Streptococcus suis is an emerging zoonotic pathogen that typically causes bacteremia or meningitis in humans, whereas vertebral osteomyelitis with epidural abscess is exceedingly rare and may be missed. We describe a 65-year-old farmer with fever and severe low back pain after long-term bare-handed handling of raw pig lungs. Pre-treatment blood cultures yielded S. suis identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). After transient improvement on empirical therapy, fever recurred with worsening lumbar pain. Contrast-enhanced magnetic resonance imaging (MRI) demonstrated multilevel thoracolumbar pyogenic spondylitis with an epidural abscess and a sub-ligamentous abscess beneath the posterior longitudinal ligament (PLL) extending from L2 to L5. Computed tomography-guided lumbar biopsy followed by tissue metagenomic next-generation sequencing (mNGS) detected S. suis, providing concordant evidence supporting pathogen involvement at the vertebral focus. The bloodstream isolate (SS-JX2025-01) was serotype 2, sequence type 7 (ST7). It remained susceptible to β-lactams and glycopeptides but was resistant to macrolide–lincosamide and tetracycline classes, consistent with erm(B), tet(O), tet(40), and ant(6)-Ia detected by whole-genome sequencing (WGS). Virulence profiling revealed an epf+/sly+/mrp pattern with multiple adhesins and immune-evasion factors, whereas canonical 89K pathogenicity island markers were absent. Core-genome phylogeny placed SS-JX2025-01 within the Chinese ST7 lineage associated with previous outbreaks. This biopsy-supported case expands the clinical spectrum of invasive S. suis infection, highlights the value of tissue mNGS as an adjunct for supporting deep-seated foci in zoonotic infections, and underscores the importance of occupational prevention in small-scale farming households. I argue that definite relative clauses, despite their stable strong island status cross-linguistically, lack island status in Adara. The structure is neither selective nor weak, but rather completely permeable, allowing the free extraction of both arguments and adjuncts. In support of the claim that A-bar movement may escape relative clause domains in Adara, I adduce evidence from crossover effects, reconstruction effects, and quantifier float. In addition, I show that relative clause-internal material may probe upwards and outwards, triggering long-distance agreement of relativized verbs with clause-external subjects in a way that is not possible with true island structures in the language. Accurate documentation of burn wounds is essential for evaluating treatment outcomes and monitoring healing progression. Traditional two-dimensional (2D) photography remains the clinical standard but lacks depth and volumetric accuracy. Three-dimensional (3D) scanning offers enhanced visualization of wound morphology and tissue vitality, potentially improving objectivity in burn assessment. This study compares two handheld 3D scanning systems—Artec Eva and Revopoint Miraco—in documenting acute and healing burn wounds, using standard clinical photography as the reference. Fifteen patients with second-degree and third-degree burns were prospectively examined at the Burn Unit of AGEL Hospital Košice-Šaca, with five representative cases selected for detailed analysis. For each patient, clinical photographs and paired 3D scans were obtained under standardized conditions and evaluated for color fidelity, wound margin clarity, representation of epithelialisation islands, necrotic tissue, and correlation with clinical findings. Across all cases, Artec Eva demonstrated superior color accuracy, clearer wound delineation, and more realistic visualization of tissue vitality and re-epithelialisation. Revopoint Miraco reliably captured wound shape but produced darker tones and exaggerated surface relief, occasionally distorting depth perception. Overall, both systems successfully identified key healing features; however, Artec Eva provided more clinically accurate and visually consistent results. Three-dimensional scanning represents a valuable adjunct to conventional burn documentation. Full article

Colorectal cancer (CRC) is a multifactorial disease increasingly recognized for its complex interplay with the gut microbiota. The disruption of microbial homeostasis—dysbiosis—has profound implications for intestinal barrier integrity and host immune function. Pathogenic bacterial species such as Fusobacterium nucleatum, Escherichia coli harboring polyketide synthase (pks) island, and enterotoxigenic Bacteroides fragilis are implicated in CRC through mechanisms involving mucosal inflammation, epithelial barrier disruption, and immune evasion. These pathogens promote pro-tumorigenic inflammation, enhance DNA damage, and suppress effective anti-tumor immunity. Conversely, commensal and probiotic bacteria, notably Lactobacillus and Bifidobacterium species, exert protective effects by preserving epithelial barrier function and priming host immune responses. These beneficial microbes can promote the maturation of dendritic cells, stimulate CD8+ T cell cytotoxicity, and modulate regulatory T cell populations, thereby enhancing anti-tumor immunity. The dichotomous role of the microbiota underscores its potential as both a biomarker and a therapeutic target in CRC. Recent advances in studies have explored microbiota-modulating strategies—ranging from dietary interventions and prebiotics to fecal microbiota transplantation (FMT) and microbial consortia—as adjuncts to conventional therapies. Moreover, the composition of the gut microbiome has been shown to influence the responses to immunotherapy and chemotherapy, raising the possibility of microbiome-informed precision oncology therapy. This review synthesizes the current findings on the pathogenic and protective roles of bacteria in CRC and evaluates the translational potential of microbiome-based interventions in shaping future therapeutic paradigms. Adjunct clauses have traditionally been assumed to be syntactic configurations from which extraction is universally impossible. However, numerous studies have challenged this assumption and extraction from finite adjunct clauses has been shown to be acceptable to varying degrees in the Mainland Scandinavian languages, as well as in English. The relative acceptability of extraction appears to depend on a number of factors, including the type of adjunct clause and the type of extraction dependency. Research on L2 learning has shown that learners often transfer properties of their L1 grammar into their L2 during the process of learning a second language. Our previous studies on L1 English and L1 Danish found a surprising contrast in which L1 English users found relativization out of adverbial clauses to be better than L1 Danish users did. Based on these findings, we conducted an L2 acceptability judgment experiment on extraction from three types of finite adjunct clauses in Danish (corresponding to English if-, when- and because-clauses) in order to test whether language-specific parameters related to extractability are transferred from L1 to L2. Our results show that the judgments from L2 Danish speakers are intermediate between and significantly different from L1 English and L1 Danish, which does not suggest a parameter resetting. This paper discusses the status of island phenomena in Avatime, an endangered Kwa language of Ghana. We focus on clausal adjuncts, specifically noun complement clauses (NCCs). We show that while standard adjuncts are strong islands in Avatime, NCCs allow argument extraction. We suggest that this is related to the fact that NCCs in Avatime are not a type of relative clause. Instead, NCCs involve a kind of serial verb construction, which independently allows for extraction. Full article

The distinction between weak and strong islands has been extensively explored in the literature from both a descriptive and analytical perspective. In this paper, I document and analyze island constructions and constraints in Mende, an understudied Mande language spoken in Sierra Leone. Mende has both weak islands (left branch and wh-islands) and strong islands (adjunct clauses, sentential subjects, and coordinate structures). Intriguingly, it has a third class of islands, that I call mixed islands which show a subject–non-subject asymmetry in allowing for movement out of relative clauses, only when they modify the subject. As such, subject-modifying RCs cannot be classified as (strong/weak) islands in Mende. This is the first systematic work on islands and island constraints in the Mande language family, and, as such, it brings novel data from an understudied language family to bear on our understanding of A-bar dependencies and the study of island escape in African languages. It also calls into question a neat paradigm of cross-linguistic island constraints. Importantly, this work also lays down a baseline for future research on island constraints in the broader Mande language family. In order to discuss island constraints, this paper also lays out the first analysis of relative clauses in Mende, while integrating new research on the left periphery, focus constructions, and wh-constructions. Primary ciliary dyskinesia (PCD) is an inherited disorder that impairs motile cilia, essential for respiratory health, with a reported prevalence of 1 in 16,309 within Hispanic populations. Despite 70% of Puerto Rican patients having the RSPH4A [c.921+3_921+6del (intronic)] founder mutation, the characterization of the ciliary dysfunction remains unidentified due to the unavailability of advanced diagnostic modalities like High-Speed Video Microscopy Analysis (HSVA). Our study implemented HSVA for the first time on the island as a tool to better diagnose and characterize the RSPH4A [c.921+3_921+6del (intronic)] founder mutation in Puerto Rican patients. By applying HSVA, we analyzed the ciliary beat frequency (CBF) and pattern (CBP) in native Puerto Rican patients with PCD. Our results showed decreased CBF and a rotational CBP linked to the RSPH4A founder mutation in Puerto Ricans, presenting a novel diagnostic marker that could be implemented as an axillary test into the PCD diagnosis algorithm in Puerto Rico. The integration of HSVA technology in Puerto Rico substantially enhances the PCD evaluation and diagnosis framework, facilitating prompt detection and early intervention for improved disease management. This initiative, demonstrating the potential of HSVA as an adjunctive test within the PCD diagnostic algorithm, could serve as a blueprint for analogous developments throughout Latin America. Triple-negative breast cancer (TNBC) is the most aggressive molecular subtype, with a poor survival rate compared to others subtypes. For a long time, chemotherapy was the only systemic treatment for TNBC, and the identification of actionable molecular targets might ultimately improve the prognosis for TNBC patients. We performed a genome-wide analysis of DNA methylation at CpG islands on a collection of one hundred ten breast carcinoma samples and six normal breast tissue samples using reduced representation bisulfite sequencing with the XmaI restriction enzyme (XmaI-RRBS) and identified a subset of TNBC samples with significant hypomethylation at the LTB4R/LTB4R2 genes’ CpG islands, including CpG dinucleotides covered with cg12853742 and cg21886367 HumanMethylation 450K microarray probes. Abnormal DNA hypomethylation of this region in TNBC compared to normal samples was confirmed by bisulfite Sanger sequencing. Gene expression generally anticorrelates with promoter methylation, and thus, the promoter hypomethylation detected and confirmed in our study might be revealed as an indirect marker of high LTB4R/LTB4R2 expression using a simple methylation-sensitive PCR test. Analysis of RNA-seq expression and DNA methylation data from the TCGA dataset demonstrates that the expression of the LTB4R and LTB4R2 genes significantly negatively correlates with DNA methylation at both CpG sites cg12853742 (R = −0.4, p = 2.6 × 10−6; R = −0.21, p = 0.015) and cg21886367 (R = −0.45, p = 7.3 × 10−8; R = −0.24, p = 0.005), suggesting the upregulation of these genes in tumors with abnormal hypomethylation of their CpG island. Kaplan–Meier analysis using the TCGA-BRCA gene expression and clinical data revealed poorer overall survival for TNBC patients with an upregulated LTB4R. To this day, only the leukotriene inhibitor LY255283 has been tested on an MCF-7/DOX cell line, which is a luminal A breast cancer molecular subtype. Other studies compare the effects of Montelukast and Zafirlukast (inhibitors of the cysteinyl leukotriene receptor, which is different from LTB4R/LTB4R2) on the MDA-MB-231 (TNBC) cell line, with high methylation and low expression levels of LTB4R. In our study, we assess the therapeutic effects of various drugs (including leukotriene receptor inhibitors) with the DepMap gene effect and drug sensitivity data for TNBC cell lines with hypomethylated and upregulated LTB4R/LTB4R2 genes. LY255283, Minocycline, Silibinin, Piceatannol, Mitiglinide, 1-Azakenpaullone, Carbetocin, and Pim-1-inhibitor-2 can be considered as candidates for the additional treatment of TNBC patients with tumors demonstrating LTB4R/LTB4R2 hypomethylation/upregulation. Finally, our results suggest that the epigenetic status of leukotriene B4 receptors is a novel, potential, predictive, and prognostic biomarker for TNBC. These findings might improve individualized therapy for TNBC patients by introducing new therapeutic adjuncts as anticancer agents. Full article

First described by Duckett in 1981, and initially employed for the surgical correction of mid-penile hypospadias, the onlay preputial island flap urethroplasty has progressively gained increasing popularity, extending its indication to proximal forms. However, with the complexity of the penile anomaly, the rate of postoperative complications related to poor tissue perfusion (including skin and glans dehiscence, urethral stenoses, and fistulas) has also increased. Conventionally, the visual assessment of the onlay preputial island flap is the only option available to establish the appropriate tissue vascularization during surgery. To this end, we have first introduced the EleVision IR system (Medtronic Ltd., Hong Kong, China) to assess the vascular perfusion of the preputial island flap in a 13-month-old boy undergoing the onlay urethroplasty for the surgical correction of a mid-shaft hypospadias. This was possible 80 s after the intravenous injection of indocyanine green (ICG, 0.15 mg/kg, Diagnostic Green GmbH, Munich, Germany). ICG-based laser angiography helped define the proximal resection margin of the preputial flap, and proved to be safe, effective, and easy to employ. This innovative intraoperative imaging modality can be considered a useful adjunct for tissue perfusion evaluation and intraoperative decision-making during the onlay preputial island flap urethroplasty in children. Adjunct clauses are traditionally assumed to be strong islands for extraction across languages. However, the universal island status of adjunct clauses has been challenged by studies showing that extraction is possible from finite adjunct clauses in the Mainland Scandinavian languages. The possibility of extraction in these languages appears to be affected by various factors, including the type of adjunct clause, the type of extraction dependency, and the presence of contextual facilitation. These findings call for a re-evaluation of the islandhood of adjunct clauses in English. We conducted an acceptability judgment study on relativization from three types of finite adjunct clauses in English (if-, when-, and because-clauses) in the presence of supporting context. We found that the three clause types showed rather non-uniform acceptability patterns: extraction from when- and because-clauses both yielded significantly lower ratings than extraction from if-clauses, which patterned with non-island that-clauses. Our results suggest that at least for relativization, if- and when-adjuncts are not invariably strong islands in English, and that extra-grammatical factors may be key in understanding island structures traditionally assumed to be purely syntactic in nature. Recent research suggests that island effects may vary as a function of dependency type, potentially challenging accounts that treat island effects as reflecting uniform constraints on all filler-gap dependency formation. Some authors argue that cross-dependency variation is more readily accounted for by discourse-functional constraints that take into account the discourse status of both the filler and the constituent containing the gap. We ran a judgment study that tested the acceptability of wh-extraction and relativization from nominal subjects, embedded questions (EQs), conditional adjuncts, and existential relative clauses (RCs) in Norwegian. The study had two goals: (i) to systematically investigate cross-dependency variation from various constituent types and (ii) to evaluate the results against the predictions of the Focus Background Conflict constraint (FBCC). Overall we find some evidence for cross-dependency differences across extraction environments. Most notably wh-extraction from EQs and conditional adjuncts yields small but statistically significant island effects, but relativization does not. The differential island effects are potentially consistent with the predictions of the FBCC, but we discuss challenges the FBCC faces in explaining finer-grained judgment patterns. Full article

In this article, I will argue that many of the theoretical approaches to extraction from participle adjunct islands suffer from the fact that the focus of investigation lies on perceived grammaticality differences in interrogative structures. Following approaches which make an explicit connection between extraction asymmetries and properties of the underlying proposition, I will argue that there is good evidence for the existence of similar differences in declarative adjunct constructions which can explain most of the grammaticality patterns observed for interrogatives. A crucial distinction to the majority of previous theories is the focus on acceptability rather than grammaticality, and the assumption that acceptability in declaratives is determined by a variety of semantic and syntactic complexity factors which do not influence how strongly extraction degrades the structure. This line of argumentation is more compatible with approaches to island phenomena that explain the low acceptability of some extractions by independent effects such as processing complexity and discourse function instead of syntactic principles blocking the extraction. I will also discuss a partially weighted, multifactorial model for the acceptability of declarative and interrogative participle adjunct constructions, which explains the judgment patterns in the literature without the need for additional, complex licensing conditions for extraction. Island Extractions in the Wild: A Corpus Study of Adjunct and Relative Clause Islands in Danish and English

Adjuncts and relative clauses are traditionally classified as strong islands for extraction across languages. However, the Mainland Scandinavian (MSc.) languages have been reported to differ from e.g., English in allowing extraction from adjunct and relative clauses. In order to investigate the distribution of possible island extractions in these languages based on naturally produced material, we conducted two exploratory corpus studies on adjunct and relative clause extraction in Danish and in English. Results suggest that both extraction from finite adjuncts and from relative clauses appears at a non-trivial rate in naturally produced Danish, which supports the claim that these structures are not strong islands in Danish. In English, we also found a non-trivial amount of examples displaying extraction from finite adjuncts, as well as a small number of cases of relative clause extraction. This finding presents a potential challenge to the claim that English differs from MSc. in never allowing extraction from strong islands. Furthermore, our results show that both languages appear to share certain trends that can be observed in the extraction examples regarding the type of extraction dependency, the type of adjunct clause featured in adjunct clause extraction, and the type of matrix predicate featured in relative clause extraction.
