Dual-energy computed tomography (DECT) is a promising advancement for detecting bone edema-like lesions (BME). However, its application in horses remains limited. The aim of this study was to evaluate DECT virtual-non-calcium (VNCa) imaging in the equine foot and establish which confounding factors could influence its applicability in clinical practice. The DECT VNCa map of 14 standing and 5 cadaveric (recumbent) cases with foot-related lameness was scored in consensus by two readers in comparison to MRI. Overall, 17/19 cases demonstrated BME on MRI, whereas 2 did not. Agreement between DECT VNCa and MRI was found in 15/19 feet (78.9%). Disagreement in 4/19 cases with BME was due to sclerosis (1/19), mild BME extent on MRI (2/19), or scan artifacts (1/19). The extent of BME was significantly underestimated using DECT VNCa compared to MRI (p = 0.016). No significant correlation was found between sclerosis score and the BME extent underestimation on DECT (p = 0.056). Between standing and post-mortem cases, there was no significant difference in the agreement between DECT and MRI (p = 0.53) or DECT VNCa image quality (p = 0.22). In conclusion, DECT VNCa effectively identified moderate and severe BME, and its use was feasible in standing positioning. In case of sclerosis, a case-by-case assessment is recommended. Full article

(1) Background: Tendon and ligament injuries are a leading cause of lameness in horses, with significant economic implications. Platelet-rich plasma (PRP) has gained attention for its regenerative potential, but its efficacy remains uncertain due to inconsistent study designs and reporting. (2) Methods: This systematic review, following the PRISMA guidelines, evaluated 22 studies (clinical and experimental) to assess the safety and efficacy of PRP in treating equine tendon and ligament injuries. The risk of bias was analyzed using the ROBINS-I and RoB 2.0 tools. (3) Results: PRP demonstrated a favorable safety profile, with no severe adverse effects reported. Clinical outcomes included improved lameness scores, ultrasonographic tissue organization, and return-to-work rates. However, variability in PRP formulations (e.g., leukocyte-rich vs. leukocyte-reduced) and activation methods (e.g., calcium chloride, thrombin) contributed to inconsistent results. Experimental studies supported PRP’s role in collagen synthesis and neovascularization, but comparative trials with stem cells or other therapies (e.g., extracorporeal shockwave) showed mixed results. The methodological quality of studies varied, with only 27% achieving “good” scores for PRP reporting. (4) Conclusions: PRP is a safe and potentially effective treatment, but its clinical application is hindered by a lack of standardization. Future research should focus on large, randomized controlled trials with uniform PRP protocols, long-term (≥2 years) efficacy assessments, comparative studies with MSC combinations, and cost-effectiveness analyses. Lameness detection in horses is a critical challenge in equine veterinary practice, particularly when symptoms are mild. This study aimed to develop a predictive system using a support vector machine (SVM) to identify the affected limb in horses trotting in a straight line. The system analyzed data from inertial measurement units (IMUs) placed on the horse’s head, withers, and pelvis, using variables such as vertical displacement and retraction angles. A total of 287 horses were included, with 256 showing single-limb lameness and 31 classified as sound. The model achieved an overall accuracy of 86%, with the highest success rates in identifying right and left forelimb lameness. However, there were challenges in identifying sound horses, with a 54.8% accuracy rate, and misclassification between forelimb and hindlimb lameness occurred in some cases. The study highlighted the importance of specific variables, such as vertical head and withers displacement, for accurate classification. Future research should focus on refining the model, exploring deep learning methods, and reducing the number of sensors required, with the goal of integrating these systems into equestrian equipment for early detection of locomotor issues. Synovitis is present before and during osteoarthritis in horses and can result in performance-limiting lameness. Twenty-four horses with lameness localized to the metacarpo-/metatarsophalangeal joint or a single joint of the carpus were enrolled in this study. We evaluated the response of intra-articular injection with 10 million activated (aMSC) or non-activated (naMSC) allogeneic equine umbilical cord blood-derived mesenchymal stromal cells (MSCs). Subjective and objective lameness was assessed on Days 0, 1, 21, and 42. The treatment injection was randomly assigned and performed following the baseline assessment on Day 0. naMSC-treated horses had straight-line lameness scores that were significantly lower on Day 21 (1.0 ± 1.15) and Day 42 (1.13 ± 1.00) than on Day 0 (p = 0.0098 and p = 0.0418, respectively). aMSC-treated horses had straight-line lameness scores that were significantly lower on Day 21 (0.96 ± 1.03) and Day 42 (0.79 ± 1.05) than on Day 0 (p = 0.0011 and p < 0.0001, respectively). There was no significant difference between the treatment groups for any parameter at any timepoint. In conclusion, both aMSC and naMSC allogeneic MSCs resulted in significantly improved subjective lameness scores in horses when compared to baseline lameness scores. Full article

Equine-assisted services (EAS) is a relatively new area of study within higher education requiring the development of a curriculum that best prepares the undergraduate student for the demands of the industry. Further, coursework targeting the specific management challenges for ensuring the welfare of horses utilized within EAS is limited. Therefore, the objective of this study was to apply an industry-based survey assessment utilizing EAS professionals within the United States to document management practices associated with healthcare and pain management challenges in the EAS horse. Suggested curriculum for undergraduate students targeting a career in EAS is given, utilizing data collected from the survey. Participant responses (n = 30) indicated that all of the surveyed facilities cared for more than three geriatric horses within their program, with the majority (60% responding “definitely yes”) utilizing regularly functionally lame horses, although the prevalence of lameness was not correlated with number of geriatric horses within a program (0.20, p = 0.22). While lameness is a common healthcare challenge within EAS, gastrointestinal conditions (73%) were one of the other leading health concerns for these facilities. The most common equine pain detection method for EAS facilities was behavior assessment (93%), in which this method was positively correlated with years of horse experience (0.51, p = 0.01). Horse pain was primarily managed through annual alternative therapeutic interventions (76%), daily prescriptions (73%), and nutritional supplements (73%). As such, the curriculum for undergraduate EAS degree programs should cover healthcare practices targeting geriatric horse management and associated health issues such as gastrointestinal conditions and lameness, including early detection and pharmaceutical, nutritional, and alternative therapeutic treatments. Scintigraphy and Positron Emission Tomography (PET) are both nuclear medicine imaging techniques, providing functional information of the imaged areas. Scintigraphy is a two-dimensional projected imaging technique that was introduced in equine imaging in the late 1970s. Scintigraphy allows imaging of large body parts and can cover multiple areas, remaining the only technique commonly used in horses for whole body imaging. PET is a cross-sectional imaging technique, first used in horses in 2015, allowing higher resolution three-dimensional functional imaging of the equine distal limb. This manuscript will cover current use and values of these two modalities in equine lameness diagnosis. Full article

The Ridden Horse Pain Ethogram (RHpE) was developed to facilitate the identification of musculoskeletal pain. The aim of the current study was to collate behavioural data using the RHpE from horses at competitions assumed by their owners and/or riders to be fit for competition. The objectives were to quantify the frequency of occurrence of behaviours in pain-free horses and those with lameness or abnormalities of canter and to determine any differences between disciplines and levels of competition. The RHpE was applied to 1358 horses competing in Grand Prix (GP) dressage (n = 211), 5* three-day events (TDE) (n = 137), or low-level one-day events (ODE) (n = 1010). The median RHpE score for all horses was 4 (interquartile range [IQR] 2, 5; range 0, 12) and the median lameness grade was 0 (IQR 0, 1; range 0, 4). The Kruskal–Wallis test, followed by Dunn’s test for pairwise comparisons, found a difference in median RHpE scores between low-level ODE and GP dressage (p = 0.001), but not between 5* TDE and low-level ODE (p = 0.09) or between GP dressage and 5* TDE (p = 1.00). The median RHpE score was highest for low-level ODE. The Chi-square/Fisher’s exact test identified a significant difference in prevalence of most of the 24 behaviours of the RHpE in non-lame compared with lame horses. Recognition of the behaviours of non-lame horses may improve equine welfare and performance, and rider comfort, confidence, and safety. Full article

This literature review analyzes the historical development of polo, its organizational structure, the course of the game, as well as the breeding, rearing, and training practices of polo horses. Frequently occurring ailments, such as musculoskeletal injuries, respiratory diseases, and internal illnesses, are highlighted. Lameness is a major problem, with injuries to the superficial digital flexor tendon being the most common cause. Other notable diseases include exercise-induced pulmonary hemorrhage (EIPH), myositis, rhabdomyolysis, and equine infectious anemia (EIA). To ensure the welfare of polo horses, effective prevention and management strategies are crucial. These include proper training, the adaptation of the ground surface, appropriate shoeing, and compliance with animal welfare guidelines and association rules. Collaboration between associations, players, organizers, and veterinarians is crucial. Promoting responsible management practices and raising awareness among stakeholders can help ensure that polo continues to thrive while maintaining high animal welfare standards. Full article

The use of therapeutic ultrasounds (TUs) is widespread in both human and veterinary medicine. In fact, mechanical vibration is the simplest and purest form of vibratory energy that is applied either in physical therapies or in rehabilitation medicine. In particular, the use of low-frequency TUs to treat equine conditions is a new and evolving field. In the equine industry, osteoarthritis (OA) is one of the most challenging causes of lameness. Despite its prevalence and the advancements in its treatment, there is still no therapy whose results are completely decisive. Little is described in the literature about the use of TUs in horses’ joints, particularly regarding its use to treat OA. For these reasons, the aim of this study was to preliminarily assess the efficacy of low-frequency ultrasound in two horses with metacarpo/metatarso-phalangeal joint OA. The reduction in lameness was significant in both treated cases, pointing to the effective therapeutic action of TUs. However, to better evaluate the long-term effects in athlete horses, it is necessary to include in the research a greater number of cases and a control group. Full article

Osteoarthritis is a leading cause of lameness and joint disease in horses. A simple, economical, and accurate diagnostic test is required for routine screening for OA. This study aimed to evaluate infrared (IR)-based synovial fluid biomarker profiling to detect early changes associated with a traumatically induced model of equine carpal osteoarthritis (OA). Unilateral carpal OA was induced arthroscopically in 9 of 17 healthy thoroughbred fillies; the remainder served as Sham-operated controls. The median age of both groups was 2 years. Synovial fluid (SF) was obtained before surgical induction of OA (Day 0) and weekly until Day 63. IR absorbance spectra were acquired from dried SF films. Following spectral pre-processing, predictive models using random forests were used to differentiate OA, Sham, and Control samples. The accuracy for distinguishing between OA and any other joint group was 80%. The classification accuracy by sampling day was 87%. For paired classification tasks, the accuracies by joint were 75% for OA vs. OA Control and 70% for OA vs. Sham. The accuracy for separating horses by group (OA vs. Sham) was 68%. In conclusion, SF IR spectroscopy accurately discriminates traumatically induced OA joints from controls. Full article

Working equids are often used to exhaustion, living and dying in conditions below minimal welfare standards. Due to their poor welfare status, euthanasia should be considered in certain conditions. The study aimed to describe the population and the disease frequency of the working equids admitted at an equine clinic in Cairo (i.e., Egypt Equine Aid (EEA)) from 2019 to 2022 and identify possible associations between physical parameters at admission and the outcome. Records of 1360 equids admitted at EEA were reviewed. The majority of the admitted equids were horses (65.6%), followed by donkeys (33%), in particular stallions (68.7%), from 1 to 15 years old (74.8%). Hospitalisation was mainly due to wounds (28.9%), orthopaedic problems (27.4%), colic (8.5%), or infectious diseases (7.4%). The majority of the equids were discharged, but 5.1% died on their own, without human intervention, and 23% were euthanised. Text mining revealed the anamnesis’s most frequent words were ‘accident’, ‘lameness’, and ‘wound’. In addition, owners sometimes reported using inappropriate remedies (e.g., firing) before hospitalisation. Multivariable ordinal regression analysis performed between physical parameters and the outcome (ordered based on severity: discharged, euthanasia, and dead) revealed that sex (male vs. female: OR = 1.33; p < 0.05), colour of the mucous membrane (pathological vs. physiological: OR = 1.72; p < 0.01), and capillary refill time (pathological vs. physiological: OR = 1.42; p = 0.02) increased the likelihood of a non-survival outcome. In conclusion, early euthanasia should be considered for these equids, to minimise prolonged suffering. Moreover, owners’ education is recommended to guarantee minimal welfare standards to the working equids. Full article

Back pain is common in ridden horses. Back diseases in horses include Impinging Dorsal Spinous Processes, Ventral Spondylosis, Osteoarthritis of Articular Process, Intervertebral Discs Disease, Vertebral Fractures, Conformational Abnormalities, Desmopathy of the Supraspinous Ligament, Desmopathy of the Intraspinous Ligament, and Longissimus Muscle Strain. Back pain may also develop as a result of lameness (particularly hindlimb lameness). A poorly fitting saddle and an unbalanced rider are also considered important factors influencing the development of back pain in horses. The conventional diagnosis of equine back pain includes a clinical examination and diagnostic imaging examination using ultrasound, radiography, and thermography. Advanced diagnostic modalities of equine back pain involve the objectification of standard procedures through the use of algometers, a lameness locator, biometric mats, and the geometric morphometrics method. In addition to modern diagnostic methods, such as computed tomography and scintigraphy, advances in the diagnosis of equine back pain include the use of electromyography and functional electrical stimulation. The aim of this review article is to familiarize clinicians with the usefulness and capabilities of conventional diagnostic protocols and advanced diagnostic modalities. Although orthopedic examination and traditional diagnostic methods will remain the foundation of the diagnosis of back diseases, modern methods meet the growing expectations towards high-performance horses and allow for deeper diagnostics and objective monitoring of rehabilitation and training progress. The study was conducted as a blinded laboratory study using seven equine stifle specimens. The 2D (T2W TSE) and 3D (3D VIEW T2W HR, T2 3D mFFE, T1W VISTA SPAIR, 3D PDW SPAIR) 3-tesla MRI sequences of each stifle were obtained. Cartilage thickness was measured at 30 locations on MRI and on gross pathology. Thickness measurements were compared using a Bland–Altman plot and post hoc analysis tests. The 3D sequences were found to be generally more accurate than the 2D sequence (p < 0.001). The smallest difference to macroscopic measurements was observed in the 3D VIEW T2W HR and T1W VISTA SPAIR sequences with no statistical difference between each other. Knowing the accuracy of different sequences will improve the evaluation of equine cartilage and the early detection of cartilage pathologies. This would promote MRI as a noninvasive imaging modality for horses suffering from stifle lameness with no findings in conventional imaging methods. Furthermore, since 3D sequences seem to have better accuracy in depicting cartilage, they may replace 2D sequences, thereby shortening scanning times. Full article

Treatment of Naturally Occurring Tendon Disease with Allogeneic Multipotent Mesenchymal Stromal Cells: A Randomized, Controlled, Triple-Blinded Pilot Study in Horses

The treatment of tendinopathies with multipotent mesenchymal stromal cells (MSCs) is a promising option in equine and human medicine. However, conclusive clinical evidence is lacking. The purpose of this study was to gain insight into clinical treatment efficacy and to identify suitable outcome measures for larger clinical studies. Fifteen horses with early naturally occurring tendon disease were assigned to intralesional treatment with allogeneic adipose-derived MSCs suspended in serum or with serum alone through block randomization (dosage adapted to lesion size). Clinicians and horse owners remained blinded to the treatment during 12 months (seven horses per group) and 18 months (seven MSC-group and five control-group horses) of follow-up including clinical examinations and diagnostic imaging. Clinical inflammation, lameness, and ultrasonography scores improved more over time in the MSC group. The lameness score difference significantly improved in the MSC group compared with the control group after 6 months. In the MSC group, five out of the seven horses were free of re-injuries and back to training until 12 and 18 months. In the control group, three out of the seven horses were free of re-injuries until 12 months. These results suggest that MSCs are effective for the treatment of early-phase tendon disease and provide a basis for a larger controlled study. Full article

Equine metabolic syndrome (EMS) has become an important issue in modern veterinary medicine and is linked to the common, extremely painful, most-of-the-time performance-terminating hoof laminitis. The growing knowledge in the field of genetic background, inducing environmental factors, diagnosis, treatment and maintenance of affected equines led us to summarise the available information to be used not only for scientific purposes but for fieldwork. In horses, the clinical presentation of EMS includes: obesity or local fat deposition, bilateral lameness or hoof rings attributed to ongoing or previous (pasted) laminitis with the key feature of the occurrence of insulin dysregulation, disturbing the homeostasis within insulin, glucose and lipid metabolism. The management of EMS is based on dietary and fitness discipline; however, intensive research is ongoing in the field of regenerative medicine to develop modern and promising therapies. Full article
