Rare Duplication in the RYR1 Gene Causing Malignant Hyperthermia and Clinical Variability

Background/Objectives: Variants in the RYR1 gene are associated mainly with Malignant Hyperthermia. Missense variants are largely the most common, while insertions and duplications account for less than 10%. We aimed to investigate the effect of a rare duplication in the RYR1 gene with the variability of the Malignant Hyperthermia susceptibility phenotype. Methods: We used exome variant screening, in vitro contracture test, anatomopathological examination of the muscle biopsy, RT-qPCR analysis for RYR1 relative expression. Results: We identified a family with two affected siblings carrying an insertion of 18 pair bases in exon 91 of the RYR1 gene, resulting in an in-frame duplication of 6 amino acids (c.12835_12852 dupGAGGGCGCGGCGGGGCTC: 162 p.G4279_T4284insAAGLEG). This variant was found at a frequency of 0.0007% in gnomAD and was absent in 1200 Brazilian controls. First classified as a Variant of Uncertain Significance (VUS), with the molecular and physiological data from our family, we were able to reclassify it, reaching 5 points, which is still a VUS but borderline likely pathogenic. Muscle relative mRNA expression of RYR1 in the two patients identified a ~50% reduction, suggesting a possible hypomorphic allele. Conclusions: The pathomechanisms of RYR1 gene variants in Malignant Hyperthermia are mainly associated with gain-of-function mechanisms, but small insertions can often lead to loss of function or improper folding protein. This study adds evidence to the possibility that duplication in this region can cause structural defects and a more severe phenotype in the patients. Full article

X-ray and electron-beam (E-beam) sterilization technologies were assessed to supplement gamma sterilization, the most common radiation technology used today for biopharmaceutical product sterilization. The mechanical properties of a PE/EVOH/PE film were studied using tensile tests and dynamical mechanical analysis after each irradiation technology (i.e., gamma, electron beam and X-ray irradiations). The effects of each irradiation were compared using two statistical methods. The results indicate that the three irradiation technologies induce no difference in mechanical properties in the investigated dose range for this material. Full article

The Ladle Shroud has become an important part of secondary steelmaking, with its role in reducing liquid steel contamination and process improvements. Due to the inherent negative pressure at the lower nozzle–Ladle Shroud joint, it is well known that Ladle Shrouds, protecting steel flows between a Ladle and a tundish below, can suffer from inadvertent ingress of air. Therefore, there is a need to apply inert gas injection at the joint. In the present paper, 3D transient multi-phase simulations of flows occurring for a Reverse Tapered Ladle Shroud during start-up were studied using CFD software ANSYS Fluent 19.1. This allowed us to study the initial multi-phase flow developed during the start-up and potential steel reoxidation, based on a first principles approach. Time-dependent phase fields as well as attendant velocity and turbulence fields were obtained, resulting in the prediction of a turbulent multi-phase flow during start-up and filling. Additionally, some transient phenomena like steel splashing and air suction were observed mathematically. A full-scale water model of the Ladle Shroud was used to qualitatively validate the initial multi-phase turbulent flow inside the Ladle Shroud, in the absence of inert gas injection. Full article

The reported prevalence of autoantibodies (AAB) (ANA, SMA, LKM, SLA) after pediatric liver transplantation (pLTX) varies considerably from 26–75%, but their clinical impact on outcome is uncertain. We aimed to study the prevalence of AAB after pLTX, their association with donor-, transplant-, and recipient-characteristics, and their relation to outcome. In our multicenter retrospective study, we aimed to clarify conflicting results from earlier studies. Six ERN TransplantChild centers reported data on 242 patients, of whom 61% were AAB positive. Prevalence varied across these centers. Independent of the interval between pLTX and AAB analysis, a one-hour increase in CIT resulted in an odds ratio (OR) of 1.37 (95% CI 1.11–1.69) for SMA positivity and an OR of 1.42 (95%CI 1.18–1.72) for ANA positivity. Steroid-free immunosuppression (IS) versus steroid-including IS (OR 5.28; 95% CI 1.45–19.28) was a risk factor for SMA positivity. Liver enzymes were not associated with ANA or SMA positivity. We did not observe an association of rejection activity index with ANA or SMA. However, the liver fibrosis score in follow-up biopsies was associated with ANA titer and donor age. In conclusion, this first multicenter study on AAB after pLTX showed high AAB prevalence and varied widely between centers. Longer CIT and prednisolone-free-IS were associated with AAB positivity, whereas AAB were not indicative of rejection, but instead were associated with graft fibrosis. The detection of AAB may be a marker of liver fibrosis and may be taken into consideration when indications for liver biopsy and immunosuppressive regimes, or reduction of immunosuppression in long-term follow-up, are being discussed. Prospective immunological profiling of pLTX patients, including AAB, is important to further improve our understanding of transplant immunology and silent graft fibrosis. Full article

Nuclear rivalry, as well as terrorism and the war against terror, exemplify the dangerous escalation of violence that is threatening our world. Gandhi’s militant nonviolence offers a possible alternative that avoids a complacent indifference toward injustice as well as the imitation of violence that leads to its escalation. The French-American cultural anthropologist René Girard discovered mimetic rivalries as one of the main roots of human conflicts, and also highlighted the contagious nature of violence. This article shows that Gandhi shares these basic insights of Girard’s anthropology, which increases the plausibility of his plea for nonviolence. Reading Gandhi with Girard also complements Girard’s mimetic theory by offering an active practice of nonviolence as a response to violent threats, and by broadening the scope of its religious outreach. Gandhi’s reading of the Sermon on Mount not only renounces violence and retaliation like Girard but also underlines the need to actively break with evil. Both Gandhi and Girard also address the religious preconditions of nonviolent action by underlining the need to prefer godly over worldly pursuits, and to overcome the fear of death by God’s grace. This congruence shows that Girard’s anthropology is valid beyond its usual affinity with Judaism and Christianity. Full article

Chemically and biologically safe storage of solutions for medical uses is a daily concern for industry since decades and it appeared even more dramatic during the last two years of pandemia. Biological safety is readily reached by sterilization using γ-irradiation process. However, such a type of irradiation induces the degradation and the release of chemicals able to spoil the biological solutions. Surprisingly, there are no investigations on multi-layer films combining multi-technique and multi-method approaches to unveil the events occurring during γ-irradiation. Furthermore, our investigations are focuses on properties/events occurring at product, macromolecular, and molecular levels. Full article

In this study, the oxidation of methionine is used as a proxy to model the gamma radiation-induced changes in single-use bags; these changes lead to the formation of acids, radicals, and hydroperoxides. The mechanisms of formation of these reactive species and of methionine oxidation are discussed. With the help of reaction kinetics, the optimal conditions for the use of these single-use bags minimizing the impact of radical chemistry are highlighted. Biopharmaceutical bags gamma irradiated from 0 kGy to 260 kGy and aged from 0 to 36 months were filled with a methionine solution to follow the oxidation of the methionine. The methionine sulfoxide was measured with HPLC after different storage times (0, 3, 10, 14, 17, and 21 days). Three main results were analyzed through a design of experiments: the oxidative induction time, the methionine sulfoxide formation rate, and the maximum methionine sulfoxide concentration detected. A key aspect of the study is that it highlights that methionine is oxidized not necessarily directly by hydro(gen) peroxide but throughperacid, and likely peracetic acid. The answers to the design of experiments were considered to obtain the desirability domain for the optimization of the conditions of use for the single-use bags limiting the oxidation of methionine as well as the release of reactive species thereof. The annual Eastern Canadian Gastrointestinal Cancer Consensus Conference 2016 was held in Montreal, Quebec, 5–7 February. Experts in radiation oncology, medical oncology, surgical oncology, and infectious diseases involved in the management of patients with gastrointestinal malignancies participated in presentations and discussion sessions for the purpose of developing the recommendations presented here. This consensus statement addresses multiple topics: (1) Follow-up and survivorship of patients with resected colorectal cancer; (2) Indications for liver metastasectomy; (3) Treatment of oligometastases by stereotactic body radiation therapy; (4) Treatment of borderline resectable and unresectable pancreatic cancer; (5) Transarterial chemoembolization in hepatocellular carcinoma; (6) Infectious complications of antineoplastic agents.
