Background/Objectives: This study investigated the association between cochlear implant (CI) users’ assessed perception of musical sound quality and their subjective music perception and music-related quality of life (QoL). The aim was to provide a comprehensive evaluation by integrating a relatively objective Turkish Multiple Stimulus with Hidden Reference and Anchor (TR-MUSHRA) test and a subjective music questionnaire. Methods: Thirty CI users and thirty normal-hearing (NH) adults were assessed. Perception of sound quality was measured using the TR-MUSHRA test. Subjective assessments were conducted with the Music-Related Quality of Life Questionnaire (MuRQoL). Results: TR-MUSHRA results showed that while NH participants rated all filtered stimuli as perceptually different from the original, CI users provided similar ratings for stimuli with adjacent high-pass filter settings, indicating less differentiation in perceived sound quality. On the MuRQoL, groups differed on the Frequency subscale but not the Importance subscale. Critically, no significant correlation was found between the TR-MUSHRA scores and the MuRQoL subscale scores in either group. Conclusions: The findings demonstrate that TR-MUSHRA is an effective tool for assessing perceived sound quality relatively objectively, but there is no relationship between perceiving sound quality differences and measures of self-reported musical engagement and its importance. Subjective music experience may represent different domains beyond the perception of sound quality. Therefore, successful auditory rehabilitation requires personalized strategies that consider the multifaceted nature of music perception beyond simple perceptual judgments. Full article

Objective: This observational cross-sectional study aimed to comprehensively evaluate sensory–cognitive performance in children diagnosed with Attention-Deficit/Hyperactivity Disorder (ADHD), with a focus on auditory processing, visual–perceptual abilities, visual–motor integration, and oculomotor function. The study further examined how hyperactivity, age, and gender may influence these domains. Methods: A total of 70 non-medicated children with clinically diagnosed ADHD (mean age = 9.1±2.4 years; 67.1% male), all with normal visual acuity, were assessed using four standardized instruments: the Test of Auditory Processing Skills, Third Edition (TAPS-3), the Test of Visual Perceptual Skills, Fourth Edition (TVPS-4), the Beery–Buktenica Developmental Test of Visual–Motor Integration, Sixth Edition (VMI-6), and the Developmental Eye Movement (DEM) Test. Statistical analyses included one sample and independent samples t-tests, one-way ANOVA, and Pearson correlation coefficients. Results: Participants demonstrated significantly above-average performance in auditory processing (TAPS-3: μ=108.4, std=7.8), average visual–perceptual abilities (TVPS-4: μ=100.9, std=7.2), slightly below-average visual–motor integration (VMI-6: μ=97.1, std=9.0), and marked deficits in oculomotor efficiency (DEM ratio: μ=87.3, std=18.1). Statistically significant differences were observed across these domains (t-values ranging from 2.9 to 7.2, p<0.01). Children with hyperactive-impulsive presentations exhibited lower horizontal DEM scores (μ=73.4, std=12.3) compared to inattentive counterparts (μ=82.9, std=16.2; p=0.009). Age and sex influenced specific subtest scores, with boys and children aged 8–9 years achieving higher outcomes in word memory (p=0.042) and visual discrimination (p=0.034), respectively. Moderate correlations were identified between auditory and visual–perceptual skills (r=0.32, p=0.007), and between visual–perceptual and oculomotor performance (r=0.25, p=0.035). Conclusions: The findings from this sample reveal a distinct sensory–cognitive profile in children with ADHD, characterized by relatively preserved auditory processing and pronounced oculomotor deficits. These results underscore the value of a multimodal assessment protocol that includes oculomotor and visual efficiency evaluations. The conclusions pertain specifically to the cohort studied and should not be generalized to all populations with ADHD without further validation. Full article

Misophonia is a condition characterized by intense, involuntary distress or anger in response to specific sounds, often leading to irritation or aggression. While the condition is recognized for its emotional and behavioral impacts, little is known about its physiological and perceptual effects. The current study aimed to explore the physiological correlates and perceptual consequences of misophonia through a combination of electrophysiological, perceptual, and cognitive assessments. Seventeen individuals with misophonia and sixteen control participants without the condition were compared. Participants completed a comprehensive battery of tests, including (a) cortical event-related potentials (ERPs) to assess neural responses to standard and deviant auditory stimuli, (b) the spatial release from the speech-on-speech masking (SRM) paradigm to evaluate speech segregation in background noise, and (c) the flanker task to measure selective attention and cognitive control. The results revealed that individuals with misophonia exhibited significantly smaller mean peak amplitudes of the N1 and N2 components in response to oddball tones compared to controls. This suggests a potential underlying neurobiological deficit in misophonia patients, as these components are associated with early auditory processing. However, no significant differences between each group were observed in the P1 and P2 components regarding oddball tones or in any ERP components in response to standard tones. Despite these altered neural responses, the misophonia group did not show differences in hearing thresholds, speech perception abilities, or cognitive function compared to the controls. These findings suggest that while misophonia may involve distinct neurophysiological changes, particularly in early auditory processing, it does not necessarily lead to perceptual deficits in speech perception or cognitive function. Chinese classical gardens serve as restorative environments that epitomize the essence of Chinese aesthetics. Despite their recognized Outstanding Universal Value, research on the multisensory experiences and associated well-being benefits they offer remains insufficiently explored. This study examines the Humble Administrator’s Garden through a multisensory framework incorporating visual, auditory, and tactile dimensions. By employing a mixed methods approach that integrates questionnaires and physiological measurements, we assessed the psychophysiological responses of 78 participants before and after entering the garden, as well as at distinct landscape points. Key findings reveal the following: (1) the traditional garden environment evoked significant positive perceptual effects; (2) the garden, with its diverse combination of landscape elements—including architecture, water features, rock formations, and plants—was strongly associated with enhanced well-being outcomes. A higher diversity of visual landscapes yielded more pronounced benefits, with water features exerting the most substantial positive influence on health, while rock formations showed a comparatively lesser effect; and (3) physiological and psychological responses of participants varied within the same landscape setting, with tactile experiences eliciting the most profound psychological changes. These findings underscore the pivotal role of multisensory engagement in Chinese classical gardens for fostering well-being, offering valuable health-related benefits beyond their cultural and ecological significance. Background/Objectives: Previous studies have examined the role of working memory in cognitive tasks such as syntactic, semantic, and phonological processing, thereby contributing to our understanding of linguistic information management and retrieval. However, the real-time processing of phonological information—particularly in relation to suprasegmental features like tone, where its contour represents a time-varying signal—remains a relatively underexplored area within the framework of Information Processing Theory (IPT). This study aimed to address this gap by investigating the real-time processing of similar tonal information by native Cantonese speakers, thereby providing a deeper understanding of how IPT applies to auditory processing. Methods: Specifically, this study combined assessments of cognitive functions, an AX discrimination task, and electroencephalography (EEG) to investigate the discrimination results and real-time processing characteristics of native Macau Cantonese speakers perceiving three pairs of similar tones. Results: The behavioral results confirmed the completed merging of T2–T5 in Macau Cantonese, and the ongoing merging of T3–T6 and T4–T6, with perceptual merging rates of 45.46% and 27.28%, respectively. Mismatch negativity (MMN) results from the passive oddball experiment revealed distinct temporal processing patterns for the three tone pairs. Cognitive functions, particularly attention and working memory, significantly influenced tone discrimination, with more pronounced effects observed in the mean amplitude of MMN during T4–T6 discrimination. Differences in MMN peak latency between T3–T6 and T4–T6 further suggested the use of different perceptual strategies for these contour-related tones. Specifically, the T3–T6 pair can be perceived through early signal input, whereas the perception of T4–T6 relies on constant signal input. Conclusions: This distinction in cognitive resource allocation may explain the different merging rates of the two tone pairs. This study, by focusing on the perceptual difficulty of tone pairs and employing EEG techniques, revealed the temporal processing of similar tones by native speakers, providing new insights into tone phoneme processing and speech variation. Background: Speech production is a possible way to monitor bulbar and respiratory functions in patients with amyotrophic lateral sclerosis (ALS). Moreover, the emergence of smartphone-based data collection offers a promising approach to reduce frequent hospital visits and enhance patient outcomes. Here, we studied the relationship between bulbar and respiratory functions with voice characteristics of ALS patients, alongside a speech therapist’s evaluation, at the convenience of using a simple smartphone. Methods: For voice assessment, we considered a speech therapist’s standardized tool—consensus auditory-perceptual evaluation of voice (CAPE-V); and an acoustic analysis toolbox. The bulbar sub-score of the revised ALS functional rating scale (ALSFRS-R) was used, and pulmonary function measurements included forced vital capacity (FVC%), maximum expiratory pressure (MEP%), and maximum inspiratory pressure (MIP%). Correlation coefficients and both linear and logistic regression models were applied. Results: A total of 27 ALS patients (12 males; 61 years mean age; 28 months median disease duration) were included. Patients with significant bulbar dysfunction revealed greater CAPE-V scores in overall severity, roughness, strain, pitch, and loudness. They also presented slower speaking rates, longer pauses, and higher jitter values in acoustic analysis (all p < 0.05). The CAPE-V’s overall severity and sub-scores for pitch and loudness demonstrated significant correlations with MIP% and MEP% (all p < 0.05). In contrast, acoustic metrics (speaking rate, absolute energy, shimmer, and harmonic-to-noise ratio) significantly correlated with FVC% (all p < 0.05). Conclusions: The results provide supporting evidence for the use of smartphone-based recordings in ALS patients for CAPE-V and acoustic analysis as reliable correlates of bulbar and respiratory function. As important recreational spaces for urban residents, urban microgreen parks enhance the urban living environment and alleviate psychological pressure on residents. The visual, auditory, and olfactory senses are crucial forms of perception in human interaction with nature, and the sustainable perceptual design of miniature green parks under their interaction has become a recent research hotspot. This study aimed to investigate the effects of the visual, acoustic, and olfactory environments (e.g., aromatic green vegetation) on human perception in miniature green parks. Participants were evenly divided into eight groups, including single-sensory groups, multi-sensory interaction groups, and a control group. Eye-tracking technology, blood pressure monitoring, and the Semantic Differential (SD) scales and Profile of Mood State (POMS) were used to assess the effectiveness of physical and mental perception recovery in each group. The results revealed that in an urban microgreen space environment with relatively low ambient noise, visual–auditory, visual–olfactory, and visual–auditory–olfactory interactive stimuli were more effective in promoting the recovery of visual attention than single visual stimuli. Additionally, visual–auditory–olfactory interactive stimuli were able to optimize the quality of spatial perception by using positive sensory inputs to effectively mask negative experiences. Simultaneously, environments with a high proportion of natural sounds had the strongest stimuli, and in the visual–auditory group, systolic blood pressure at S7 and heart rate at S9 significantly decreased (p < 0.05), with reductions of 18.60 mmHg and 20.15 BPM, respectively. Aromatic olfactory sources were more effective in promoting physical and mental relaxation compared to other olfactory sources, with systolic blood pressure reductions of 24.40 mmHg (p < 0.01) for marigolds, 23.35 mmHg (p < 0.01) for small-leaved boxwood, and 27.25 mmHg (p < 0.05) for camphor trees. Specific auditory and olfactory conditions could guide visual focus, such as birdsong directing attention to trees, insect sounds drawing attention to herbaceous plants, floral scents attracting focus to flowers, and leaf scents prompting observation of a wider range of natural vegetation. In summary, significant differences exist between single-sensory experiences and multi-sensory modes of spatial perception and interaction in urban microgreen parks. Compared to a silent and odorless environment, the integration of acoustic and olfactory elements broadened the scope of visual attention, and In the visual–auditory–olfactory interactive perception, the combination of natural sounds and aromatic camphor tree scents had the best effect on attention recovery, thereby improving the quality of spatial perception in urban microgreen parks. Background: This scoping review had as a primary goal a review of the literature and the an analysis of the possible effectiveness of the LSVT LOUD approach in children with voice and speech deficits. Methods: A search was conducted in the Scopus and PubMed databases in May of 2024. Eleven articles were obtained from the search. The standards of PRISMA recommendations were used for scoping reviews and the PCC framework was used for the eligibility criteria. Furthermore, the study used the instructions in the Cochrane Handbook for a quality assessment. The Mendeley Reference Manager software collected the studies and removed duplicates. Results: The reviewed studies employed formal and informal measures to assess voice and speech abilities in the children. Regarding the sample’s characteristics, the studies mostly included children with Cerebral Palsy (CP) and also those with Down Syndrome (DS). All the studies reported that children with CP and DS undertook a total dose of the LSVT LOUD treatment. Significant post-treatment findings indicated increased speech function and sound pressure level, regarding the auditory–perceptual ratings of voice and speech improvement. In many studies, parents’ and expert listeners’ ratings of voice, perception of vocal loudness, speech, and communication indicated improvement. Conclusions: The majority of the included studies provide positive evidence for the LSVT as an approach. However, the small sample size that featured in the studies, as well as their limitations, made these conclusions uncertain. Moreover, the study’s findings provided recommendations that speech language therapists and other clinicians need to follow when setting a treatment plan with children with CP and DS. Full article

Noise reduction (NR) algorithms are employed in nearly all commercially available hearing aids to attenuate background noise. However, NR processing also involves undesirable speech distortions, leading to variability in hearing outcomes among individuals with different noise tolerance. Leveraging 30 participants with normal hearing engaged in speech-in-noise tasks, the present study examined whether the cortical measure of neural signal-to-noise ratio (SNR)—the amplitude ratio of auditory evoked responses to target speech onset and noise onset—could predict individual variability in NR outcomes with varying strength, thus serving as a reliable indicator of individual noise tolerance. In addition, we also measured subjective ratings of noise tolerance to see if these measures could capture different perspectives on individual noise tolerance. Results indicated a significant correlation between neural SNR and NR outcomes that intensified with increasing strength of NR processing. While subjective ratings of noise tolerance were not correlated with the neural SNR, noise-tolerance ratings could predict outcomes with stronger NR processing and account for additional variance in the regression model, although the effect was limited. Our findings underscore the importance of accurately assessing an individual’s noise tolerance characteristics in predicting perceptual benefits from various NR processing methods and suggest the advantage of incorporating both cortical and subjective measures in the relevant methodologies. Childhood apraxia of speech (CAS) represents a significant diagnostic and therapeutic challenge within the field of clinical neuropsychology, characterized by its nuanced presentation and multifactorial nature. The aim of this study was to distil and synthesize the broad spectrum of research into a coherent model for the assessment and diagnosis of CAS. Through a mixed-method design, the quantitative phase analyzed 290 studies, unveiling 10 clusters: developmental apraxia, tabby talk, intellectual disabilities, underlying speech processes, breakpoint localization, speech characteristics, functional characteristics, clinical practice, and treatment outcome. The qualitative phase conducted a thematic analysis on the most cited and recent literature, identifying 10 categories: neurobiological markers, speech motor control, perceptual speech features, auditory processing, prosody and stress patterns, parent- and self-report measures, intervention response, motor learning and generalization, comorbidity analysis, and cultural and linguistic considerations. Integrating these findings, a descriptive and prescriptive model was developed, encapsulating the complexities of CAS and providing a structured approach for clinicians. This model advances the understanding of CAS and supports the development of targeted interventions. This study concludes with a call for evidence-based personalized treatment plans that account for the diverse neurobiological and cultural backgrounds of children with CAS. Its implications for practice include the integration of cutting-edge assessment tools that embrace the heterogeneity of CAS presentations, ensuring that interventions are as unique as the children they aim to support. Objective: This study examined the relationship between gender and auditory-perceptual evaluation of tracheoesophageal (TE) speech. Method: We collected auditory-perceptual judgments of two features, speech acceptability and listener comfort, from normal-hearing young adult listeners (n = 16) who were naïve to TE speech. Auditory-perceptual judgments were made for 12 TE speakers (6 men and 6 women) on two occasions separated by between 7 and 14 days. During the first session, listeners were deceived about the gender of the voice samples presented, and in the second session, listeners were informed of the true gender of the voice samples. Results: The findings suggest that a gender bias exists in perceptions of TE speech, and that female TE speakers tend to be disproportionately penalized when compared to their male counterparts when gender is known. Conclusions: These data provide insights into the potential influence of speaker gender on listener judgments of TE speech and the impact that such factors may have on communication. Our data indicate that listeners rate female TE speaker samples as less acceptable and less comfortable to listen to when the samples are known to be female speakers. Full article

Post-thyroidectomy syndrome (PTS), characterized by voice issues after thyroidectomy without recurrent laryngeal nerve injury, was investigated in this study. The Voice Fatigue Index (VFI) and cepstral analysis were employed for subjective and objective voice evaluation. Retrospective analysis involved 96 patients (37 males, 59 females) who underwent thyroidectomy without nerve injury from April 2018 to June 2022. Assessments pre- and post-thyroidectomy included the Voice Handicap Index (VHI) and VFI, along with auditory perceptual, acoustic (including cepstral), aerodynamic, and glottal vibration analyses. In females, although the GRBAS scale showed no significant change, both VHI and VFI increased post-thyroidectomy. Significant correlations were observed between the VHI and VFI in females. Acoustic analysis indicated a decrease in the cepstral peak prominence (CPP) of vowels (/a/) and sentences in females, with significant correlations between changes in the CPP/a/ and VHI/VFI. The maximum fundamental frequency (F0max) exhibited a significant decrease, correlating with the VHI and VFI changes. The VFI demonstrated effectiveness in subjective PTS voice evaluation, comparable to the VHI. The present study highlights the potential of cepstral analysis as an index reflecting subjective voice discomfort, suggesting its promise for a comprehensive PTS voice evaluation. Full article

Temporal and Spectral Models as Correlates to Auditory-Perceptual Judgments of Overall Severity and Listener Comfort in Tracheoesophageal Voice

Introduction: This study pursued two objectives: (1) to determine the potential association between listener (n = 51) judgments of 20 male tracheoesophageal speaker samples for two auditory-perceptual dimensions of voice, overall severity (OS) and listener comfort (LC); and (2) to assess the temporal and spectral acoustic correlates for these auditory-perceptual dimensions. Methodology: Three separate correlation analyses were performed to evaluate the association between OS and LC. First, scores of OS and LC from all listeners were pooled together, and then the correlation between OS and LC was computed. Second, scores of OS and LC were averaged over all listeners to derive a single estimate of OS and LC for each TE speaker sample; the correlation between the average OS and LC was then computed. Third, listener-to-listener variability in the association between OS and LC was evaluated by computing the correlation between OS and LC scores from each listener across all TE samples. Finally, two stepwise multiple regression models were created to relate the average LC score to spectral and temporal variation in the acoustic signal. Results: While the pooled OS and LC scores had a moderate positive correlation (r = 0.66, p < 0.00001), the averaged OS and LC exhibited a near perfect positive correlation (r = 0.99, p < 0.00001). The significant differences between the pooled and averaged scores were explained by significant listener-to-listener variability in the association between OS and LC. OS and LC scores from 5 listeners had non-significant correlations, 10 had moderate correlations (r < 0.7), 35 listeners had high correlations (0.7 < r < 0.9), and 1 listener had a very high correlation (r < 0.9 < 1). Finally, the acoustic models created based on the spectral and temporal variations in the signal were able to account for 87.7% and 61.8% of variation in the average LC score. Conclusions: The strong correlations between OS and LC suggest that LC may, in fact, provide a more comprehensive auditory-perceptual surrogate for the voice quality of TE speakers. Although OS and LC are distinct conceptual dimensions, LC appears to have the advantage of assessing the social impact and potential communication disability that may exist in interactions between TE speakers and listeners. Background: While several acoustic voice metrics are available for clinical voice assessment, there remains a significant need for reliable and ecologically valid tools. The Acoustic Voice Quality Index version 03.01 (AVQI-3) and Acoustic Breathiness Index (ABI) hold potential due to their comprehensive assessment approach, incorporating diverse voice aspects. However, these tools still need to be validated in English-speaking populations. Methods: This study assessed the discriminatory accuracy and validity of AVQI-3 and ABI in 197 participants, including 148 with voice disorders. Voice samples were collected, followed by AVQI-3 and ABI calculations. Additionally, auditory-perceptual assessments were conducted by a panel of speech-language pathologists. Results: AVQI-3 and ABI effectively identified disordered voice quality, evidenced by high accuracy (AUCs: 0.84, 0.89), sensitivity, and specificity (thresholds: AVQI-3 = 1.17, ABI = 2.35). Strong positive correlations were observed with subjective voice quality assessments (rs = 0.72, rs = 0.77, p < 0.001). Conclusions: The study highlights AVQI-3 and ABI as promising instruments for clinically assessing voice disorders in U.S. English speakers, underscoring their utility in clinical practice and voice research. Background and Objectives: Transcutaneous electrical nerve stimulation (TENS), a pain-alleviating and muscle-relaxing treatment used in physio-therapeutic clinical practice, has recently appeared to be just as effective in dysphonia. This review aimed at clarifying whether TENS can be an effective practice in dysphonia therapy and/or management on its own or combined with other types of interventions and, hence, whether its practice can be a useful, more widespread establishment to speech and language therapy intervention methods. Materials and Methods: A search was conducted on the PubMed database using specific terms based on the PICO search strategy. Eventually, four randomized controlled studies and four clinical trials were included. The methodological quality of the included studies was evaluated using the physiotherapy evidence-based database (PEDro) assessment tool, and this indicated high-quality research with an average score of 8.43. Results: The studies utilized various TENS devices, predominantly the Dualpex 961 device (frequency of 10 Hz, phase of 200 ms). The assessment methods varied, including auditory perception, vocal therapy, electrostimulation, audio and video perceptual assessments, and laryngeal evaluations. The clinical outcomes of TENS showed a reduction in musculoskeletal pain in various areas, while the acoustic analysis results were significant in only one study. TENS was compared to manual laryngeal therapy (LMT), placebo TENS, and vocal therapy in different studies with mixed results. Conclusions: This review supports the idea that a multidimensional approach, incorporating various therapeutic modalities (TENS, LMT, speech therapy, and vocal training) can yield positive outcomes for patients with voice disorders. Further research is needed to explore the specific mechanisms of action and optimal treatment protocols for TENS in voice therapy. Full article
