Objectives: The objective of this study was to evaluate changes in anatomical point position, cranio-cervical posture, and respiratory dimensions following conventional bimaxillary total prosthetic rehabilitation. Methods: A prospective, longitudinal, observational, analytical study was conducted on 12 patients, aged 55 to 75 years, at the Department of Dental Prosthetics at the University of Medicine and Pharmacy in Cluj-Napoca. All patients had complete bimaxillary edentulism and received removable dentures as treatment. Clinical and cephalometric analyses were performed before and after prosthetic treatment to compare changes. The cephalometric analysis was based on the guidelines of Tweed and Rocabado for evaluation. Quantitative data were described using the mean and standard deviation for normal distribution and represented by bar graphs with error bars. A paired samples t-test was used to determine differences between groups, with a significance threshold of 0.05 for the bilateral p-value. Results: When analyzing changes in cranial base inclination, the corresponding angles exhibited an increase, indicating cephalic extension. A statistically significant difference in the anteroposterior diameter of the oropharyngeal lumen with and without bimaxillary complete dentures was identified (p < 0.05). For hyperdivergent patients, modifications in the position of anatomical features on cephalometry slightly reduced the VDO and had a slight compensatory effect on skeletal typology. In contrast, for hypodivergent patients, modifications to the position of anatomical landmarks also had a compensatory effect on skeletal typology, increasing the VDO. Conclusion: Changes in the position of anatomical features on cephalometry generally have a compensatory effect on skeletal typology after complete denture placement. Complete prosthetic treatment with removable dentures can significantly influence respiratory function by reducing the oropharyngeal lumen and body posture by cephalic extension and attenuation of the lordotic curvature of the cervical spine. Background: Mandibular movement is inherently complex, involving both lateral and vertical components. In older adults, this complexity is compounded by diminished neuromuscular coordination and reduced proprioceptive feedback, often impairing effective mastication. Objectives: This study aimed to review dental literature on the influence of masticatory parameters in the formation of wear facets on acrylic resin denture teeth. Methods: A targeted literature review was conducted, analyzing peer-reviewed studies from 1991 to 2024 through PubMed and Scopus. The review focused on masticatory parameters, age-related neuromuscular adaptations, chewing patterns, and preferred-side denture tooth wear, affecting older adults using complete removable dentures. Results: Mandibular movement characteristics—particularly velocity, duration, and cycle frequency—were found to significantly influence denture tooth wear, often resulting in reduced bite force and masticatory efficiency. Contemporary denture materials demonstrated improved wear resistance, aiding in the regulation of chewing frequency and velocity. These factors are increasingly important as age-related changes impact the stomatognathic system. Notably, wear facets were most prominent on the maxillary first molars, regardless of the occlusal scheme. Conclusions: This review provided a comprehensive overview of existing research, analyzed current evidence, and offered insights into the role of masticatory function and wear characteristics in older complete denture wearers. Objectives: This study aimed to establish reference values for bite force in individuals with various prosthetic restorations and to examine the relationship between prosthetic treatment groups (PTGs) and bite force as an indicator of masticatory muscle function. Materials and Methods: In a cross-sectional study from November 2021 to March 2023, 198 participants aged 18 to 95 years were recruited from multiple dental and geriatric centers. The participants were assigned to seven PTGs based on their dental and prosthetic statuses. Bite force was measured using the Occlusal Force Meter GM10, with three recordings on each side of the jaw, and analyzed using ANOVA. Results: The bite force decreased with fewer teeth and the transition from fixed to removable dentures. Fully dentate participants exhibited the highest bite forces, differing significantly from the other groups (p < 0.001). For the fully dentate individuals (547 ± 240 N), the bite force decreased progressively with the extent of prosthetic restoration, reaching 55 ± 45 N in edentulous individuals with complete dentures in both jaws. However, edentulous participants with two interforaminal implants demonstrated higher bite forces than those with partial dentures. Conclusions: Bite force is significantly impacted by prosthetic restoration type. Fully dentate individuals have the highest bite forces, while edentulous patients with implant-supported dentures also show considerable bite forces, similar to those with partial dentures. Dentures Used for Rehabilitation of Mastication After Loss of Teeth Maintain Proper Biomechanical Properties of Masseter Muscles—A Comparative Study

Background: Various types of dentures are applied in the treatment of missing teeth. The research carried out so far has proven that both the absence of teeth and the replacement of missing teeth with dentures affect the functional condition of masseter muscles. The purpose is, thus, to find out how the tone, elasticity, and stiffness of masseter muscles change after prosthetic treatment. Material and methods: The material for the study consisted of 137 individuals, who were divided into group 1, with dentures (n = 87) and undergoing prosthetic treatment, and group 2, without dentures (n = 50) with teeth preserved, and not undergoing prosthetic treatment. Additionally, group 1 was divided into subgroups, in accordance with the type of prosthetic treatment applied: group 1a—removable complete dentures (n = 14), group 1b—removable partial dentures (n = 48), group 1c—fixed partial dentures (n = 22), group 1d—fixed complete dentures (n = 3). The biomechanical properties of masseter muscles, namely: tone, stiffness, and elasticity were assessed by means of myotonometry. The measurements were taken under muscle relaxation and under maximum muscle contraction. Results: The values of the parameters of tone, stiffness, and elasticity were higher in group 2, without dentures, in comparison with group 1, with dentures, yet they did not differ with statistical significance. The type of prosthesis used affected the results obtained. Masseter muscles demonstrated the best biomechanical parameters in the case of applying fixed partial dentures. Conclusions: Prosthetic replacement of missing teeth contributes to maintaining the good condition of masseter muscles, which has been confirmed by the results of myotonometric measurements taken. Background and Objectives: A randomized, double-blind clinical trial was conducted based on the CONSORT study protocol for randomized clinical trials (NCT06531720) to compare the effectiveness of oral mucosa healing properties of 0.2% chlorhexidine digluconate (CHX) and 8.7% choline salicylate (CHS), as well as a control group (CON) with no intervention, in patients with delivered partial removable dentures (PRDs). Materials and Methods: Patients (n = 27) who were enrolled in the study were healthy subjects according to the inclusion/exclusion criteria, and they received new PRDs to complement Kennedy’s class III and IV deficiencies. During the process of adaptation to new prosthetic restorations, OMLs were formed and treated with one of two selected preparations, either CHX = 0.2% or CHS = 8.7%, in relation to the control group (CON). The wound surface area (WSA) (mm2) was measured on repeatable intraoral images taken in accordance with the examination protocol on the first control visit on day 1, day 3, day 7, day 10, and day 14 with the assistance of computer software. Results: There were no statistically significant differences between groups. The fastest effect of WSA complete reduction was observed in the CHX group after 7 days (WAS = 0.78, SD = 1.18) in comparison to CHS = 10 days (WAS = 0.44, SD = 0.90) and CON = 14 days (WAS = 0.22, SD = 0.67). The decrease in the WSA after 7 days of observation was 85.1% in the CHX group, 70.1% in the CHS group, and 59.2% in the CON group. Conclusions: The WSA decreased most rapidly after 7 days of treatment with 0.2% chlorhexidine digluconate (CHX), slightly more slowly after 10 days of treatment with 8.7% choline salicylate (CHS), and relatively most slowly in the CON group, who were not treated with any topical medication after 14 days. Oral mucosa lesions (OMLs) therapy during the process of adaptation to new removable prosthetic restorations is a very important element supporting the whole process. Topical medications containing 0.2% chlorhexidine digluconate are indicated as adjunctive therapy in the process of the supportive treatment and disinfection of oral mucosa lesions. However, this does not release the dentist from liability for the careful adjustment of the removable prosthetic restoration. Full article

Background/Objectives: Defects in maxillary and mandibular alveolar ridges are common in maxillofacial practice. Reconstruction with microvascular bone grafts and subsequent prosthetic rehabilitation is the gold standard treatment. This study investigated patients’ quality of life (QoL) after microvascular alveolar ridge reconstruction with subsequent dental rehabilitation. The effect of the underlying disease and success rates of the prosthetic treatment on QoL were analysed. Methods: OHIP-49 was used to evaluate oral health-related QoL (OHrQoL). The SF-36 was used to assess disease-nonspecific QoL. Results: Fifty-eight patients were enrolled and divided into four diagnostic (malignancy, osteoradionecrosis, benign disease, and cleft palate) and five prosthetic groups (no prosthetics, removable partial dentures, complete dentures, implant-supported removable dentures, and implant-supported fixed dentures). There was a significant difference between the diagnostic groups in the total score of their OHIP-49 (p = 0.008). Patients with malignant disease and osteoradionecrosis had worse QoL scores than those with benign diseases and cleft palate. Implant-supported prostheses had the best OHrQoL. Removable partial dentures and patients in whom dental rehabilitation was not possible had the worst OHrQoL (p = 0.042). The SF-36 subscale score showed no statistically significant differences between the diagnostic and prosthetic groups (p > 0.05). Conclusions: OHrQoL after microvascular alveolar ridge reconstruction differs significantly based on underlying diagnoses and prosthetic restorations. Benign diseases and implant-supported dentures have the highest scores. Full article

Background and Objectives: With increases in cancer incidence and the number of cancer survivors, the demand for cancer management is growing. However, studies on dental prosthetic treatment for patients with cancer are rare. We aim to investigate the dental prosthetic treatment in patients with cancer aged ≥65 years after expanded health insurance coverage. Materials and Methods: This retrospective study included patients who were treated with implants and removable dentures at Ulsan University Hospital in South Korea between June 2015 and June 2023. Data on age, sex, cancer location, comorbid systemic diseases, number of remaining teeth, dental prosthetic treatment history, type of dental prosthetic treatment, and insurance coverage status were extracted from patient medical records and panoramic radiographs. The influence of multiple variables on dental prosthetic treatment was analyzed using the Chi-square and Fisher’s exact tests. Results: The study included 61 patients with cancer (32 men, 29 women; average age: 70.9 years). Among them, 56 (91.8%) had insurance coverage benefits, and 34 (55.7%) received treatments such as implants, removable partial dentures, or complete dentures for the first time. Treatment types included 37 (60.7%) cases of implant prostheses and 24 (39.3%) conventional removable dentures. No statistical differences were observed in the type of dental prosthetic treatment according to sex, age, cancer location, number of systemic diseases, and dental prosthetic treatment history (p > 0.05). Patients with <10 remaining teeth received treatment with conventional removable dentures, which was statistically significant (p < 0.001). Conclusions: Of the 61 patients, 56 (91.8%) received insurance benefits, and 34 (55.7%) underwent dental prosthetic treatment for the first time. Within the limitations of this retrospective study, the expanded health insurance coverage alleviated the unmet demand for dental prosthetic treatment. As cancer prevalence continues to increase, expanding customized health insurance coverage is crucial to meet this demand. Full article

Background: Removable prostheses have seen a fundamental change recently because of advances in polymer materials, allowing improved durability and performance. Despite these advancements, notable differences still occur amongst various polymer materials and processing technologies, requiring a thorough grasp of their mechanical, physical, and therapeutic implications. The compressive strength of dentures manufactured using various technologies will be investigated. Methods: Traditional, injection molding, and additive and subtractive CAD/CAM processing techniques, all utilizing Polymethyl methacrylate (PMMA) as the main material, were used to construct complete dentures. The specimens underwent a compressive mechanical test, which reveals the differences in compressive strength. Results: All the specimens broke under the influence of a certain force, rather than yielding through flow, as is characteristic for plastic materials. For each specimen, the maximum force (N) was recorded, as well as the breaking energy. The mean force required to break the dentures for each processing technology is as follows: 4.54 kN for traditional packing-press technique, 17.92 kN for the injection molding technique, 1.51 kN for the additive CAD/CAM dentures, and 5.9 kN for the subtractive CAD/CAM dentures. Conclusions: The best results were obtained in the case of the thermoplastic injection system and the worst results were recorded in the case of 3D printed samples. Another important aspect depicted is the standard deviation for each group, which reveal a relatively unstable property for the thermoplastic injected dentures. Good results here in terms of absolute property and stability of the property can be conferred to CAD/CAM milled group. Full article

A common challenge encountered with both traditional and digitally produced dentures involves the extraction of artificial teeth from the denture base. This narrative review seeks to present an updated perspective on the adherence of synthetic teeth for denture base materials, employing diverse methods. Dental technicians often employ chemical approaches and mechanical techniques (including abrasion, laser treatment, and abrasive blasting) to augment the retention of denture teeth. However, the efficacy of these treatments remains uncertain. In certain instances, specific combinations of Denture Base Resin (DBR) materials and artificial teeth exhibit improved performance in conventional heat-cured dentures following these treatments. The primary reasons for failure are attributed to material incompatibility and inadequate copolymerization. As new denture fabrication techniques and materials continue to emerge, further research is imperative to identify optimal tooth-DBR combinations. Notably, 3D-printed tooth–DBR combinations have demonstrated reduced bond strength and less favorable failure patterns, while utilizing milled and traditional combinations appears to be a more prudent choice until advancements in additive manufacturing enhance the reliability of 3D-printing methods. This study compared the clinical experiences of foreign-trained dentists (FTDs) enrolled in an Advance Standing DMD Dental Program (DMDAS) with those of the domestic dental students (DMD) at the University of Illinois Chicago, College of Dentistry (UIC-COD). A cross-sectional retrospective chart review of patients treated by 295 DMD and 253 DMDAS predoctoral dental students was completed at the UIC-COD. The data were retrieved from the electronic health record system (axiUm) for the graduated classes of 2018, 2019, 2020, 2021, and 2022 on various performed clinical procedures as measured by relative value units (RVUs). The retrieved data were used to compare the clinical experiences of DMDAS vs. DMD students. Descriptive (mean) and statistical (independent t-test) analyses were used (α = 0.05). The results indicated that DMD and DMDAS students had comparable clinical experiences in several disciplines, including diagnosis, prevention, direct/indirect restorations, endodontics, periodontics, complete dentures, removable partial dentures, implants/fixed partial dentures, and oral surgery. There was a statistical difference in total RVUs for diagnosis (p = 0.002) and direct restorations (p < 0.001), in which DMD students had more experience. The 28 month program for FTDs appeared to be a reasonable timeframe to obtain an adequate number of varied clinical experiences as compared with the traditional four-year program at the UIC-COD. Denture stomatitis (DS) is a very common disease in wearers of removable complete and partial dentures with a worldwide prevalence in the range of 20–67%. Both industrially developed and impoverished nations are affected by the illness. DS is often associated with ill-fitting dentures or a fungal infection with Candida spp. Candida is normally found in the oral cavity microbiota, but it can be harmful to the health of elderly people with underlying diseases. Therefore, the purpose of the present study is to offer the most recent information about the epidemiology, etiology, and global distribution of Candida species associated with DS through a systematic review. Several databases, including Medline, Web of Science, and Scopus, were used to conduct an extensive search of the literature published in the previous 20 years. The selection of studies was performed by two authors. The extracted data were as follows: author, year of publication, country, sample, frequency of DS, method of diagnosing stomatitis, species of Candida, risk factors, and etiology of the disease. The JBI Critical appraisal tools were used to assess the quality of the studies. Eventually, twenty-eight studies were included in the systematic review. Twenty-one studies investigated DS, while seven studies examined Candida colonization in patients using removable dentures. The results show that the main causes of DS include the type of dentures, continuous wearing of dentures, and the formation of a Candida biofilm, which is facilitated by poor dental hygiene. Additionally, previous studies have pinpointed the significance of the salivary flow, saliva composition, and salivary pH. The findings of the current review indicate that it is crucial to monitor denture wearers for the appearance of DS, especially the patients whose immunity has been impaired due to a systemic condition. A flipper is an interim removable partial denture used for immediate esthetic restoration and space maintenance for a limited period before definitive treatment. Traditional methods for fabricating flippers are labor-intensive and manual. With the advent of technological advancement in dentistry, fully digitalized fabrication including computer designing and three-dimensional (3D) printing can be used. This study aimed to demonstrate the complete digital workflow for fabricating flippers and compare the features of different digital methods. In a partially edentulous patient, three flippers were fabricated using different digital protocols: individual printing (tooth and base parts) and combination, i.e., one-body printing and gingival layering and one-body printing and gingival coloring. Each flipper was tried into the oral cavity, and the adaptation of flippers was evaluated using the triple-scan technique. This study confirms that fully digital fabrication of a flipper is an effective method compared with traditional methods and shows various digital workflows available in 3D printing. Thus, an appropriate fabrication method must be selected for each situation. Full article

Oral infections occur due to contact between biofilm rich in Candida albicans formed on the inner surface of complete dentures and the mucosa. This study investigated historical advances in the prevention and treatment of oral mucosal infection and identified gaps in the literature. Bibliographic research was conducted, looking at PubMed, Embase, Web of Science, and Scopus, where 935 articles were found. After removing duplicates and excluding articles by reading the title and abstract, 131 articles were selected for full reading and 104 articles were included. Another 38 articles were added from the gray literature. This review followed the PRISMA-ScR guidelines. The historical period described ranges from 1969 to 2023, in which, during the 21st century, in vitro and in vivo studies became more common and, from 2010 to 2023, the number of randomized controlled trials increased. Among the various approaches tested are the incorporation of antimicrobial products into prosthetic materials, the improvement of oral and denture hygiene protocols, the development of synthetic and natural products for the chemical control of microorganisms, and intervention with local or systemic antimicrobial agents. Studies report good results with brushing combined with sodium hypochlorite, and new disinfectant solutions and products incorporated into prosthetic materials are promising. Full article

This in vitro study aimed to confirm the effect of the additional features of Kirei Keep Light (KKL), a commercial UV-C irradiation system that was originally created for coating the surface of removable dentures with photoreactive 2-methacryloyloxyethyl phosphorylcholine (MPC), on the antimicrobial efficacy aspect against Candida albicans biofilm on a denture base material (PMMA) and SARS-CoV-2. Materials and Methods: The antimicrobial efficacy on C. albicans biofilm was evaluated through quantitative (CFU) and qualitative (SEM images) analysis of three groups: no treatment (control), KKL, and immersion in a disinfectant solution, MCAE. The quantitative evaluation on SARS-CoV-2 was performed by comparing the untreated (control) group and the KKL group. Results: In comparison with the control group (2.39 × 106 CFU/mL), KKL irradiation resulted in a 91.01% reduction in C. albicans biofilm (2.15 × 105 CFU/mL), whereas for the MCAE group, this reduction was 99.98% (4.64 × 102 CFU/mL). The SEM image results also corroborate the CFU results, which showed that the fewest clean surfaces were found in the control, and this gradually increased with KKL and MCAE. SARS-CoV-2 inhibition, indicated by its TCID50 value, demonstrated that KKL almost completely inhibited SARS-CoV-2 infection and replication (99.99% reduction). Conclusion: KKL possesses antimicrobial efficacy on C. albicans biofilm on PMMA and SARS-CoV-2. Full article

With the life expectancy increasing, there is a growing need for prosthetic dental treatments to restore the oral health, function, and quality of life of edentulous patients. Presently, only a few articles are available describing the oral rehabilitation of patients with severely resorbed ridges with milled complete dentures. This clinical case report provides a straightforward protocol consisting of a combination of analog and digital techniques for the rehabilitation of edentulous patients with severely resorbed ridges with milled fixed and removable complete dentures. This technique permits the minimization of the number of appointments, improves patient comfort, allows for the digital archiving of important clinical data, and permits the manufacture of prostheses with improved mechanical properties. These favorable outcomes were achieved by using the patient’s existing PMMA complete denture as a custom tray for a final impression with light-bodied Polyvinylsiloxane. Subsequently, the resulting models were digitized, and a digital complete denture was designed and manufactured in an expedited manner using CAD-CAM techniques. Therefore, this case report highlights the potential of CAD/CAM technology to predictably restabilize oral functions and improve patients’ quality of life. Full article
