In this report, we present the case of a talented anorectic patient whose artistic expression of his anorexia gave us an exceptional insight into the pain he experienced. Based on his artwork, we designed an illustrated visual analogue scale (VAS) that allowed us to assess the control of the disease over his mind and thus the clinical progress from a psychotherapeutic point of view. By comparing the progression of the VAS with the Body Mass Index (BMI), we were able to identify the tipping point at which the patient regained control and provide targeted support to maintain it, perceived as very helpful by the patient. A VAS assessing anorexia's control over the mind is a simple and readily available tool that factors in the psychiatric background of the disease. It could be a good complement to the BMI as a clinical outcome parameter and should be further explored. Full article

Introduction: In Switzerland, specialists in psychiatry and psychotherapy are highly needed for about one third of all current psychiatrists will retire over the next ten years. With this survey we wanted to investigate positive and negative motivational aspects for becoming a psychiatrist in residents in the canton of Bern. Methods: From January to February 2023, we conducted an online survey of psychiatric residents in the canton of Bern in Switzerland. The survey comprised questions on demographics and motivational aspects of applying for a psychiatric residency. Results: We contacted 150 psychiatric residents in the canton of Bern. 92 (61%) participated in the survey. The most important motivational aspects to apply for a psychiatric residency were interest for mental health, the physician-patient relationship and acquiring psychotherapy skills. The most demotivational aspects were poor salaries and the high administrative burden. Only 36% of the surveyed physicians aimed at working in private practice five years from now. Conclusions: Most surveyed residents liked psychotherapy and mental health aspects in being a psychiatrist. The authors conclude that for improving the attractiveness, a psychiatric residency programs led by the government should be initiated to intensify residential education by common efforts of private and institutional psychiatrists. Hospitalized patients can be difficult to meet with and understand while going through their suffering. Patients and their caregivers can find themselves locked into a kind of “non-meeting” during the hospital stay. In other cases, there may be a “hyper-meeting” in the hospital, with problematic attitudes and behavior or adverse reactions to hospital staff. Faced with such situations and others, where something is preventing the usual process of discussion within the team from working, a patient identification psychodrama has been set up for caregiver staffs. It is about having a place where the group can work together, help each other, and discuss the experiences they have had with a patient. A place that involves both the emotional and the physical; a place to look at the tensions between the team and the patient and to discuss them openly. It is also a place for integration and development. It utilizes the capacities of healthcare professionals, who as humans and as individuals possess the capacity to identify with suffering, to recognize pain and put it into words as if they were a spokesperson. Background: Only a few patients with strokes following a suicide attempt (SFSA) are described in the literature. The best-known mechanism is the dissection of cervical arteries. We aimed to determine the frequency, clinical presentation, mechanisms, and outcomes of SFSA patients in one academic institution and in all patients reported in the literature. Method: We retrospectively identified all SFSA in the Acute Stroke Registry and Analysis of Lausanne (ASTRAL) from 2003 to 2021. A thorough workup was performed to establish the stroke mechanism. We also searched for all published SFSA cases in the worldwide literature for further analysis of demographics, comorbidities, and medium-term outcome. Results: In our center, SFSA frequency was six out of 6,767 patients (0.0009%), with 83% being male. Comparing all reported 22 patients (own and published) to non-SFSA patients in ASTRAL, SFSA patients were younger (median 51 vs 75 years), had higher National Institutes of Health Stroke Scale scores on admission (median 13 vs 6) and higher three-months mortality (36 vs 15%). The most frequent mechanism was carotid dissection from hanging (41%), however, multiple other stroke mechanisms have been found or suspected. Conclusions: SFSA are rare and seem to affect younger patients presenting with more severe strokes and higher medium-term mortality. While carotid dissection emerged as the predominant cause, several other stroke mechanisms have been identified. Full article

Healthcare professionals take care of victims of excessive jealousy, but also deal with jealous people who suffer and want to change their behavior. Most of the time, patients do not come to us to talk specifically about jealousy, but the subject can be raised in appropriate contexts, for example following a violent incident (e.g., in the emergency department, traumatology, gynecology or pediatrics) during a consultation with a specialist on alcohol, psychiatry or geriatrics, at a family planning clinic, during a prenatal consultation or during couples therapy.
