Cardiovascular Risk in Rheumatic Patients Treated with JAK Inhibitors: The Role of Traditional and Emerging Biomarkers in a Pilot Study

Background: Despite therapeutic advances, morbidity and mortality remain high in patients with rheumatoid arthritis (RA) and psoriatic arthritis (PsA), primarily due to increased cardiovascular risk. Objectives: Our study aimed to evaluate the cardiovascular risk profile and biomarker dynamics in patients with RA and PsA treated with Janus kinase inhibitors (JAKis). To our knowledge, this is the first study assessing Lp(a) levels in this context. Methods: This prospective, observational study assessed 48 adult patients. The follow-up period was 12 months. Traditional cardiovascular risk factors and biological markers, including lipid profile, lipoprotein(a) [Lp(a)], and uric acid (UA), were assessed at baseline and follow-up. Correlations between JAKi therapy, lipid profile changes, and cardiovascular risk factors were investigated. Cox regression analysis was used to identify predictors of non-major cardiovascular events. Results: A strong positive correlation was observed between baseline and 12-month Lp(a) levels (r = 0.926), despite minor statistical shifts. No major cardiovascular events occurred during follow-up; however, 47.9% of patients experienced non-major cardiovascular events (e.g., uncontrolled arterial hypertension, exertional angina, and new-onset arrhythmias). Active smoking [hazard ratio (HR) 9.853, p = 0.005], obesity (HR 3.7460, p = 0.050), and arterial hypertension (HR 1.219, p = 0.021) were independent predictors of these events. UA (HR 1.515, p = 0.040) and total cholesterol (TC) (HR 1.019, p = 0.034) were significant biochemical predictors as well. Elevated baseline Lp(a) combined with these factors was associated with an increased event rate, particularly after age 60. Conclusions: Traditional cardiovascular risk factors remain highly prevalent and predictive, underscoring the need for comprehensive cardiovascular risk management. Lp(a) remained stable and may serve as a complementary biomarker for risk stratification in JAKi-treated patients. Full article

Despite targeting mainly the respiratory tract, SARS-CoV-2 disrupts T cell homeostasis in ways that may explain both acute lethality and long-term immunological consequences. In this study, we aimed to evaluate the T-cell-mediated chain of immunity and formation of TCR via TREC assessment in COVID-19 and long COVID (LC). For this study, we collected 231 blood samples taken from patients with acute COVID-19 (n = 71), convalescents (n = 51), people diagnosed with LC (n = 63), and healthy volunteers (n = 46). With flow cytometry, we assessed levels of CD4+ and CD8+ minor T cell subpopulations (i.e., naïve, central and effector memory cells (CM and EM), Th1, Th2, Th17, Tfh, Tc1, Tc2, Tc17, Tc17.1, and subpopulations of effector cells (pE1, pE2, effector cells)). Additionally, we measured TREC levels. We found distinct changes in immune cell distribution—whilst distribution of major subpopulations of T cells was similar between cohorts, we noted that COVID-19 was associated with a decrease in naïve Th and CTLs, an increase in Th2/Tc2 lymphocyte polarization, an increase in CM cells, and a decrease in effector memory cells 1,3, and TEMRA cells. LC was associated with naïve CTL increase, polarization towards Th2 population, and a decrease in Tc1, Tc2, Em2, 3, 4 cells. We also noted TREC correlating with naïve cells subpopulations. Our findings suggest ongoing immune dysregulation, possibly driven by persistent antigen exposure or tissue migration of effector cells. The positive correlation between TREC levels and naïve T cells in LC patients points to residual thymic activity. The observed Th2/Th17 bias supports the hypothesis that LC involves autoimmune mechanisms, potentially driven by molecular mimicry or loss of immune tolerance. Full article

Background: Polycystic Ovary Syndrome (PCOS) is a prevalent endocrine disorder characterized by excess body weight, hyperandrogenism, hyperglycemia, and insulin resistance often resulting in hirsutism and infertility. Dietary strategies have been shown to ameliorate metabolic disturbances, hormonal imbalances, and inflammation associated with PCOS. Recent evidence indicates that intermittent fasting (IF) could effectively enhance health outcomes and regulate circadian rhythm; however, its impact on PCOS remain unclear. Objective: Therefore, this systematic review and meta-analysis aims to examine the effect of IF on women diagnosed with PCOS. Methods: Comprehensive research was conducted across three major databases including PubMed, Scopus, and Web of Science without date restrictions. Meta-analysis was performed using Cochrane Review Manager Version 5.4 software. Results: Five studies fulfilled the inclusion criteria. IF significantly reduced body weight (MD = −4.25 kg, 95% CI: −7.71, −0.79; p = 0.02), BMI (MD = −2.05 kg/m2, 95% CI: −3.26, −0.85; p = 0.0008), fasting blood glucose (FBG; MD = −2.86 mg/dL, 95% CI: −4.83, −0.89; p = 0.004), fasting blood insulin (FBI; MD = −3.17 μU/mL, 95% CI: −5.18, −1.16; p = 0.002), insulin resistance (HOMA-IR; MD = −0.94, 95% CI: −1.39, −0.50; p < 0.0001), triglycerides (TG; MD = −40.71 mg/dL, 95% CI: −61.53, −19.90; p = 0.0001), dehydroepiandrosterone sulfate (DHEA-S; MD = −33.21 μg/dL, 95% CI: −57.29, −9.13; p = 0.007), free androgen index (FAI; MD = −1.61%, 95% CI: −2.76, −0.45; p = 0.006), and C-reactive protein (CRP; MD = −2.00 mg/L, 95% CI: −3.15, −0.85; p = 0.006), while increasing sex hormone-binding globulin (SHBG; SMD = 0.50, 95% CI: 0.22, 0.77; p = 0.004). No significant changes were observed in waist-to-hip ratio (WHR), total cholesterol (TC), LDL, HDL, total testosterone (TT), or anti-Mullerian hormone (AMH). Conclusions: IF represents a promising strategy for improving weight and metabolic, hormonal, and inflammatory profiles in women with PCOS. However, the existing evidence remains preliminary, necessitating further robust studies to substantiate these findings. Full article

Aspirin Eugenol Ester Ameliorates HFD-Induced NAFLD in Mice via the Modulation of Bile Acid Metabolism

Non-alcoholic fatty liver disease (NAFLD) is a highly prevalent condition worldwide and represents a major global health challenge. Pharmacological and pharmacodynamic results indicate that aspirin eugenol ester (AEE) performs various pharmacological activities. However, it is unclear whether AEE can ameliorate the NAFLD. This study investigated the ameliorative effects of AEE on glucose and lipid metabolism disorders by in vitro and in vivo experiments. In the cellular model, TC increased to 0.104 μmol/mg and TG increased to 0.152 μmol/mg in the model group, while TC decreased to 0.043 μmol/mg and TG decreased to 0.058 μmol/mg in the AEE group. In the model group, the area occupied by lipid droplets within the visual field was significantly elevated to 17.338%. However, the administration of AEE resulted in a substantial reduction in this area to 10.064%. AEE significantly reduced the lipid droplet area and TC and TG levels (p < 0.05), increased bile acids in the cells and in the medium supernatant (p < 0.05), and significantly up-regulated the expression of LRH-1, PPARα, CYP7A1, and BSEP mRNA levels (p < 0.05) compared to the model group. In the animal model, different doses of AEE administration significantly down-regulated the levels of TC, TG, LDL, GSP, and FBG (p < 0.05) compared to the high-fat-diet (HFD) group, and 216 mg/kg of AEE significantly improved hepatocellular steatosis, attenuated liver injury, and reduced the area of glycogen staining (p < 0.05). In the HFD group, the glycogen area within the visual field exhibited a significant increase to 18.250%. However, the administration of AEE resulted in a notable reduction in the glycogen area to 13.314%. Liver and serum metabolomics results show that AEE can reverse the metabolite changes caused by a HFD. The major metabolites were involved in seven pathways, including riboflavin metabolism, glycerophospholipid metabolism, tryptophan metabolism, primary bile acid biosynthesis, biosynthesis of unsaturated fatty acids, nicotinate and nicotinamide metabolism, and tryptophan metabolism. In conclusion, AEE had a positive regulatory effect on NAFLD. Full article

Background and Objectives: Gestational diabetes mellitus (GDM) is a major public health concern associated with adverse maternal and neonatal outcomes. It was found that even physiological pregnancy is followed by a significant shift in serum lipid profile, and even more pronounced in GDM pregnancies. We aimed to comprehensively assess lipid parameters among pregnant women with and without GDM. Materials and Methods: A systematic review, covering PubMed, WoS, and SCOPUS until 23 July 2024, with meta-analysis and meta-regression, was conducted, comprising studies measuring TG, TC, LDL-C, HDL-C, VLDL-C, and TG/HDL ratio in pregnant women diagnosed with GDM, and those with normal glucose tolerance. The overall effect size measure was the SMD. NOS and JADAD scales were used for quality assessment, I2 statistics for heterogeneity evaluation, and funnel plots for publication bias inspection. Results: A total of 457 studies were included in the qualitative analysis, and 74, 277, and 122 studies were included in the quantitative analysis for the 1st 2nd, and 3rd trimester, respectively. TG and TG/HDL levels were significantly elevated in all three trimesters (TG: SMD = 0.61, 0.57, and 0.48, p < 0.001 for all, and TG/HDL: SMD = 0.44, 0.66, and 0.49; p < 0.001 for all), while TC and LDL-C levels showed significant increases in the 1st and 2nd trimesters (TC: SMD = 0.38, 0.27, p < 0.001 for both, LDL-C: SMD = 0.33, 0.20, p < 0.001 for both), in pregnant women with GDM compared to those without the condition. Conclusions: GDM is associated with significant lipid abnormalities, particularly elevated TG and decreased HDL-C levels. These lipid changes are most pronounced in the first and second trimesters, highlighting the importance of early detection and management. Full article

Environmental costs (carbon emissions) have come with China’s economic rise, and its forestry sector now faces difficulties in maintaining both its profit and the health of its ecosystems. This study assesses the impact of carbon emissions on forestry efficiency and total factor productivity (TFFP) in China’s 31 provinces between 2001 and 2021. Using the data envelopment analysis (DEA) model through the slack-based measure (SBM framework) and Malmquist–Luenberger index (MLI), we examine the efficiency and productivity growth of forestry, both with and without accounting for carbon emissions. The study reveals that when carbon emissions are not taken into account, traditional measures of productivity tend to overstate both efficiency and total factor forestry productivity (TFFP) growth, resulting in an average of 7.7 percent higher efficiency and 1.6 percent of additional TFFP growth per year. If we compare the regions, coast provinces with stricter technical regulations have improved efficiency in usage, but places like Tibet and Qinghai, with more vulnerable ecosystems, endure harsher consequences. Regardless of incorporating bad output into the TFFP estimation, China’s growth in forestry productivity primarily depends on efficiency change (EC) rather than technological change (TC). This study investigates the influence of cerium nitrate (Ce(NO3)3·6H2O) content on the performance of Ce(III)/CF/BN/EPN coatings intended for heat exchangers. A series of Ce(III)/carbon fibre (CF)/boron nitride (BN)/epoxy phenolic (EPN) coatings are fabricated with varying concentrations of Ce(NO3)3·6H2O. The results of SEM and EDS show that the dissolution of cerium nitrate in acetone due to the particulate form causes it to be distributed in a diffuse state in the coating. This diffuse distribution does not significantly alter the porosity or structural morphology of the coating. With the increase in cerium nitrate content, both the EIS test results and mechanical damage tests indicate a progressive improvement in the corrosion resistance and self-healing properties of the coatings, while the thermal conductivity (TC) remains largely unaffected. The Ce in the coating reacts with the water molecules penetrating into the coating to generate Ce2O3 and CeO2 with protective properties to fill the permeable pores inside the coating or to form a passivation film at the damaged metal–coating interface, which enhances the anticorrosive and self-repairing properties of the coating. However, the incorporation of Ce(NO3)3·6H2O does not change the distribution structure of the filler inside the coating. As a result, the phonon propagation path, rate, and distance remain unchanged, leading to negligible variation in the thermal conductivity. Therefore, at a cerium nitrate content of 2.5 wt%, the coating exhibits the best overall performance, characterised by a |Z|0.1Hz value of 6.08 × 109 Ω·cm2 and a thermal conductivity of approximately 1.4 W/(m·K). Accurate estimation of forest aboveground carbon stock (AGC) is critical for climate change mitigation and ecological management. This study develops a high-resolution AGC estimation workflow for the Pinglu Canal basin, integrating Sentinel-2, Sentinel-1, ALOS PALSAR, and SRTM data with field survey measurements. Feature selection via Recursive Feature Elimination and modeling with a Random Forest algorithm—optimized through hyperparameter tuning—yielded high predictive accuracy under the ALL data combination (R2 = 0.818, RMSE = 11.126 tC/ha), enabling the generation of a 10 m-resolution AGC map. The total AGC in 2024 was estimated at 2.26 × 106 tC. To evaluate human-induced changes, we established a baseline scenario based on historical AGC trends (2002–2021) and climate data. Comparisons revealed that afforestation and vegetation restoration during canal construction led to higher AGC values than projected under natural conditions. This positive deviation highlights the effectiveness of targeted ecological interventions in mitigating carbon loss and promoting forest recovery. Our results demonstrate a cost-effective, scalable method for AGC mapping using freely accessible remote sensing data and machine learning. The findings also provide insights into balancing large-scale infrastructure development with ecosystem conservation. Full article

In the context of climate change, it becomes of utmost importance to limit the negative impact of industrial activities on carbon emissions, water stress, biodiversity loss, and natural resources depletion. Whether we consider the situation from a product-centered perspective (life cycle, R&D&I process, tools, methods, design, production, etc.) or from a human-centered perspective (habits, practices, fixation, strategic orientations, emotional sensitivity, etc.), coming years will represent a formidable upheaval for companies. To support this transition, various tools assessing products’ impact have been developed over the past decade. They aim at guiding decision makers, integrating new criteria to assess project success, and promoting the development and industrialization of solutions answering pressing environmental issues. If assessment is a key factor of success, it has become clear that processes and practices also need to evolve for practitioners to properly integrate sustainable requirements from the initial stages of their project. In that context, biomimicry, the approach aimed at taking nature as a model to support the design of more sustainable solutions, has been the center of growing interest. However, no integrated methods exist in the cosmetics sector to assess if a product is properly developed through biomimicry. This missing framework led to difficulties for cosmetic companies to support eco-design through biomimicry. In this article, we present a method called Biomim’Index developed by L’Oréal research and innovation sustainable development team to address three objectives: (i) to characterize cosmetic technologies according to whether they are based on bioinspiration, biomimetics or biomimicry; (ii) to guide the project’s leaders to identify key steps to improve existing cosmetic technologies through biomimicry; and (iii) to support the integration of biomimicry as an operational approach towards the development of new sustainable cosmetic technologies. This method, focusing on the problem-driven biomimetic approach is based on a combination of procedural requirements from the biomimetics TC288 18458:2015 ISO norm and environmental design requirements from L’Oréal for the Future (L4TF) commitments. Results present a proof of concept to outline the method’s efficiency and limits to support innovative eco-designed projects and value cosmetic technologies designed through biomimicry. Full article

This study investigated how distinct ACN-rich fruit supplements impact the gut microbiome over time in the context of a Western-style diet. We hypothesized that ACN-induced microbial shifts would occur rapidly, differ by ACN source, and require continued intake to persist. Methods: C57BL/6J mice were fed the total Western diet (TWD) supplemented with freeze-dried powders from bilberry (BB), tart cherry (TC), chokeberry (CB), elderberry (EB), black currant (BC), or black raspberry (BRB) for 0, 1, 3, or 7 days. Cocoa polyphenols (CPs) were included as a comparator with a distinct polyphenol profile. Fecal microbiota were collected at 0, 1, 3, and 7 days post exposure and analyzed by 16S rRNA sequencing. Results: ACN-rich supplements induced rapid microbial shifts detectable within one day of exposure. However, most changes reverted toward the baseline within days of supplement withdrawal, indicating limited persistence. Among the ACNs, BRB produced the most sustained microbiome alterations. Microbial responses varied by ACN source, suggesting that differences in glycoside and aglycone structures influence the community composition. Conclusions: ACN-rich foods can induce rapid but largely transient alterations in the gut microbiome, with variability linked to the polyphenol structure. These findings highlight the ecological sensitivity of the microbiome to specific dietary components and underscore the importance of sustained intake for maintaining microbial shifts. Full article

Timing and Amplitude of Light Exposure, Not Photoperiod, Predict Blood Lipids in Arctic Residents: A Circadian Light Hypothesis

In Arctic residents, blood lipids fluctuate seasonally in response to changes in light exposure (LE) patterns. This study investigates which aspects of LE (timing, dynamic range, and duration) are related to lipid profiles. We analyzed actigraphy data measuring LE and blue light exposure (BLE) along with blood determinations from 27 residents across different seasons. We assessed circadian parameters of melatonin in a subset of participants. We found that features of BLE and melatonin significantly predicted lipids: An earlier BLE acrophase was associated with higher HDL-C (β = −0.246, p = 0.013). Nighttime BLE was positively associated with TC (β = 0.290, p = 0.008) and LDL-C (β = 0.253, p = 0.025). The normalized circadian amplitude of BLE was inversely associated with the TG/HDL ratio (β = −0.384, p < 0.001). Finally, earlier melatonin was associated with lower TG/HDL (β = 0.464, p = 0.007). Results remained significant after adjusting for co-factors of photoperiod duration, age, sex, and indigeneity. These findings suggest that patterns of LE (circadian light hygiene) could be a way to improve cardiovascular health. Full article

Background: Gallbladder hypomotility is a key pathogenic factor in cholelithiasis. Non-invasive interventions to enhance gallbladder contractility remain limited. Ultrasound therapy has shown promise in various muscular disorders, but its effects on gallbladder function are unexplored. Methods: This study employed low-intensity pulsed ultrasound (LIPUS) at a 3 MHz frequency and 0.8 W/cm2 intensity with a 20% duty cycle to irradiate the gallbladder region of fasting guinea pigs. Gallbladder contractile function was evaluated through multiple complementary approaches: in vivo assessment via two-dimensional/three-dimensional ultrasound imaging to monitor volumetric changes; quantitative functional evaluation using nuclear medicine scintigraphy (99mTc-HIDA); and ex vivo experiments including isolated gallbladder muscle strip tension measurements, histopathological analysis, α-smooth muscle actin (α-SMA) immunohistochemistry, and intracellular calcium fluorescence imaging. Results: Ultrasound significantly enhanced gallbladder emptying, evidenced by the volume reduction and increased ejection fraction. Scintigraphy confirmed accelerated bile transport in treated animals. Ex vivo analyses demonstrated augmented contractile force, amplitude, and frequency in ultrasound-treated smooth muscle. Histological examination revealed smooth muscle hypertrophy, α-SMA upregulation, and elevated intracellular calcium levels. Extended ultrasound exposure produced sustained functional improvements without tissue damage. Conclusions: Ultrasound effectively enhances gallbladder contractile function through mechanisms involving smooth muscle structural modification and calcium signaling modulation. These findings establish the experimental foundation for ultrasound as a promising non-invasive therapeutic approach to improve gallbladder motility and potentially prevent gallstone formation. Effects of Personalized Nutrition Education on Lipid Profiles in Chinese Adults: A Medical Student-Implemented Community Intervention Study

Background: Dyslipidemia affects over 30% of Chinese adults, with awareness rates below 20%. Promoting nutrition education programs in the general population is important, but few studies have investigated the specific strategies and their efficacy. Methods: This longitudinal study was conducted in a representative sample of community-dwelling adults in Lu’an City, central-eastern China. After recruitment and propensity score matching, the personalized and conventional nutrition education groups included 306 and 612 participants, respectively. We provided standardized nutrition education based on the Dietary Guidelines for Chinese Residents (2022) for the conventional group, and personalized nutrition advice based on individual food intake and the guidelines for the personalized group. Serum total cholesterol (TC), high-density lipo-protein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) and triglycerides (TGs) were measured at baseline and again after follow-up. Results: Three years after the intervention, the average levels of TC, LDL-C, and TG in both groups all increased. After adjusting for covariates, an increase in HDL-C was greater in the personalized group than in the convention group, while the increment in TG was less pronounced. LDL-C changes were similar between the groups. The beneficial effect of personalized nutrition education on HDL-C levels was more pronounced in women (pinteraction < 0.05). Similar results were observed among participants with dyslipidemias and after excluding all lipid-lowering medication users. Conclusions: We conducted personalized nutrition education through medical students’ community practice projects among Chinese community residents, revealing that personalized nutrition education based on dietary surveys could significantly improve blood lipid profiles in general residents and individuals with dyslipidemias compared to conventional nutrition education. Background: The increased presence of metabolic syndrome (MetS) indicators globally is considered a public health problem, and the dose-response of exercise is not clarified. Objectives: This purpose of this study was to analyze the effectiveness of changes in biochemical, physiological, and anthropometric indicators of MetS based on distinct types of exercise in adults. Methods: Based on PRISMA guidelines, we searched the following databases—PubMed, Cochrane, LILACS, Springer Link, and Science Direct— for clinical trials investigating the effect of exercise in MetS indicators, without date or language restrictions. The quality of evidence and risk of biases were assessed using the PEDro scale. The impact of aerobic training “AT,” resistance training “An-T”, concurrent training “CT”) on MetS indicators (body mass index (BMI), waist circumference (WC), total body weight (TBW), total cholesterol (TC), LDL-c, HDL-c, fasting blood glucose (FBG), triglycerides (TG), and systolic and diastolic blood pressure (SBP, DBP)) were included in this analysis. Results: A total of 10 clinical trials was selected. Results demonstrate a heterogeneity of over 50% (p < 0.001). A mean difference was found for TC by AT (−23.70 mg/dL, p= 0.003) and An-T (3.91 mg/dL, p= 0.003); on HDL by CT (0.12 mg/dL, p = 0.004); on FBG by AT (−0.66 mg/dL, p = 0.02), CT (−1.42 mg/dL, p = 0.01); on DBP by AT (−0.79 mmHg, p = 0.02). Conclusions: There is a dominance of concurrent exercise over other types of exercise, considering the greater effectiveness and significance of the effect of seven MetS indicators, including HDL and fasting blood glucose, with a major effect size. Background/Objectives: Hypertension is strongly linked to changes in vascular function and lipid metabolism. This study aimed to examine the relationship between lipid profiles, various metabolic biomarkers, and pulse wave analysis in patients with hypertension. Methods: A group of 66 hypertensive patients, aged 64 ± 10 years, participated in pulse wave analysis utilizing an oscillometric device. Multiple lipid serum biomarkers were assessed, such as total cholesterol (TC), triglycerides (TG), and non-HDL cholesterol (non-HDL). Lipid balance index (LBI) was determined by considering TG, LDL, HDL levels, and lipid-lowering medications. Results: Notable correlations were observed for SBP, DBP, and early vascular aging (EVA) with lipid biomarkers. In addition to serum lipids, metabolic syndrome, insulin resistance, and non-alcoholic fatty liver disease (NAFLD) were significantly linked to pulse wave analysis variables. Multiple regression analysis showed that only TC continued to have a significant association with DBP. Conclusions: Total cholesterol, triglycerides, non-HDL cholesterol, and lipid balance index provide information about systolic and diastolic blood pressure, as well as early vascular aging in hypertensive patients. LBI offers valuable vascular insights in hypertensive individuals with cardiovascular risk factors, early vascular aging, insulin resistance, and NAFLD. The connection between metabolic biomarkers and pulse wave measurements in individuals with hypertension offers a comprehensive method for the early identification of vascular injury and could enhance the prediction of major cardiovascular events.
