Background: The pursuit of longevity has long been central to nutritional science, with growing evidence underscoring the profound influence of dietary patterns on lifespan and overall health. While various diets have been associated with improved well-being, their comparative effects on longevity remain to be synthesized comprehensively. Main Findings: This review examines the scientific evidence linking major dietary patterns such as the Mediterranean, DASH (Dietary Approaches to Stop Hypertension), plant-based, Blue Zones, intermittent fasting, caloric restriction, and Nordic diets, to longevity and mortality outcomes. The Mediterranean and DASH diets consistently demonstrate reduced cardiovascular and all-cause mortality, while plant-based and Blue Zones diets emphasize whole, minimally processed foods that enhance metabolic and cardiovascular health. Intermittent fasting and caloric restriction emerge as metabolic modulators with anti-aging potential. The Nordic diet, rich in locally sourced foods like berries and fish, also contributes to lower mortality and improved cardiovascular function. Conclusions: This paper uniquely integrates comparative insights from diverse dietary frameworks, emphasizing their shared principles of nutrient density, moderation, and metabolic balance. By adopting evidence-based elements from these dietary models, individuals can optimize health span and longevity, reinforcing the pivotal role of diet as a cornerstone of preventive and personalized nutrition. Background: China’s rapidly aging population has led to a growing burden of dementia, marked by cognitive decline and heavy social and economic costs. Dietary patterns have been identified as a critical means for prevention. Methods: This study drew on data from the China Longitudinal Health and Longevity Survey (CLHLS). Three logistic regression models were applied to examine the association between the Chinese version of the Mediterranean-Dietary Approaches to Stop Hypertension Intervention for Neurodegenerative Delay (cMIND) diet and dementia. To test the stability of the results, we conducted two sensitivity analyses. Restricted cubic spline (RCS) models were used to assess the potential for a nonlinear relationship. Subgroup and interaction analyses were conducted to explore heterogeneity across covariates and main effects. Propensity score matching (PSM) was performed as a secondary analysis to minimize the influence of confounding factors. Results: The study included 9142 participants, with a dementia prevalence of 10.7% among Chinese older adults. After adjusting for all covariates, each one-unit increase in the cMIND diet score was associated with an 11% lower prevalence of dementia (OR = 0.89; 95% CI: 0.84–0.93). After full adjustment, the RCS model confirmed a significant and linear dose–response association between adherence to the cMIND diet and dementia. Comparable associations were observed across most subgroups. Conclusions: Adherence to the cMIND diet was significantly associated with a lower prevalence of dementia in Chinese older adults, with evidence of a clear dose–response effect. These findings highlight the potential of the cMIND diet as a preventive strategy against dementia in this population. Full article

Background/Objectives: Type 2 diabetes mellitus (T2DM) represents a growing public health burden in the European Union (EU), largely driven by modifiable lifestyle factors such as diet. This systematic review aimed to synthesize observational evidence on the associations between nutritional exposures and incident T2DM across the EU-28, with a focus on regional heterogeneity and implications for EU-level nutrition policy. Methods: The review followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD42020219994). Searches were conducted in different databases (PubMed, Embase, Scopus, Web of Science) identifying 23,437 records, from which 104 observational studies were included. Eligible studies involved adults (≥18 years) without T2DM at baseline and assessed dietary exposures in relation to T2DM incidence. Data extraction and methodological quality assessment were performed in duplicate using the NHLBI tool in Covidence. Results: Most included studies were cohort studies (77.9%), primarily conducted in Western, Northern, and Southern Europe. Diets characterized by high consumption of whole and minimally processed foods, such as fruits, vegetables, legumes, whole grains, and fermented dairy, consistently showed associations with lower T2DM risk. In contrast, high intake of red and processed meats, sugar-sweetened beverages, and ultra-processed foods was linked to higher risk. Adherence to Mediterranean or plant-based dietary patterns was associated with lower risk, whereas high animal-protein dietary patterns were detrimental. Conclusions: Nutritional determinants play a decisive role in shaping T2DM risk in the EU. Evidence supports prioritizing dietary patterns rich in plant-based and minimally processed foods while reducing ultra-processed and meat-based products. Tailored region-specific policies are needed to address the T2DM epidemic and guide effective prevention strategies. Full article

Background/Objectives: Nutrition during the reproductive years shapes women’s immediate health, fertility, pregnancy outcomes, and long-term offspring well-being. This position paper narratively synthesizes and critically appraises evidence on how dietary patterns, macro-/micronutrients, and supplementation influence women’s health, female fertility, and reproductive outcomes, to inform practical recommendations. Methods: We narratively reviewed recent reviews, cohort studies, clinical trials, and public-health guidance on macronutrients, key micronutrients, dietary patterns (with emphasis on the Mediterranean diet), ultra-processed food (UPF) intake, and targeted supplementation relevant to menstrual, metabolic, cardiovascular, skeletal, and reproductive outcomes. Results: Balanced, diverse diets rich in whole and minimally processed foods support hormonal regulation, ovulatory function, healthy gestation, and chronic-disease risk reduction. Priority nutrients include iron, folate, calcium, vitamin D, zinc, vitamin B12, and long-chain omega-3s (DHA), with supplementation considered when dietary intake or bioavailability is inadequate. Evidence consistently links Mediterranean-style eating to improved metabolic health, insulin sensitivity, IVF success, lower gestational diabetes risk, and favorable neonatal outcomes. High UPF consumption is associated with poorer diet quality, inflammation, adverse pregnancy outcomes, and potential reproductive impairment, warranting a reduction in favor of nutrient-dense foods. Diet also influences cardiovascular and bone health through effects on lipids, glycemia, blood pressure, and mineral/vitamin status, with fiber-rich carbohydrates, unsaturated fats (notably olive oil), and adequate calcium–vitamin D emerging as central levers. Conclusions: For women of childbearing age, a Mediterranean-aligned, minimally processed dietary pattern—tailored to individual needs and complemented by prudent use of folate, iron, vitamin D, calcium, B12, and DHA when indicated—offers robust benefits across reproductive, metabolic, cardiovascular, and skeletal domains. Public-health actions should improve access to healthy foods, curb UPF marketing, and embed personalized nutrition counseling in routine care; further longitudinal research from preconception through postpartum is needed. Hypertension (HTN) and chronic kidney disease (CKD) are significant global health burdens, with microalbuminuria (MA) serving as a key early marker of renal damage and cardiovascular risk. While nutritional interventions are pivotal for management, the evidence for specific nutrients is often complex and inconsistent, creating challenges for clinical guidance. This review critically evaluates current evidence on the interaction among macronutrients, micronutrients, and established dietary approaches and their influence on the development and course of HTN and MA. Strong consensus is present regarding sodium restriction, increased intakes of potassium, and the implementation of dietary patterns like Dietary Approaches to Stop Hypertension (DASH) and the Mediterranean diet to improve blood pressure and renal outcomes. Evidence favors protein moderation (approximately 0.8 g/kg/day), especially from plant sources, and emphasizes carbohydrate quality (e.g., high fiber, low glycemic index) over absolute quantity. The role of micronutrients is more nuanced; maintaining vitamin D sufficiency is protective, but intervention trials for many supplements, including B vitamins and antioxidant vitamins (C and E), have yielded inconsistent results. Several minerals, such as iron and selenium, exhibit a U-shaped risk curve where both deficiency and excess are detrimental, highlighting the risks of unselective supplementation. Ideal nutrition care prioritizes holistic dietary patterns over a focus on single nutrients. Clinical guidance should be founded on sodium reduction and potassium-rich foods, with personalized recommendations for protein and micronutrient supplementation based on an individual’s specific cardiovascular and renal profile. Future research must target nutrients with conflicting evidence to establish clear, evidence-based intake guidelines. Full article

Diet influences brain health through many connected metabolic and molecular pathways, and these effects are stronger in obesity. This review links diet quality with cognitive decline and dementia risk. Ultra-processed, high-fat, high-sugar diets drive weight gain, insulin resistance, and chronic inflammation. These changes trigger brain oxidative stress, reduce DNA repair, deplete NAD+, disturb sirtuin/PARP balance, and alter epigenetic marks. Gut dysbiosis and leaky gut add inflammatory signals, weaken the blood–brain barrier, and disrupt microglia. Mediterranean and MIND diets, rich in plants, fiber, polyphenols, and omega-3 fats, slow cognitive decline and lower dementia risk. Trials show extra benefit when diet improves alongside exercise and vascular risk control. Specific nutrients can help in certain settings. DHA and EPA support brain health in people with low omega-3 status or early disease. B-vitamins slow brain shrinkage in mild cognitive impairment when homocysteine is high. Vitamin D correction is beneficial when levels are low. A practical plan emphasizes healthy eating and good metabolic control. It includes screening for deficiencies and supporting the microbiome with fiber and fermented foods. Mechanism-based add-ons, such as NAD+ boosters, deserve testing in lifestyle-focused trials. Together, these measures may reduce diet-related brain risk across the life span. At the same time, artificial intelligence can integrate diet exposures, adiposity, metabolic markers, multi-omics, neuroimaging, and digital phenotyping. This can identify high-risk phenotypes, refine causal links along the diet–obesity–brain axis, and personalize nutrition-plus-lifestyle interventions. It can also highlight safety, equity, and privacy considerations. Translationally, a pattern-first strategy can support early screening and personalized risk reduction by integrating diet quality, adiposity, vascular risk, micronutrient status, and microbiome-responsive behaviors. AI can aid measurement and risk stratification when developed with privacy, equity, and interpretability safeguards, but clinical decisions should remain mechanism-aligned and trial-anchored. Background/Objectives: Metabolic dysfunction-associated steatotic liver disease (MASLD) is rapidly attracting growing concern around the world. While there has been progress in the development of pharmacologic treatments, lifestyle and dietary interventions remain as the first-line approach for management. This scoping review aimed to identify dietary strategies for managing MASLD and to highlight current research gaps and challenges. Methods: A systematic search of PubMed and Science Direct was conducted up to 10 July 2025, for relevant studies on dietary modifications and MASLD. Data extracted included types of interventions, outcomes related to liver health, and research limitations. Results: Dietary interventions were shown to consistently improve hepatic and metabolic outcomes. In a randomized controlled trial of 12 weeks (n = 259), a Mediterranean diet reduced hepatic steatosis by 39% and improved insulin sensitivity. A calorie-restricted lifestyle program in adults with MASLD (n = 196) reduced liver fat by 25% over 52 weeks. Resistant starch supplementation (n = 200) lowered intrahepatic triglyceride content by 8% through gut microbiome modulation. A pilot RCT of medically tailored meals in cirrhosis (n = 40) reduced ascites symptoms and improved quality of life. Finally, prebiotic supplementation in MASLD (n = 200) lowered systemic inflammation and increased immune-regulating microbes. In contrast, Western dietary patterns and ultra-processed foods were consistently linked to lipotoxicity and inflammation. Conclusions: Dietary interventions remain critical for the management of chronic liver disease and continue to play a vital role even as pharmacotherapy options emerge. Further research should explore precision nutrition and microbiome-based therapies while also addressing the methodological limitations like the underutilization of causal inference frameworks. Finally, it is also important to consider culturally tailored interventions to account for barriers in access and equity in underserved populations. Full article

Background/Objectives: In FY 2024, 12.3% of the U.S. population was enrolled in the Supplementary Nutrition Assistance Program or SNAP, the principal food and nutrition program of the U.S. government. Herein, we analyze the cost of the Harvard (HHEP), Mediterranean (MED) and Dietary Approaches to Stop Hypertension (DASH) eating patterns (EPs) to ascertain if they are affordable with the maximum allowable SNAP benefit. Methods: We utilize the 2021 Thrifty Food Plan (TFP) framework to analyze the cost of each of these alternative EPs across 15 age/sex groups. Results: We find that the MED and DASH EPs’ costs typically exceed the daily maximum SNAP benefit for those consuming more than 2100 calories. Conclusions: Our result suggests that reliance on fluid milk, allowance of more refined grains, starchy vegetables, red meat, added sugar and sodium and indifference toward food quality and freshness when calculating the maximum SNAP benefit reduces its cost in comparison to diets less reliant on fluid milk and more reliant on whole grains, fish, nuts, and fresh fruits and vegetables, a commonality shared by the HHEP, MED and DASH EPs. Health-centered alternative diets such as MED and DASH cannot be purchased with maximum allowable allocations of SNAP benefits, meaning that a poverty tax is placed on SNAP beneficiaries. Tomatoes are among the most consumed vegetables within the Mediterranean diet, recognized as one of the healthiest dietary patterns in the world. This study evaluated the effects of four fertilization treatments on the antioxidant and metabolic profiles of four local tomato varieties: Cornabel (V1), Cuban Pepper (V2), Corno Andino (V3), and Roli Rosa (V4). Treatment 1 (T1) used 1.28 kg/m2 of commercial woody compost (C/N ratio 13), while Treatment 2 (T2) served as the control, initially without fertilization. Treatments 3 and 4 (T3, T4) incorporated ramial chipped wood (RCW) at 15 kg/m2 and 7.5 kg/m2, respectively, without tillage. Each treatment × variety combination included four biological replicates (n = 4). Untargeted metabolomic profiling via UHPLC-QToF and statistical analyses identified 163 compounds, 37 of which showed significant varietal differences (p < 0.05). The flavonoid eriodictyol was more abundant in the Pebroter varieties (V1, V2, V3), whereas luteolin-8-O-glucoside (orientin) predominated in V4, likely due to differential expression of biosynthetic genes. RCW (T3) yielded higher levels of antioxidant compounds like phenolic acids, and flavonoids compared to compost and control; however, no dose–response was observed using different doses of RCW (T3 and T4). These findings demonstrate that RCW may modulate the antioxidant metabolite profile of tomatoes, highlighting its potential as a sustainable fertilization strategy for enhancing antioxidant compounds. Background: Migraine is a common, disabling neurological disorder with substantial genetic and environmental contributions. Dietary exposures are widely discussed by patients and clinicians as potential triggers or modifiers of attack frequency and severity. We synthesized contemporary evidence on dietary patterns, specific nutrients, and elimination strategies relevant to migraine prevention and management. Methods: We performed a narrative review of PubMed and Google Scholar (inception–August 2025) using combinations of “migraine”, “diet”, “nutrition”, “ketogenic”, “Mediterranean”, “omega-3”, and “gluten”. We prioritized randomized/controlled studies, recent systematic reviews/meta-analyses, and representative observational studies; evidence quality and applicability were appraised descriptively. Results: Higher adherence to a Mediterranean-style diet is associated with lower migraine frequency and disability in observational cohorts. Very low-calorie ketogenic diets significantly reduced monthly migraine attack frequency compared with isocaloric non-ketogenic comparators in an adult randomized controlled trial of participants with overweight or obesity (≥50% responder rate: 74% vs. 6%). Additional supportive evidence from uncontrolled studies, including those involving medium-chain triglyceride supplementation, further corroborates these findings. Omega-3 fatty acids (EPA/DHA) show prophylactic benefit in randomized trials and network meta-analyses, with favorable tolerability. Gluten-free diets may improve headaches in celiac disease and may help selected non-celiac patients. Alcohol (especially red wine) and high, irregular caffeine intake are frequently reported triggers, while evidence for specific foods/additives remains inconsistent. Weight loss and regular physical activity may further reduce burden in people with obesity. Conclusions: Current evidence supports recommending Mediterranean-style eating, consideration of omega-3 supplementation, and selective trials of ketogenic or elimination approaches in appropriate patients, alongside weight management and lifestyle optimization. High-quality, longer-duration RCTs using standardized dietary protocols and adherence biomarkers are needed to define dose–response relationships and enable personalized nutrition in migraine. This study focused on investigating the effects of dietary counseling on antioxidant intakes and how diet can influence reactive oxygen species (ROS) levels and consequently oxidative stress (OS) during pregnancy. At the end of the first trimester of pregnancy (12–13 week of pregnancy), 80 women were randomized into a control group and to an intervention group to receive individual dietary counseling. In the intervention group (n = 60), the dietary counseling about the Mediterranean diet (MD) was carried out during six online meetings every 15 days and focused on antioxidant nutrients and their intake. They were encouraged to follow the antioxidant-rich diet for 12 weeks, including increased consumption of antioxidant-rich foods such as ≥5 servings/day of vegetables, ≥2 servings/day of fruit, ≥8.5 servings/day of wholegrains and 3–4 servings/week of lean meat. In the control group (n = 20) dietary counseling included generic, standard of care guidance about nutrition in pregnancy and was only discussed during a single session. OS biomarkers, particularly (ROS) and Total Antioxidant Capacity (TAC) levels, were analyzed before and after the study duration in women’s serum. The results of the study showed lower ROS and higher TAC levels after intervention compared to the levels before intervention, as well as compared to the levels of the control group. In conclusion, dietary counseling improved the intake of antioxidant nutrients from food during pregnancy, as depicted by oxidative stress biomarkers’ levels, and may influence the observed impacts of these dietary patterns on pregnancy outcomes. Background: Insulin resistance (IR) is a key pathophysiological mechanism linking obesity, type 2 diabetes, and cardiovascular disease. Office workers, due to prolonged sedentary behavior and suboptimal lifestyle patterns, may be particularly susceptible to IR. However, large-scale studies in this occupational group remain scarce. Objective: To evaluate the prevalence of elevated IR risk using non–insulin-based indices—TyG, METS-IR, and SPISE—and their associations with sociodemographic and lifestyle factors in a large sample of Spanish office workers. Methods: This cross-sectional study included 82,020 office workers from Spain (2021–2022). IR risk was assessed using the TyG index, METS-IR, and SPISE, all derived from fasting biochemical and anthropometric data. Sociodemographic and lifestyle variables were self-reported using validated questionnaires. Sex-stratified analyses and multivariate logistic regression models were performed. Results: Men showed significantly higher odds of elevated IR risk compared to women across all indices: TyG (OR = 2.48, 95% CI: 2.37–2.60), METS-IR (OR = 1.47, 95% CI: 1.38–1.57), and SPISE (OR = 1.88, 95% CI: 1.78–1.99). Smoking, physical inactivity, and low adherence to the Mediterranean diet were independently associated with elevated IR scores, regardless of sex or age. Conclusions: A substantial proportion of office workers exhibit elevated insulin resistance risk, particularly among men and those with unhealthy lifestyles. TyG, METS-IR, and SPISE are valuable, low-cost tools for early IR detection in occupational health settings. These findings support the implementation of preventive strategies targeting modifiable behaviors in sedentary working populations. Full article

Background/Objectives: The aging population is associated with an increased incidence of dementia, which deteriorates the quality of life of adults, leading to an elevated socioeconomic burden. This review aimed to extensively examine which dietary patterns favorably influence cognitive outcomes based on prospective cohort studies of adults. Methods: A literature search was performed in the PubMed®/MEDLINE® database up to 30 October 2024. Results: One hundred and eighteen publications were included. In a comparison of high and low categories, the Mediterranean-Dietary Approaches to Stop Hypertension Intervention for Neurodegenerative Delay (MIND) diet increased cognitive function and reduced the risk of cognitive impairment. The Mediterranean (MED) diet improved cognitive function but showed no beneficial effects on cognitive impairment or dementia. The Dietary Approaches to Stop Hypertension (DASH) diet lowered the risk of cognitive impairment but showed no alteration in either cognitive function or dementia. The Healthy Eating Index (HEI) did not alter cognitive function, cognitive impairment, or dementia. The healthy plant-based dietary index (hPDI) decreased the risk of cognitive impairment and dementia, whereas the unhealthy plant-based dietary index (uPDI) elevated the risk of cognitive impairment. The Western dietary pattern (WDP) decreased cognitive function. Conclusions: The MIND diet and hPDI appeared to be effective, while the WDP appeared to be detrimental for cognitive health. Further prospective cohort studies of healthy and unhealthy dietary patterns are required to confirm the association between diverse diets and cognitive health. Full article

Background: The United States Dietary Guidelines (USDG) form the basis of federal nutrition programs/policies for Americans. There has been little work to ensure that the presentation of the USDG are culturally acceptable and relevant for African Americans (AAs). This study aimed to explore the acceptability and perceptions of a randomized intervention among AA adults adhering to unmodified dietary patterns outlined in the USDG (Healthy US, Mediterranean, Vegetarian). Methods: Qualitative focus groups were conducted with participants from a USDG-based intervention, the Dietary Guidelines: 3 Diets study (DG3D). Six focus group discussions were conducted with AA adults in the Southeastern US in December 2021, after completion of the 12-week DG3D intervention. Verbatim transcripts were coded thematically and analyzed in Nvivo12 using an iterative constant comparative method. Results: Participants (n = 42; median age 52 years, range 26–65, 16 Healthy US, 17 Mediterranean, 9 Vegetarian) shared their perspectives and experiences adopting USDG dietary patterns and reported barriers and facilitators to adopting dietary change. Discussions elicited insights into the cultural relevance of the USDG and dietary intervention. Participants also described their recommended changes to the USDG-based intervention to enhance program implementation and successful dietary change. Conclusions: Study findings suggest that adaptations to the USDG dietary patterns are needed to ensure cultural relevance for AA adults in the US. This study can inform the development of culturally relevant dietary guidelines and intervention programs. Full article

Background: Chronic skin ulcers are characterized by an impaired and delayed wound healing process, posing a major economic and healthcare burden. These multifactorial conditions are influenced by both biological, clinical and psychosocial factors. The aim of our cross-sectional study was to investigate the influence of psychosocial and lifestyle factors—specifically adherence to the Mediterranean diet, emotional health, sleep quality and demographic characteristics—on physical symptoms and clinical severity in patients with skin ulcers, using a multidisciplinary approach to identify key predictors of disease burden. Methods: A cross-sectional analysis was conducted on patients with skin ulcers, using data from a monocentric pathology registry. Collected variables included gender, age, dietary habits (specifically, adherence to the Mediterranean diet), sleep disturbances, educational level, anxiety and depressive symptoms, Physician Global Assessment (PGA), Patient Global Assessment (PtGA), and Skindex-17 (a dermatology-specific quality of life measure). Hierarchic multivariate linear regression models were applied to identify predictors of physical symptoms and clinical severity, while simultaneously controlling for potential confounders. Results: Older age, poorer adherence to the Mediterranean diet, and elevated anxiety levels emerged as the strongest predictors of worse physical symptoms, as measured by the Skindex-17. Male sex and more severe depressive symptoms were significantly associated with higher PGA scores. Our data also suggest older age and poorer Mediterranean diet adherence to influence clinical severity. Lastly, sleep disturbances were also found to correlate with patient-reported severity. Conclusions: Our study underscores the impact of psychosocial and behavioral/lifestyle factors on the clinical burden of skin ulcers through a comprehensive multidisciplinary approach. In particular, our data indicate that dietary patterns and emotional health appear to shape both symptom perception and clinical evaluation, emphasizing the need for holistic management strategies.
