Round 1

Reviewer 1 Report

line 221+222: I would advise to make a difference between the short and long term results. In long term results the evidence for benefit is not very clear (ref74, gerritsen 2002).

My compliments for your extensive, precise and very relevant work!

Author Response

Dear Reviewer,

Thank you! We have added a clarification as suggested, please see line 223.

Reviewer 2 Report

I think it is an excellent review work. It is well thought out and well structured. The reference bibliography is adequate and recent. I think, in short, that it is a good job. However, I would encourage the authors to consider a meta-analysis, not a review. On the other hand, I would ask them for some further contribution to the conclusions.

Author Response

Dear Reviewer 2,

Thank you! For this special issue, the invitation was to write a review, which is why our paper has this format. We have elaborated the conclusions as asked, please see lines 323-326.
