Thyroid cancer (TC) is the most common endocrine malignancy, with a rising global incidence. In Ecuador, TC rates are among the highest worldwide. Generally, fine-needle aspiration (FNA) remains the standard diagnostic tool; however, due to its limitations, alternative or complementary approaches are required. In this context, liquid biopsy, particularly circulating tumor DNA (ctDNA), offers a promising, minimally invasive option for tumor genotyping. Objective: This study evaluated the concordance between genetic variants identified in ctDNA and tumor tissue. Thirty-six women with papillary thyroid cancer were included. Tumor tissue and blood samples were collected, and DNA was extracted. Next-Generation Sequencing (NGS) using the TruSight Tumor 15 panel identified genetic variants in both ctDNA and tumor DNA. Variant pathogenicity was assessed following ACMG guidelines. Genetic ancestry was determined using Ancestry Informative Markers (AIMs). A total of 71 cancer-associated variants were detected, with 81.69% concordance between tumor DNA and ctDNA. TP53 was the most frequently mutated gene. While most pathogenic variants were found in tumor tissue, some variants appeared exclusively in ctDNA samples on specific patients, suggesting tumor heterogeneity. Ancestry analysis revealed a predominant Native American component (62.4%). Liquid biopsy demonstrates high concordance with tumor tissue analysis and holds potential as a complementary diagnostic tool for thyroid cancer. However, challenges such as low ctDNA yield and underrepresentation in genetic databases highlight the need for improved protocols and increased inclusion of admixed populations in genomic studies. Background/Objectives: Prostate cancer (PCa) is diagnosed at an earlier median age, more advanced stage, and has worse clinical outcomes in African American (AA) men compared to European Americans (EA). Methods: To investigate the role of aberrant DNA methylation in tumor-adjacent stroma (TAS), methyl binding domain sequencing (MBD-seq) was performed on AA (n = 17) and EA (n = 15) PCa patients. This was independently confirmed using the long interspersed nuclear element-1 (LINE-1) assay. Pathway analysis was performed on statistically significantly differentially methylated genes for AA and EA TAS. DNA methylation profiles of primary cultured AA and EA carcinoma-associated fibroblasts (CAFs) were compared with AA and EA TAS. AA and EA CAFs were treated with demethylating agent 5-Azacytidine (5-AzaC). Results: AA TAS exhibited higher global DNA methylation than EA TAS (p-value < 0.001). Of the 3268 differentially methylated regions identified (DMRs, p-value < 0.05), 85% (2787 DMRs) showed increased DNA methylation in AA TAS, comprising 1648 genes, of which 1379 were protein-coding genes. Based on DNA methylation levels, two AA subgroups were identified. Notably, AA patients with higher DNA methylation were predominantly those with higher Gleason scores. Pathway analysis linked methylated genes in AA TAS to several key signaling pathways (p-value < 0.05), including immune response (e.g., IL-1, IL-15, IL-7, IL-8, IL-3, and chemokine), Wnt/β-catenin, androgen, PTEN, p53, TGF-β, and circadian clock regulation. A total of 168 concordantly methylated genes were identified, with 109 genes (65%) showing increased methylation in AA CAFs and TAS (p-value < 0.05). Treatment with 5-AzaC significantly reduced DNA methylation of concordant genes in AA CAFs (p-value < 0.001). Conclusions: These findings suggest a distinct stromal methylome in AA, providing a foundation for integrating demethylating agents into standard therapies. This approach targets the tumor microenvironment, potentially addressing PCa disparities in AA men. Full article

The white snook (Centropomus viridis) is an emerging aquaculture species with high market acceptance, exhibiting catadromous and protandric hermaphroditic characteristics in adulthood. This study aimed to preliminarily characterize certain hematological and biochemical parameters, as well as blood cell morphology, for identifying possible variations between sexes maintained under aquaculture recirculating system (RAS) conditions. The white snook broodstock was anesthetized with clove oil, and biometric values, as well as sex classification, were measured. Then, blood samples were collected from 14 females (7132 ± 1610 g) and 20 males (2200 ± 0.963 g) via caudal vessel puncture to analyze selected hematological parameters, blood biochemistry, and cellular morphology. Fulton’s condition factor (K) showed no differences between sexes, indicating a healthy fish status. Females showed significantly higher serum cholesterol, glucose, and triglyceride levels than males. Also, hematocrit (HCT) and mean corpuscular volume (MCV) were elevated in females. No sex-related differences were observed in red or white cell counts or in blood cell dimensions. Morphological characterization identified erythrocytes, thrombocytes, and three types of leukocytes: lymphocytes (small and large lymphocytes), neutrophils, and monocytes, with no eosinophils or basophils detected in either sex. These findings provide fundamental reference values for the hematological and biochemical profiles of C. viridis broodstock in captivity and highlight sex-specific differences relevant for reproductive and health monitoring. However, it should be considered that the sample size used to establish reference ranges for the species is small, so it is recommended to implement a monitoring plan for this and other broodstocks of this emerging species. Pediatric delirium (PD) is an acute neuropsychiatric syndrome marked by fluctuating disturbances in attention and cognition, frequently observed in pediatric intensive care units (PICUs) and associated with increased morbidity, mortality, and long-term cognitive impairment. Despite its clinical significance, PD remains underdiagnosed due to challenges inherent in assessing consciousness and cognition in children at varying developmental stages. Several bedside tools have been developed and validated in recent years, including the Cornell Assessment of Pediatric Delirium (CAPD), PreSchool Confusion Assessment Method for the Intensive Care Unit (psCAM-ICU); Pediatric Confusion Assessment Method for the Intensive Care Unit (pCAM-ICU), and Sophia Observation Withdrawal Symptoms—Pediatric Delirium Scale (SOS-PD), enhancing early recognition and management of PD in critically ill children. This narrative review explores the historical background, epidemiology, risk factors, pathophysiology, clinical subtypes, diagnostic tools, and current prevention and treatment strategies for PD from newborns to 21 years old. The screening tools available and the integration of non-pharmacological interventions, such as environmental modifications and family-centered care, as well as cautious and selective pharmacological management, are emphasized in this review. Early identification and targeted interventions are essential to mitigate the adverse outcomes associated with PD. Full article

The heritage of public works is composed of networks that are strongly linked to the territory where they are built. With the aim of deepening our knowledge of the appearance and subsequent development of the transport and supply systems in Madrid, we present a study of the main hydraulic works, bridges and railway stations. Based on historical and technological documentation, works and networks are analysed and georeferenced in order to relate their traces and evolution to the city. These built elements define recognisable physical and cultural traces in the form and identity of the city. The documentation and technological and social analysis work was completed with a dissemination and heritage education process. The results show that the physiographic and lithological reality of Madrid, characterised by its intense link with water, has determined the configuration of the urban network and the expansion of the city. Bridges span obstacles and set milestones. Stations are spaces for exchange and connection. The water supply network feeds the urban grid. The city changes, but the traces remain. Urban growth has smoothed, absorbed or hidden the original relief and watercourses, but they are still present in public works and even in the collective memory of the citizens through the force of their cultural and social values. Background: A significant proportion of post-myocardial infarction (MI) patients do not reach target low-density lipoprotein cholesterol (LDL-C) levels. Suboptimal LDL-C reduction is often attributed to poor adherence to pharmacological therapy and lifestyle recommendations. Methods: In a prospective registry of 179 post-MI patients who completed a Phase 2 Cardiac Rehabilitation Program (CRP), we evaluated the characteristics and predictors of suboptimal LDL-C reduction. Key indicators were assessed before and after CRP: adherence to the Mediterranean diet (using the PREDIMED questionnaire), weekly physical activity (via the IPAQ questionnaire), therapeutic adherence (using the Morisky–Green questionnaire), and peak oxygen consumption (VO2) on exercise testing. Lipid-lowering therapy (LLT) and LDL-C were recorded prior to MI and both before and after Phase 2 CRP. At the end of Phase 2, we analyzed the difference between measured and theoretical LDL-C (basal LDL-C minus expected LDL-C reduction by LLT), which was defined as “residual difference in LDL-C” (RD-LDL-C). We analyzed the predictors of positive RD-LDL-C (lower than theoretically expected). Results: After CRP, 54 (30.2%) patients exhibited positive RD-LDL-C. Within this subgroup, LLT was uptitrated, and patients received more potent LLT at the conclusion of CRP (theoretical potency: 69.81 ± 7.07 vs. 66.41 ± 7.48%, p = 0.005). However, they were less likely to reach the target LDL-C level <55 mg/dL (66.7% vs. 93.6%, p < 0.001). Male sex (HR 17.96 [2.15, 149.92], p = 0.008) and higher lipoprotein (a) levels (HR 1.02 [1.01, 1.03] per mg/dL, p = 0.001) were associated with a positive RD-LDL-C. Conversely, diabetes mellitus (HR 0.17 [0.06, 0.51], p = 0.002), higher corrected basal LDL-C levels (HR 0.98 [0.97, 0.99] per mg/dL, p = 0.001), and supervised in-hospital training during CRP (HR 0.28 [0.09, 0.86], p = 0.03) were associated with a reduced probability of positive RD-LDL-C. No association was found with adherence to the Mediterranean diet (88.1%), therapeutic adherence (89.1%), reported weekly physical activity (median 3545 [1980, 6132] metabolic equivalents per week), or change in peak VO2. Conclusions: More than one-third of post-MI patients demonstrated lower than expected LDL-C reduction (positive RD-LDL-C) following CRP, a finding that could not be attributed to poor adherence to pharmacological therapy or lifestyle recommendations. These findings suggest that a personalized approach to prescribing and uptitrating LLT may help achieve LDL-C targets, particularly in MI patients with healthy lifestyle habits who exhibit a lower response to LLT. Background/Objectives:H5N1 influenza viruses are spreading worldwide and threaten global public health. Preparedness is necessary to mitigate the worst-case scenario should an H5N1 influenza pandemic occur and justify the development of vaccines against circulating H5N1 viruses of concern. Methods: The production and characterization of egg-based split and inactivated H5Nx of three distinct monovalent antigens from clades 2.3.4.4b, 2.3.2.1c, and 2.3.4 were performed at an industrial scale. These antigens were formulated and their immune responses, when combined or not with IB160 squalene-based oil-in-water emulsion adjuvant in a rat model, were evaluated in a one- or two-dose immunization schedule. IgG antibodies, hemagglutination inhibitions, and microneutralization titers were measured for vaccine-induced immunity and cross-reactivity. Results: Three monovalent vaccines from clades 2.3.4.4b, 2.3.2.1c, and 2.3.4 were produced at an industrial scale and characterized. The immune responses against the monovalent vaccines showed a clade-specific antibody response and the need to combine with IB160 adjuvant for a required immune response. Conclusions: Considering the candidate vaccine viruses (CVVs) with the testing potency reagents available and that the antibody response obtained against the CVVs produced was clade-specific, IDCDC RG-71A is the indicated CVV for the predominant currently circulating H5N1 influenza virus of clade 2.3.4.4b and must be combined with adjuvant to induce a higher and efficacious immune response in a two-dose immunization protocol. Tilapia (Oreochromis niloticus) is one of the most important species in aquaculture, so the optimization of its culture by using sustainable strategies is fundamental. The aim of this study was to evaluate the effects of incorporating natural pigments (carrot and beetroot meal) as carbon sources in a biofloc system on the growth, immune response, and oxidative stress of O. niloticus. The experiment comprised four treatments: clear water as control (CT), biofloc with molasses (TBM), biofloc with carrot meal (TBC), and biofloc with beetroot meal (TBB). Results showed that biofloc helped to maintain optimal water quality and high survival rates, but specifically, TBC treatment achieved the highest growth rates and feed conversion ratio, along with elevated leukocyte counts, indicating improved health and immunity. Gene expression analysis revealed enhanced antioxidant activity (sod, gpx) and modulated immune responses (tnf-α, il-1β), particularly under thermal stress. Carrot meal emerged as an effective carbon source in biofloc systems, promoting growth, immune resilience, and oxidative stress resistance in tilapia, while beetroot meal improved pigmentation. These findings highlight the potential of natural pigments to enhance aquaculture sustainability and productivity. Full article

This paper presents a methodology for optimizing an aeronautical propeller to minimize power consumption. A multi-objective approach using blade element momentum (BEM) theory and evolutionary algorithms is employed to optimize propeller design by minimizing power consumption during takeoff and top-of-climb. Three different evolutionary algorithms generated a Pareto front, from which the optimal propeller design is selected. The selected propeller design is evaluated under optimal operational conditions for a specific mission. In this context, two operational approaches for the optimized propellers during flight missions are evaluated. The first approach considers the possibility of only three values for the propeller rotation, while the second allows continuous changes in the rotational speed and pitch angle values, known as the multi-rotational-speed approach. In the second approach, a modal analysis of the propeller is performed using rotating beam theory. The natural frequencies of vibration, constrained by the Campbell diagram, enable an operational analysis and ensure structural integrity by preventing resonance between propeller blades and the rotational procedures. The multi-rotational approach is conducted with and without frequency constraints, resulting in general flight energy reductions of 1.40% and 1.47%, respectively. However, substantial power savings are achieved, namely up to 10% during critical flight states, which can have a significant impact on future engine design and operability. The main contributions of the research lie in analyzing the multi-rotational approach with vibrational constraints of the optimized propeller. This research advances sustainable aviation practices by focusing on reducing power consumption while maintaining performance. Full article

Background: Health consciousness refers to an individual’s level of knowledge and concern regarding the impact of food on personal health; sensory appeal to the influence of attributes such as taste, aroma, appearance, and texture on food preference; and perception of front-of-package (FOP) labels refers to how the presentation of nutritional information on the package affects product choice. Given the increasing concerns about unhealthy food consumption among university students and the role of FOP labels in guiding food choices, it is essential to understand how these factors influence purchase intentions. Objective: This study was to examine the relationship between health consciousness, sensory appeal, and perception of FOP labels with purchase intentions for unhealthy foods in university students. Methods: A cross-sectional predictive study involved 361 students from public and private universities using a non-probability purpose-sampling approach. Data were collected through a previously validated questionnaire and analyzed using multiple linear regression. Results: The results revealed a significant positive association between sensory appeal and purchase intentions for unhealthy foods (β = 0.339; p < 0.001). In contrast, health consciousness (β = −0.296; p < 0.001) and perception of FOP labels (β = −0.237; p < 0.001) were inversely related to purchase intentions. Conclusion: These findings suggest that promoting health consciousness, improving perceptions of FOP labels, and addressing sensory appeal could effectively encourage healthier eating habits and prevent diet-related diseases among university students. Anaerobic digestion (AD) offers a sustainable solution by treating biodegradable waste while recovering bioenergy, enhancing the share of renewable energy. Thus, this study aims to investigate the AD for managing and valorizing residues from the potato chip industry: potato peel (PP), potato offcuts (OC), waste cooking oil (WCO), wastewater (WW), and sewage sludge (SS). In particular, the biochemical methane potential (BMP) of each residue, anaerobic co-digestion (AcoD), and greenhouse gas (GHG) emissions of an AD plant are assessed. WW, OC, and SS present a BMP of around 232–280 NmLCH4/kg of volatile solids (VS). PP and WCO reach a BMP slightly lower than the former substrates (174–202 NmLCH4/gVS). AcoD results in methane yields between 150 and 250 NmLCH4/gVS. An up-scaled anaerobic digester is designed to manage 1.60 Mg/d of PP. A residence time of 12 days and a digester with 165 m3 is estimated, yielding 14 Nm3CH4/MgVS/d. A simulated AD plant integrated with a combined heat and power unit results in a carbon footprint of 542 kg of CO2-eq/Mgdb PP, primarily from biogenic GHG emissions. These findings highlight the potential of AD to generate renewable energy from potato industry residues while reducing fossil fuel-related GHG emissions and promoting resource circularity. Universal accessibility in cultural heritage is a key challenge to ensure the inclusion of all visitors in historical and tourist environments. The aim of this research is to analyze the current state of research on universal accessibility to cultural heritage in Spain, identifying the key institutions, themes, and trends within this field. Additionally, it seeks to assess how these findings relate to national priorities and highlight gaps in the implementation of accessibility measures. This study employed the PRISMA 2020 framework to select and analyze 1035 scientific articles published between 2010 and 2025, retrieved from the Scopus and Web of Science databases. Bibliometrix (an R package) and Biblioshiny were used to analyze the data. The results show an exponential growth in publications since 2018, highlighting key areas such as accessible tourism, digitalization, the use of technologies for heritage documentation, urban planning, and geo-heritage conservation. It is concluded that accessibility in cultural heritage is an expanding interdisciplinary field with increasing international significance. It is recommended to continue exploring innovative technological solutions, strengthen governance strategies, and expand studies to rural and natural environments to ensure truly universal accessibility. The identification of filamentous fungi by matrix-assisted laser desorption/ionization–time of flight mass spectrometry (MALDI–TOF MS) represents a challenge due to their complex taxonomy and the lack of comprehensive databases. The aim of this study was to evaluate the current status of available MALDI–TOF MS databases for the identification of dermatophytes, including commercial, in-house, and web-based databases. We collected 289 dermatophyte strains from different centers and analyzed them using four databases and a combination of them. The combination of commercial and in-house databases was shown to improve the identification rate and accuracy at the species level. For Trichophyton rubrum, the concordance among all databases was above 90.0%. For the T. mentagrophytes group, correct identification at the species level ranged from 30.0 to 78.9%, depending on the database, and showed very low agreement among them. The addition of the novel species T. japonicum to our in-house database resulted in the successful identification of this species. On the other hand, T. interdigitale and T. tonsurans were the species most frequently misidentified by MALDI–TOF MS. Through deep spectra analysis of both species, up to 29 protein peaks were found to be suitable for their differentiation, demonstrating the potential of peak analysis in differentiating closely related species. In conclusion, improvements of the databases with new strains resulted in increased identification accuracy at the species level. This, combined with peak analysis, could improve the overall identification of dermatophytes by MALDI–TOF MS in clinical laboratories. Full article

Urinary actinomycosis is a rare condition, often mimicking a urinary tract tumor. Due to its low prevalence, it can be challenging to diagnose and may be mistaken for malignancies. A 33-year-old female patient with a history of type 2 Diabetes Mellitus and recurrent urinary tract infections presented to the emergency room with right renal fossa pain radiating to the right hypochondrium, fever with chills, nausea, and vomiting. Physical examination revealed a positive Giordano sign and tenderness at the ipsilateral middle and upper ureteral points. A contrast-enhanced CT scan showed a mass infiltrating the distal third of the right ureter, causing retrograde dilatation and hydronephrosis. Additionally, a liver injury with both liquid and solid components was observed. Therefore, given the suspicion of a urothelial tumor, a diagnostic cystoscopy and ureteroscopy were performed. Using interventional radiology, an abscessed liver lesion was drained, yielding purulent fluid. The histopathological examination revealed no evidence of malignancy. However, due to the strong suspicion of upper urinary tract urothelial carcinoma, a right radical nephroureterectomy with bladder cuff excision and retroperitoneal lymphadenectomy was performed. Histopathological examination ultimately confirmed urinary actinomycosis. Consequently, antibiotic therapy with oral amoxicillin 2 g every 12 h was initiated, leading to a good clinical response. Despite its low incidence, urinary actinomycosis should be considered as a differential diagnosis in cases suspected of urothelial tumors in the upper urinary tract. Increased awareness of this rare condition may help prevent unnecessary surgical interventions. Full article

Hemerocallis citrina is an herbaceous perennial plant used in Asian cuisine and Traditional Chinese Medicine. Here, we tested the therapeutic potential of extracts (HCE30%, HCE50%, and HCN) in vivo, using models of two human genetic neurodegenerative diseases—Machado–Joseph Disease/Spinocerebellar Ataxia type 3 (MJD/SCA3) and Frontotemporal Dementia with Parkinsonism associated to chromosome 17 (FTDP-17). Chronic treatment with HCE30% extract ameliorated the motor deficits typically observed in these models. Interestingly, we found that the effect on the motor phenotype of the MJD/SCA3 model was dependent on serotonergic signaling and on the action of the HLH-30/TFEB transcription factor, known to regulate the cellular response to amino acid starvation, the autophagy and mitophagy pathways, lysosome localization and biogenesis, exocytosis, and mitochondrial biogenesis. Altogether, our findings reinforce the idea that phytochemicals act through the modulation of serotonergic neurotransmission and introduce a novel layer to the HLH-30/TFEB regulatory network. Thus, it also strengthens the use of these pathways as therapeutic targets for protein-related neurodegenerative disorders and confirms the utility of medicinal plants as a source of innovation in the quest for new therapeutic agents.
