In this study, a new and straightforward process for the preparation of budesonide 21-phosphate (Bud-21P) and its disodium salt (Bud-21P-Na2) is described. The method results in a yield comparable to those obtained by diphosphoryl chloride, but it is more manageable, less expensive, and safer. The new compounds are characterized by better water solubility compared to the parent compound. Moreover, they have been evaluated for their anti-inflammatory activity and the obtained results clearly evidence that Bud-21P and Bud-21P-Na2 retained anti-inflammatory activity like the parent compound budesonide (Bud) in mice with cutaneous induced edema. Network traffic analysis, and specifically anomaly and attack detection, call for sophisticated tools relying on a large number of features. Mathematical modeling is extremely difficult, given the ample variety of traffic patterns and the subtle and varied ways that malicious activity can be carried out in a network. We address this problem by exploiting data-driven modeling and computational intelligence techniques. Sequences of packets captured on the communication medium are considered, along with multi-label metadata. Graph-based modeling of the data are introduced, thus resorting to the powerful GRALG approach based on feature information granulation, identification of a representative alphabet, embedding and genetic optimization. The obtained classifier is evaluated both under accuracy and complexity for two different supervised problems and compared with state-of-the-art algorithms. We show that the proposed preprocessing strategy is able to describe higher level relations between data instances in the input domain, thus allowing the algorithms to suitably reconstruct the structure of the input domain itself. Furthermore, the considered Granular Computing approach is able to extract knowledge on multiple semantic levels, thus effectively describing anomalies as subgraphs-based symbols of the whole network graph, in a specific time interval. Interesting performances can thus be achieved in identifying network traffic patterns, in spite of the complexity of the considered traffic classes. Full article

Background: Bronchial asthma is an inflammatory airway disease with an ever-increasing incidence. Therefore, innovative management strategies are urgently needed. MicroRNAs are small molecules that play a key role in lungs cellular functions and are involved in chronic inflammatory diseases, such as bronchial asthma. This study aims to compare microRNA serum expression between subjects with asthma, obesity, the most common co-morbidity in asthma, and healthy controls to obtain a specific expression profile specifically related to lung inflammation. Methods: We collected serum samples from a prospective cohort of 25 sex-matched subjects to determine circulating miRNAs through a quantitative RT-PCR. Moreover, we performed an in silico prediction of microRNA target genes linked to lung inflammation. Results: Asthmatic patients had a significant lower expression of hsa-miR-34a-5p, 181a-5p and 146a-5p compared to both obese and healthy ones suggesting microRNAs’ specific involvement in the regulation of lungs inflammatory response. Indeed, using in silico analysis, we identified microRNAs novel target genes as GATA family, linked to the inflammatory-related pathway. Conclusions: This study identifies a novel circulating miRNAs expression profile with promising potentials for asthma clinical evaluations and management. Further and larger investigations will be needed to confirm the potential role of microRNA as a clinical marker of bronchial asthma and eventually of pharmacological treatment response. Full article

In the Adriatic Sea (Mediterranean Sea), during monitoring of Production Water (PW) discharged from offshore platforms, high contamination levels of metals, PAH, and aliphatic hydrocarbons are detected in the sediment close to some installations. Here, we investigate for the first time, the transport and fate of the total suspended solids (TSS) associated with PW discharged by selected platforms, considering the separation of particulate matter from the water plume. We apply a 3D hydrodynamic model and a Lagrangian module to simulate the dispersion of PW suspended substances, then we relate the numerical results to the sediment contamination measured data. The TSS released with PW determines a negligible contribution along the water column and seems poorly related to the anomalies observed in the sediment contamination levels. This approach paves the way to assess the potential risks arising from TSS discharged with the PW on marine ecosystems and to optimize the environmental monitoring tools. Full article

S1P is the final product of sphingolipid metabolism, which interacts with five widely expressed GPCRs (S1P1-5). Increasing numbers of studies have indicated the importance of S1P3 in various pathophysiological processes. Recently, we have identified a pepducin (compound KRX-725-II) acting as an S1P3 receptor antagonist. Here, aiming to optimize the activity and selectivity profile of the described compound, we have synthesized a series of derivatives in which Tyr, in position 4, has been substituted with several natural aromatic and unnatural aromatic and non-aromatic amino acids. All the compounds were evaluated for their ability to inhibit vascular relaxation induced by KRX-725 (as S1P3 selective pepducin agonist) and KRX-722 (an S1P1-selective pepducin agonist). Those selective towards S1P3 (compounds V and VII) were also evaluated for their ability to inhibit skeletal muscle fibrosis. Finally, molecular dynamics simulations were performed to derive information on the preferred conformations of selective and unselective antagonists. Full article

European legislative framework supports a multidisciplinary strategy of environmental monitoring because the environment is a complex system of abiotic and biotic interactions, and it should not be studied and protected by looking at one single aspect. The resulting heterogeneous data request to be carefully processed, and the application of Weight of Evidence (WOE) approaches is, thereby, an integrated validated tool. In this perspective, the present study aims to: (i). apply a specific model (Sediqualsoft) based on the WOE approach for processing multidisciplinary data related to four Lines Of Scientific Evidence (LOEs: chemical analyses, ecotoxicological bioassays, bioaccumulation tests and biomarkers) regarding sediments from an area of the Adriatic Sea; (ii). evaluate the usefulness of this specific integrated approach to estimate the potential environmental hazard due to the presence of gas production platforms respect to the traditional approach of sediment chemical characterization. This latter recognized a more contaminated area within 100 m of the platforms in which the Sediqualsoft model showed the presence of a chemical hazard, ranging from moderate to severe, and identified the contaminants (e.g., some metals, benzo(a)pyrene and acenaphthene) most responsible for it. A significant hazard also appeared in some of the sampled stations by analyzing the LOEs dedicated to the biological responses. The choice of different reference values (regulatory limits, threshold values or concentrations measured in the control area) influenced only the chemical hazard but not the overall integration with other LOEs, showing a moderate hazard for the majority of stations. Here, the concentrations measured in a control area are firstly proposed as possible reference values in Sediqualsoft model applications; this could be of particular relevance when Sediment Quality Guidelines are not available for all the measured substances. Moreover, the limitations of a conventional pass-to-fail approach or worst-case scenario were overcoming interpreting whole chemical and ecotoxicological results. All data analyzed and discussed confirm Sediqualsoft as a suitable tool for processing environmental data, including those first processed here on a monitoring scenario of gas platforms that discharge Produced Water into the sea. The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has led to widespread use of hydroxychloroquine and azithromycin despite the lack of conclusive evidence for their safety and efficacy. We evaluated the association between treatment with hydroxychloroquine and/or azithromycin and hospital mortality as the primary outcome. We compared the hospital mortality of patients treated with hydroxychloroquine alone, azithromycin alone, or their combination to the mortality of patients who received neither drug. A logistic multivariate model with overlap weight propensity score was used for estimation of odds ratios (ORs) with 95% confidence intervals (95% CIs). One thousand four hundred and three patients with SARS-CoV-2 infection were admitted to the hospital. At the time of the analysis, the outcome was available for 1376 (98%) of them. Five hundred and eighty-seven patients (42%) received azithromycin and 377 patients (27%) received hydroxychloroquine, alone or in combination. In-hospital mortality was 26%. After the adjusted analysis, azithromycin alone was associated with lower mortality (OR 0.60, 95% CI 0.42–0.85) compared to no treatment. Hydroxychloroquine alone (OR 0.76, 95% CI 0.53–1.08) and the combination of azithromycin and hydroxychloroquine (OR 1.13, 95% CI 0.77–1.69) were not associated with hospital mortality. In this cohort of patients, azithromycin alone was associated with lower hospital mortality but hydroxychloroquine was not associated with increased or reduced mortality. While we await randomized clinical trials, these data support the use of azithromycin in novel coronavirus disease 2019 (COVID-19) and can contribute to better understanding of its role in further meta-analyses. The incidence of hepatocellular carcinoma deriving from metabolic dysfunctions has increased in the last years. Sirtuin- (SIRT-3), phospho-mammalian target of rapamycin (p-mTOR) and hypoxia-inducible factor- (HIF-1α) are involved in metabolism and cancer. However, their role in hepatocellular carcinoma (HCC) metabolism, drug resistance and progression remains unclear. This study aimed to better clarify the biological and clinical function of these markers in HCC patients, in relation to the presence of metabolic alterations, metformin therapy and clinical outcome. A total of 70 HCC patients were enrolled: 48 and 22 of whom were in early stage and advanced stage, respectively. The expression levels of the three markers were assessed by immunohistochemistry and summarized using descriptive statistics. SIRT-3 expression was higher in diabetic than non-diabetic patients, and in metformin-treated than insulin-treated patients. Interestingly, p-mTOR was higher in patients with metabolic syndrome than those with different etiology, and, similar to SIRT-3, in metformin-treated than insulin-treated patients. Moreover, our results describe a slight, albeit not significant, benefit of high SIRT-3 and a significant benefit of high nuclear HIF-1α expression in early-stage patients, whereas high levels of p-mTOR correlated with worse prognosis in advanced-stage patients. Our study highlighted the involvement of SIRT-3 and p-mTOR in metabolic dysfunctions that occur in HCC patients, and suggested SIRT-3 and HIF-1α as predictors of prognosis in early-stage HCC patients, and p-mTOR as target for the treatment of advanced-stage HCC. Neoplasms of the ovary present an increasing challenge to the physician. Neoplastic ovarian cysts can resemble endometriomas in ultrasound imaging and need to be carefully considered in the differential diagnosis. We report the case of a woman with a strong family history of hereditary breast and ovarian cancer, who presented with a pelvic mass. The young girl refused oncogenetic counseling and genetic testing, even though she had a 50% a priori probability of being a BRCA1 mutation carrier. Pelvic magnetic resonance imaging (MRI) and a comparative analysis of the serum concentration of HE-4 and CA125 biomarkers provided accuracy and sensitivity in the diagnosis of a benign ovarian pathology. Based on this experience, we propose that the sensitivity of a screening program based on a HE4 and CA125 assay and MRI in high risk patients with mutations in the BRCA1 and BRCA2 genes may be considered a useful pre-operative tool for the differential diagnosis of pelvic masses.
