Variations in the Diagnosis and Management of Benign Paroxysmal Positional Vertigo Among Physician Specialties in Saudi Arabia: Influence of Clinical Experience and Case Exposure

Background/Objectives: Benign paroxysmal positional vertigo (BPPV) is the most prevalent vestibular disorder encountered in clinical settings and is highly responsive to evidence-based diagnostic and therapeutic interventions. However, variations in practice patterns among physician specialties can compromise timely diagnosis and effective treatment. Understanding these variations is essential for improving clinical outcomes and standardizing care. This study aimed to assess the diagnostic and treatment practices for BPPV among Ear, Nose, and Throat (ENT) specialists, neurologists, general practitioners, and family physicians in Saudi Arabia and to examine how these practices are influenced by clinical experience and patient case exposure. Methods: A cross-sectional, questionnaire-based study was conducted between April 2023 and March 2024 at King Khalid University, Abha, Saudi Arabia. A total of 413 physicians were recruited using purposive sampling. Data were analyzed using IBM SPSS version 24.0. Parametric tests, including one-way ANOVA and chi-square tests, were used to assess differences across groups. A p-value of <0.05 was considered statistically significant. Results: Overall, all physician groups exhibited limited adherence to guideline-recommended positional diagnostic and therapeutic maneuvers. However, ENT specialists and neurologists demonstrated relatively higher compliance, particularly in performing the Dix–Hallpike test, with 46.97% and 26.79% reporting “always” using the maneuver, respectively (p < 0.001, Cramér’s V = 0.22). Neurologists were the most consistent in conducting oculomotor examinations, with 73.68% reporting routine performance (p < 0.001, Cramér’s V = 0.35). Epley maneuver usage was highest among neurologists (86.36%) and ENT specialists (77.14%) compared to family physicians (50.60%) and GPs (67.50%) (p = 0.044). Physicians with 11–15 years of experience and >50 BPPV case exposures consistently showed a greater use of diagnostic maneuvers, repositioning techniques, and guideline-concordant medication use (betahistine 76.67%; p < 0.001). Continuing medical education (CME) participation and the avoidance of unnecessary imaging were also highest in this group (46.67% and 3.33%, respectively; p < 0.001). Conclusions: Significant inter-specialty differences exist in the management of BPPV in Saudi Arabia. Greater clinical experience and higher case exposure are associated with improved adherence to evidence-based practices. Targeted educational interventions are needed, particularly in primary care, to enhance guideline implementation. Full article

Background/Objectives: Salmonella enterica subsp. enterica serovar Enteritidis (S. Enteritidis) is a major pathogen associated with poultry products, and the rise of antimicrobial-resistant strains has intensified the need for effective natural control strategies. Essential oils (EOs) are recognized for their antimicrobial potential, but their volatility, instability, and risk of toxicity at high concentrations limit their practical application. Therefore, the objective of this study was to evaluate the antimicrobial efficacy of EOs in broilers infected with S. Enteritidis and to characterize potential synergistic or antagonistic interactions between the oils. Methods: To achieve this, the oils were first assessed through Minimum Inhibitory Concentration (MIC), Minimum Bactericidal Concentration (MBC), and Fractional Inhibitory Concentration (FIC) assays, and the most effective ones against S. Enteritidis were selected. These selected oils were then microencapsulated and incorporated into the broiler feed for the in vivo assay. Results: The encapsulated formulation retained key bioactive compounds and significantly reduced bacterial shedding and intestinal colonization when administered to broilers experimentally infected with S. Enteritidis. Broilers receiving the optimized half-dose supplementation exhibited a 36% reduction in fecal shedding and a 4 log10 decrease in cecal bacterial counts compared to untreated controls. A transient reduction in liver colonization was also observed, while feed intake remained unaffected. Conclusions: These findings demonstrate that microencapsulated EOs can serve as an effective natural strategy to control S. Enteritidis in poultry. The results support the broader application of lipid-based encapsulation technologies for improving the functional performance of phytobiotics in animal production. Full article

Remote support in general is a method that saves time and resources. A relatively new and promising technology for remote support that combines video conferencing and physical mobility is that of telepresence systems. The remote assistant, that is, the user of said technology, gains both presence and maneuverability in the distant location. As telepresence systems vary greatly in their design, the question arises as to whether the design influences the perception of the remote assistant. Unlike pure design studies, the present work focuses not only on the design and evaluation of the telepresence system itself, but especially on its perception during a collaborative task involving a human partner visible through the telepresence system. This paper presents two studies in which participants performed an assembly task under the guidance of a remote assistant. The remote assistant was visible through differently designed telepresence systems that were evaluated in terms of social perception and trustworthiness. Four telepresence systems were evaluated in study 1 (N = 32) and five different systems in study 2 (N = 34). The results indicated that similarly designed systems showed only marginal differences, but a system that was designed to transport additional loads and was therefore less agile and rather bulky was rated significantly less positively regarding competence than the other systems. It is particularly noteworthy that it was not the height of the communication medium that was decisive for the rating, but above all, the agility and mobility of the system. These results provide evidence that the design of a telepresence system can influence the social perception of the remote assistant and therefore has implications for the acceptance and use of telepresence systems. Heterogeneous nuclear ribonucleoprotein K (hnRNPK) is an RNA-binding protein containing low-complexity domains (LCDs), which are known to regulate protein behavior under stress conditions. This study demonstrates the ability to control hnRNPK’s transitions into four distinct material states—monomer, soluble aggregate, liquid droplet, and fibrillar hydrogel—by modulating environmental factors such as temperature and protein concentration. Importantly, the phase-separated and hydrogel states are newly identified for eGFP-hnRNPK, marking a significant advancement in understanding its material properties. A combination of biophysical techniques, including DLS and SEC-LS, were used to further characterize hnRNPK in monomeric and soluble aggregate states. Structural methods, such as SANS, SAXS, and TEM, revealed the elongated morphology of the hnRNPK monomer. Environmental perturbations, such as decreased temperature or crowding agents, drove hnRNPK into phase-separated or gel-like states, each with distinct biophysical characteristics. These novel states were further analyzed using SEM, X-ray diffraction, and fluorescence microscopy. Collectively, these results demonstrate the complex behaviors of hnRNPK under different conditions and illustrate the properties of the protein in each material state. Transitions of hnRNPK upon condition changes could potentially affect functions of hnRNPK, playing a significant role in regulation of hnRNPK-involved processes in the cell. Full article

Background/Objectives: Accurate head positioning is essential for diagnostics of benign paroxysmal positional vertigo (BPPV). This study aimed to quantify the head angles and angular velocities during traditional manual BPPV diagnostics in patients with positional vertigo. Methods: A prospective, observational cohort study was conducted at a tertiary university hospital outpatient clinic. One trained examiner performed the Supine Roll Test (SRT) and the Dix–Hallpike test (DHT) on 198 adults with positional vertigo. The primary outcomes included head angle variability and accuracy and angular velocity variability. The secondary outcomes examined the relationship between the head angle accuracy and participant-reported limitations. Results: The absolute variability for all head angles ranged from ±8.7° to ±11.0°. The yaw axis head angles during the DHT, particularly on the left side, had the highest relative variability (left DHT: coefficient of variance 0.29). Systematic errors included the yaw axis head angles undershooting the target (90°) by 19.7–23.8° during the SRT and the pitch axis head angles undershooting the target (120°) by 7.8–8.7° during the DHT. The left-sided yaw axis in the DHT was undershot by 11.8°, while the right-sided DHT angle was slightly overshot (2.5°). Right-sided yaw axis angles in the SRT and DHT were more accurate than the left-sided ones (right SRT: 19.9°; left SRT: 23.9°; p < 0.0001) (right DHT: 7.0°; left DHT: 13.2°; p < 0.0001). The regression analysis found no association between the participant-reported limitations and head angle accuracy. Conclusions: This study highlights the substantial variability and inaccuracies in head positioning during traditional manual BPPV diagnostics, supporting the relevance of a guidance system to improve BPPV diagnostics. This work presents a nano-watt digital output temperature sensor featuring a supply-insensitive, self-biased current source. Second-order temperature dependencies of the MOS diode are canceled to produce a stable reference and a linear temperature-sensitive voltage. The sensor integrates a sensing unit, voltage-controlled differential ring oscillators, and a low-power frequency-to-digital converter, utilizing a resistor-less design to minimize power and area. The delay element in the ring oscillator reduces stage count, improving noise performance and compactness. Fabricated in 65 nm CMOS, the sensor occupies 0.02 mm2 and consumes 60 nW at 25 °C and 0.8 V. Measurements show an inaccuracy of +1.5/−1.6 °C from −20 °C to 120 °C after two-point calibration, with a resolution of 0.2 °C (rms) and a resolution FoM of 0.022 nJ·K−2. Consuming 0.55 nJ per conversion with a 9.2 ms conversion time, the sensor was tested in a battery-less wireless sensor node, demonstrating its suitability for wireless sensing systems. Full article

A low-power, high-speed adder circuit topology based on current-starved inverters is presented to provide a basic arithmetic function for low-power, high-frequency signal processing systems. The adder is designed in 22 nm FDSOI (Fully-Depleted Silicon-on-Insulator) technology and is suitable for operation up to 5 GHz (Giga-Hertz). The proposed adder utilizes current-starved inverters to implement low-power operation while still maintaining signal integrity for high-frequency sine signals. The circuit uses a differential input and output structure to mitigate potential noise coupling onto any high-frequency signal pathways. The proposed solution differs from standard adder architectures by utilizing a fully analog signal processing design, accepting analog inputs while outputting an analog signal, and offering suitable functionality at Giga-Hertz level signals as compared to other relevant works. The simulated experimental results show the power consumption to be approximately 150 nW at 0.8 V supply with an input-referred noise of 6.091 nV/Hz at 5 GHz. Full article

This study investigated how the DNACJ6 gene variation related to RMR alteration affects risk factors of obese environments in children with obesity aged 8–9. Methods: Over a three-year follow-up period, 63.3% of original students participated. Changes in the variables (anthropometrics, blood biochemistry, and dietary intakes) were analyzed and compared between those without obesity (non-OB) and with obesity (OB) classified at the study endpoint. Result: The average MAF of nine SNPs (D-1 to D-IX) was defined as 18.1%. The OB group showed greater increases in RMR, BMI, WC, and SBP, while the non-OB group had significantly greater increases in HDL and intakes of nutrients (e.g., total calories, vitamins B2, C, folate, A, retinol, iron, and zinc). Increased RMR, BMI, BW, and RMR/BW changes were observed with mutant allele of D-I SNP, which was also associated with a higher prevalence of obesity. Greater increases in animal fat intake, including saturated fatty acids and retinol, were noted in the minor alleles of D-VI, D-VII, D-VIII, and D-IX SNPs compared to those of the major alleles. The odds ratio for BMI risk was significantly higher in the mutant alleles of D-I (rs17127601), D-VII (rs1334880), and D-VIII (rs7354899) compared to the wild type, with increases of 2.59 times (CI; 1.068–6.274), 1.86 times (CI; 1.012–3.422), and 1.85 times (CI; 1.008–3.416), respectively. RMR was a mild risk factor in minors of the D-1, D-VII, and D-VIII; however, a higher RMR/BW ratio significantly correlated with decreased BMI risk, and this effect was found in only the major alleles of D-I, D-VII, and D-VIII SNPs, not in the minor alleles. High retinol intake appeared to reduce obesity risk in the minor alleles of the D-I, D-VII, and D-VIII SNPs, even though intake of animal fats and retinol remained higher among minors over the three years. Conclusions: These findings suggest that the RMR/BW ratio and dietary fat/retinol intake should be considered in DNACJ6-gene-based precision medicine approaches for pediatric obesity prevention, particularly for boys. Background/Objectives: In the framework of the ValueCare project (funded by EC, ref 875215), the Valencia pilot site assessed the comprehensive health of 240 older people with frailty. ValueCare aims to deliver personalised integrated health and social care and better outcomes for older people. Methods: For the health evaluation, a comprehensive approach was adopted, based on validated questionnaires that address not only mental and physical health but also other key dimensions in older people’s well-being, namely unwanted loneliness and nutrition. Results: This study provides an overview of the overall health status of 60-year-old people from March to December 2022 in the city of Valencia, allowing for a comparison between national and European health indicators. Conclusions: Older people in the city of Valencia reported good health, quality of life, independency, nutrition and social wellbeing, with some differences between gender. Full article

Fibroblast growth factor receptor 2 (FGFR2) has been associated with breast cancer. We performed in silico analyses to investigate the FGFR2 mRNA expression and splice variants associated with breast cancer subtypes. Online databases, including cBioPortal and TCGA SpliceSeq, were used to examine the association between the FGFR2 expression and splice variants with breast cancer subtypes. A higher FGFR2 mRNA was significantly associated with luminal, oestrogen receptor (ER)-positive breast cancers, and invasive lobular carcinomas, whereas a lower FGFR2 was associated with human epidermal growth factor receptor 2 (HER2)-positive breast cancer and invasive ductal carcinomas. The epithelial alternatively spliced FGFR2 IIIb isoform was significantly enriched in ER+ breast cancer, while the mesenchymal FGFR2 IIIc isoform was significantly prevalent in HER2+ cancer. Increased levels of FGFR2 and IIIb splice isoforms are associated with less aggressive breast cancer phenotypes, while decreased levels of FGFR2 and increased IIIc splice isoform are associated with more aggressive phenotypes. The design features single-mode fibre coupling of the free-space beam, assisted by a fast-steering, tip/tilt mirror that enables first-order turbulence suppression and fine target tracking. The total power throughput over the free-space link and the scintillation index in fibre are optimised. The optimal tip/tilt correction bandwidth and range, aperture size, and focal length for a given link are derived using analytical atmospheric turbulence modelling and numerical simulations. Wnt signaling is involved in embryo development and cancer. The binding between the DIX domains of Axin1/2, Dishevelled1/2/3, and Coiled-coil-DIX1 is essential for Wnt/β-catenin signaling. Structural and biological studies have revealed that DIX domains are polymerized through head-to-tail interface interactions, which are indispensable for activating β-catenin Wnt signaling. Although different isoforms of Dvl and Axin proteins display both redundant and specific functions in Wnt signaling, the specificity of DIX-mediated interactions remains unclear due to technical challenges. Using AlphaFold2(AF2), we predict the structures of 6 homodimers and 22 heterodimers of DIX domains without templates and compare them with the reported X-ray complex structures. PRODIGY is used to calculate the binding affinities of these DIX complexes. Our results show that the Axin2 DIX homodimer has a stronger binding affinity than the Axin1 DIX homodimer. Among Dishevelled (Dvl) proteins, the binding affinity of the Dvl1 DIX homodimer is stronger than that of Dvl2 and Dvl3. The Coiled-coil-DIX1(Ccd1) DIX homodimer shows weaker binding than the Axin1 DIX homodimer. Generally, heterodimer interactions tend to be stronger than those of homodimers. Our findings provide insights into the mechanism of the Wnt signaling pathway and highlight the potential of AF2 and PRODIGY for studying protein–protein interactions in signaling pathways. Coccoloba uvifera L. (seagrape) is a primarily dioecious neotropical tree species which often grows in the beach–forest transitional ecotone of coastal strand vegetation. We used five maternally inherited non-coding chloroplast regions to characterize the phytogeography of C. uvifera collected across the Caribbean Basin and Florida. Bayesian analysis revealed divergence between the Aruba–Trinidad–Tobago–Antigua–Jamaica island group and the continental Belize–Florida–US Virgin Islands (USVI) group at 1.78 million years before present (mybp), divergence between the Belize and Florida–USVI groups at 1.08 mybp, and a split of Antigua–Jamaica from Aruba–Trinidad–Tobago at 0.217 mybp. Haplotype network analysis supports the three clades, with the island group possessing the oldest haplotype. Based on geology and proximity, these clades correspond to South American (oldest), Central American, and North American (most recent). Coccoloba uvifera demographic expansion occurred during the Pleistocene epoch and peaked near the end of the last glacial maximum (ca. 0.026–0.019 mybp) when the global sea levels were 125 m lower than today. Our findings also reveal that tropical cyclones, which often impact coastal strand vegetation, did not affect genetic diversity. However, there was a positive association between latitude and the average number of substitutions, further enriching our understanding of the species’ phytogeography. Full article

Plant pathogenic nematodes play an important role in crop production and are difficult to control. One of them is Heterodera schachtii—a cyst nematode, pathogenic to sugar beet. Nature suggests a natural way to resolve this problem by using nematode-trapping fungi, one of which is Pleurotus ostreatus. It is one of the most widespread mushrooms in the world. It is a valuable and widely cultivated edible mushroom with nematocidal properties. The mycelium of this mushroom produces toxin droplets that paralyze nematodes, after which the nematodes become infected with the hyphae filament, resulting in their death. This feature can be put to practical use as a natural nematocidal agent. In this paper, we have described studies on the variability of the nematocidal properties in the progeny of three wild strains of P. ostreatus (Po1, Po2, Po4), obtained by crossing dikaryons with monokaryons (Buller phenomenon). The toxicity of mycelium was tested against the model organism Caenorhabditis elegans and against sugar beet pest—H. schachtii. The results of the study allowed the selection of the progeny heterokaryotic mycelia with the best parameters useful for in vitro control of both nematodes. They differed in the activity against C. elegans and H. schachtii, and in the toxic knobs production. The results indicated that the progeny of the Po1 strain presented a good ability to produce hyphal knobs in the presence of C. elegans, and progenies of the Po4 strain presented good quality of growth in preferred temperatures (10–25 °C). Three strains Po1 5dix27, Po2 20dix21, and Po4 2dix1, as well as a maternal strain Po4 controlled H. schachtii by entwining cysts better than other strains. These strains were moderately effective against C. elegans. Strains Po2 15dix17, Po4 1dix18, and Po4 1dix30 may be considered good controlling isolates against both tested organisms. The results of the research also clearly suggest that the killing properties of P. ostreatus mycelia should not be assessed only by their ability to produce toxic hyphal knobs. Their nematocidal properties also depend on other mechanisms developed by mycelia, which is observed as the lethality of nematodes. The results of this research will lead to a natural way to protect plants against nematodes. The research also proved the nematocidal properties of the wild strains to reduce the H. schachtii population in the soil. Full article

Hydrodynamic linear guides in machine tools offer a high load capacity and excellent damping characteristics, improving stability, precision, and vibration reduction. This study builds on previous research where floating heights were verified with a simulation model limited to measured floating heights. Advancements include incorporating pressure sensors into a fixed steel rail, enabling simultaneous measurement of oil film pressure and floating heights for a comprehensive understanding of lubrication conditions within the lubrication gap. The experimental results explore the effects of different lubrication methods, providing valuable insights into cavitation and lubrication adequacy. The results demonstrate the feasibility of utilizing pressure sensors to measure oil film pressure within the lubrication gap, providing a nuanced understanding of lubrication dynamics. By measuring both floating heights and pressure measurement, distinctions between hydrodynamic lubrication, mixed friction regions, and instances of lubricant deficiency become readily discernible. The variations in real-time oil film pressure and floating heights help to optimize the lubrication cycle for hydrodynamic linear guides, enhancing system performance and longevity. Full article
