Background and Objectives: Femoral malunion, defined as healing of a femoral fracture in an anatomically incorrect position, can lead to significant biomechanical and functional impairment despite modern fixation techniques achieving union rates near 99%. The lack of a universal definition and standardized management approach continues to hinder optimal outcomes. This review aims to synthesize the literature on the causes, clinical presentation, radiologic assessment, surgical indications, corrective procedures, and outcomes of femoral malunion to guide clinical decision-making and future research. Materials and Methods: A narrative review of peer-reviewed orthopedic literature was conducted, focusing on adult femoral malunions across anatomical regions. Articles detailing deformity thresholds, imaging modalities, corrective osteotomies, and fixation strategies were included. Particular emphasis was placed on region-specific deformities—femoral head, neck, intertrochanteric, diaphyseal, and distal femur—and their corresponding surgical correction methods, including valgus intertrochanteric osteotomy, clamshell osteotomy, and lengthening with external or magnetic intramedullary devices. Results: Malunion most commonly presents as angular, rotational, or length deformity, with thresholds of >5–10° angulation, >10° rotation, or >1–2 cm shortening being clinically significant. Patients may experience pain, limp, gait asymmetry, and early-onset arthritis. Corrective techniques tailored to the anatomical site yield favorable results: valgus intertrochanteric osteotomy restores leg length and alignment; diaphyseal malunions respond well to single- or multi-plane osteotomies with internal fixation or gradual correction; distal femoral malunions often require multiplanar osteotomy to reestablish the joint line. Most series report high union rates and functional improvement, though complications such as infection and hardware failure may occur. Conclusions: Femoral malunion remains a complex but treatable condition. Successful outcomes rely on accurate deformity characterization, patient-specific surgical planning, and restoration of mechanical alignment. Standardized deformity criteria and long-term functional outcome studies are needed to refine management algorithms and improve patient care. Full article

Background: Femoral neck fractures are rare but serious injuries in children and adolescents, often resulting from high-energy trauma and prone to complications like avascular necrosis (AVN) and nonunion. Even rarer is the development of slipped capital femoral epiphysis (SCFE) following femoral neck fracture, which presents unique diagnostic and treatment challenges. SCFE can destabilize the femoral head, with severe cases requiring complex surgical interventions. Case presentation: This report details a case of a 15-year-old male with autism spectrum disorder (ASD) who developed severe SCFE one month after treatment for a Delbet type III femoral neck fracture. The condition was managed with an Imhäuser intertrochanteric osteotomy (ITO), in situ fixation (ISF), and osteochondroplasty (OChP), supported by virtual surgical planning (VSP) and 3D-printed patient-specific instruments (PSIs) for precise correction and fixation. Discussion: The surgery was completed without complications. Six months after the operation, the patient exhibited a pain-free, mobile hip with radiographic evidence of fracture healing and no signs of AVN. Functional outcomes were favorable despite rehabilitation challenges due to ASD. Conclusions: The Imhäuser ITO, combined with ISF and OChP, effectively addressed severe SCFE after femoral neck fracture, minimizing AVN risk. VSP and PSIs enhanced surgical accuracy and efficiency, demonstrating their value in treating rare and complex pediatric orthopedic conditions. Full article

Epineural neurorrhaphy is a standard nerve repair method, but it is rarely reported in veterinary literature. Epineural neurorrhaphy in canine sciatic nerve injury are described in this report. An 11-month-old, castrated male Maltese dog, presented with an one-month history of non-weight bearing lameness and knuckling of the right pelvic limb. The dog showed absence of superficial and deep pain perception on the dorsal and lateral surfaces below the stifle joint. The dog had undergone femoral head and neck osteotomy in the right pelvic limb one month prior to referral at a local hospital. Based on physical and neurological examinations, peripheral nerve injury of the right pelvic limb was suspected. Radiography showed irregular bony proliferation around the excised femoral neck. Magnetic resonance imaging revealed sciatic nerve injury with inconspicuous continuity at the greater trochanter level. A sciatic nerve neurotmesis was suspected and surgical repair was decided. During surgery, non-viable tissue of the sciatic nerve was debrided, and epineural neurorrhaphy was performed to bridge a large, 20-mm defect. The superficial and deep pain perception was progressively improved and restored at 3 weeks postoperatively, and the dog exhibited a gradual improvement in motor function. At 10 weeks postoperatively, the dog showed no neurological deficit including knuckling but the tarsal joint hyperextension did not improve due to ankylosis. The dog had undergone tarsal arthrodesis and exhibited almost normal limb function without any neurologic sequela until the last follow-up at 2.5 years postoperatively. Full article

Traumatic hip dislocation might lead to serious complications and a poor outcome. Fortunately, it is a rare condition in pediatric patients. The purpose of this study is to establish and describe the complications caused by hip dislocations associated with transphyseal femoral neck fractures. Therefore, we conducted a literature review that resulted in 11 articles, including 32 patients, older than 10 years of age, suffering from traumatic hip dislocation associated with a transphyseal femoral neck fracture. We presented a case series of three patients with hip fracture-dislocation treated in our clinic that were also evaluated and included in the study. For the 35 patients included in the study group, the percentage of avascular osteonecrosis after hip fracture-dislocation was 88.57%. Traumatic hip dislocation associated with transphyseal femoral neck fracture is a rare condition and has a poor prognosis because of the high incidence of femoral head avascular necrosis (AVN). Reduction should be attempted within six hours the from injury, but this may not minimize the risk of AVN if transphyseal separation occurs. The approach may influence the development of AVN; lateral approach of the hip with great trochanter osteotomy seems to have the lowest number of cases of AVN. Full article
