Instant messaging (IM) applications are ubiquitous, and while end-to-end encryption protects message content, traffic metadata remains observable. This paper proposes a traffic correlation framework for IM call services under a passive ISP-level threat model to infer communication parties from encrypted traffic. The framework extracts and matches metadata from sustained, bidirectional call flows and jointly analyzes endpoint identifiability, shared server connectivity, symmetry in call duration and traffic volume, and service type indicators to derive correlation artifacts for matching. The framework is instantiated and evaluated on WhatsApp, Facebook Messenger, and Snapchat across diverse user behavior scenarios and commonly deployed network settings. Experimental results show that the method reliably links caller and callee flows, revealing edges in users’ social graphs without decrypting any packets. Under typical data retention regimes, these findings indicate that metadata-based correlation provides a practical basis for deanonymization and represents a persistent privacy risk for users of IM calling. Background: Healthcare communication often relies on complex digital infrastructures, yet clinicians increasingly adopt general-purpose Instant Messaging Apps (IMAs) such as WhatsApp® to meet unmet needs. DonnaRosa, an Italian community of breast cancer specialists founded in 2017, is a Community of Practice (CoP), where experts exchange second opinions, guidelines, and trial opportunities. This paper examines its origins, practices, and implications. Methods: A mixed-methods design was applied: (1) qualitative analysis of chat logs to identify interaction patterns and rules; (2) a 2024 online survey of 54 members (92.5% response rate) exploring demographics, usage, and perceived value; (3) ongoing semi-structured interviews with founders and participants to reconstruct history, recruitment, and professional impact. Results: The group has grown through personal invitations, creating a friendly network of oncologists. Communication is concise, colloquial, and collegial. Activities focus on case discussions, reassurance, interpretation of guidelines, and exchange of research opportunities. This article presents data from an online survey conducted in 2024, showing that the group is widely used for second opinions, often consulted even on weekends and holidays, and perceived as a source of professional support and learning. Members report that participation frequently changes or refines their clinical judgement, especially when guidelines are incomplete or ambiguous. The community also promotes resilience, reduces professional isolation, supports informal collaboration in research projects, and encourages interaction on organisational and healthcare management issues. Conclusions:DonnaRosa illustrates how informal IMAs can evolve into robust infrastructures of care and professional solidarity, complementing formal systems. In the era of artificial intelligence, CoPs like DonnaRosa may become even more relevant: AI tools, especially large language models, can accelerate literature retrieval and data synthesis, while the CoP provides the critical, experience-based interpretation needed for safe and meaningful application. Such a dual infrastructure—technological and human—offers a promising path for oncology, where complexity requires both computational breadth and the depth of expert clinical judgement. Taken together, these findings and the evolving role of AI in clinical communication underscore the need for oncology societies to develop governance frameworks that ensure the safe, accountable, and clinically appropriate use of instant-messaging tools in professional practice. Amid growing threats of image data leakage and misuse, image encryption has become a critical safeguard for protecting visual information. However, many recent image encryption algorithms remain constrained by trade-offs between security, efficiency, and practicability. To address these challenges, this paper first proposes a novel two-dimensional variable fractional-order coupled quadratic hyperchaotic map (2D-VFCQHM), which incorporates a state-dependent dynamic memory effect, wherein the fractional-order is adaptively determined at each iteration by the mean of the system’s current state. This mechanism substantially enhances the complexity and unpredictability of the underlying chaotic dynamics. Building upon the superior hyperchaotic properties of the 2D-VFCQHM, we further develop a high-performance image encryption algorithm that integrates a novel fusion strategy within a dynamic vector-level diffusion-scrambling framework (IEA-VMFD). Comprehensive security analyses and experimental results demonstrate that the proposed algorithm achieves robust cryptographic performance, including a key space of 2298, inter-pixel correlation coefficients below 0.0018, ciphertext entropy greater than 7.999, and near-ideal plaintext sensitivity. Crucially, the algorithm attains an encryption speed of up to 126.2963 Mbps. The exceptional balance between security strength and computational efficiency underscores the practical viability of our algorithm, rendering it well-suited for modern applications such as telemedicine, instant messaging, and cloud computing. Adolescence is a developmental stage characterized by increased vulnerability to technology use. Several models have been proposed to explain the psychological processes involved in addictive use. In response to this evidence, therapeutic and preventive intervention programs aim to reduce key symptoms in order to promote health and protect adolescents. This study presents a systematic review and meta-analysis on the effectiveness of psychological therapeutic and preventive interventions for problematic use of Information and Communication Technologies (ICTs) in adolescents (aged 10–21). A total of nine studies (five RCTs and four non-RCTs) with 744 participants were analyzed. The search was conducted following PRISMA guidelines and using the PICO framework. Included studies involved face-to-face or online psychological therapeutic and preventive interventions targeting adolescents, with a particular focus on cognitive behavioral therapy (CBT). Results indicate significant effects in favor of the experimental group in reducing symptoms associated with Internet, video game, social media, and instant messaging addiction, with pooled effect sizes of SMD = −1.53 (RCTs) and SMD = −1.13 (non-RCTs). Despite heterogeneity and potential publication bias, the evidence supports the effectiveness of these interventions, particularly CBT, family therapy, and executive function training. A multidisciplinary approach, early detection, and treatment personalization are recommended. Methodological limitations were identified, highlighting the need for more rigorous future research with attention to gender differences and cultural adaptation. Full article

In an era where digitalization shapes economic and social landscapes, the intersection of digital skills, ethics, and integrity plays a crucial role in understanding the vulnerability of youth classified as NEET (Not in Education, Employment, or Training). This study explores how risky internet use and digital skill gaps contribute to socio-economic exclusion, integrating a multivariate and spatial approach to assess regional disparities in Europe. This study adopts a systems thinking perspective to explore digital exclusion as an emergent outcome of multiple interrelated subsystems. The research employs logistic regression, Principal Component Analysis (PCA) with Promax rotation, and Geographic Information Systems (GIS) to examine the impact of digital behaviors on NEET status. Using Eurostat data aggregated at the country level for the period (2000–2023) across 28 European countries, this study evaluates 24 digital indicators covering social media usage, instant messaging, daily internet access, data protection awareness, and digital literacy levels. The findings reveal that low digital skills significantly increase the likelihood of being NEET, while excessive social media and internet use show mixed effects depending on socio-economic context. A strong negative correlation between digital security practices and NEET status suggests that youths with a higher awareness of online risks are less prone to socio-economic exclusion. The GIS analysis highlights regional disparities, where countries with limited digital access and lower literacy levels exhibit higher NEET rates. Digital exclusion is not merely a technological issue but a multidimensional socio-economic challenge. To reduce the NEET rate, policies must focus on enhancing digital skills, fostering online security awareness, and addressing regional disparities. Integrating GIS methods allows for the identification of territorial clusters with heightened digital vulnerabilities, guiding targeted interventions for improving youth employability in the digital economy. This paper presents the design and implementation of an Internet-based instant messaging application that leverages post-quantum cryptographic algorithms founded on isogenies of elliptic curves. The system employs the CSIDH cryptosystem for key exchange and SeaSign for digital signatures, integrating these with the X3DH and Double-Ratchet protocols to enable end-to-end encryption for both text messages and binary file transfers. Key generation is supported for new users upon registration, ensuring robust cryptographic foundations from the outset. The performance of the CSIDH and SeaSign algorithms is evaluated at various security levels using a Python-based prototype, providing practical benchmarks. By combining isogeny-based cryptographic schemes with widely adopted secure messaging protocols, this work presents an illustration of a selected quantum-resistant communication solution and offers insights into the feasibility and practicality of deploying such protocols in real-world applications. Face-to-face interaction is one of the most natural forms of human communication. Unsurprisingly, Video Conferencing (VC) Applications have experienced a significant rise in demand over the past decade. With the widespread availability of cellular devices equipped with high-resolution cameras, Instant Messaging Video Call Applications (IMVCAs) now constitute a substantial portion of VC communications. Given the multitude of IMVCA options, maintaining a high Quality of Experience (QoE) is critical. While content providers can measure QoE directly through end-to-end connections, Internet Service Providers (ISPs) must infer QoE indirectly from network traffic—a non-trivial task, especially when most traffic is encrypted. In this paper, we analyze a large dataset collected from WhatsApp IMVCA, comprising over 25,000 s of VC sessions. We apply four Machine Learning (ML) algorithms and a Large Multimodal Model (LMM)-based agent, achieving mean errors of 4.61%, 5.36%, and 13.24% for three popular QoE metrics: BRISQUE, PIQE, and FPS, respectively. Ratchet protocols are a class of secure protocols based on ratcheting encryption mechanisms, widely employed in instant messaging. Against the backdrop of frequent incidents of communication privacy breaches, ratchet protocols have become a vital technology for ensuring secure end-to-end communication. This paper presents a formal analysis framework for ratchet protocols grounded in Logic of Events theory (LoET). We further extend LoET by introducing dedicated Diffie–Hellman (DH) and ratchet event classes, along with tailored axioms and inference rules, to support precise modeling of ratcheted encryption. Using the Signal protocol as a case study, we construct a bidirectional authentication model and rigorously prove that both its symmetric and asymmetric ratchet phases satisfy strong authentication properties. Compared with existing formal approaches, our method enables more expressive modeling of key update sequences and supports structured reasoning over causality and authentication flows. The proposed framework lays a theoretical foundation for analyzing the security of modern ratcheted protocols and holds potential for future automated verification. Full article

Objectives: We aimed to investigate how receiving integrated healthcare services from a case manager via instant messaging affected patients with disabilities. Methods: This database-matched case–control study was conducted at one medical center. Patients with officially certified disabilities were recruited and assigned to either the LINE-based group or the control group, which accessed services in the traditional manner. Their baseline characteristics were collected through chart reviews. Medical service utilization data—including their number of outpatient visits, prescribed medications, and hospitalizations—were obtained at baseline and 3, 6, and 12 months into the intervention. In the LINE group, quality of life, caregiver burden, and perceived social support were also assessed. A repeated-measures ANOVA was used to analyze within- and between-group differences over time. Results: Both the LINE group and the control group contained 66 patients. The number of outpatient visits (p < 0.001) and quantity of medication taken (p = 0.026) were significantly lower in the LINE group than in the control group. Furthermore, the caregiver burden in the LINE group (p = 0.024) was significantly lower 12 months after receiving integrated healthcare services. Conclusions: Providing integrated healthcare services via instant messaging enabled patients with disabilities to access medical services promptly and efficiently, thus enhancing the accessibility of healthcare and improving care for the disabled population. Background/Objectives: The outcomes of diabetes therapy depend largely on how well patients can implement medical advice in their lives. The main aim of the DiabPeerS study was to evaluate a peer support instant messaging service (IMS) approach to diabetes self-management education and support (DSMES) for people with type 2 diabetes mellitus (T2DM). Methods: Participants with T2DM took part in a randomized controlled trial. Both the intervention group (IG) and the control group (CG) received standard therapy, but the IG additionally participated in the peer support IMS intervention. The duration of the intervention was 7 months, succeeded by a follow-up 7 months later. Eleven biochemical, six behavioral, and six psychosocial parameters were measured at four times. Results: The targeted sample size could not be reached, and 68 participants took part. The following results have been found for the main hypotheses: No influence on HbA1c was detected (IG: −0.27, CG: +0.06, p > 0.05). Diabetes self-management behaviors were unaffected (IGdiet: +0.02, CGdiet: +0.46, p > 0.05; IGexercise: −0.72, CGexercise: +0.44, p > 0.05; IGbloodsugar: −0.21, CGbloodsugar: +0.65, p > 0.05; IGfootcare: +0.37, CGfootcare: +1.13, p > 0.05). Quality of life increased during the intervention in both the IG (KSK: +8.92, PSK: +7.41, p < 0.001) and the CG (KSK: +8.73, PSK: +7.48, p < 0.001). Medication adherence increased in the IG (+3.31, p < 0.01), although these participants were still classified as non-adherent. Conclusions: A peer support IMS intervention is a promising approach, but we recommend combining the online setting with an initial face-to-face situation. Full article

Building upon the job demands-resources model, this study examines how communication technology (CT) use in remote work relates to role clarity, coworker support, work overload, and, in turn, burnout to enhance sustainable worker and organizational workplace practices. By analyzing non-experimental survey data from 447 U.S. workers transitioning to remote work in 2020, we found that job demands/resources mediated the relationships between CTs and burnout, with each CT linked to specific demands/resources. Phone calls, email, and instant messaging were associated with role clarity and coworker support, mitigating burnout. Video calls were linked to higher work overload and increased burnout which can decrease worker and organizational sustainability. Our findings highlight the importance of CT use in relation to employee well-being. Supported by affordance theory, we found that each CT had unique associations with job resources and demands when CTs served as key communication channels during organizational transitions. Background: Digital technologies are increasingly being used to promote effective health interventions in the older adult population. This study aimed to develop and validate instant messages with educational content related to glycemic control, delivered via an instant messaging application, as a type 2 DM control strategy for older adults. Methods: This was a methodological study that developed instant messages containing text and images. The validation process was conducted by a panel of experts composed of nursing, nutrition, and physical education professionals. SPSS version 25.0 was used for the statistical analysis. A CVI was used to measure the experts’ agreement regarding the validity of the content of the educational messages. An exact test of binomial distribution with p > 0.05, indicating statistical significance, and a 0.95 proportion of agreement was used to estimate the statistical reliability of the CVI. Results: Sixty-one text messages were prepared with illustrations as support, and were divided into three themes. The educational messages were validated by experts, with an average CVI score above 0.80 for all indicators. Conclusions: The educational messages developed in this study were considered relevant and clear for older adults and could be applied in the digital environment, with the objective of helping older adults manage type 2 DM. The purpose of this paper is to present the creation, challenges and difficulties of four utility apps to increase production on Android handheld devices. The motivation of creating the apps and the problems encountered during creation are highlighted. The first app is a fast emoji app for WhatsApp. The motivation for creating such an app is that usually phone users do not like to type too much; with this app, they can communicate with each other just by sending emojis. The second app is a standalone emoji sender app, which does the same thing as the previous app but uses its own database, not the WhatsApp database. Full article

Background: Prostate cancer is one of the most common cancers among men worldwide. Management options include active surveillance, surgery, radiation, and chemotherapy, while self-management and behavioral interventions have shown promise in improving health-promoting behaviors and addressing barriers to care. Mobile health interventions, particularly instant messaging platforms, offer a growing opportunity for effective therapeutic support, but evidence on their role in self-management remains limited. The objective was to investigate the efficacy of self-management interventions based on instant messaging on quality of life and self-efficacy in patients diagnosed with prostate cancer through a systematic review and meta-analysis. Methods: A search was conducted of three databases from their inception to November 2024. Randomized controlled trials were included. Two reviewers performed independent data extraction and methodologic quality assessment of the studies. Results: A total of seven studies were included in the review. Instant messaging interventions were recognized by a previously published taxonomy of collaborative technologies. The meta-analysis showed that self-management interventions based on instant messaging have an effect on quality of life and self-efficacy. Conclusions: This systematic review highlights the potential benefits of self-management interventions incorporating instant messaging for improving quality of life and self-efficacy in prostate cancer patients. Female sex workers (FSWs) in South Africa have a high HIV prevalence. To reduce the rate of new infections, pre-exposure prophylaxis (PrEP) was introduced to FSWs; however, studies show that FSWs’ uptake and adherence to PrEP has been inconsistent. This study explored existing evidence related to strategies used to encourage PrEP uptake and adherence. Utilizing the scoping review framework, seven peer-reviewed articles were analyzed thematically. The themes were (1) PrEP promotion and distribution, (2) PrEP counselling and using educational resources, and (3) using instant messaging and rewards programs. The suggestions from the literature include a generalized PrEP promotion and distribution approach not aimed towards high-risk groups to avoid stigma. PrEP pick-up points should include sex work-friendly healthcare facilities as well as community-based venues. PrEP counselling should be conducted prior to PrEP administration to prepare users on how to deal with side effects. Information, education, and communication materials should be colorful and consist of catchy phrases targeted to the whole population. Instant messaging has been found to be effective in encouraging adherence. FSWs should be instrumental in informing intervention best practices. Future interventions aimed at FSWs should focus on holistic wellness that incorporates the psychosocial aspects of HIV prevention. Full article
