Objective: Several studies have compared the accuracy of digital scans and conventional impressions for post space capture. However, only a few have specifically investigated the precision of intraoral scanners in measuring post spaces of varied lengths. This study aimed to evaluate the accuracy of various intraoral scanning techniques in capturing long and short post spaces. Material and Methods: This study grouped samples into eight categories based on four techniques and two post space depths (7 and 11 mm). After tooth preparation, root canal treatment, and post space preparation, laboratory scans were performed using Duralay. Intraoral scans were obtained directly and indirectly with the Trios fourth generation scanner using the Duralay and PVS techniques. The accuracies, in terms of trueness, and precisions were compared after ten repetitions for each group using the Kruskal–Wallis or Mann–Whitney U tests. Results: The Duralay Intraoral Scan groups demonstrated a high consistency, while the Direct Intraoral Scan groups showed moderate consistency. Variability was higher for the Duralay Lab Scan and PVS Intraoral Scan groups for short post spaces. Conclusions: The capture technique significantly affected the accuracies of the post space measurements. The techniques also demonstrated varying consistency and precision. These findings provide critical insights to guide their selections for clinical and research applications. Clinical Significance: This study is one of the few to compare the accuracy of intraoral scanners for the capture of both short and long post spaces. It addresses a key gap in current dental research and offers practical guidance for clinicians and researchers in selecting appropriate scanning techniques for various clinical scenarios. The findings have the potential to enhance the accuracy and reliability of post space measurements and improve patient outcomes. Full article

This critical narrative review presents the concepts and methods that have been or are currently applied in the treatment of periapical tissue changes in mature and immature permanent teeth. Treatment success is defined as the healing of the inflammatory lesion in permanent teeth and, additionally, the completion of root development in immature teeth. Background/Objectives: Endodontics focuses on the prevention and treatment of diseases affecting the dental pulp and periapical tissues. Periapical changes have been managed using various methods depending on factors such as the extent of the lesion and the stage of root and apical development. Conventional root canal treatment, revitalization, and apexification have all been employed. Methods: Three databases (PubMed, Scopus, and Web of Science) were searched for studies discussing different treatment approaches, materials, and the efficacy of techniques used over time in mature and immature permanent teeth. Results: This review includes seven case reports, seven case series, and three cohort studies, each detailing the treatment method, case characteristics, follow-up period, and treatment outcomes. Conclusions: Modern materials have significantly improved the outcomes of revitalization and apexification procedures. These methods can now compete with or even surpass the clinical effectiveness of conventional root canal therapy. Full article

Chemical endodontic irritants can lead to the demineralization of the inorganic tooth structure, its loss of integrity, microhardness changes, erosion, and an increased risk of fractures. We investigated the action of iron oxide nanomagnet particles (IONPs) as an irrigant solution for improving hardness and identifying the concentration of element ions in the root canal. There were six groups in total: a control group (no treatment) and experimental groups (UN: ultrasound agitation normal saline, UI: ultrasound agitation IONPs, MSI: magnetic field and endodontic needle with syringe agitation IONPs, MUI: magnetic field and ultrasound agitation IONPs, and EDTA: ethylenediaminetetraacetic acid). We hypothesized that IONPs with magnetic agitation would preserve microhardness better than EDTA. Vickers hardness testing was used to evaluate microhardness, which was then analyzed using energy-dispersive X-ray spectroscopy (EDS) to investigate the calcium/phosphorus ratio and the presence of iron. The IONP groups exhibit a higher VHN value than the EDTA group (p < 0.05). These results support our hypothesis, indicating that utilizing an IONP irrigant solution with an external magnetic field does not change microhardness but enhances it compared to the EDTA group, suggesting that employing an external magnetic field to deliver nanoparticles to the root canal wall does not affect the properties of the tooth structure compared to conventional instrumentation techniques, which lead to unnecessary loss of root structure. Background and objectives: Post-operative pain (POP) is a common complication after root canal treatment and is influenced by various clinical and patient-related factors. The present study evaluated the incidence and intensity of POP following root canal treatment using a bioceramic sealer performed by operators with different levels of experience. Methods: A total of 115 patients were included in this prospective observational study. Patients were treated by operators with different levels of experience: postgraduate students (PGSs) and endodontic specialists (ESs). Standardized protocols were used in two distinct appointments: an instrumentation visit and an obturation visit. Obturation was performed using the continuous wave condensation technique and Total Fill Hi-Flow BC Sealer (TFHF). POP, mastication discomfort, and sleep disturbance were assessed at 24, 48, and 72 h after the instrumentation and obturation phases using a numeric rating scale (NRS). Results: The results indicate significantly higher POP after the instrumentation phase, compared to the obturation phase (p < 0.001). The pain intensity progressively decreased over time for both phases. No significant differences were observed between the PGS and ES groups regarding POP, mastication discomfort, or sleep disturbance at any time. Sealer extrusion did not significantly impact POP (p > 0.05). Conclusions: This study found that operator experience does not significantly influence POP when a standardized protocol is followed. The use of TFHF with the continuous wave condensation technique was associated with minimal POP. Full article

Objectives: Although many studies have demonstrated the effectiveness of intraosseous anesthesia (IOA), the experience of clinicians administering IOA has rarely been reported. Some endodontists may never have received formal training in IOA, raising questions about whether inexperienced clinicians can achieve similar results. Although the previous studies suggest that limited experience may increase the risk of complications, the extent to which clinician experience influences IOA outcomes remains uncertain. This study sought to assess whether clinician experience affects the effectiveness, patient comfort, and safety of IOA in endodontic treatment. Methods: This retrospective cohort study included 72 patients who had previously undergone endodontic treatment or retreatment for mandibular molars. Of these, 37 were treated by an endodontic resident with limited IOA experience, and 35 by an endodontist with extensive IOA experience. IOA was administered using the QuickSleeper 5 system (Dental Hi Tec, Cholet Cedex, France) as the sole anesthetic technique before the start of the endodontic treatment or retreatment. Pain assessments were recorded preoperatively (baseline), during IOA administration, and during the treatment. Statistical analyses included binomial and chi-square tests to compare the success rates and associations between the clinician experience and the IOA outcomes. Statistical significance was set at p < 0.05. Results: Pain during IOA administration was reported as none or mild in 98.6% of the patients, with no significant difference between the endodontist (97.2%) and resident (100%) groups (p = 0.378). Profound anesthesia was achieved in 94.1% of the cases, with no significant difference between the endodontist (96.8%) and resident (91.9%) groups (p = 0.738). Complications were rare, with two cases of perforator separation (5.4%) in the resident group; nevertheless, clinician experience was not significantly associated with complications (p = 0.553). Conclusions: Within the limitations of this study, clinician experience did not significantly affect the IOA outcomes, including effectiveness, patient comfort, and safety. These findings suggest that IOA can be used effectively by clinicians with different levels of experience, thus supporting its broader implementation in endodontic practice. Full article

Background/Objectives: The removal of a fiber post (FP) during endodontic retreatment can be the source of significant complications. This study evaluated two commonly used techniques in removing a fiber post from an endodontically treated tooth by investigating three metrics: volume of dentin removed, efficiency, and temperature increase. Methods: Thirty extracted, single-rooted teeth were decoronated at the CEJ, then underwent endodontic treatment and post-space preparation. Fiber posts were bonded within the canal space. Teeth were pair-matched and randomly assigned to undergo post removal via Munce bur (MB) or diamond-coated ultrasonic tip (US). Teeth were scanned with micro-CT prior to post placement and after post removal. Results: The volume of dentin removal was not statistically significant between groups (p > 0.05), but the Munce bur resulted in eccentric removal patterns. There was a statistically significant difference in the time required to remove the fiber post between MB and US (p < 0.05). Removal of a fiber post with a Munce bur took an average of 58 s. Removal of a fiber post with an ultrasonic tip took an average of 502 s. There was no statistically significant difference in maximum temperature generated during post removal between MB and US (p > 0.05). Conclusions: Removal of a FP with a Munce bur requires significantly less time when compared to using an ultrasonic tip, with reduced risk of generating excessive heat for either technique with adequate coolant. US can stay more centered in the canal during FP removal when compared to Munce burs, potentially reducing unfavorable outcomes. Full article

Background and Clinical significance: Apical fenestration is a rarely reported clinical finding that may be associated with apical periodontitis. However, its diagnosis can often be complicated by overlapping clinical and radiographic features. While management traditionally involves a combination of endodontic and surgical interventions, there is limited documentation regarding successful outcomes achieved through non-surgical treatment alone. Therefore, further reporting and investigation of such cases are warranted to enhance clinical understanding and inform decision-making. Case Presentation: This case report describes the non-surgical management of a 20-year-old patient presenting with symptomatic apical periodontitis and a labial apical fenestration in a previously treated maxillary left central incisor (tooth #21) exhibiting an open apex. Diagnosis was confirmed using cone-beam computed tomography (CBCT), which revealed a bone defect in the facial cortical plate. The treatment protocol involved conservative canal debridement, intracanal placement of calcium hydroxide, and final obturation using an apical plug of calcium silicate-based hydraulic cement (CSBHC) and the monoblock technique. Over a follow-up period of two years and eight months, clinical and radiographic assessments demonstrated resolution of symptoms, healing of the sinus tract, and complete regeneration of the buccal cortical bone. Conclusions: This case highlights the potential for complete healing of apical fenestration associated with apical periodontitis in an open apex tooth through non-surgical endodontic treatment alone. Background and Objectives: Successful endodontic treatment requires thorough disinfection and removal of the smear layer to prevent reinfection. However, conventional irrigants like sodium hypochlorite (NaOCl) and ethylenediaminetetraacetic acid (EDTA) can compromise dentin integrity. This study assessed the efficacy of the Synergistic Microbicidal and Ablative Root canal Technique (SMART), which integrates AromaRoot, a biocompatible irrigation solution based on quaternary ammonium compounds, with 980 nm diode laser activation, to enhance bacterial reduction and smear layer removal. Materials and Methods: Sixty extracted single-rooted human teeth were inoculated with Enterococcus faecalis and divided into four treatment groups using NaOCl, AromaRoot, and 980 nm laser, either alone or in combination. Bacterial counts were measured as colony-forming units per milliliter (CFU/mL). For smear layer analysis, 56 extracted teeth were prepared and irrigated using EDTA, AromaRoot, and laser activation, followed by scanning electron microscopy to evaluate dentinal tubule exposure. Data were analyzed using Kruskal–Wallis and ANOVA. Results: The combination of AromaRoot, NaOCl, and laser activation achieved a 99.00% bacterial reduction (from 8082 to 60 CFU/mL, p < 0.001), outperforming NaOCl alone (98.34%, 131 CFU/mL). For smear layer removal, AromaRoot with laser achieved 78.5% open dentinal tubules in the apical third, significantly higher than EDTA alone (64.5%, p < 0.05), though EDTA remained superior in the coronal third (89.0% vs. 81.0%, p > 0.05). Conclusions: The SMART technique significantly improves both disinfection and smear layer removal in root canal therapy, particularly in the apical region. These findings suggest that AromaRoot, especially when laser-activated, may serve as a safe and effective alternative to conventional irrigants, warranting further clinical evaluation. Full article

Objectives: Endodontic microsurgery has become an integral part of daily endodontic practice. However, research on the correlation between the lesion characteristics observed via cone beam computed tomography (CBCT) and pain and swelling after endodontic microsurgery (EMS) is still lacking in the literature. The present study aims to examine the relationship between the radiographic characteristics of preoperative periapical lesions obtained from CBCT images and post-surgical symptoms such as pain and swelling. Materials and Methods: A total of 61 patients undergoing EMS utilizing modern techniques were asked to report their level of pain and swelling at 8, 24, 48, and 72 h after EMS using VAS. Independent variables such as age, gender, tooth location, CBCT periapical index, endodontic diagnosis, cortical bone perforation by the lesion, duration of the EMS, preoperative analgesic consumption, antibiotic prescription, and pre-/postoperative mouthwash were analyzed using the Fisher Exact test. Multivariate regression analysis was also conducted to determine the independent significant factors associated with pain and swelling. A p-value of ≤0.05 was considered statistically significant. Results: The maximum pain score was recorded at 8 h (4.26 ± 3.13), while peak swelling was measured after 24 h (6.46 ± 2.87). The risk of swelling was more likely to decrease by 75.7% for patients with a CBCT index score of >3 than those with a CBCT index score of ≤3 (AOR = 0.243; CI = 0.071–0.831; p = 0.024). The effects of all other factors on pain, including cortical bone perforation by the lesion (p = 0.290), swelling (p = 0.071), postoperative mouthwash use (p = 0.062), and swelling (p = 0.934), did not reach statistical significance. Conclusions: Patients with periapical lesions larger than 4 mm will likely experience less swelling after EMS, while pain is not affected by lesion size, cortical bone perforation, or mouthwash use. Clinical Relevance: This study identified a new predictor of swelling after EMS based on the size of the periapical lesion. These results will improve the management of post-surgical sequelae after EMS and support shared decision making. Full article

Background/Objectives: Evaluation of artifact expression in CBCT images caused by different intracanal medications (IMs) considering variations in scanning and reconstruction protocols. Reconstruction protocols refer to the specific parameters and image processing techniques applied during CBCT acquisition, including voxel size, slice thickness, and artifact reduction settings. MicroCT was used as the reference standard. Methods: Root canal preparation (45./05) of upper canines was performed, and the teeth were divided into four groups (n = 10) according to the IM used: G1: Ultracal XS (UC); G2: Bio-C Temp (BCT); G3: Metapex (MT); and G4: Metapaste (MP). The specimens were sealed with temporary provisional material and stored at 37 °C and 100% humidity for 7 days. Then, they were scanned using microCT (SkyScan 1174) and two CBCTs in high and low resolutions: EagleX3D and OP300. Image registration was performed using FIJI ImageJ software (v. 1.54k). Axial, sagittal, and coronal reconstructions were quali-quantitatively evaluated by two calibrated examiners following the scores for the artifacts (dark streaks, hypodense areas, and distortion): definitely absent; probably absent; not sure; probably present and definitely present; and the possibility of using the images for endodontic diagnosis: No/Yes. Statistical analysis was performed using Fleiss’ kappa test and two-way ANOVA (α = 95%). Results: CBCT images showed greater volume distortion of intracanal medication (p < 0.05) compared to microCT images. X3D CBCT showed the highest values of distortion, regardless of resolution, compared to OP300 (p < 0.05). The highest and lowest volume distortion for intracanal medications was observed in the UC and BCT groups, respectively (p < 0.05). Conclusions: Intracanal medication generates metallic artifact expression in CBCT images, hampering endodontic diagnosis. Full article

Maxillary first premolars pose challenges due to their anatomical characteristics and positioning within the dental arch. This study aimed to retrospectively analyze cone-beam computed tomography (CBCT) data to determine the horizontal X-ray beam angulations for maxillary first premolars at which root canals overlap and neighboring tooth superimposition occur, providing clinically relevant guidance for optimizing IO radiographic techniques. Methods: CBCT scans from 85 patients were analyzed using ImageJ software to measure the angles at which maxillary first premolar root canals overlap or become obscured by adjacent teeth. The mean angles for canal overlap and neighboring tooth superimposition were determined. Statistical analysis was performed using SPSS Version 29.0, and the level of significance was set to 5%. Results: The mean angle for root canal overlap was 93.56° (±10.08). The angles at which neighboring teeth began to superimpose were 124.38° (±9.91) for the distal contour of the canine and 63.46° (±9.38) for the mesial root contour of the second premolar. No significant differences were observed between apical and coronal measurements for root canal overlap but tapering of the roots led to significant differences in neighboring tooth superimposition (p < 0.05). Conclusions: A mesial beam shift within a calculated safe corridor (98.5–129.5°) optimizes canal separation without superimposition from adjacent teeth. For ideal visualization, a mesial angulation close to 40° is recommended. These findings support improved IO radiographic techniques while minimizing the risk of retakes in adherence to the ALARA principle. Full article

Background/Objectives: Bacterial biofilms on root surfaces outside the apical foramen are linked to refractory apical periodontitis, as microorganisms can survive in extraradicular areas and cause persistent infections. This study aimed to precisely evaluate the relationship between extraradicular biofilm and persistent periapical periodontitis through an overview of case reports. Methods: A systematic search of PubMed, Web of Science, Scopus, Embase and ScienceDirect databases was conducted up to June 2023. Keywords included “extraradicular infection”, “wet canal”, “wet canals”, “extraradicular mineralized biofilms”, and “calculus-like deposit”. Only case reports meeting the inclusion criteria were analyzed. Results: Fifteen cases of extraradicular infection were identified, involving eight women and six men aged between 18 and 60 years. These cases included nine failed treatments confirmed through complementary methods such as histobacteriologic analysis, scanning electron microscopy (SEM), or polymerase chain reaction (PCR). Among these, four patients (six teeth) exhibited calculus-like deposits. Conclusions: Extraradicular biofilm is strongly associated with failed endodontic treatments, leading to persistent infections. A structured decision-making approach is essential. Before considering apical surgery, clinicians should prioritize intraradicular infection control through thorough irrigation, antimicrobial medicaments, and adjunctive disinfection techniques. When extraradicular biofilms or mineralized calculus are present, and symptoms persist after optimal intracanal disinfection, apical surgery should be performed. Full article

Background/Objectives: C-shaped canals represent a challenge in endodontic procedures. The present study aimed to investigate the prevalence and characteristics of C-shaped canals in mandibular second molars in Guayaquil, Ecuador. Methods: CBCT records from two radiology centers were examined (2020–2022). A total of 400 CBCT scans (800 mandibular second molars) were analyzed using Fan’s classification. Data on the presence or absence of C-shaped canals, gender, and bilateral occurrence were collected. Statistical analysis included the test for differences in proportions and the chi-squared test to assess the significance of correlations between variables. Results: C-shaped canals were found in 28% of mandibular second molars, affecting 33.75% of the patient sample. The prevalence was significantly higher in females (42.9%) compared to males (20.4%). Bilateral occurrences were observed in 63% of affected patients. Morphologic variability was pronounced, with Fan C1 and C4 dominating in the coronal and apical thirds, respectively, while a shift toward Fan C2 and C3 was observed in the middle third. Only 19.2% of the teeth maintained a consistent morphology. Conclusions: C-shaped canals were found in 28% of mandibular second molars and occurred predominantly in females. Notably, most cases were bilateral, highlighting the importance of effectively adapting endodontic techniques to treat this unique anatomical variation. Further research into genetic and environmental influences could deepen our understanding and help develop refined diagnostic and therapeutic strategies. Full article

Enterococcus faecalis is strongly associated with secondary/persistent root canal infections, being the most prevalent bacterium in cases of apical periodontitis in previously treated teeth. This study was elaborated to evaluate the anti-planktonic, antibiofilm, and synergistic effects of Nasturtium officinale and Psidium guajava hydroethanolic extracts against standard and clinical strains of E. faecalis. Firstly, the N. officinale extract was prepared from watercress leaves, and P. guajava extract was prepared from guava tree leaf shoots. Then, the content of soluble solids was quantified in both. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) values of the isolated N. officinale and P. guajava extracts for each bacterial strain were determined using the broth microdilution method, following the Clinical and Laboratory Standards Institute (CLSI) guideline M7-A9. The MTT assay was used to evaluate the antibiofilm activity, and the fractional bactericidal concentration index (FBCI) was utilized to evaluate the synergistic effect of the N. officinale and P. guajava extracts using the checkerboard technique. Again, the MTT assay was used to evaluate the antibiofilm activity of the combined extracts this time. The data were subjected to statistical analysis using ANOVA and Tukey’s test, with a significance level of p ≤ 0.05. It was found that the soluble solid content of N. officinale was 50 mg/mL, and of P. guajava was 33.5 mg/mL. The MBC value of N. officinale was 12.5 mg/mL, and of P. guajava was 0.52 mg/mL against all the tested strains of E. faecalis. The combined 0.1 mg/mL N. officinale + 0.1 mg/mL P. guajava, and 0.1 mg/mL N. officinale + 0.5 mg/mL P. guajava hydroethanolic extracts effectively reduced the biofilm formation of the standard and clinical strain 4 of E. faecalis. Therefore, these combined extracts may be considered as endodontic irrigants in future studies. Full article

Quality of Single-Cone Obturation Using Different Sizes of Matching Gutta-Percha Points of Two Reciprocating Single-File Systems in Curved and Straight Root Canals

Background and Objectives: Endodontic success depends on eliminating infection and creating a durable seal to prevent recontamination. The goal of this study was to assess the impact of different ISO sizes on the obturation quality using two reciprocating single-file systems, WaveOne® Gold and Procodile®, in two different canal morphologies. Material and Methods: Overall, 140 root canals from human permanent teeth were randomly assigned to 14 groups based on selected ISO sizes and straight and curved canal curvatures, and the two file systems, WaveOne® Gold files in ISO sizes 20, 25, and 45, and Procodile® files in ISO sizes 20, 25, 40, and 45, were employed for canal preparation. These 140 canals were obturated using corresponding gutta-percha points and AH-Plus sealer and the quality of the obturation was assessed after sectioning the roots (apical, middle, coronal third) by evaluating the resulting 420 sections under a digital fluorescence microscope with regard to the proportion of gutta-percha, sealer, and unfilled areas. The results were analyzed using nonparametric tests. Results: For both systems, there was a significant difference in the percentage of gutta-percha-filled areas (PGFA, p < 0.001) and sealer-filled areas (PSFA, p < 0.001 among the different ISO sizes). However, no significant difference was observed in the percentage of unfilled areas (PUA, p = 0.354). ISO 40 demonstrated the best results, with the highest percentage of gutta-percha-filled areas (87%) and the lowest percentages of sealer-filled areas (13%) and voids (0.5%). In contrast, the lowest percentages of gutta-percha filled areas were observed in root canal fillings with ISO 20 (81%) and ISO 25 (81%). Regarding both reciprocating file system sizes, ISO 45 in WaveOne® Gold and ISO 40 in Procodile® demonstrated significantly improved (p < 0.05) filling quality, with PGFA of 85% and 87%, respectively. The differences between both systems were not significant. Conclusions: The results presented suggest that larger sizes provide better filling results, especially in the apical region. These results underline the importance of selecting appropriate preparation sizes adjusted to the initial anatomical specifications to optimize root canal obturation and ensure a high quality and durable seal.
