using a gravimetric method, and the data were analyzed using an F-test, setting the significance threshold at p < 0.05. Results: The results revealed statistically significant differences in absorption, particularly for aluminum chloride and ferric sulfate (p < 0.001). Ultrapak demonstrated the highest absorption, followed by the coreless cotton thread, while the monofilament thread absorbed the least, especially at 1200 s. Conclusions: These findings indicate that Ultrapak’s superior absorption could enhance moisture control during procedures, highlighting the importance of selecting an appropriate retraction cord for optimal clinical outcomes. Further research is needed to confirm these findings in a clinical setting. Full article

Dental zirconias have been broadly utilized in dentistry due to their high mechanical properties and biocompatibility. Although initially introduced in dentistry as an infrastructure material, the high rate of technical complications related to veneered porcelain has led to significant efforts to improve the optical properties of dental zirconias, allowing for its monolithic indication. Modifications in the composition, processing methods/parameters, and the increase in the yttrium content and cubic phase have been presented as viable options to improve zirconias’ translucency. However, concerns regarding the hydrothermal stability of partially stabilized zirconia and the trade-off observed between optical and mechanical properties resulting from the increased cubic content remain issues of concern. While the significant developments in polycrystalline ceramics have led to a wide diversity of zirconia materials with different compositions, properties, and clinical indications, the implementation of strong, esthetic, and sufficiently stable materials for long-span fixed dental prostheses has not been completely achieved. Alternatives, including advanced polycrystalline composites, functionally graded structures, and nanosized zirconia, have been proposed as promising pathways to obtain high-strength, hydrothermally stable biomaterials. Considering the evolution of zirconia ceramics in dentistry, this manuscript aims to present a critical perspective as well as an update to previous classifications of dental restorative ceramics, focusing on polycrystalline ceramics, their properties, indications, and performance. Over the past two decades, dental ceramics have experienced rapid advances in science and technology, becoming the fastest-growing field of dental materials. This review emphasizes the significant impact of translucent zirconia in fixed prosthodontics, merging aesthetics with strength, and highlights its versatility from single crowns to complex bridgework facilitated by digital manufacturing advancements. The unique light-conducting properties of translucent zirconia offer a natural dental appearance, though with considerations regarding strength trade-offs compared to its traditional, opaque counterpart. The analysis extends to the mechanical attributes of the material, noting its commendable fracture resistance and durability, even under simulated physiological conditions. Various zirconia types (3Y-TZP, 4Y-TZP, 5Y-TZP) display a range of strengths influenced by factors like yttria content and manufacturing processes. The study also explores adhesive strategies, underlining the importance of surface treatments and modern adhesives in achieving long-lasting bonds. In the realm of implant-supported restorations, translucent zirconia stands out for its precision, reliability, and aesthetic adaptability, proving suitable for comprehensive dental restorations. Despite its established benefits, the review calls for ongoing research to further refine the material’s properties and adhesive protocols and to solidify its applicability through long-term clinical evaluations, ensuring its sustainable future in dental restorative applications. Background: The risk of palatally displaced canines (PDCs) rises in patients with tooth agenesis. The orthodontic extrusion and alignment of PDCs require adequate anchorage to enable tooth movement and control the side effects. There is no paper presenting treatment in the case of severe oligodontia with simultaneous PDCs and the use of mini-implants (MIs) for their orthodontic extrusion. Case presentation: A 15-year-old patient presented with non-syndromic oligodontia and bilateral PDCs. Cone beam computed tomography revealed that both PDCs were in proximity to the upper incisors’ roots. There was no evident external root resorption of the incisors. The “canines first” approach was chosen. MIs were used both as direct and indirect anchorage. First, the extrusive forces of cantilevers were directed both occlusally and distally. Next, the buccal directions of forces were implemented. Finally, fixed appliances were used. PDCs were extruded, aligned, and torqued. Proper alignment and occlusion were achieved to enable further prosthodontic restorations. Conclusions: The use of MIs made it possible to avoid collateral effects, reduce the risk of complications, and treat the patient effectively. MIs provide adequate anchorage in demanding cases. The use of MIs for the extrusion of PDCs made it possible to offer this treatment option to patients with severe oligodontia. The presented protocol was effective and served to circumvent treatment limitations associated with an inadequate amount of dental anchorage and a high risk of root resorption. Full article

Direct restorations are currently the most popular restorations used in dental prosthodontics. Due to the increased requirements for materials used in the fabrication of fixed restorations, there is a need for evaluation of strength parameters of these materials, including dental cements. The present study investigated the change in selected strength parameters of four dual-cured composite cements as a function of storage temperature. The following were investigated: three-point flexural strength (FS), flexural modulus in bending (FM), diametral tensile strength (DTS) and Vickers hardness (HV). Four dual-cured composite cements were tested, i.e., Multilink Automix (Ivoclar Vivadent), seT PP (SDI), MaxCem (Kerr), and Bifix Hybrid Abutment (VOCO). Each of the tested cements was stored for 7 days at one of the selected temperatures: 8 °C, 15 °C, 25 °C, or 35 °C, before the samples were made. Strength properties (DTS, FS) are not strongly dependent on the storage temperature in the range of 8–35 °C. Some statistical differences were observed between the hardness of MaxCem and Multilink Automix storage in various temperatures. FS and FM were lowest for Bifix Hybrid Abutment, MaxCem and Multilink Automix storage at 25 °C, and highest for Bifix Hybrid Abutment, MaxCem, and seT PP stored in 35 °C. The cement with the highest filler content (70% by weight) showed the highest FS and HV. Full article

This patient series reports the outcomes of CAD/CAM prosthetic reconstructions in patients with cleft lip and palate (n = 9, aged 27 to 76) who have experienced significant failure with conventional restorative and fixed prosthodontic treatments. The objective of the protocol is to establish a functional and patient-friendly prosthetic structure for individuals with unilateral/bilateral cleft lip and palate (UCLP/BCLP) while minimising the requirement for specialised follow-up care in the cleft unit. The study data were obtained from a retrospective cohort at Helsinki University Hospital. Prosthetic reconstructions were performed using CAD/CAM bar structures by the Atlantis 2in1 system or Createch removable telescope structures, supported by four to eight maxillary dental implants. Out of the nine patients, seven experienced no complications. One prosthesis fracture occurred after 16 months due to a design error in the original framework, and one patient experienced failure of osseointegration in a dental fixture (specifically, one fixture out of the eight maxillary implants in this patient). In total, 56 implants were successfully placed. The maxillary dentition of elderly patients with cleft lip and palate often poses challenges due to periodontal and reconstructive issues. An implant-supported CAD/CAM bar with a removable telescope suprastructure offers an easily maintained and functional solution for dental rehabilitation. Full article

The success of implant-supported fixed complete dental prostheses (ISFCDPs) depends on multiple factors: some are related to the fixtures, such as fixture material, surface characteristics, positioning, and type of connection to prosthetic components; others are related to the prostheses, such as design and materials used. Zirconia is a material widely used in fixed prosthodontics, whether on natural teeth or on implants, with excellent results over time. Regarding the use of zirconia for ISFCDPs, the 2018 ITI Consensus Report stated that “implant-supported monolithic zirconia prostheses may be a future option with more supporting evidence”. Since CAD/CAM technology and zirconia are being continuously innovated to achieve better results and performances over time, a narrative review of the literature seems necessary to focus research efforts towards effective and durable solutions for implant-supported, full-arch rehabilitations. The objective of the present narrative review was to search the literature for studies regarding the clinical performance of zirconia-based ISFCDPs. According to the results of this review, the use of zirconia for ISFCDPs showed good clinical outcomes, with high survival rates ranging from 88% to 100% and prosthetic complications that were restorable by the clinicians in most cases. Digital applications have changed therapy in prosthodontics. In 2017, a systematic review reported on complete digital workflows for treatment with tooth-borne or implant-supported fixed dental prostheses (FDPs). Here, we aim to update this work and summarize the recent scientific literature reporting complete digital workflows and to deduce clinical recommendations. A systematic search of PubMed/Embase using PICO criteria was performed. English-language literature consistent with the original review published between 16 September 2016 and 31 October 2022 was considered. Of the 394 titles retrieved by the search, 42 abstracts were identified, and subsequently, 16 studies were included for data extraction. A total of 440 patients with 658 restorations were analyzed. Almost two-thirds of the studies focused on implant therapy. Time efficiency was the most often defined outcome (n = 12/75%), followed by precision (n = 11/69%) and patient satisfaction (n = 5/31%). Though the amount of clinical research on digital workflows has increased within recent years, the absolute number of published trials remains low, particularly for multi-unit restorations. Current clinical evidence supports the use of complete digital workflows in implant therapy with monolithic crowns in posterior sites. Digitally fabricated implant-supported crowns can be considered at least comparable to conventional and hybrid workflows in terms of time efficiency, production costs, precision, and patient satisfaction. Tooth preparation for a metal-ceramic crown with a subgingival finish line can lead to inflammatory changes in the gingival tissue, often accompanied by cell damage. This study aimed to evaluate the clinical signs of inflammation and the cytomorphological status of the gingival tissue before and after tooth preparation. The research included a homogeneous group of 19 patients with an indication for upper canine preparation. Before and after treatment, the gingival and the gingival bleeding indexes were determined, gingival swabs were taken, and direct smears prepared on slides for cytomorphometric analysis. The values of the measured gingival indexes were statistically significantly higher (p < 0.001) after tooth preparation. They decreased over time, which indicated the reversibility of the resulting changes. Cytological examination showed no statistically significant difference between the values of nuclear area, perimeter, Feret diameter, Feret angle, integrated optical density, MinFeret, and roundness, before and after the treatment. Significantly higher values of circularity, integrated optical density, MinFeret (p < 0.05), as well as roundness (p < 0.001) were found after 72 h, compared to those taken 15 min after tooth crown preparation. This study is a pioneering attempt to show gingival changes during fixed prosthodontic treatment and may shed new light on pathogenetic events in prosthodontic patients. Full article

Fixed restorations are now among the most common restorations in modern dental prosthodontics. The current view in prosthodontics of maximum preparation economy is causing an increased interest in the mechanical properties of cements. Among the most important properties of materials used for indirect cementation are mechanical properties, i.e., hardness and compressive strength. These properties can change as a result of changes in physical factors. The purpose of this study was to analyze the available literature on the effect of conditioning temperature of cements used for cementation of indirect fixed restorations on the durability of their bonding to dental tissues and their mechanical and physical properties. The following databases were used: Mendeley, PubMed, ResearchGate, National Library of Medicine, and Google Scholar. Analysis of the available literature was carried out according to the Prisma diagram program. Forty-eight articles were selected, which were the following types of studies: clinical reports, research article, and review articles. Some studies indicated that mechanical properties, such as flexural strength, polymerization shrinkage, and conversion factor, did not change after heating the composite material. According to some researchers, preheating the composite material increased its conversion degree, which consequently led to an increase in hardness and fracture toughness, an increase in flexural strength and an increase in elastic modulus, and an increase in abrasion resistance. Studies on changes in the mechanical and physical properties of composite materials, as well as composite cements, have not always provided clear answers, as there are still no laboratory and clinical studies that fully confirm the benefits of heating composite cements. Conducting studies evaluating the effect of elevated storage temperature on the strength parameters of cements, in conjunction with the type of material and its composition, could provide answers to many clinical questions that are still unresolved. If the benefits of heating cements were unequivocally confirmed in laboratory studies, this could open up many possibilities for improving the retention of fixed prosthetic restorations. Full article

Background and objectives: Porcelain-fused-to-metal (PFM) prostheses are considered the gold standard for the replacement of missing teeth, however, these have several drawbacks. Therefore, lithium disilicate (LDS) prostheses have been introduced for the construction of fixed crowns and bridges. The aim of this systematic review was to ascertain the long-term survival of LDS fixed prostheses in comparison to other materials. Materials and methods: The focused question was ‘In patients who have undergone prosthodontic treatment (participants), what are the overall survival rate of lithium disilicate (LDS) crowns and fixed bridges; and how do they relate to survival rates of non-LDS similar restoration are the survival and com-plication rates (outcomes) of LDS-based fixed prostheses with complete coverage (intervention) higher or lower when compared to non-LDS materials (controls)?’. An electronic search was conducted in PubMED/Medline, EMBASE, Google Scholar, and ClinicalTrials.gov for articles published between January 2006 and August 2022 using appropriate MeSH terms and keywords. The following types of studies were included: (1) All types of prospective clinical studies; (2) Clinical studies focusing on the survival of fixed LDS bridges and crowns; (3) Studies using natural teeth with complete coverage as abutment for fixed LDS bridges and crowns; and (4) Studies in English. The following studies were excluded: (1) Laboratory/in vitro studies and studies on LDS prostheses with no description of outcomes or survival rates; (2) Commentaries; (3) Letters to the editor; (4) Reviews; and (5) Internal data from manufacturers. The data from included studies were extracted and the risk of bias was assessed within the studies using ROBINS-I. Results: A total of 25 studies were included in this systematic review. The overall 5-year and 10-year survival rates were of 95–100% and 71.4–100%, respectively. Generally, three-unit bridges had a significantly lower survival rate over 5 and 10 years compared to single-unit crowns. Overall, the risk of bias in the included studies was moderate. Conclusions: The LDS-based complete coverage prostheses have a survival rate ranging between 48.6% and 100%. Furthermore, due to the lack of comparative studies, the long-term function and survival of LDS prostheses compared to other material prosthesis (PFM and ZrO) is debatable. Full article

Social media like YouTube are increasingly used by students as a learning tool. The aim of this study was to examine the use of YouTube videos as a means of learning clinical procedures among dental students in Riyadh, Saudi Arabia. A cross-sectional observational study was conducted among dental students from six dental colleges in Riyadh, Saudi Arabia. Among the total of 331 dental students who responded to this survey, 93.9% (n = 308) reported that they had used YouTube for dental learning. A total of 65.30% (n = 201) of the respondents strongly agreed that they find videos of clinical procedures on YouTube helpful as a learning tool. A total of 54.40% (n = 180) agreed that they always refer to YouTube videos to prepare for a clinical procedure that they have never done before. A total of 75.3% (n = 232) reported that they most commonly watched clinical procedures related to restorative dentistry, 67.2% (n = 207) for fixed prosthodontics procedures, 65.3% (n = 201) for endodontic clinical procedures, and 62.3% (n = 192) for removable prosthodontics videos. A total of 50.60% (n = 156) strongly agreed that YouTube videos are helpful in relating theoretical knowledge with clinical knowledge. A total of 50.30% (n = 155) reported that it is important to have faculty guidance regarding useful YouTube videos on dental procedures. A total of 91.90% (n = 283) participants would like their dental school to post tutorials or videos for clinical procedures on YouTube. Even after increased availability of online videos, a majority of dental students felt that YouTube videos suggested by faculty were more valuable than videos identified through normal searching. Long-Term Treatment Outcomes of Implant Prostheses in Partially and Totally Edentulous Patients

Implant dental therapy is a clinical procedure used for treating patients with tooth loss with known clinical success. This clinical study aimed to evaluate the long-term clinical outcomes of dental implants in partially and totally edentulous patients. A total of 544 Microdent (Microdent SU, Implant Microdent System®, Santa Eulàlia de Ronçana Barcelona, Spain) screw implants were placed in 111 patients using a two-stage surgical technique and a conventional loading protocol (lasting 3 months). Implant and prosthetic clinical findings were evaluated during a 15-year follow-up. A total of 6 implants were lost during the healing period, and 124 prostheses were placed over the 538 implants that remained: 20 single crowns, 52 partially fixed bridges, 45 full-arch fixed restorations, and 7 overdentures. A total of 20 of these were lost during the follow-up period. The cumulative survival rate for all implants was 96.4%. The data underwent statistical analysis (significance level: p < 0.05). The mean marginal bone loss was 1.82 ± 0.54 mm, ranging from 1.2 to 3.1 mm. The most frequent complications were mechanical prosthodontic complications (16.2%). In all, 11.8% of implants showed periimplantitis as the primary biological complication. Dental implants inserted in both the maxillary and mandibular areas produce long-term favorable outcomes and stable tissue conditions when a delayed loading protocol is followed. Full article

Microleakage is a persistent problem despite advancement in materials and techniques in fixed prosthodontics. This leads to the importance of sound crown foundation material and luting agents used to maintain the marginal seal. The literature is deficient with studies, comparing microleakage under various crown foundation materials and luting agents, especially with CAD-CAM (computer-aided design and computer-aided manufacturing) metal crowns. This study was aimed to compare microleakage in a nanocomposite/dentinal bonding agent and amalgam/cavity varnish as crown foundation materials luted with two different luting cements: resin-reinforced glass ionomer cement and self-adhesive resin cement, under both dry and contaminated conditions. A hundred intact, caries-free human molars were prepared to receive crown foundation material and extra coronal restorations. Amalgams with cavity varnish and nanocomposites with dentinal bonding agent in both ideal and contaminated conditions were used as crown foundation materials. After restoration, each sample was cemented with a CAD-CAM milled metal crown using two different luting agents—resin-reinforced glass ionomer cement and self-adhesive resin cements both in ideal and contaminated conditions. Cementation was followed by thermocycling of samples, immersion in erythrosine B dye, embedding in clear auto polymerizing acrylic resin and sectioning to evaluate microleakage using stereomicroscope. The mean microleakage between different luting cements on the experimental side of the facial surface was 137.64 μm and 211.01 μm for resin-reinforced GIC and for self-adhesive resin cement was 119.78 μm and 150.42 μm, under ideal and contaminated condition, respectively. There was a significant difference in mean micro-leakage between different crown foundation material and cement groups used in the study. The composites and amalgam, both when used as crown foundation material and luted with use of technically advanced CAD-CAM metal crown with self-adhesive resin cement (in both ideal or contaminated condition), showed less microleakage than in resin-reinforced glass ionomer cement. Overall, the self-adhesive resin cement showed comparatively reduced microleakage in all combinations with different crown foundations. Thus, this combination can be used in daily clinical practice to provide better protection from further decay. Full article

Periodontal health plays an important role in the longevity of prosthodontic restorations. The issues of comparative assessment of prosthetic constructions are complicated and not fully understood. The aim of this article is to review and present the current knowledge regarding the various technical, clinical, and molecular aspects of different prosthetic biomaterials and highlight the interactions between periodontal health and prosthetic restorations. Articles on periodontal health and fixed dental prostheses were searched using the keywords “zirconium”, “CAD/CAM”, “dental ceramics”, “metal–ceramics”, “margin fit”, “crown”, “fixed dental prostheses”, “periodontium”, and “margin gap” in PubMed/Medline, Scopus, Google Scholar, and Science Direct. Further search criteria included being published in English, and between January 1981 and September 2021. Then, relevant articles were selected, included, and critically analyzed in this review. The margin of discrepancy results in the enhanced accumulation of dental biofilm, microleakage, hypersensitivity, margin discoloration, increased gingival crevicular fluid flow (GCF), recurrent caries, pulp infection and, lastly, periodontal lesion and bone loss, which can lead to the failure of prosthetic treatment. Before starting prosthetic treatment, the condition of the periodontal tissues should be assessed for their oral hygiene status, and gingival and periodontal conditions. Zirconium-based restorations made from computer-aided design and computer-aided manufacturing (CAD/CAM) technology provide better results, in terms of marginal fit, inflammation reduction, maintenance, and the restoration of periodontal health and oral hygiene, compared to constructions made by conventional methods, and from other alloys. Compared to subgingival margins, supragingival margins offer better oral hygiene, which can be maintained and does not lead to secondary caries or periodontal disease. Full article
