Leadless Pacemaker Implantation During Extraction in Patients with Active Infection: A Comprehensive Analysis of Safety, Patient Benefits and Costs

Background: Cardiac implantable electronic device (CIED) infections necessitate extraction and subsequent pacing interventions. Conventional methods after removing the infected CIED system involve temporary or semi-permanent pacing followed by delayed permanent pacemaker (PPM) implantation. Leadless pacemakers (LPs) may offer an alternative, allowing immediate PPM implantation without increasing infection risks. Our objective is to evaluate the safety and cost-effectiveness of LP implantation during the same procedure of CIED extraction, compared to conventional two-stage approaches. Methods: Pacemaker-dependent patients with systemic or pocket infection undergoing device extraction and LP implantation during the same procedure at Sheba Medical Center, Israel, were compared to a historical group of patients undergoing a semi-permanent (SP) pacemaker implantation during the procedure, followed by a permanent pacemaker implantation. Results: The cohort included 87 patients, 45 undergoing LP implantation and 42 SP implantation during the extraction procedure. The LP group demonstrated shorter intensive care unit stay (1 ± 3 days vs. 7 ± 12 days, p < 0.001) and overall hospital days (11 ± 24 days vs. 17 ± 17 days, p < 0.001). Rates of infection relapse and one-year mortality were comparable between groups. Economic analysis revealed comparable total costs, despite the higher initial expense of LPs. Conclusions: LP implantation during CIED extraction offers significant clinical and logistical advantages, including reduced hospital stays and streamlined treatment, with comparable safety and cost-effectiveness to conventional approaches. Effective management and handling of construction and demolition waste (CDW) can yield significant technical and environmental benefits for road pavement construction. This article aims to provide a comprehensive and up-to-date chronological review of studies on the mechanical performance of asphalt mixtures—primarily hot mix asphalt (HMA)—incorporating recycled concrete aggregate (RCA). Since the main limitation of RCA is the presence of residual adhered mortar, the review also includes studies that applied various surface treatments (mechanical, chemical, and thermal, among others) to enhance mixture performance. The article summarizes the experimental procedures used and highlights the key findings and conclusions of the reviewed research. Although the results are varied and sometimes contradictory—mainly due to the source variability and heterogeneity of RCA—the use of these materials is technically viable. Moreover, their application can provide environmental, social, and economic advantages, particularly in the construction of low-traffic roadways. Finally, the article identifies research gaps and offers recommendations for future researches. Full article

A Prediction Method for Technically Recoverable Reserves Based on a Novel Relationship Between the Relative Permeability Ratio and Saturation

Upon reaching stabilized production in waterflooded reservoirs, waterflood performance curves are conventionally used to predict technically recoverable reserves (TRRs). However, as reservoirs enter high water-cut stages, the relationship between the relative permeability ratio and saturation becomes nonlinear, causing deflection in waterflood performance curves. This leads to systematic overestimation of both predicted TRR and ultimate recovery factors. To overcome these limitations in conventional TRR prediction methods, this study establishes a novel relative permeability ratio-saturation relationship based on characteristic relative permeability curve behaviors. The proposed model is validated for three distinct fluid-rock interaction types. We further develop a permeability-driven forecasting model for oil production rates and water cuts. Comparative analyses with a conventional waterflood curve methodology demonstrate significant accuracy improvements. The results show that while traditional methods predict TRR ranging from 78.40 to 92.29 million tons, our model yields 70.73 million tons—effectively resolving overestimation issues caused by curve deflection during high water-cut phases. This approach establishes a robust framework for determining critical development parameters, including economic field lifespan, strategy adjustments, and ultimate recovery factor. Full article

We need to educate students and the public about addressing natural resource challenges to maintain civilization moving into a sustainable future. Because US mineral and energy resources are found in its continental crust and sedimentary basins, introductory geology students need to be well-informed about US crust and basins. We think that creating effective videos about these topics is the best way to engage students to want to learn more. In preparation for making these videos, we researched what introductory geology students are taught and what they learn about these topics. Student interviews informed us about learned curriculum, and taught curriculum was analyzed using a novel keyword-counting method applied to textbook indices. We found that geophysics is stressed twice as much as geology, radiometric dating, and sedimentary basins. We expected that students would have learned more about geophysics and less about the other topics; however, this was not the case. Students knew more about geology, and less about geophysics, radiometric dating, and sedimentary basins. To make effective videos on these topics, we need to explain the following threshold concepts: seismic refraction to scaffold student understanding of crustal geophysics, as well as radiometric dating and deep time to understand crustal geology and the economic importance of sedimentary basins. Understanding the entrepreneurial profile is essential for developing effective regional policies that promote business growth. The path of an entrepreneur is shaped not only by individual decisions but also by the inherent risks of managing a business. This study aims to identify the characteristics of entrepreneurs at the regional level, specifically highlighting the impact of accumulated experience in their fields. Our central hypothesis asserts that entrepreneurial experience significantly influences how business owners perceive and respond to economic challenges. Utilizing survey data from 120 entrepreneurs in Romania’s Center Region (a NUTS 2 area), we reveal that entrepreneurial experience profoundly affects perceptions of key business challenges, such as legislative instability, taxation predictability, governmental support strategies, and access to SME financing. Importantly, our findings demonstrate that entrepreneurs with less than 10 years of experience express greater concerns about these challenges compared to their more seasoned peers. This novel insight highlights the need for tailored policy interventions aimed at enhancing regional economic resilience and fostering entrepreneurial sustainability. By addressing the specific needs of less experienced entrepreneurs, our study contributes to a deeper understanding of how experience shapes business dynamics in the region. Full article

Mangoes are an important part of Hainan’s tropical characteristic agriculture. In response to the requirements of building an ecological civilization pilot demonstration zone in Hainan, China, green and sustainable development will be the future development trend of the mango planting system. However, the economic benefits and environmental impact during its planting and management process remain unclear. This paper combines emergy, life cycle assessment (LCA), and economic analysis to compare the system sustainability, environmental impact, and economic benefits of the traditional mango cultivation system (TM) in Dongfang City, Hainan Province, and the early-maturing mango cultivation system (EM) in Sanya City. The emergy evaluation results show that the total emergy input of EM (1.37 × 1016 sej ha−1) was higher than that of TM (1.32 × 1016 sej ha−1). From the perspective of the emergy index, compared with TM, EM exerted less pressure on the local environment and has better stability and sustainability. This was due to the higher input of renewable resources in EM. The LCA results showed that based on mass as the functional unit, the potential environmental impact of the EM is relatively high, and its total environmental impact index was 18.67–33.19% higher than that of the TM. Fertilizer input and On-Farm emissions were the main factors causing environmental consequences. Choosing alternative fertilizers that have a smaller impact on the environment may effectively reduce the environmental impact of the system. The economic analysis results showed that due to the higher selling price of early-maturing mango, the total profit and cost–benefit ratio of the EM have increased by 55.84% and 36.87%, respectively, compared with the TM. These results indicated that EM in Sanya City can enhance environmental sustainability and boost producers’ annual income, but attention should be paid to the negative environmental impact of excessive fertilizer input. These findings offer insights into optimizing agricultural inputs for Hainan mango production to mitigate multiple environmental impacts while enhancing economic benefits, aiming to provide theoretical support for promoting the sustainable development of the Hainan mango industry. This study examines the development of the digital economy and society in the Republic of Kazakhstan by combining international benchmarking with a context-specific national framework. It highlights the limitations of existing global indices such as DESI, NRI, and EGDI in capturing the structural and institutional dimensions of digital transformation in emerging economies. To address this gap, the study introduces a novel composite metric, the Index of Digital Economy Development (IDED), which integrates five sub-indices: infrastructure, usage, human capital, economic digitization, and transformation effectiveness. The methodology involves comparative index analysis, the construction of the IDED, and statistical validation through a public opinion survey and regression modeling. Key findings indicate that cybersecurity is a critical yet under-represented component of digital development, showing strong empirical correlations with DESI scores in benchmark countries. The results also highlight Kazakhstan’s strengths in digital public services and internet access, contrasted with weaknesses in business digitization and innovation. The proposed IDED offers a more comprehensive and policy-relevant tool for assessing digital progress in transitional economies. This study contributes to the literature by proposing a replicable index structure and providing empirical evidence for the inclusion of cybersecurity in national digital economy assessments. The aim of the study is to assess Kazakhstan’s digital economy development by addressing limitations in global measurement frameworks. Methodologically, it combines comparative index analysis, the construction of a national composite index (IDED), and statistical validation using a regional survey and regression analysis. The findings reveal both strengths and gaps in Kazakhstan’s digital landscape, particularly in cybersecurity and SME digitalization. The IDED introduces an innovative, context-sensitive framework that enhances the measurement of digital transformation in transitional economies. Full article

The plant-based oil industry contributes significantly to food waste/by-products in the form of underutilized biomass, including oil pomace, cake/meal, seeds, peels, wastewater, etc. These waste/by-products contain a significant quantity of nutritious and bioactive compounds (phenolics, lignans, flavonoids, dietary fiber, proteins, and essential minerals) with proven health-promoting effects. The utilization of them as natural, cost-effective, and food-grade functional ingredients in novel food formulations holds considerable potential. This review highlights the potential of waste/by-products generated during plant-based oil processing as a promising source of bioactive compounds and covers systematic research, including recent studies focusing on innovative extraction and processing techniques. It also sheds light on their promising potential for valorization as food ingredients, with a focus on specific examples of food fortification. Furthermore, the potential for value creation in the food industry is emphasized, taking into account associated challenges and limitations, as well as future perspectives. Overall, the current information suggests that the valorization of plant-based oil industry waste and by-products for use in the food industry could substantially reduce malnutrition and poverty, generate favorable health outcomes, mitigate environmental concerns, and enhance economic profit in a sustainable way by developing health-promoting, environmentally sustainable food systems. Equestrian sports play a significant economic role in the horse industry. In recent years, numerous equine viruses have emerged, among which are equine Pegiviruses and the re-emerging Equine coronavirus (ECoV). These viruses are distributed globally and primarily cause subclinical infections with unknown morbidity, even if ECoV can occasionally induce febrile and diarrheic episodes. To broaden the data on the Italian equine population, a study was conducted to assess their prevalence in two distinct horse populations belonging to the Carabinieri Corps (CC) and the Italian Army (IA) of the Italian Armed Forces (IAF). Samples consisted of blood serum and rectal swabs of 436 horses collected within the national surveillance program for equine infectious anemia and gastrointestinal parasite monitoring and analyzed for Pegivirus (caballi and equi) and ECoV by Real-Time RT PCR. The prevalence detected were 6.56% and 3.53%, respectively, for Pegivirus caballi and equi for the IA, while for the CC, they were 10.13% and 0.84%. Only one sample tested positive for ECoV belonging to a horse of the CC. Phylogenetic analyses were carried out on the PCR-positive samples that were sequenced using Sanger protocols. Understanding the epidemiology of these viruses is essential for evaluating the implementation of effective prevention strategies. Full article

A systematic sensitivity analysis using three-dimensional discrete element models with discrete fracture networks (DEM-DFN) was conducted to evaluate underground excavation support in jointed rock masses at the CLAB2 site in Southeastern Sweden. The site features a joint network comprising six distinct joint sets, each with unique geometrical properties. The study examined 10 DFNs and 19 rock bolt patterns, both conventional and unconventional. It covered 200 scenarios, including 10 unsupported and 190 supported cases. Technical and economic criteria for stability were assessed for each support system. The results indicated that increasing rock bolt length enhances stability up to a certain point. However, multi-length rock bolt patterns with similar consumption can yield significantly different stability outcomes. Notably, the arrangement and properties of rock bolts are crucial for stability, particularly in blocks between bolting sections. These blocks remain interlocked in unsupported areas due to the induced pressure from supported sections. Although equal-length rock bolt patterns are commonly used, the analysis revealed that triple-length rock bolts (3, 6, and 9 m) provided the most effective support across all ten DFN scenarios. This study examines the role of human capital, technological investment, and institutional quality in promoting economic complexity in South Africa, with implications for sustainable development and the strategic role of Small and Medium Enterprises. Motivated by the growing importance of productive sophistication for long-term development in emerging economies (notably SDG 8 and SDG 9), the study examines both long-run and short-run dynamics using the Autoregressive Distributed Lag approach, with robustness checks via Fully Modified Least Squares, Dynamic Least Squares, and Canonical Cointegration Regression. Structural Vector Autoregression is employed to assess the persistence of shocks, while the Toda–Yamamoto causality test evaluates causality. The results reveal that institutional quality significantly enhances economic complexity in the long run, while technological investment exhibits a negative long-run impact, potentially indicating absorptive capacity constraints within industries. Though human capital and income per capita do not influence complexity in the long run, they have short-term effects, with income per capita having the most immediate influence. Variance decomposition shows that shocks to technological investment are essential for economic complexity, and are the most persistent, followed by human capital and institutional quality. These findings show the need for institutional reforms that lower entry barriers for SMEs in industries, targeted innovation policies that support upgrading, and human capital strategies aligned with driven industrial transformation. The study offers insights for policymakers striving to influence structural drivers to advance sustainable industrial development and achieve the SDGs. Full article

Agricultural production in southern Angola faces challenges due to unsustainable practices, including inefficient use of water, fertilizers, and machinery, resulting in low yields and environmental degradation. Therefore, clear and measurable indicators are needed to guide farmers toward more sustainable practices. The scientific literature insufficiently addresses this issue, leaving a significant gap in the evaluation of key performance indicators (KPIs) that can guide good agricultural practices (GAPs) adapted to the context of southern Angola, with the goal of promoting a more resilient and sustainable agricultural sector. So, the objective of this study is to identify and assess KPIs capable of supporting the selection of GAPs suitable for maize, potato, and tomato cultivation in the context of southern Angolan agriculture. A systematic literature review (SLR) was conducted, screening 2720 articles and selecting 14 studies that met defined inclusion criteria. Five KPIs were identified as the most relevant: gross margin, net profit, water use efficiency, nitrogen use efficiency, and machine energy. These indicators were analyzed and standardized to evaluate their contribution to sustainability across different GAPs. Results show that organic fertilizers are the most sustainable option for maize, drip irrigation for potatoes, and crop rotation for tomatoes in southern Angola because of their efficiency in low-resource environments. A clear, simple, and effective representation of the KPIs was developed to be useful in communicating to farmers and policy makers on the selection of the best GAPs in the cultivation of different crops. The study proposes a validated KPI-based methodology for assessing sustainable agricultural practices in developing regions such as southern Angola, aiming to lead to greater self-sufficiency and economic stability in this sector. In this study, we examine the impact of mergers and acquisitions (M&As) on firm environmental performance, aiming to address the gap in research and guide firms, investors, and policymakers toward more environmentally conscious decision-making in M&A. Using panel data from Chinese A-share listed firms (2008–2022), we estimate a two-way fixed effect model. The Propensity Score Matching and the instrumental variable method address potential endogeneity concerns, and robustness checks validate the findings. We found that M&As have a significantly positive effect on firm environmental performance, with heterogeneous impacts across regions, industries, and M&A types. The environmental benefits are most pronounced in heavily polluting industries and hybrid M&A deals. Eastern China shows more modest improvements. The results of mechanism tests revealed that M&As enhance environmental performance primarily by boosting total factor productivity and fostering innovation. This study offers a novel perspective by linking M&A activities to environmental sustainability, enriching the literature on both M&As and corporate environmental performance. We show that even conventional M&A deals (not sustainability-focused) can improve environmental performance through operational synergies. Expanding beyond polluting industries, we reveal how sector characteristics shape M&A’s environmental impacts. We identify practical mechanisms through which standard M&A activities can advance sustainability goals, helping firms balance economic and environmental objectives. It provides empirical evidence from China, an emerging market with distinct institutional and regulatory contexts. The findings offer guidance for firms engaging in M&A to strategically improve sustainability performance. Policymakers can leverage these insights to design incentives for M&A in pollution-intensive industries, aligning economic growth with environmental goals. By demonstrating that M&As can enhance environmental outcomes, this study supports the potential for market-driven mechanisms to contribute to broader societal sustainability objectives, such as reduced industrial pollution and greener production practices. Soil salinization severely limits plant growth and productivity. Mulberry (Morus alba L.), an economically and ecologically important tree, is widely cultivated, yet its salt-tolerance mechanisms at the seedling stage remain insufficiently understood. This study investigated the physiological and biochemical responses of two-year-old mulberry (‘Tailai Sang’) seedlings subjected to six NaCl treatments (0, 50, 100, 150, 200, and 300 mmol L−1) for 28 days. Results showed that growth parameters and photosynthetic gas exchange exhibited dose-dependent declines. The reduction in net photosynthetic rate (Pn) was attributed to both stomatal limitations (decreased stomatal conductance) and non-stomatal limitations, as evidenced by a significant decrease in the maximum quantum efficiency of photosystem II (Fv/Fm) under high salinity. To cope with osmotic stress, seedlings accumulated compatible solutes, including soluble sugars, proteins, and proline. Critically, mulberry seedlings demonstrated effective ion homeostasis by sequestering Na+ in the roots to maintain a high K+/Na+ ratio in leaves, a mechanism that was compromised above 150 mmol L−1. Concurrently, indicators of oxidative stress—malondialdehyde (MDA) and H2O2—rose significantly with salinity, inducing the activities of antioxidant enzymes (SOD, CAT, APX, and GR), which peaked at 150 mmol L−1 before declining under extreme stress. A biomass-based LC50 of 179 mmol L−1 NaCl was determined. These findings elucidate that mulberry salt tolerance is a coordinated process involving three key mechanisms: osmotic adjustment, selective ion distribution, and a robust antioxidant defense system. This study establishes an indicative tolerance threshold under controlled conditions and provides a physiological basis for further field-based evaluations of ‘Tailai Sang’ mulberry for cultivation on saline soils. China’s economy is shifting from an era of rapid expansion to one focused on high-quality development, making it imperative to tackle environmental degradation linked to energy use. Understanding how New Quality Productive Forces (NQPF) interact with energy efficiency, along with the mechanisms driving this relationship, is essential for economic transformation and long-term sustainability. This study establishes an evaluation framework for NQPF, integrating technological, green, and digital dimensions. We apply fixed-effects models, the spatial Durbin model (SDM), a moderation model, and a threshold model to analyze the influence of NQPF on Green Total Factor Energy Efficiency (GTFEE) and its spatial implications. This underscores the necessity of distinguishing it from traditional productivity frameworks and adopting a new analytical perspective. Furthermore, by considering dimensions such as input, application, innovation capability, and market efficiency, we reveal the moderating role and heterogeneous effects of artificial intelligence (AI). The findings are as follows: The development of NQPF significantly enhances GTFEE, and the conclusion remains robust after tail reduction and endogeneity tests. NQPF has a positive spatial spillover effect on GTFEE; that is, while improving the local GTFEE, it also improves neighboring regions GTFEE. The advancement of AI significantly strengthens the positive impact of NQPF on GTFEE. AI exhibits a significant U-shaped threshold effect: as AI levels increase, its moderating effect transitions from suppression to facilitation, with marginal benefits gradually increasing over time.
