Background. Extensive partial edentulism alters the biological and functional balance of the stomatognathic system, requiring targeted pre-prosthetic procedures to optimize treatment outcomes. Objectives. The aim of this study was to assess the extent of improvement in the clinical–biological scores of the prosthetic field in patients with extensive edentulism, following pre-prosthetic interventions. Materials and Method. This prospective, cross-sectional study investigated 194 subjects with extensive partial edentulism. Clinical–biological scores, initially and following the pre-prosthetic phase, were recorded using a scoring system that evaluated dental and periodontal status, bone and mucosal support, occlusion, and mandibulo-cranial relationships. Statistical comparisons of clinical–biological scores were related to the type of prosthetic therapy. Statistical significance was considered at a p-value < 0.05. Results. There was an overall significant improvement in the clinical–biological scores initially (mean value 20.2) and after pre-prosthetic procedures (mean value 23.22) (p < 0.001). When treatment groups were divided, the implant-assisted prosthesis group showed the best improvement in all domains, followed by the conventional fixed-prostheses group (p < 0.01). Dental support improved significantly in those with semi-rigid composite prostheses (p = 0.014), while periodontal support was improved in both fixed- and hybrid-implant groups. Mucosal and bone support improved mostly in the fixed-implant groups (p = 0.014). Conclusions. Pre-prosthetic procedures significantly enhance the biological and functional readiness of the prosthetic field, with the degree of improvement influenced by the complexity and type of planned prosthetic rehabilitation. The findings underscore the value of individualized pre-prosthetic protocols as an essential component of prosthetic treatment planning. Full article

Background/Objectives: Multiple factors may contribute to the development of open bite malocclusion, including genetics and environmental factors. Anterior open bite is usually related to the interposition of the tongue between the dental arches during swallowing or at rest. But how important is the role of the tongue in the development of anterior open bite? And how does anterior open bite malocclusion influence tongue behavior? With this study, we would like to offer a better understanding on the importance of tongue function in the context of this malocclusion. Methods: In this narrative review, a comprehensive electronic search was conducted via PubMed, Google Scholar, and ScienceDirect. The inclusion criteria were original research articles published between 2014 and 2024 with full text access. The exclusion criteria were articles older than 10 years and articles with restricted access or without full text access. Results: Out of the initial 1231 articles, 9 articles were found to be eligible for the present study. The tongue is a part of the neutral zone of the oral cavity, together with the lips and cheek musculature, forming a corridor of equilibrium. If this neuromuscular balance is altered, the teeth will move out of the neutral zone, and various malocclusions may develop. Patients with anterior open bite experience difficulties in closing the anterior portion of the oral cavity during swallowing; therefore, adaptative changes occur in an attempt to compensate by changing the dynamics of the tongue. Conclusions: The cause–effect relationship between tongue malfunction and anterior open bite remains controversial. This review article mentions the possible role of the tongue in anterior open bite etiology, but evidence is still needed on this subject, as it is yet unclear whether the influence of the tongue is a primary cause, an influencing factor, or just a consequence of an already-existing vertical occlusal pathology. The human tooth’s morphology, which includes variations in cusp numbers and patterns, is of tremendous interest to anthropologists, morphologists, and dentists. Cusp 6 is an additional cusp that is very seldom encountered in primary or permanent mandibular molars, especially first molars. A supernumerary cusp located lingual to the distobuccal cusp at the crown’s distal border is cusp 6. According to the literature, cusp 6 is also known by other anthropologic designations, such as “Tuberculum Sextum” or “Entoconulid”. This case offers a unique instance of a bilateral tuberculum sextum in the maxillary permanent first molars, characterized by an additional cusp on the lingual surface. The patient, an adolescent, exhibited no associated symptoms. Early detection of such dental anomalies is essential for treatment planning, particularly in maintaining occlusal balance and preventing future complications. Regular monitoring is necessary to manage a bilateral tuberculum sextum in order to avoid enamel fractures and occlusal interference. To preserve functional occlusion and avoid problems, prophylactic sealants or selective grinding may be taken into consideration. This report highlights the importance of recognizing a tuberculum sextum for proper clinical management. Full article

Evaluating Surgical Approaches for Hemimandibular Hyperplasia Associated with Osteochondroma: A Systematic Literature Review

Background/Objectives: Hemimandibular hyperplasia (HH) associated with osteochondroma presents complex challenges in maxillofacial surgery, including facial asymmetry, occlusal instability, and temporomandibular joint (TMJ) dysfunction. Surgical interventions vary widely in approach and outcomes, underscoring the need for a systematic evaluation of effectiveness. This systematic review assesses the effectiveness of surgical approaches for managing HH associated with osteochondroma, focusing on techniques including condylectomy, orthognathic surgery, distraction osteogenesis, total joint replacement (TJR), and genioplasty. Methods: Following PRISMA 2020 guidelines, a comprehensive search was conducted in PubMed, Scopus, and Web of Science for studies published from 2000 to 2023. Eligibility criteria were based on the PICO framework, with primary outcomes evaluated for facial symmetry, occlusal correction, mandibular function, and recurrence rates. The Cochrane Risk of Bias Tool assessed study quality, while the GRADE framework evaluated the certainty of evidence. This review was not registered due to exclusion criteria for certain dental topics in PROSPERO. Results: Of 145 studies identified, 18 met inclusion criteria, totaling 214 patients. High and low condylectomy both effectively corrected asymmetry, with high condylectomy reducing recurrence risk but often requiring reconstruction. Orthognathic surgery, combined with condylectomy, significantly enhanced facial symmetry and occlusal function. Distraction osteogenesis proved valuable for mandibular lengthening in cases of severe deformities, while TJR offered definitive solutions for extensive joint involvement. Genioplasty corrected chin asymmetry, contributing to improved facial balance. Limitations included small sample sizes and variable follow-up durations. Conclusions: Surgical approaches tailored to individual patient needs show effectiveness in treating HH associated with osteochondroma, achieving functional and esthetic outcomes. Future studies should prioritize larger cohorts and standardized follow-up protocols to better assess long-term efficacy. Advances in 3D surgical planning and individualized treatment strategies show promise for optimized patient-specific care. Full article

Background: The widespread prevalence of obstructive sleep apnea (OSA) underscores the necessity for effective therapies. Mandibular advancement devices (MADs) have emerged as valid treatment for mild to moderate cases, despite the associated dental side effects. Methods: This study evaluates the nature, onset, and long-term manifestation of these side effects. In the prospective group (n = 12), dental impressions were taken pre-MAD-insertion and at intervals of three, six, nine, and twelve months post-insertion to monitor occlusal alterations. In the retrospective group, participants (n = 8) wearing MADs for 7 years at average underwent lateral cephalogram assessments to compare with pre-treatment X-rays. All participants completed a specific questionnaire. Statistical analysis was performed via t-test and with p < 0.05 as the significance level. Results: The majority of participants in both groups consistently used MADs and reported significant sleep quality improvements, rating common symptoms like jaw tension as negligible. In both the prospective group and the retrospective group, significant reductions in overjet were observed at multiple time points, with the prospective group showing reductions at six months (p = 0.001), nine months (p > 0.001), and twelve months (p = 0.019), while the retrospective group indicated a notable decrease between baseline and follow-up assessments after a mean of seven years of device wear (p = 0.004). A slight overbite increase of 0.2 mm was prospectively observed after one year, whereas a trend towards a minimal decrease over the long term was observed in the retrospective sample (p = 0.003). Noteworthy changes in angle class or lower incisor inclination were absent. Cephalograms revealed significant IOK-NL angle alterations with a mean of 98.2° before and 95.2° upon long-term treatment (p = 0.020). Conclusions: These findings suggest that MADs are effective in treating OSA with minor adverse effects. This study advocates for moderate mandibular protrusion to balance therapeutic efficacy with dental health considerations, crucial for optimizing treatment outcomes. Nonetheless, the limited sample size warrants caution when generalizing these results to the broader population. Full article

Objectives: The purpose of this study was to determine the stability of occlusal contacts based on the left–right distribution of the occlusal contact area, divide participants into well-balanced and unbalanced groups, and clarify the effect of light clenching on the balance ability. Methods: Forty-one healthy men completed occlusal contact examinations with pressure-sensitive films, and the participants were allocated to the balanced occlusal contact (BOC) group or the unbalanced occlusal contact (UOC) group. The balance ability was measured using a center of gravity sway meter. The static balance in standing and dynamic balance using the cross test were assessed. Measurements were performed in the mandibular rest position (RP) or with light clenching (LC). Differences in the balance ability between the participant groups due to clenching and correlations between the static and dynamic balance were analyzed. Results: Differences in the balance ability due to clenching were observed only in the BOC group, with the static balance higher with LC, and the dynamic balance higher in the RP condition (p < 0.01). Significant correlations were observed between the static and dynamic balance except for the UOC group with LC (p < 0.01, p < 0.05). Conclusions: The results of this study suggested that occlusion affects the postural control when occlusal contact is stable but does not affect the postural control when occlusal contact is unstable. Background and objectives: This study analyzed and compared the distribution patterns of occlusal forces using T-Scan III before and after the hydrostatic temporary oral splint (Aqualizer Ultra) therapy in healthy subjects and subjects with temporomandibular disorders (TMDs). Materials and Methods: Fifty-one subjects were divided into groups based on anamnesis and responses to the Fonseca questionnaire. The first group, non-TMDs group (n = 19), and the second group, TMDs group (n = 32), had mild-to-severe TMDs, as identified by the Fonseca questionnaire. The non-TMDs group had an average age of 25.4 years (SD = 4.8, range 20–38) with 15 females (78.95%) and 4 males (21.05%). The TMDs group had an average age of 27.4 years (SD = 7.0, range 22–53) with 25 females (78.125%) and 7 males (21.875%). T-Scan III device was used for occlusal analysis before and after hydrostatic splint usage. Results: Significant differences were observed in the TMDs group for anterior and posterior right percentages of forces before and after hydrostatic splint usage. The analysis of force distribution per sector before and after hydrostatic splint therapy showed no significant differences in the non-TMDs group. Analysis of force distributions in the entire study population before and after hydrostatic splint therapy showed significant differences in the anterior and posterior right regions. Occlusal force increased by 32–56% in the front region and decreased in the posterior area after hydrostatic splint usage. Conclusions: Hydrostatic splint therapy is recommended as a part of full-mouth rehabilitation treatment for all patients regardless of the severity of TMDs. Full article

The association between craniofacial muscles and postural control is well-known because of numerous anatomical connections. However, there are a few conflicting studies that correlated the activity of the masticatory muscles with the distribution of body weight pressure on the feet, which can strongly influence balance. Therefore, the purpose of our study was to evaluate the association between the masseter and temporalis muscle activity and foot pressure distribution. Fifty-two women were recruited, and baropodometric and EMG analyses of the masseter and temporalis baseline activities were analyzed. An ipsilateral association was found between the right temporal muscle activity and the right rearfoot load (r = 0.29, p < 0.05) and right forefoot load (r = −0.29; p < 0.05), as well as the percentage of muscle activation of the right masseter muscles with the percentage of pressure on the right forefoot (r = 0.31, p < 0.05) and right rearfoot (r = −0.31, p < 0.05). While further studies are needed, an ipsilateral association was found between masticatory muscles and foot pressure distribution. Full article

Introduction: An altered dental occlusion can also affect balance stability or mobility. Thus, the aim of this study was to examine whether wearing a mandibular splint, which retains the occlusion close to the centric occlusion, can increase or decrease balance stability and the range of motion (ROM) of the cervical spine as opposed to the habitual occlusion, and if there is a difference between men and women. Material and methods: In this study, 41 male (34.7 ± 11.4 years) and 50 female subjects (29.3 ± 12.7 years) participated. Cervical spine ROM was recorded using the Zebris CMS 70P system. For balance stability, a pressure measuring platform integrated into the treadmill system (FDM-T) of the company Zebris® Medical GmbH was used. Here, the area of the ellipse and the length of the Center of Pressure (CoP) was recorded whilst in the bipedal and unipedal stance. Results: The sex comparison showed significant differences for the area of the ellipse of the right leg and ROM extension in the habitual occlusion: females showed a better balance stability and a larger ROM extension. When wearing the splint, only the CoP of the left leg was significant with a better balance stability in female subjects. Within the male subjects, the ellipse area in the bipedal and unipedal (left/right) stance showed mostly significant reductions, while the rotation left and right as well as the lateral flexion (left/right) improved when wearing the splint. Female subjects, when wearing the splint, showed a significant decrease of the ellipse area and the CoP length when standing on either leg. Flexion, rotation to the right and lateral flexion to the left/right, all increased significantly. Conclusion: Wearing a splint that keeps the jaw close to the centric relation improved balance stability and increased the ROM of the cervical spine for both male and female subjects. Women may have marginally different basic balance stability strategies than men, with regard to bipedal and unipedal standing. Nevertheless, there are scarcely any differences between the two sexes in the adaptation when wearing a splint. Changing the jaw relation in healthy adults can possibly support the release of movement potentials that simplify the performance of everyday activities or sports movements. Full article

Aim: The authors provided an accurate, simple, and noninvasive method for matching the intraoral scan with facial scan of a patient, without the need of a cone beam computed tomography (CBCT). Materials and methods: Three different facial scans were acquired: the first one with the mouth closed, the second one with a voluntary “social smile”, and the last one scanned the face with a lip-and-cheek retractor with dental arches in occlusion. The base of this method is to superimpose the area of the dental arches acquired by a face scanner with the same area derived by an intraoral scanner. Result: An accurate matching of intraoral and facial scans can be achieved without the risks of radiation exposure. Conclusions: The virtual patient helps the orthodontist to improve both diagnosis and treatment planning: a three-dimensional digital smile design can be performed, the patient’s smile can then be analyzed in the context of the lips, and the teeth can be moved to achieve a consonant and balanced smile. All this information can be integrated in a clear aligner therapy or an indirect bonding procedure, enhancing outcomes in the facial esthetics. Full article

Background and Objectives: The long face type is associated with excessive vertical facial growth and most often with anterior open bite. In many cases of anterior open bite of high severity associated with bimaxillary dento-alveolar protrusion, lips are unable to form an adequate seal at rest. This leads to many issues, including facial dysmorphism. The aim of this study was to describe the case of a 15 year old girl who addressed the orthodontist in November 2015, having complaints related to the skeletal and dental open bite. Case Description: A 15.7 year old patient required a consultation with the orthodontic service for impaired dento-facial aesthetics at rest, smile and speech due to an exaggerated superior protrusion of the upper frontal teeth, labial incompetence with excessive gingival exposure at rest and smile associated with upper and lower anterior teeth crowding. The orthodontic diagnostic consisted of skeletal open bite with a hyperleptoprosop morphological facial type, high degree of hyperdivergence, bimaxillary dento-alveolar protrusion, 7 mm skeletal open-bite, 3 mm vertical inocclusion of the anterior teeth, skeletal class II relationships, bilateral half cusp class II molar and canine relationships, labial incompetence, highly increased interlabial gap, facial asymmetry, excessive gingival exposure of 7 mm at smiling and bimaxillary anterior crowding. Because the patient initially refused orthognathic surgery, prior to starting the orthodontic treatment, the patient was recommended to receive a bilateral extraction of the first upper premolars. Key objectives of pre-surgical orthodontic treatment were to achieve a retroclined position of the upper incisors under their normal inclination for the planned upward maxillary rotation, to maintain slightly lower incisor proclination. The orthognathic surgery consisted of Le Fort I impaction osteotomy with 8 mm anterior impactation, bilateral sagittal split osteotomy, and mandibular repositioning using occlusal splint. Conclusions: At the end of the orthodontic-surgical treatment, the patient presented significant improvement in dento-facial aesthetics, and optimal skeletal, muscular and dental balance. Background: Cranial surface electromyography is assumed to analyze the correlation between the stomatognathic apparatus and the muscular system and its implications on the physical status of professional athletes. The purpose of this paper is to evaluate surface cranial electromyography as an aid in the diagnosis and treatment of occlusal and muscular disorders in professional athletes. Methods: A sample of 25 athletes (mean age 23 years, 20 men and 5 women) underwent electromyographic recording; among them, 13 had a sports injury condition (symptomatic athletes), while 12 were in perfect physical condition (asymptomatic athletes). At odontostomatological examination, 6 showed cranio-mandibular disorders (dysfunctional athletes), while 19 showed no disorders (functional athletes). The treatment plan to resolve the symptoms of the dysfunctional athletes was chosen based on the electromyographic data. One month after the start of therapy with an occlusal splint, a follow-up was performed, and the results were compared with the initial data. Results: Statistical analysis showed that the chosen therapy following the use of electromyography was effective in 72% of cases, while 28% of patients did not respond to therapy (p = 0.028). Conclusions: The use of cranial electromyography in competitive athletes is a valuable tool in therapeutic choice aimed at balancing occlusal loads and improving the patient’s global tonic postural attitude, resulting in positive feedback in the qualitative assessment of sports performance. A 3D finite element model of the mandible dentition was developed, including 14 teeth, a periodontal ligament (PDL), and a splint made of polymethylmethacrylate (PMMA). The study considered three design options: 1—the case of splint absence; 2—the case of the splint presence installed after manufacture; and 3—the case of splint presence installed after correction (grinding) performed to ensure a uniform distribution of occlusal force between the teeth. For cases of absence and presence of splint, three measurements of the functional load were performed using the T-Scan III software and hardware complex (TekScan, Boston, MA, USA). It was found that the presence of a splint led to a decrease in the total value of the occlusive load and to a uniform distribution between all the mandible teeth. The occlusal force was considered as a static vertical force evenly distributed between the nodes belonging to the occlusive surface of the corresponding tooth for the first design option and the occlusal surface of the splint for the second and third ones, respectively. As a result of the study, it was concluded that the splint usage was effective in order to change the distribution of the functional load during the treatment of proved masticatory muscles’ parafunction; the safety of using a splint for teeth and surrounding tissues under the influence of the considered functional load was shown; the potential applicability of PMMA as a structural material of a splint that had been used for the treatment of masticatory muscles’ parafunction was established. Choosing an adequate occlusion is challenging during the construction of artificial dentures, and critical for patient satisfaction. However, there is no conclusive evidence to support which occlusal design is more appropriate. The aim of this systematic review was to evaluate canine-guided occlusion in comparison to other occlusal schemes in removable complete denture wearers. The review was conducted according to the Preferred Reported Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. An extensive search was carried out on (PubMed (National Library of Medicine)), Scopus (Elsevier), Cochrane collaboration (Wiley), Science Direct (Elsevier) and Europe PMC (European Bioinformatics Institute). English and non-English studies were identified using keywords on canine-guided occlusion, denture occlusion, dental occlusion and artificial occlusal schemes. Out of 1759 articles searched, 18 were selected based on the inclusion criteria. The included studies were interpreted for the descriptive analysis for the calculation of occlusal schemes, occlusion assessment parameters, the outcome of the occlusal schemes and types of removable prosthesis used. The systematic analysis of occlusal schemes revealed that canine-guided occlusion is viable compared with other occlusal designs in terms of patient satisfaction, mastication, retention, esthetics, phonetics, oral-health-related quality of life and muscle activity. The type of occlusal scheme influences the complete denture patient’s’ masticatory efficiency, satisfaction, retention, phonetics and esthetics. Nevertheless, physical, physiological, mechanical and psychological factors play a vital role in the success of removable complete dentures. The canine-guided occlusal scheme is preferred because of its simplicity, less time consumption, good masticatory performance, ease of fabrication and modification into bilateral balanced occlusion if required. Further studies are required to reduce the controversies related to jaw relations, gnathology and occlusal schemes in complete dentures. Full article

The purpose of this investigation was to show how to manage an anterior crossbite in early mixed dentition with an eruption guidance appliance (EGA). The analyzed clinical case reported an anterior crossbite, a bimaxillary retrusion tendency, and a horizontal growth pattern. The anterior crossbite was an unfavorable occlusal condition that could lead to a class III malocclusion growth pattern. An early treatment approach was suggested to reach a correct sagittal jaw relationship. Hence, the selected approach acted on the dentoalveolar sector, aiming to have effects on the posterior vertical dimension and to improve the sagittal jaw’s relation. An EGA was selected to treat the patient in early mixed dentition. After 7 months of therapy with night-time use, the dental malocclusion was completely resolved. The patient continued to be treated with the same device, used as active retention. With the EGA treatment, the erupting forces, rather than the active forces, were used to resolve the dental malocclusion. This approach allowed a low compliance requirement and had a minimum psychosocial and psychological impact on the patient. The early treatment was essential to give a functional occlusion and a good balance of the soft perioral tissues and muscles. Full article
