This study investigates how the timing of self-employment relative to first childbirth shapes long-term entrepreneurial outcomes among Israeli mothers. Drawing on rich administrative panel data from the Israeli National Insurance Institute (N = 73,141 woman-years), we follow a cohort of women who gave birth for the first time in 2010, tracking their employment trajectories over 15 years (2005–2019). Using random-effect logistic regressions, OLS models, and fixed subgroup analyses, this study compares women who entered self-employment before childbirth with those who did so afterward. The results reveal that postnatal entrants are more likely to operate smaller businesses and exit self-employment earlier, yet often earn higher income from wage employment, compared to their prenatal counterparts. By tracing these outcomes over time, this study demonstrates how key life events, such as childbirth, structure women’s employment paths and contribute to differentiated patterns of labor market participation. Situated in a context of near-universal motherhood and limited public support for working parents, the findings offer insight into the dynamic links between family formation, employment timing, and entrepreneurial sustainability. By adopting a life-course perspective, this study demonstrates how the sequencing of family and employment transitions intersect to shape access to economic resources and entrepreneurial sustainability. Food-Derived Compounds Extend the Shelf Life of Frozen Human Milk

Breastmilk is known to provide optimal nutrition for infant growth and development. A cross-sectional analysis of nationally representative US data from 2016 to 2021 revealed that >90% of lactating mothers reported using breast pumps to express milk. We conducted a survey of n = 1049 lactating or recently lactating individuals from a US nationally representative population to explore breastmilk storage practices among this group. The data revealed that 83% of respondents store breastmilk in their homes, with 68% using freezers to do so for >1 month. The lowest available temperature in most household freezers is −20 °C, a temperature that is inadequate to maintain human milk’s emulsified structure, leading to separation, degradation of fats, loss of key vitamins, and changes in palatability. We developed a first-of-its-kind high-throughput screening platform to identify food-derived compounds and combinations of compounds that, when added to human breastmilk, preserve fat content, retain antioxidant capacity, and reduce production of rancid-associated free fatty acids during extended freezer storage. Our screening identified pectin (0.5% w/v) and ascorbic acid (100 μg/mL) as optimal preservation agents. Compared to untreated controls, this formulation reduced glycerol production by approximately 60% and maintained antioxidant capacity after 6 months of storage at −20 °C. Lysozyme and protease activity were maintained at >75% of the levels in fresh breastmilk. This formulation represents a lead for the development of safe and affordable frozen breastmilk shelf-life extenders for at-home use to increase the longevity of stored breastmilk. Full article

Background: Recently, vitamin D deficiency (VDD) has been described as a pandemic, affecting all groups of the population. Pregnant women and preterm infants are particularly vulnerable to vitamin D deficiency. Objectives: We aimed to evaluate the maternal and neonatal vitamin D status in relation with maternal vitamin D supplementations during pregnancy and to identify demographic, social, and healthcare risk factors for maternal VDD and vitamin D insufficiency in women delivering at ≤32 weeks of gestation. Methods: This prospective observational study was developed in the regional level III maternity unit of the Clinical County Emergency Hospital Sibiu. It included all admitted mothers who delivered at ≤32 weeks of gestation and their infants between 1 March 2022 and 28 February 2025. Infant deaths in the first 24 h of life, major congenital defects, chromosomal abnormalities, the admission of outborn infants without their mothers, or the transfer of the mother more than 48 h after birth were used as exclusion criteria. Maternal and neonatal data were collected from medical records. Data on maternal vitamin D supplementation were collected through interviews. Univariate and multivariate logistic regressions, linear regression, and predictive models were performed for data analysis. Results: A total of 146 mothers (median (IQR) age 30 (24–35) years) and their 164 preterm infants born at ≤32 weeks of gestation (median gestational age of 30 (27–31) weeks and birth weight of 1200 (900–1527) g) were included in this study. Only 43.15% of the mothers used multivitamins containing vitamin D during pregnancy, and 10.96% used specific vitamin D supplements. Vitamin D supplementation was used for a median of 4 (3–5) months at a median dose of 800 (250–1500) IU/day. Severe VDD (25(OH)D < 10 ng/mL), VDD (25(OH)D < 20 ng/mL), VDI (25(OH)D 20–29 ng/mL) were found in 19.86%, 55.48%, and 23.97% of the mothers and 16.46%, 58.53%, and 25.61% of their infants, respectively. A significant correlation was found between the maternal and neonatal status (r = 0.684, r2 = 0.468, p < 0.001, B = 0.62). Both the maternal and neonatal vitamin D status were correlated with the vitamin D duration and dose used for supplementation during pregnancy. The logistic regression analysis showed that birth during a cold season and increased parity are independently associated with severe maternal VDD, while birth during the cold season and a lower educational status were independently associated with maternal VDD. Only an absent vitamin D supplementation (in the form of a multivitamin or specific vitamin D supplements) has been proven as an independent risk factor for VDI. Conclusions: Our findings revealed a worrisome prevalence of severe VDD, VDD, and VDI in mothers delivering very prematurely and in their infants. Additionally, less than half of the mothers in this study used vitamin D supplements during pregnancy despite the national recommendations. The professionals involved in advising pregnant women and policymakers should find solutions to improve the vitamin D status in these vulnerable groups of the population. Full article

Background: The role of vitamin D deficiency (VDD) in both mothers and neonates has been suggested as a possible factor in the development of allergic conditions in early infancy, however limited research has investigated this link in relation to allergic rhinitis (AR). This study investigates whether VDD in the mother–newborn dyad is associated with the onset of AR in neonates within the first three days after birth. The aim is to contribute to the understanding of neonatal allergic outcomes related to vitamin D status, which may inform future preventive strategies. This study investigates the role of vitamin D in the early onset of allergic rhinitis (AR) in neonates, specifically within the first three days of life. Although AR typically develops after years of allergen exposure and is rare in children under two, we aimed to explore its occurrence at this early stage. While no clear link was found between vitamin D and early AR onset, further research is needed to explore vitamin D levels at later ages and over longer time frames to clarify the relationship. Methods: A cross-sectional study was conducted between September 2019 and January 2022 in a single hospital. The study involved 248 infants born at ≥37 weeks of gestation and their mothers, who were of Greek nationality. The study included mother–infant pairs who met the inclusion criteria. Chi-square tests were applied to analyze the association between maternal or neonatal VDD and the presence of AR in neonates within the first three days after birth. In addition, multiple regression analysis was used to control other potential factors contributing to AR. Results: The results showed an unclear relationship between VDD and the onset of AR in neonates within the first three days of life. Although several factors were analyzed, the effect of VDD on the development of AR remained unclear. Conclusions: The findings highlight the lack of clarity regarding the effect of maternal and neonatal VDD on the incidence of AR in the immediate neonatal period. Few studies to date have specifically examined the role of VDD in neonatal AR. Further research with larger sample sizes is needed to verify these associations and to guide potential interventions aimed at reducing allergic outcomes in neonates. Full article

Background/Objectives: Habits such as a diet high in sugars and poor dental biofilm control are linked to a higher prevalence of caries and low socioeconomic status. This study aimed to analyze the nutrition habits of schoolchildren in Mallorca and their relationship with the presence of dental caries, depending on the type of school, geographic location, and parents’ education level. Methods: A cross-sectional study was conducted to examine the prevalence of dental caries based on World Health Organization (WHO) standards and nutritional practices following guidelines from the Food and Agriculture Organization of the United Nations (FAO) and the European Food Safety Authority (EFSA). This study included 718 students from three age groups: first-year elementary students (ages 5–6), sixth-year elementary students (age 12), and fourth-year secondary school students (age 15). Relevant sociodemographic factors were also considered in the analysis. Results: In schoolchildren aged 5–6 years, higher monthly consumption of processed and sugary foods, such as sweets (rural: 24.66 (CI 95%: 20.30–29.02); urban: 19.29 (CI 95%:16.27–22.304); p = 0.044), was noted in schoolchildren from rural sectors compared to those residing in urban areas. At 15 years of age, there was a higher consumption of potato chips in public schools than in subsidized/private schools (public: 26.95 (CI 95%: 24.42–29.49); subsidized/private: 18.29 (CI 95%: 13.92–22.65) p = 0.004). A high consumption of sweets is associated with an increased risk of caries (OR sweets: 1.76 CI: 1.04–2.98; p = 0.035). Fewer students with mothers with a lower education level eat dinner (elementary: 75%; secondary 91%; higher: 98%; p = 0.003). Conclusions: Higher consumption of sweets in rural areas and potato chips in public schools, along with the association between sweet consumption and caries risk, highlight how geographic location, school type, and parents’ education level influence children’s nutrition habits and caries. Vaccination Status and Influencing Factors of Delayed Vaccination in Toddlers Born to Hepatitis B Surface Antigen-Positive Mothers

Background: This study aims to analyze the vaccination status and factors influencing delayed vaccination among toddlers born to hepatitis B surface antigen (HBsAg)-positive mothers. Methods: Data of HBsAg-positive mothers between 1 January 2021 and 31 December 2022 were provided by the Suzhou Maternal and Child Health Care and Family Planning Service Center. The vaccination records were obtained from the Jiangsu Province Immunization Service Management Information System. Logistic regression analysis was used to analyze influencing factors of delayed vaccination. Results: A total of 4250 toddlers born to HBsAg-positive mothers were documented. The data revealed that the first dose of the hepatitis B vaccine was administered to 100% of the toddlers. In addition, the coverage of the National Immunization Program (NIP) vaccines among these toddlers ranged from 92.9% to 99.4%. The proportion of delayed NIP vaccination varied between 0% and 12.2%. The proportion of delayed Bacillus Calmette–Guérin (BCG) vaccination was 11.3%, with the delay predominantly observed between 4 and 6 months. Notably, the proportion of delayed BCG vaccination among the toddlers born to HBsAg-positive mothers was significantly higher than that in the general population. Additionally, the proportion of the first dose of non-NIP vaccines was 3.3–36.4%, and the proportion of DTaP-IPV/Hib was 27.0%. Logistic regression analysis revealed that the regional level, the mother’s human papillomavirus (HPV) vaccination status, and the infant’s birth weight were significant factors influencing the timeliness of vaccination. Conclusions: Although the vaccination status of toddlers born to HBsAg-positive mothers in Suzhou city remains stable, the issue of delayed vaccination requires attention. It is essential to continue strengthening targeted vaccine education to reduce vaccine hesitancy and improve the rate of timely vaccination. Full article

Kreol Morisien is a vibrant and expressive language that reflects the multicultural heritage of Mauritius. There are different versions of Kreol languages. While Kreol Morisien is spoken in Mauritius, Kreol Rodrige is spoken only in Rodrigues, and they are distinct languages. Being spoken by only about 1.5 million speakers in the world, Kreol Morisien falls in the category of under-resourced languages. Initially, Kreol Morisien lacked a formalised writing system, with many people using different spellings for the same words. The first step towards standardisation of writing Kreol Morisien was after the publication of the Kreol Morisien orthography in 2011 and Kreol Morisien grammar in 2012 by the Kreol Morisien Academy. Kreol Morisien obtained a national position in the year 2012 when it was introduced in educational organisations. This was a major breakthrough for Kreol Morisien to be recognised as a national language on the same level as English, French, and other oriental languages. By providing a means for Kreol Morisien speakers to connect with others, a translation system will help to preserve and strengthen the identity of the language and its speakers in an increasingly globalized world. The aim of this paper is to develop a translation system for Kreol Morisien and English. Thus, a dataset consisting of 50,000 parallel Kreol Morisien and English sentences was created, where 48,000 sentence pairs were used to train the models, while 1000 sentences were used for evaluation and another 1000 sentences were used for testing. Several machine translation systems such as statistical machine translation, open-source neural machine translation, a Transformer model with attention mechanism, and Marian machine translation are trained and evaluated. Our best model, using MarianMT, achieved a BLEU score of 0.62 for the translation of English to Kreol Morisien and a BLEU score of 0.58 for the translation of Kreol Morisien into English. To our knowledge, these are the highest BLEU scores that are available in the literature for this language pair. A high-quality translation tool for Kreol Morisien will facilitate its integration into digital platforms. This will make previously inaccessible knowledge more accessible, as the information can now be translated into the mother tongue of most Mauritians with reasonable accuracy. Full article

Aim: To understand the factors that influence family violence towards First Nations women, to inform practises and policies to support these women and improve their engagement in maternal child health services. Design: A qualitative study, using narrative inquiry integrated with the Indigenous philosophy ‘Dadirri’, and thematic analysis of the data. Materials and Methods: Survey of 10 Maternal Child Health nurses in 2019, and interviews of 35 Aboriginal mothers in 2021. Results: The nurses identified drugs, alcohol, socio-economic issues, the history of effects of colonisation on First Nations peoples, and stress as perceived factors influencing family violence, and acceptance, fear, cultural beliefs, and mistrust, for women’s low reporting of violence. Factors that influenced nurses’ ability to identify family violence were mistrust and understanding of Aboriginal culture. Low self-esteem, lack of belonging, and not being heard were identified by the mothers as factors that influence family violence. Fear of child protective services, shame, mistrust, and poor rapport with the nurses contributed to their low reporting of violence. The most significant factor for the mothers to disclose violence is fear of losing her child, mistrust, and the questioning process. Conclusions: Nurses’ understanding of Indigenous culture is critical to develop trust and improve the engagement of First Nations women. A significant difference in the synthesis of data between the nurses and their First Nations consumers was conspicuous. Research regarding the benefit of models and interventions that recognise the social determinants of health and well-being on health outcomes as well as the value of culturally strong health services aimed to encourage an earlier identification of risk, ideally from the antenatal period to the child’s fifth birthday, is imperative. The implications of this research are of international importance for First Nations families and challenge current nursing practises to address the human rights challenge of the inequity in health outcomes between First Nation and non-First Nation children, their exposure to family violence, and their over-representation in child protection services. Background and Objectives: Global research has shown that 60–90% of school children have caries, and that oral health care is of great public health significance. We present the data of an oral health promotion Program conducted in the Primorje-Gorski Kotar County (PGC), Croatia, from 2008 to 2019. The Program includes comprehensive preventive oral status assessments of first- and fifth-grade elementary school students, as well as oral health promotional and preventative activities for preschool children, pregnant women, and new mothers. Here, we aimed to analyze the Program data and determine its applicability and sustainability. Materials and Methods: We assessed the changes in caries prevalence in first- and fifth-grade PGC students by comparing the 2008–2019 dental registry data on decayed, missing, and filled teeth for primary teeth (dmft)/decayed, missing, and filled teeth for permanent teeth (DMFT) index means. We also analyzed the data from the administrative Program reports. Results: We analyzed the dental registry data of 44,422 children in the PGC (21,714 first and 22,708 fifth grade). The average Program response rate was 83%. We noted a dmft/DMFT index decrease from 4.66 to 3.73 (first graders) and from 2.50 to 1.00 (fifth graders). The 2017–2019 dmft was significantly smaller than that of 2008–2009. There were 2336 workshops conducted in kindergartens, 1240 in first grades, and 1015 in fifth grades; health visitors educated 26,559 women. There was an increasing trend in the number of insured people under the age of six using pediatric oral health care. Conclusions: The Program improved the behavior and oral health of children and included various stakeholders, avoided additional financial expenses, increased the number of children in care, and proved its necessity and sustainability. It has been recognized on a national level and has led to the creation of two oral health care programs. Full article

Introduction: Bethlem myopathy is a rare genetic disease caused by a variant mapped to 21q22, which harbors the collagen type VI alpha 2 chain (COL6A2) and collagen type VI alpha 1 chain (COL6A1) genes, and 2q37, which harbors the collagen type VI alpha 3 chain (COL6A3) gene. Disease onset can occur at any age, and the symptoms are related to those of muscular dystrophy. Since Bethlem myopathy is a rare disease, no previous studies have been conducted in Arab countries, including Saudi Arabia. Its variable presentation of nonspecific muscular contractions and severity represents a diagnostic dilemma. Case presentation: Here, we report a Saudi pediatric patient, who is 9 years old (proband), brought to the pediatric clinic of King Saud’s Hospital by his mother. The boy presented with difficulty standing, walking, and running with his classmates and unaffected siblings. He has a younger sibling, aged 6 years old, who reported having a limping gait and difficulty bending his right knee. Laboratory results for the proband were unremarkable except for a slight increase in creatine kinase (CK). Whole-exome sequencing (WES) was performed for five family members, including the proband and his symptomatic brother, their mother and two asymptomatic siblings. A very rare 3′ splice site acceptor intronic variant, NM_001849.4: c.1817-3C>G, located three nucleotides before exon 25, was identified in COL6A2. Bioinformatics tools (SpliceAI, dbscSNV, FATHMM-MKL, and MaxEntScan) predicted this variant as pathogenic. The proband and his 6-year-old sibling presented a homozygous genotype for the variant, whereas the mother and one asymptomatic sibling were heterozygous, and the other sibling carried homozygous wild-type alleles. Conclusions: This is the first study to report a case of Bethlem myopathy confirmed by WES in Saudi Arabia and all Arab nations. The identified variant is rare, and its segregation pattern suggests autosomal recessive inheritance. The segregation pattern and bioinformatics tool results may qualify this variant to be annotated as pathogenic, addressing the reported uncertainty of its classification. Our findings contribute to linking and filling the knowledge gap of diagnosing and managing patients with collagen VI-related myopathies, providing greater clinical and genetic understanding to the existing knowledge. Background/Objectives: In regions of Africa with a high prevalence of malaria, pregnant women in their first or second trimester should be administered intermittent preventive treatment in pregnancy (IPTp). However, infants may contract malaria despite the IPTp therapy that their mothers have received. The objective of the present investigation was to assess the symptoms and various treatments for neonatal malaria. Methods: Entropy weight and TOPSIS were used to achieve the study goal. The TOPSIS multi-attribute decision-making system was used to assess newborn malaria symptoms and select the optimal treatment, even for mothers receiving IPTp medication during pregnancy. The entropy weight approach calculated TOPSIS attribute weights. The present research used UNICEF data for 14 African nations in 2023. Results: The results indicated that neonates whose mothers received IPTp therapy ultimately contracted malaria, with diarrhea being the primary symptom. It is important to note that health providers administer a combination of zinc and oral rehydration solution (ORS) to infants as the most effective treatment for malaria symptoms, thereby abandoning the first-line treatment for malaria, artemisinin-based combination therapy (ACT). Conclusions: The most effective treatment for neonatal malaria is a combination of zinc and ORS, although less than half of children in Africa have access to ORS. Therefore, the findings of this study may encourage African countries to prioritize co-pack therapy in their procurement and supply, healthcare provider training, and expenditures. This therapy will also help alleviate the symptoms of malaria in neonates. Full article

Chlamydia trachomatis (C. trachomatis) screening and treatment in pregnancy allows the opportunity to reduce adverse pregnancy and neonatal outcomes worldwide. Although C. trachomatis infection is easily treated and cured with antibiotics, only some countries have routine pregnancy screening and treatment programs. We therefore evaluated whether just one maternal screening for C. trachomatis is enough to prevent adverse pregnancy and negative neonatal outcomes. Among the 4087 first-time gynecological–obstetric consultations granted at the National Institute of Perinatology in 2018, we selected the study population according to a case–cohort design. Antenatal C. trachomatis screening and treatment interventions were performed on 628 pregnant women using COBAS® TaqMan CT. C. trachomatis DNA was also detected in samples from 157 infants of these mothers. In the maternal cohort, incidence of C. trachomatis infection was 10.5%. The vertical transmission rate was 1.5% for the cohort of mothers who tested positive for C. trachomatis and received treatment, and 29.7% for those with a negative test. By evaluating symptomatic neonatal infection, the hazard rate of perinatal pneumonia was 3.6 times higher in C. trachomatis-positive babies than in C. trachomatis-negative babies. Despite the low rate of mother-to-child transmission in women positive for C. trachomatis, possible maternal infection that is not detected in pregnancy significantly increases the risk of neonatal infection with consequent perinatal pneumonia. Taking the debate about cultural appropriation as a starting point, this article explores the relationship different members of the South Asian Australian diaspora have to the Australian multicultural project. Specifically, this article employs an archive of interviews from 2018 with second-generation South Asian Australian women and their (first-generation) mothers and/or grandmothers and explores how they feel about the cultural appropriation of South Asian cultural artefacts (hereafter, ‘Indo chic’). These interviews revealed that first-generation respondents were generally uncritical of Indo chic and perceived non-South Asian Australians consuming South Asian cultural artefacts as a sign of positive cross-cultural exchange that is emblematic of Australian multiculturalism. The second-generation respondents, however, felt threatened by Indo chic and felt appropriation was a racist microaggression that served to reiterate their racial difference (and inferiority) in a white settler society. The generational difference in sentiment reveals a productive fissure within migrant Australian communities to interrogate our feelings about being and feeling ‘Australian’. This article argues that conversations and debates about ‘Indo chic’ within South Asian Australian diasporas reveal the contours of what it is like to be a South Asian woman in contemporary multicultural Australia while also revealing flaws in the Australian multicultural project. In this article, I employ an affective analysis of their responses, drawing on Lauren Berlant’s idea of ‘cruel optimism’ and Sara Ahmed’s conceptions of love and the nation to fully explore the complicated somatics ‘Indo chic’ debates reveal for my respondents. Fetal and neonatal thyrotoxicosis occurs in up to 5% of pregnancies in mothers with Graves’ disease (GD). This condition is caused by stimulating antibodies against the thyrotropin receptor (TRAbs) that cross the placenta and may stimulate the fetal thyroid, typically in the second half of pregnancy. GD is often treated with radioiodine, resulting in hypothyroidism in most patients, but TRAbs can persist for several years. Even if a pregnant mother is hypothyroid after radioiodine therapy or surgery, her TRAbs can still, although rarely, induce fetal hyperthyroidism. In this review, we first present two cases of neonatal hyperthyroidism in mothers with GD who became hypothyroid after prior radioiodine therapy, identified through a 10-year analysis of the National Perinatal System in Slovenia. Based on these cases, we provide an overview of existing data on this rare clinical condition in neonates. We also discuss the underlying mechanisms and clinical outcomes based on currently available data. In conclusion, our review highlights the importance of careful monitoring during pregnancy in all women with GD, even in those well managed after radioiodine therapy or surgery. Full article

Perinatal cohort studies with a prospective longitudinal design are critical for determining the effects of early-life exposures on offspring’s health outcomes. The Sri Lanka Mother and Newborn Growth cohort study aims to investigate the impact of maternal nutritional and psychosocial factors on newborns’ birth weight in the Sri Lankan context. This paper presents the methodology of participant recruitment, follow-ups, an overview of measurements, and planned data analyses. This study included a nationally representative sample of Sri Lankan pregnant women recruited in their first trimester of pregnancy. Follow-up assessments were conducted once during the second and third trimesters of pregnancy and after the baby’s birth, prospectively tracking the women’s dietary intake, mental health, hemoglobin concentrations, and gestational weight gain data. Once the participants delivered their babies, the data on gestational age, sex of the newborn, birth weight, length and occipitofrontal circumference at birth, and mode of delivery were collected. Between August 2022 and August 2023, we recruited 2000 first-trimester pregnant women to the cohort and continued to follow up with them until the baby’s birth. The response rates were 90.4%, 81.4%, and 75.2% in the first, second, and third follow-ups. We plan to analyze the data in July 2024. We expect this study to provide valuable insights into various early-life exposures affecting neonatal birth weight. The study’s findings will serve as a valuable information resource for a broader scientific community, enabling the development of effective policies to prevent low-birth-weight deliveries in low-resource settings.
