Branched-Chain Amino Acids Combined with Exercise Improves Physical Function and Quality of Life in Older Adults: Results from a Pilot Randomized Controlled Trial

This pilot, randomized, double-blind, placebo-controlled trial investigated the effects of branched-chain amino acids (BCAAs)—provided in a 2:1:1 ratio of leucine:isoleucine:valine—combined with exercise on fatigue, physical performance, and quality of life in older adults. Twenty participants (63% female; BMI: 35 ± 2 kg/m2; age: 70.5 ± 1.2 years) were randomized to 8 weeks of either exercise + BCAAs (100 mg/kg body weight/d) or exercise + placebo. The program included moderate aerobic and resistance training three times weekly. Physical function was assessed using handgrip strength, chair stands, gait speed, VO2 max, and a 400 m walk. Psychological health was evaluated using the CES-D, Fatigue Assessment Scale (FAS), Insomnia Severity Index (ISI), and global pain, fatigue, and quality of life using a visual analog scale (VAS). Significant group x time interactions were found for handgrip strength (p = 0.03), chair stands (p < 0.01), and 400 m walk time (p < 0.01). Compared to exercise + placebo, exercise + BCAAs showed greater improvements in strength, mobility, and endurance, along with reductions in fatigue (−45% vs. +92%) and depressive symptoms (−29% vs. +5%). Time effects were also observed for ISI (−30%), FAS (−21%), and VAS quality of life (16%) following exercise + BCAA supplementation. These preliminary results suggest that BCAAs combined with exercise may be an effective way to improve physical performance and reduce fatigue and depressive symptoms in older adults. Full article

This study evaluated the differential effects of an advanced Vivifrail D-level exercise intervention on arterial stiffness, functional fitness, and body composition in robust older women with varying baseline fitness levels. A total of 41 participants were assigned to a higher-fitness group (HFG, n = 22) or a lower-fitness group (LFG, n = 19) based on their 30 s chair stand performance. Over 12 weeks, both groups completed five weekly sessions incorporating strength, balance, aerobic, and dual-task elements. Significant within-group improvements were observed in the arm curl test (HFG: p = 0.031; LFG: p = 0.002), chair stand (LFG: p < 0.001), and 2 min step test (LFG: p = 0.002). Between-group analysis showed greater percentage gains in the LFG for the chair stand (33.8% ± 28.2% vs. 7.1% ± 21.6%, p = 0.001) and step test (13.7% ± 14.5% vs. 5.3% ± 14.7%, p = 0.040). No significant changes were found in handgrip strength, gait speed, pulse wave velocity, or muscle mass. These results suggest that the modified Vivifrail protocol enhances lower-limb endurance in lower-fitness individuals, but additional adaptations may be necessary to impact vascular and compositional outcomes. This study aimed to determine whether hand grip strength (HGS) could estimate the physical function of older women with or without obesity or sarcopenia in South Jeolla Province, South Korea. Methods: A total of 541 elderly women aged 60–95 years (mean age: 79.87 ± 6.64) participated in the study. Physical function was assessed using HGS, 10 m walk test, figure-of-8 walk test, and 30-s chair stand test. Among the participants, 400 (76.8%) were classified as obese, and 137 (26.3%) had sarcopenia. Results: Results showed that HGS was significantly lower in the sarcopenic group (16.58 ± 2.86 kg) than in the normal group (24.31 ± 3.33 kg, p < 0.001). HGS was negatively correlated with body fat (r = −0.146, p = 0.001) and positively correlated with physical performance. Additionally, exercise habits were significantly associated with reduced obesity risk (OR = 0.592, p = 0.027). This study was approved by the Institutional Review Board of Chonnam National University (IRB No. 1040198-220210-BR-009-03). The significance of hand grip strength was comparable to that of gait speed, walking capacity, coordination, and balance function. Conclusions: These findings suggest that grip strength can serve as a physical indicator of healthy condition and physical function with or without sarcopenia or obesity in older women. Full article

Background/Objectives: Sarcopenia and malnutrition are common in post-stroke patients, impairing recovery. Medium-chain triglycerides (MCT) may support muscle metabolism, while chair-stand exercises improve strength and mobility. However, their combined effects remain unclear. This study evaluated the synergistic effects of MCT supplementation and high-frequency chair-stand exercise on physical function and muscle health in post-stroke patients. Methods: A retrospective cohort study included 1080 post-stroke patients categorized into three groups: (1) MCT supplementation alone, (2) chair-stand exercise alone, and (3) both combined. MCT supplementation consisted of ~40 g/day MCT oil or powder. Functional outcomes were assessed using the Functional Independence Measure (FIM-motor), while muscle health was evaluated by handgrip strength (HGS) and skeletal muscle index (SMI). Multiple linear regression analyses were performed, adjusting for confounders. Results: The combined intervention group showed significantly greater improvements in FIM-motor scores at discharge (B = 8.79, 95% CI: 5.64–11.95, β = 0.32, p < 0.001) and FIM-motor gain (B = 6.02, 95% CI: 3.42–8.62, β = 0.29, p < 0.001) compared to the control. Increases in HGS (B = 2.441, 95% CI: 0.483–4.398, β = 0.18, p = 0.015) and SMI (B = 0.194, 95% CI: 0.102–0.419, β = 0.12, p = 0.039) were also observed. Chair-stand exercise was performed a median of 62 times/day and independently improved outcomes, while MCT alone had limited effects. Conclusions: MCT supplementation combined with chair-stand exercise enhances functional recovery and muscle health in post-stroke patients, supporting its role in rehabilitation. Further research is needed to evaluate long-term effects and to examine the pharmacokinetics of MCTs, including blood concentrations, in broader populations. Full article

Objectives: This study aimed to analyze the effect of a multimodal prehabilitation program for colorectal cancer patients in body composition, physical and cardiorespiratory fitness as well as its ability to reduce postoperative complications. Methods: A longitudinal observational study evaluated the efficacy of a prehabilitation intervention based on four components: (a) health education and self-care, (b) nutritional counseling, (c) psychological support, and (d) supervised physical exercise. Body composition was determined through bioelectrical impedance analysis; physical fitness variables such as strength was measured by a handgrip dynamometer for upper limbs, and a squat–jump test, countermovement jump test using a contact platform, and a chair–stand test for lower limbs. Flexibility was assessed with the sit-and-reach test. Cardiorespiratory fitness was assessed with the 6 min walking test (6MWT). Moreover, we measured lifestyles related to the amount of physical exercise by accelerometry. Results: The final cohort included 30 patients. Patients completed an average of 9.90 ± 5.26 exercise sessions. Statistically significant changes with varying effect sizes were observed in the following outcomes: SJ values in cm and W for both sexes (p = 0.021/d = 0.14 and p = 0.043/d = 0.10, respectively), SJ in W for women (p = 0.023/d = 0.21), all chair-stand test values (p = 0.021/d = 0.65 for men, p = 0.004/d = 2.08 for women, and p = 0.000/d = 0.84 for both sexes), and sit-and-reach for both sexes (p = 0.005/d = 0.12) and for men (p = 0.044/d = 0.08). All 6MWT values had statistically significant changes (p = 0.001/0.46). Women reduced the weekly minutes spent in sedentary behavior (p = 0.037/d = 0.65) and increased the minutes spent performing light physical activity (p = 0.037/d = 0.63). With regard to surgical outcomes, there was a tendency towards a decrease in postoperative complications and hospitalization days, as well as minutes in postoperative REA (p = 0.009/d = 0.69) in relation to the control group. Conclusions: Participation in a multimodal prehabilitation program improves several aspects of physical condition and lifestyles related to the amount of physical exercise and reduces both days of hospitalization and several complications post-surgery. Background/Objectives: Early intervention combining nutrition optimisation with exercise can potentially prevent frailty progression and reverse pre-frailty in older adults. Methods: This 4-month study examined the effectiveness of nutrition education (without oral nutrition supplement use) as part of a multi-domain intervention on the nutritional status and intake of pre-frail community-dwelling older adults and its relationship with sarcopenia markers. Results: Amongst 172 participants (≥55 years), 5.8% were malnourished, with no significant change in nutritional status throughout the study. Post-intervention, participants consumed significantly higher daily calories, protein, protein per body weight (BW), and calcium (p < 0.001); protein intake at lunch (p = 0.001) and dinner (p = 0.004) also increased. However, 6-month post-intervention daily protein (p = 0.025), protein per BW (p = 0.039), and calcium (p = 0.015) decreased significantly. Sarcopenia markers (handgrip strength (HGS), five-time chair stand test (5STS), and short physical performance battery score (SPPB)) showed no significant difference post-intervention. Well-nourished participants had better HGS (p = 0.005), 5STS (p = 0.026), and SPPB (p = 0.039). Practical nutrition education effectively improved nutritional intake, but the effect was not sustained 6-months post-intervention. Conclusions: Optimising nutritional status with a focus on improving protein intake, especially at breakfast, to meet minimal intake to stimulate muscle protein synthesis can help prevent sarcopenia and frailty. Future studies should examine factors driving sustainable improvement to prevent frailty progression in this population. Full article

Background/Objectives: Hip and knee arthroplasty relieves pain, restores mobility, and improves function in severe joint damage, though pain and strength loss may persist post-surgery. Better pre-surgery function and activity predict improved arthroplasty outcomes. Prehabilitation programs enhance functional abilities, reducing hospitalization duration, and lowering peri-surgery complication risks. This study aims to evaluate the efficacy of four weeks of different modalities of tele-home-prehabilitation programs on perceived pain and functional capacity in patients who are eligible for hip or knee arthroplasty. Methods: Forty-four patients (aged 65–80 years) eligible for elective lower limb arthroplasty were enrolled in the present randomized controlled trial study. Participants were randomly assigned to the Electrostimulation Group (EG) or the Home-Based Exercise Group (HG). The EG performed underwent teleprehabilitation program using indirect neuromuscular electrostimulation therapy, while the HG performed home-based exercise supervised by a sports specialist. Functional capacity was assessed with the Timed Up and Go test (TUG), the 30 s Chair Stand test (30CST) and the Six-Minute Walking Test (6MWT). Quality of life was assessed with the Oxford Hip Score (OHS) or Oxford Knee Score (OKS), depending on the participants’ surgery. The Technology Acceptance Model (TAM) questionnaire was completed by the EG after the intervention. Results: No significant differences were found among groups in the 30CST and TUG tests. The analyses revealed significant differences for the Oxford Questionnaires and 6MWT. Conclusions: Our study highlights the potential of teleprehabilitation using indirect neuromuscular electrostimulation to improve walking autonomy and quality of life of individuals who are candidates for lower limb arthroplasty. Background/Objectives: Exercise and nutrition may be useful strategies in dynapenic and sarcopenic obesity management, but the identification of treatment modalities aimed at improving this condition is still lacking. We compared the effect of a five-month hypocaloric diet plus resistance training (RT) with and without essential amino acids (EAAs) on body composition, physical performance, and muscle strength among older adults with dynapenic obesity (DO). Methods: Older individuals (n = 48) with DO [(BMI ≥ 30 kg/m2 and/or high waist circumference and low handgrip strength (HGS)] were randomized into two double-blind groups (RT without EAAs vs. RT+EAAs). All participants followed a hypocaloric diet (1 g of proteins/kg spread over three meals) and RT for five months. Pre- and post-intervention assessments included the body composition (DXA), Short Physical Performance Battery (SPPB), HGS, one-repetition maximum (1-RM), and maximal isometric torque with an isokinetic dynamometer. Results: Both groups reduced body mass (RT: −4.66 kg; RT+EAAs: −4.02 kg), waist circumference (RT: −4.66 cm; RT+EAAs: −2.2 cm), total fat mass (RT: −3.81 kg; RT+EAAs: −3.72 kg), and compartmental fat mass with no between-group differences. Both groups improved 1-RM strength (33–47%), isometric torque for body mass (RT: 14.5%; RT+EAAs: 10.6%), and functional performance (chair stand (RT: −3.24 s; RT+EAAs: −1.5 s) and HGS (RT: −2.7 kg; RT+EAAs: 2.9 kg)) with no between-group differences. Conclusions: A moderate hypocaloric diet combined with RT improves body composition and physical function in DO participants, but EAA supplementation did not provide additional benefits. Full article

Background: This study aimed to investigate the effects of a 12-week body-weight-based resistance training program on balance ability and fear of falling in community-dwelling older women. Methods: Twenty-three older women were assigned to either an intervention group that performed the low-load resistance training with slow movement using the body weight (LRT group; n = 12) or a control group (CON group; n = 11). The LRT group participated in the exercise session twice weekly for 12 weeks, while the CON group maintained their daily routine. The 30 s chair stand test (CS-30) was applied to measure lower-extremity muscle strength, balance ability was evaluated using one-leg standing tests with eyes open (OLST-O) and closed (OLST-C), and fear of falling among all participants was assessed using the Falls Efficacy Scale International (FES-I) before (pre) and after (post) the intervention. A two-way analysis of variance with repeated measures [group (LRT and CON) × time (pre and post)] was carried out to evaluate the intervention effects. Results: Significant interactions were observed in the CS-30 (F = 9.503, p < 0.01, ηp2 = 0.312), OLST-O (F = 5.211, p < 0.05, ηp2 = 0.199), and OLST-C (F = 5.257, p < 0.05, ηp2 = 0.200), though significant simple main effects from pre to post were observed only in the LRT group. The CS-30 scores (pre: 19.8 ± 3.8 times, post: 25.5 ± 5.6 times; p < 0.001), OLST-O time (pre: 78.8 ± 35.8 s, post: 96.2 ± 29.9 s; p < 0.01), and OLST-C time (pre: 10.2 ± 5.9 s, post: 17.4 ± 12.2 s; p < 0.01) were improved before and after the intervention. However, a significant interaction was not observed in FES-I (F = 1.335, p = 0.261, ηp2 = 0.06). Conclusions: The 12-week body-weight-based resistance training program enhanced lower-extremity muscle strength and balance ability but did not lessen the fear of falling in community-dwelling older women. The study findings offer relevant information for fall prevention in older adults. Full article

(1) Background: Aging is associated with a progressive decline in physical capacity, which is further exacerbated by conditions such as arthritis and chronic joint pain. This study aimed to compare the effects of aquatic and land-based exercise on the functional fitness of older adult women. (2) Methods: Sixty older women (mean age 66.9 ± 3.8 years) participated in this study, divided into two groups: aquatic exercise and land-based exercise. Both groups completed functional fitness tests, including flexibility (Back Scratch and Chair Sit-and-Reach Tests), lower body strength (Chair Stand Test), and dynamic balance (8-Foot Up-and-Go Test). Statistical analyses compared group performance. (3) Results: Improvements in flexibility were observed in the aquatic group, with trends toward significance for the Back Scratch Test (−0.2 ± 1.0 cm vs. −2.0 ± 0.0 cm, p = 0.08) and the Chair Sit-and-Reach Test (2.87 ± 2.0 cm vs. 0.27 ± 1.0 cm, p = 0.07). No statistically significant differences were observed between the groups in measures of lower body strength (Chair Stand Test: 19.1 ± 4.47 reps vs. 18.97 ± 3.77 reps, p = 0.9) or dynamic balance (8-Foot Up-and-Go Test: 6.28 ± 6.2 s vs. 6.03 ± 5.83 s, p = 0.07). (4) Conclusions: Aquatic exercise showed greater improvements in flexibility, particularly in the upper and lower body, although these differences did not reach statistical significance. Both training modalities were equally effective in maintaining lower body strength and dynamic balance in older adult women. Background/Objectives: This study aimed to assess and compare the effects of an adapted taekwondo (TKD) program, multicomponent training (MCT), walking exercise (WE), and inactive control group (CG) on blood pressure, morphological variables, frequency of food consumption, cognitive status, health-related quality of life (HRQoL), physical fitness tests, and postural balance in independent older females. Methods: A randomized controlled trial study was conducted with the following groups: TKD (n = 13), MCT (n = 12), WE (n = 12), and CG (n = 14), considering three/weekly 60-min/sessions for 16-weeks. A two-factor mixed analysis of the variance model with repeated measures was performed. Results: TKD improved significantly more in phonetic fluency (p = 0.021; ES = 1.89) than WE and in general health (p = 0.033; ES = 1.11) than CG. Both TKD and MCT improved significantly more than CG in the 30 s chair stand, arm curl, chair sit-and-reach, timed up-and-go, maximal isometric handgrip strength, and postural balance for the eyes closed condition in the area and anteroposterior velocity (p < 0.05). Conclusions: Only TKD improved the area (p = 0.008; ES = 1.00) and mediolateral velocity (p = 0.019; ES = 0.79) for the eyes open condition, and mediolateral velocity (p = 0.021; ES = 1.57) for the eyes closed condition. Blood pressure, morphological variables, and food consumption frequency showed no significant intragroup or intergroup interactions. TKD equivalently improved HRQoL and physical fitness to MCT, with better postural balance in older females. Full article

Introduction: Stroke is a leading cause of mortality and long-term disability worldwide, often resulting in low levels of physical activity post-discharge. A comprehensive exercise program could be a safe method to increase stroke patients’ physical activity and exercise volume. This pilot study examines the effectiveness, safety, and feasibility of a comprehensive exercise regimen for stroke patients residing in the community after discharge. Methods: This single-group pilot study included patients who received inpatient rehabilitation for motor impairment following a stroke. Community-dwelling participants engaged in a comprehensive exercise program, which was structured and individualized, consisting of stretching, aerobic exercise, strength training, and balance exercises, conducted under the supervision of a health exercise specialist in a dedicated living lab within the hospital. The program involved 20 sessions, each lasting 50 min, conducted three to five times weekly. The pre- and post-intervention evaluations were comprehensive, including safety and validity assessments, along with physical function tests such as cardiopulmonary exercise testing and various fitness tests (6 min walk test, timed up and go test, chair stand test, figure-of-8 walk test, grip strength test, and sit-and-reach test). Depression levels were assessed using the Geriatric Depression Scale (GDS). Results: A total of 22 subjects were recruited, with 21 completing the study after one dropout due to fatigue and visiting distance. Among the 21 completers, one subject experienced a mild adverse event (hypoglycemia), which was not serious, confirming the program’s safety. The exercise participation rate was 95.5%, and the adherence rate averaged 95.2%, with all completers achieving over 80% adherence. Significant improvements were observed in VO2 peak (p = 0.000), 6 min walk test (p = 0.000), timed up and go test (p = 0.000), chair stand test (p = 0.000), figure-of-8 walk test (p = 0.000), and grip strength (p = 0.001). However, changes in the peak respiratory exchange ratio (p = 0.078), sit-and-reach test (p = 0.228) and GDS scores (p = 0.275) were not significant. Conclusion: This study demonstrates that the comprehensive exercise program is sufficiently safe and valid for stroke rehabilitation patients. The program significantly enhances cardiopulmonary endurance, walking ability, balance, muscle strength, muscle endurance, and coordination. Further research with larger sample sizes and control groups is needed to confirm these findings and explore additional benefits. Full article

Background: Detraining refers to the decline in physical fitness that occurs after the cessation of exercise, compromising the adaptations resulting from regular exercise training. To understand how long the benefits acquired from an exercise program can be maintained, the present study evaluated the detraining effects of a 4–week exercise cessation period in older adults who performed combined training at various weekly frequencies for 12 weeks. Methods: This randomized controlled trial assigned participants to one of two training programs: a combined training program twice a week (CT2) or four times a week (CT4) over a period of 12 weeks, followed by a four–week detraining period. The resistance training consisted of six bodyweight exercises, while the aerobic training involved either walking or running. Both the CT2 and CT4 groups completed the same total training volume and overload each week; the only difference was the number of training sessions per week. Assessments were conducted at baseline, after the training period, and after the detraining period, and included the 30–s and five–repetition chair–stand tests, isometric handgrip strength, body mass index (BMI), waist circumference, and waist–to–height ratio. Results: Thirty–one participants completed the study (CT2: 17 and CT4: 14). The groups presented similar attendance records during the training period (CT2: 96 ± 18% versus CT4: 94 ± 19%). After the 12–week training period, CT2 and CT4 improved lower limb strength, CT2 improved upper limb strength, and CT4 reduced waist circumference and waist–to–height ratio compared to baseline. After the 4–week detraining period, the lower limb strength remained improved in both groups (CT2: 4 ± 1 repetition; p < 0.001 and CT4: 4 ± 1 repetition; p < 0.001) when compared to the corresponding baseline values. The handgrip strength decreased in CT2 compared to post–training values. And the body composition benefit in CT4 was not sustained after detraining. Conclusions: The effects of 4 weeks of detraining after 12 weeks of training performed two or four times per week are similar on some but not all health-related physical fitness parameters. (1) Background: Numerous exercise programs that improve physical capacity and the risk of falls in older adults have been proposed with varying degrees of success. A novel approach may be to use a video game system that uses real-time force feedback to train older adults. The aim of this study was to evaluate the effects of a Nintendo™ Wii-based exercise program on physical function and risk of falls in older people. (2) Methods: This 12-week randomized controlled clinical trial involved 73 participants: 36 individuals participating in a control group (CG) and 37 in an experimental group (EG) participating in a combined program. Balance was measured using the Tinetti scale, flexibility was assessed with the back scratch test and the sit-and-reach test, and lower body strength was assessed with the 30 s chair stand-up test. (3) Results: The results of this study show significant improvements in balance, gait, flexibility, and strength of the lower limbs compared to a control group. (4) Conclusions: A Nintendo™ Wii-based exercise program for seniors produces improvements in the physical health of older adults. These improvements highlight the importance of integrating physical exercise through video games as an effective strategy to improve the general health and quality of life of older adults. Introduction: This study aims to provide a safe, effective, and sustainable resistance training environment for the elderly by modifying chairs and movement systems used during training, particularly under unsupervised conditions. Materials and Methods: The research focused on investigating the effect of modified chair designs on enhancing physical stability during resistance training by involving 19 elderly participants (mean 72.1, SD 4.7). The study measured changes in the body’s acceleration during movements to compare the effectiveness of the modified chairs with those commonly used in chair-based exercise (CBE) training in maintaining physical stability. A system was developed based on experimental video data, which leverages MediaPipe to analyze the videos and compute joint angles, identifying whether the actions are executed correctly. Results and Conclusions: Comparisons revealed that modified chairs offered better stability during sitting (p < 0.001) and stand-up (p < 0.001) resistance training. According to the questionnaire survey results, compared to the regular chair without an armrest, the modified chair provided a greater sense of security and a better user experience for the elderly. Video observations indicated that the correct completion rate for most exercises, except stand-up resistance training, was only 59.75%, highlighting the insufficiency of modified chairs alone in ensuring accurate movement execution. Consequently, the introduction of an automatic system to verify proper exercise performance is essential. The model developed in this study for recognizing the correctness of movements achieved an accuracy rate of 97.68%. This study proposes a new chair design that enhances physical stability during resistance training and opens new avenues for utilizing advanced technology to assist the elderly in their training.
