This study presents a case report of a phosphaturic mesenchymal tumor with osteomalacia. The manuscript is clear, relevant, and presented in a well-structured manner. The figures are appropriate and correctly show the diagnostic sequence of the case and the conclusion is consistent with the evidence and arguments presented. However, the references are more than 5 years old.

This study presents a case report of a phosphaturic mesenchymal tumor with osteomalacia. The manuscript is clear, relevant, and presented in a well-structured manner. The article is interesting and well prepared. I found only some minor flaws:

26, 130 - use localization instead of disease

64 - sentence is confusing (which radiological features?)

178 - should be diagnosis instead of prognosis

205,206 - level of venous FGF23 can't localise the tumour. Should the sentence be: "Recently, venous sampling of FGF23 has been proposed as a method for definitive confirmation of the tumor secreting FGF23"? 

I miss latest rewiev of PMT in the reference list (https://doi.org/10.1053/j.semdp.2019.07.002)

- In addition, a benign tumor of the jaws with radiological features which represent expanding while destroying the surrounding bone, including calcifying epithelial odontogenic tumor, calcifying odontogenic cyst, ossifying fibroma, central giant cell granuloma (CGCG) and brown tumor, could be considered.

Introduction: - expand the general information about the tumor and write down how many cases there have been in the last 20 years

Second paragraph should be Case report and subtitles: Clinical presentation, Differential Diagnosis and Diagnosis and management
