Self-Awareness in Survivors of an Acquired Brain Injury and Its Impact on Caregiver Burden

Background/Objectives: After an acquired brain injury (ABI), caregiver burden in family members is a clinical concern. Prior research has demonstrated that improved self-awareness in survivors of an ABI reduces caregiver burden. We examined the relationship between caregiver burden and ABI survivors’ levels of self-awareness across a span of injury chronicity following discharge from outpatient holistic milieu neurorehabilitation. Method: This retrospective observational study analyzed data on 59 individuals with heterogeneous ABIs who participated in an outpatient holistic milieu neurorehabilitation program from 2021 to 2025. This study utilized the discrepancy model of the Mayo-Portland Adaptability Inventory-4 (MPAI-4) to measure self-awareness in survivors of an ABI by calculating a discrepancy score from the self- and caregiver-rated MPAI-4 total score. Demographic information (age, education, race/ethnicity), injury history (injury type, age at injury, chronicity), program variables (length of program participation), functionality (MPAI-4), and caregiver burden (Zarit Burden Interview) at discharge were collected. Results: In order to predict caregiver burden based on self-awareness of an ABI survivor and time since injury, a multiple linear regression analysis was used. Although the multiple regression model significantly predicted caregiver burden, only self-awareness added significantly to the prediction and accounted for a modest proportion of the variance in caregiver burden. Conclusions: Self-awareness, as measured by utilizing the MPAI-4 discrepancy model, explained a modest proportion of the variance in caregiver burden regardless of time since injury. Among family members of survivors of an ABI, self-awareness of the survivor is a predictor of burden experienced by the family and would be beneficial to address as part of neurorehabilitation. Energy healing refers to a globalising market of healing rituals that aim to stimulate the clients’ self-healing process. In the context of healing, the term ‘energy’ has multi-layered meanings and may denote physical, psychological, social, as well as spiritual principles of restoring well-being. This article demonstrates how the idea of energy as a healing agent was embedded within the occult cosmology and anthropology of the British-American post-Theosophist Alice A. Bailey (1880–1949). Applying historical discourse analysis, Bailey’s impact on the emergence of energy healing is examined. Her theory of energy is explored against the backdrop of her esoteric grand narrative. It is demonstrated how Bailey’s principles of energy formed the basis for her vision of a “medicine of the future”. A concluding discussion proves the global influence of Bailey’s ‘energetic turn’ and how it modelled the approach of major holistic actors of the later twentieth century. Full article

The epichaperome, a dynamic and integrated network of chaperone proteins, extends its roles beyond basic protein folding to protein stabilization and intracellular signal transduction to orchestrating a multitude of cellular processes critical for tumor survival. In this review, we explore the multifaceted roles of the epichaperome, delving into its diverse cellular locations, factors that modulate its formation and function, its liquid–liquid phase separation, and the key signaling and crosstalk pathways it regulates, including cellular metabolism and intracellular signal transduction. We further highlight techniques for isolating and identifying epichaperome networks, pitfalls, and opportunities. Further, we review the profound implications of the epichaperome for cancer treatment and therapy design, underscoring the need for strategic engineering that hinges on a comprehensive insight into the comprehensive structure and workings of the epichaperome across the heterogeneous cell subpopulations in the tumor milieu. By presenting a holistic view of the epichaperome’s functions and mechanisms, we aim to underscore its potential as a key target for novel anti-cancer strategies, revealing that the epichaperome is not merely a piece of protein folding machinery but a mastermind that facilitates the malignant phenotype. Combination antiretroviral treatment (cART) has revolutionized the management of human immunodeficiency virus (HIV) and has markedly improved the disease burden and life expectancy of people living with HIV. HIV enters the central nervous system (CNS) early in the course of infection, establishes latency, and produces a pro-inflammatory milieu that may affect cognitive functions, even in the cART era. Whereas severe forms of neurocognitive impairment (NCI) such as HIV-associated dementia have declined over the last decades, milder forms have become more prevalent, are commonly multifactorial, and are associated with comorbidity burdens, mental health, cART neurotoxicity, and ageing. Since 2007, the Frascati criteria have been used to characterize and classify HIV-associated neurocognitive disorders (HAND) into three stages, namely asymptomatic neurocognitive impairment (ANI), mild neurocognitive disorder (MND), and HIV-associated dementia (HAD). These criteria are based on a comprehensive neuropsychological assessment that presupposes the availability of validated, demographically adjusted, and normative population data. Novel neuroimaging modalities and biomarkers have been proposed in order to complement NCI assessments, elucidate neuropathogenic mechanisms, and support HIV-associated NCI diagnosis, monitoring, and prognosis. By integrating neuropsychological assessments with biomarkers and neuroimaging into a holistic care approach, clinicians can enhance diagnostic accuracy, prognosis, and patient outcomes. This review interrogates the value of these modes of assessment and proposes a unified approach to NCI diagnosis. Full article

In the era of extensive data acquisition from manufacturing and transportation processes, the utilization of machine learning and deep learning techniques has emerged as a potent force for informed decision-making and optimized deliveries in contemporary urban landscapes. This study presents a novel approach grounded in deep learning, where product data are systematically gathered to construct a multilayer perceptron neural network model. This model proves instrumental in efficiently classifying product flows within the urban milieu. To validate its efficacy, machine learning classifiers are deployed, and their performance is juxtaposed with the neural network model. Addressing the critical question of the paper’s significance, our experimental evaluation unequivocally demonstrates the superior classification accuracy of the proposed multilayer perceptron model when compared to traditional machine learning models operating on the same product dataset. This advancement is not merely a theoretical achievement but translates into tangible improvements in last-mile delivery processes, marked by significant cost reduction and the mitigation of delays. The transformative potential of our approach is further underscored by the strategic application of a deep learning algorithm for optimization and illustrative purposes. This holistic methodology not only positions our work as a noteworthy contribution to the realm of product classification but also establishes a concrete pathway for enhancing the sustainability and efficiency of urban logistics. This paper, thus, goes beyond the conventional application of machine learning models, offering a paradigm shift in the intersection of deep learning, urban logistics, and sustainable development. In this article I present the main findings of an empirical study about contemporary spirituality in Italy begun in 2017 by reasoning about the analysis of twelve case studies which are particularly eloquent concerning the different spiritual worlds emerging in Catholic Italy. I argue that three main narratives—the spirituality of nature, wellbeing, and mystery—are useful to synthesize the heterogeneousness of groups, communities, festivals, and organizations engaged in the Italian “holistic milieu”. In order to address this reflection, firstly I will trace the international sociological debate that has accompanied the concept of contemporary spirituality and the relationship between spirituality and religion, a couple which I have named “frenemies”. Then, I will extend the analysis to the concept of the secular, examining the intertwining of the spiritual, religious, and secular spheres. After illustrating the landscape of contemporary spirituality in Catholic Italy more broadly, I shall focus on the case studies taken as examples of the spirituality of nature, the spirituality of health and wellbeing, and the spirituality of mystery. In the Discussion and Conclusion, I shall raise some fundamental questions that the study of contemporary spirituality poses for the sociology of religion with reference to secularisation, one of its most classic and yet contested paradigms. I shall claim that future research paths could further contribute to strengthening the idea, raised in this article, that secularisation can also be understood not only as an antithetical force to religion but as the process in Western history that has led to the emergence of a secular social space in dialogue with the religious sphere. This study delves into the pivotal role of shipper satisfaction in cultivating efficacious cooperation between dry ports and seaports. The research endeavors herein present a comprehensive indicator system tailored to evaluate the efficacy of the cooperation of the sea ports and dry ports. This system encompasses 14 key indicators, thoughtfully categorized into four dimensions, offering a holistic perspective on the multifaceted factors that underscore the synergy between dry ports and seaports. The establishment of a performance evaluation model for this collaborative nexus draws upon the support vector machine (SVM) technique, a choice substantiated by its suitability for the available dataset and its relevance within the research context. Leveraging the indicator data as a training dataset, the SVM approach culminates in the construction of a cooperative matrix, ultimately facilitating the discernment of performance rankings within dry port and seaport collaboration. This research methodology not only yields valuable insights into the evaluation of this distinct collaboration but also presents a versatile framework with potential application for addressing various challenges encountered within the cooperative milieu of dry ports and seaports. Alzheimer’s disease (AD) is a growing concern for the aging population worldwide. With no current cure or reliable treatments available for AD, prevention is an important and growing area of research. A range of lifestyle and dietary patterns have been studied to identify the most effective preventive lifestyle changes against AD and related dementia (ADRD) pathology. Of these, the most studied dietary patterns are the Mediterranean, DASH, MIND, ketogenic, and modified Mediterranean-ketogenic diets. However, there are discrepancies in the reported benefits among studies examining these dietary patterns. We herein compile a narrative/literature review of existing clinical evidence on the association of these patterns with ADRD symptomology and contemplate their preventive/ameliorative effects on ADRD neuropathology in various clinical milieus. By and large, plant-based dietary patterns have been found to be relatively consistently and positively correlated with preventing and reducing the odds of ADRD. These impacts stem not only from the direct impact of specific dietary components within these patterns on the brain but also from indirect effects through decreasing the deleterious effects of ADRD risk factors, such as diabetes, obesity, and cardiovascular diseases. Importantly, other psychosocial factors influence dietary intake, such as the social connection, which may directly influence diet and lifestyle, thereby also impacting ADRD risk. To this end, prospective research on ADRD should include a holistic approach, including psychosocial considerations. Full article

The gut microbiota features a unique diurnal rhythmicity which contributes to modulation of host physiology and homeostasis. The composition and activity of the microbiota and its secreted molecules influence the intestinal milieu and neighboring organs, such as the liver. Multiple immune-related molecules have been linked to the diurnal microbiota-host interaction, including Reg3γ, IgA, and MHCII, which are secreted or expressed on the gut surface and directly interact with intestinal bacteria. These molecules are also strongly influenced by dietary patterns, such as high-fat diet and time-restricted feeding, which are already known to modulate microbial rhythms and peripheral clocks. Herein, we use Reg3γ, IgA, and MHCII as test cases to highlight the divergent effects mediated by the diurnal activity of the gut microbiota and their downstream host effects. We further highlight current challenges and conflicts, remaining questions, and perspectives toward a holistic understanding of the microbiome’s impacts on circadian human behavior. Full article

With increasing interest in automatic and intelligent systems to enhance the building and construction industry, digital twins (DT) are gaining popularity as cost-effective solutions to meet stakeholder requirements. Comprising real-time multi-asset connectivity, simulation, and decision support functionalities, many recent studies have utilised Industry 4.0 technologies with DT systems to fulfil construction-specific applications. However, there is no comprehensive review to our knowledge, holistically examining the benefits of using DT as a platform from the angles of Industry 4.0 technologies, project management, and building lifecycle. To bridge this gap, a systematic literature review of 182 papers on DT-in-construction works over the past 6 years is conducted to address the three perspectives. In this review, a unified framework is first modelled to incorporate Industry 4.0 technologies within the DT structure. Next, a Six M methodology (comprising of Machine, Manpower, Material, Measurement, Milieu, and Method) based on Ishikawa’s Diagram with building lifecycle considerations is proposed to highlight the advantages of DT in ensuring successful construction projects. Lastly, through the identification of 11 future directions, this work aims to serve as a reference for both industry and academia towards the use of DT systems as a fundamental enabler to realise the Construction 4.0 paradigm. Among eco-spiritual activists in the French-speaking part of Switzerland, gendered notions such as “Mother Earth” or gendered “nature spirits” are ubiquitous. Drawing on an in-depth ethnographic study of this milieu (2015–2020), this article presents some of the ways in which these activists articulate gender issues with reference to nature. The authors discuss the centrality of the notion of the self and ask what outputs emerge from linking environmental with spiritual action. We demonstrate that activists in three milieus—the New Age and holistic milieu, the transition network, and neo-shamanism—handle this link differently and thereby give birth to a variety of emic perspectives upon the nature/culture divide, as well as upon gender—ranging from essentialist and organicist views to queer approaches. The authors also present more recent observations on the increasing visibility of women and feminists as key public speakers. They conclude with the importance of contextualizing imaginaries that circulate as universalistic and planetary and of relating them to individuals’ gendered selves and their social, political, and economic capital. Atherosclerotic cardiovascular disease remains the leading cause of morbidity and mortality worldwide. Atherosclerosis develops over several decades and is mediated by a complex interplay of cellular mechanisms that drive a chronic inflammatory milieu and cell-to-cell interactions between endothelial cells, smooth muscle cells and macrophages that promote plaque development and progression. While there has been significant therapeutic advancement, there remains a gap where novel therapeutic approaches can complement current therapies to provide a holistic approach for treating atherosclerosis to orchestrate the regulation of complex signalling networks across multiple cell types and different stages of disease progression. MicroRNAs (miRNAs) are emerging as important post-transcriptional regulators of a suite of molecular signalling pathways and pathophysiological cellular effects. Furthermore, circulating miRNAs have emerged as a new class of disease biomarkers to better inform clinical diagnosis and provide new avenues for personalised therapies. This review focusses on recent insights into the potential role of miRNAs both as therapeutic targets in the regulation of the most influential processes that govern atherosclerosis and as clinical biomarkers that may be reflective of disease severity, highlighting the potential theranostic (therapeutic and diagnostic) properties of miRNAs in the management of cardiovascular disease. Holistic spirituality has often been characterized by academic literature as belonging to the private sphere, articulated through the market and anchored in the growth of narcissistic individualism. However, recent empirical evidence and theoretical developments suggest a more complex picture. Drawing on the analysis and comparison of two empirical cases—the organization of collective meditations in public spaces and the teaching of yoga in prisons by holistic volunteers —we explore the rise of social engagement initiatives, aiming to transform society through the promotion and use of holistic techniques. Our main conclusions revolve around four main issues (a) the move of holistic spirituality from the private to the public sphere and the increasing public resonance with (and acceptance of) the contemporary holistic milieu, (b) the emergence of an holistic imaginary of social change anchored in ethics of reciprocity and responsibility, (c) the role of the body as a central locus of resistance and social transformation and (d) the articulation of new forms of individualism that enable to make self-realization compatible with social and political commitment. Amplification of oxidative stress is present since the early stages of chronic kidney disease (CKD), holding a key position in the pathogenesis of renal failure. Induction of renal pro-oxidant enzymes with excess generation of reactive oxygen species (ROS) and accumulation of dityrosine-containing protein products produced during oxidative stress (advanced oxidation protein products—AOPPs) have been directly linked to podocyte damage, proteinuria, and the development of focal segmental glomerulosclerosis (FSGS) as well as tubulointerstitial fibrosis. Vascular oxidative stress is considered to play a critical role in CKD progression, and ROS are potential mediators of the impaired myogenic responses of afferent renal arterioles in CKD and impaired renal autoregulation. Both oxidative stress and inflammation are CKD hallmarks. Oxidative stress promotes inflammation via formation of proinflammatory oxidized lipids or AOPPs, whereas activation of nuclear factor κB transcription factor in the pro-oxidant milieu promotes the expression of proinflammatory cytokines and recruitment of proinflammatory cells. Accumulating evidence implicates oxidative stress in various clinical models of CKD, including diabetic nephropathy, IgA nephropathy, polycystic kidney disease as well as the cardiorenal syndrome. The scope of this review is to tackle the issue of oxidative stress in CKD in a holistic manner so as to provide a future framework for potential interventions. Full article

Many modern forms of yoga can be located in the holistic milieu. Discourses of health and wellbeing for mind, body and soul are central in the holistic milieu. Ideas about food and diet are frequently significant aspects of this therapeutic discourse. This paper focuses on ideas about food and diet in the Art of Living Foundation (AOL), a modern transnational yoga movement. AOL legitimises its beliefs about food through an appeal to concepts found in traditional texts on yoga and āyurveda. In particular, the concept of sattva, which can be translated as balance or harmony—both significant tropes in the holistic milieu—is central to discourses about food choices in AOL and other writers in the holistic milieu.
