Background/Objectives: Frailty is a multifactorial condition influenced by physical and psychosocial factors. Understanding longitudinal changes in these domains may guide prevention strategies. This study examines the relationship between frailty status, physical fitness, and psychosocial conditions in community-dwelling older adults using five-year longitudinal data. Methods: Participants were 52 out of 89 older adults who completed both baseline and five-year follow-up assessments (follow-up rate: 58.4%). Data were collected using 10 physical fitness indicators, the fitness age score (FAS), geriatric depression scale (GDS), Lubben social network scale short form (LSNS-6), and relevant items in the six Kihon Checklist (KCL) domains. Due to low prevalence of frailty, individuals with pre-frailty and frailty were combined into the frailty-risk group. Repeated measures ANOVA with sex as a covariate was conducted to compare groups. Logistic regression was used to identify baseline predictors of frailty status at five years. Statistical significance was set at p < 0.05. Results: GDS, LSNS-6, and KCL scores remained stable over five years. However, physical fitness significantly declined in several measures, including grip strength, vertical jump height, knee extension strength, functional reach, and FAS. A significant interaction for the timed up and go test showed that the robust group maintained function, while the frailty-risk group declined. Logistic regression identified KCL oral function as a significant predictor (OR = 5.331, 95% CI = 1.593–17.839, p = 0.007). Conclusions: Maintaining both oral function and physical fitness is vital for preventing frailty, even among health-conscious older adults. Proactive strategies may support healthy aging. In this paper, we focus on defending against distributed denial-of-service (DDoS) attacks in a low-earth-orbit (LEO) satellite network (LSN). To enhance the security of LSN, we propose the K-Bottleneck Minimize routing method. The algorithm ensures path diversity while avoiding vulnerable bottleneck paths, which significantly increases the cost for attackers. Additionally, the attacker’s detectability is reduced. The results show that the algorithm avoids the bottleneck paths that are vulnerable to attacks, improves the attacker’s cost by about 13.1% and 16.6% on average and median, and improves the detectability of attackers by 48.5% and 45.4% on average and median. The algorithm generates multiple non-overlapping inter-satellite paths, preventing the exploitation of bottleneck paths and ensuring better robustness and attack resistance. Full article

Myocardial infarction (MI) is characterized by the sudden reduction in myocardial blood flow and remains the leading cause of death worldwide. Because MI causes irreversible damage to the heart, discovering drugs that can limit the extent of ischemic damage is crucial. Liensinine (LSN) is a natural alkaloid that has exhibited beneficial effects in various cardiovascular diseases, including MI; however, its molecular mechanisms of action remain largely unelucidated. In this study, we constructed murine models of MI to examine the potential beneficial effects and mechanisms of LSN in myocardial ischemic injury. Murine models of MI in wild-type and cardiomyocyte-specific β-catenin knockout mice were used to explore the role of LSN and Wnt/β-catenin signaling in MI-induced cardiac injuries and inflammatory responses. The administration of LSN markedly improved cardiac function and decreased the extent of ischemic damage and infarct size following MI. LSN not only prevented excessive inflammatory responses but also inhibited the aberrant activation of Wnt/β-catenin signaling, two factors that are critically involved in the exacerbation of MI-induced injury. Our findings provide important new mechanistic insight into the beneficial effect of LSN in MI-induced cardiac injury and suggest the therapeutic potential of LSN as a novel drug in the treatment of MI. The integration of pedestrian movement analysis with Unmanned Aerial Vehicle (UAV)-based remote sensing enables comprehensive monitoring and a deeper understanding of human dynamics within urban environments, thereby facilitating the optimization of urban planning and public safety strategies. However, human behavior inherently involves uncertainty, particularly in the prediction of pedestrian trajectories. A major challenge lies in modeling the multimodal nature of these trajectories, including varying paths and targets. Current methods often lack a theoretical framework capable of fully addressing the multimodal uncertainty inherent in trajectory predictions. To tackle this, we propose a novel approach that models uncertainty from two distinct perspectives: (1) the behavioral factor, which reflects historical motion patterns of pedestrians, and (2) the stochastic factor, which accounts for the inherent randomness in future trajectories. To this end, we introduce a global framework named LSN-GTDA, which consists of a pair of symmetrical U-Net networks. This framework symmetrically distributes the semantic segmentation and trajectory prediction modules, enhancing the overall functionality of the network. Additionally, we propose a novel thermal diffusion process, based on signal and system theory, which manages uncertainty by utilizing the full response and providing interpretability to the network. Experimental results demonstrate that the LSN-GTDA method outperforms state-of-the-art approaches on benchmark datasets such as SDD and ETH-UCY, validating its effectiveness in addressing the multimodal uncertainty of pedestrian trajectory prediction. This paper proposes a green computing strategy for low Earth orbit (LEO) satellite networks (LSNs), addressing energy efficiency and delay optimization in dynamic and energy-constrained environments. By integrating a Markov Decision Process (MDP) with a Double Deep Q-Network (Double DQN) and introducing the Energy–Delay Ratio (EDR) metric, this study effectively quantifies and balances energy savings with delay costs. Simulations demonstrate significant energy savings, with reductions of up to 47.87% under low business volumes, accompanied by a minimal delay increase of only 0.0161 s. For medium business volumes, energy savings reach 26.75%, with a delay increase of 0.0189 s, while high business volumes achieve a 4.36% energy reduction and a delay increase of 0.0299 s. These results highlight the proposed strategy’s ability to effectively balance energy efficiency and delay, showcasing its adaptability and suitability for sustainable operations in LEO satellite networks under varying traffic loads. In recent years, LoRa technology has emerged as a solution for wide-area coverage IoT applications. Deploying a LoRa single-hop network on applications may be challenging in cases of network deployments that require the installation of linear sensor network topologies covering very large distances over unpopulated areas with limited access to cellular networks and energy grids. In such cases, multi-hop communication may provide better alternative solutions to support these challenges. This research aims to study the deployment of multi-hop linear sensor networks that are energy efficient. The focus will be on assessing the coverage, throughput, and energy consumption benefits that can be achieved and the related tradeoffs that have to be considered when using multi-hop solutions. Since monitoring systems in long-distance infrastructures may benefit from solutions based on multi-hop communication, we consider oil pipeline infrastructures in the Saudi Arabian desert as a case study. An analytical model is considered for estimating the above-stated parameters and evaluating the performance of the multi-hop LoRa WSN (MHWSN) against the single-hop LoRa WSN (SHWSN). In addition, the model is used to study the tradeoffs between throughput and energy consumption in different settings of MHWSNs. Scenarios of oil pipeline monitoring systems in Saudi Arabia are specified for studying the proposed multi-hop system’s performance. The obtained results show that when we have a large-scale network, such as an oil pipeline with medium traffic load requirements, multi-hop topologies may be an efficient deployment solution. Assessing the Impact of Multidisciplinary Collaboration on Quality of Life in Older Patients Receiving Primary Care: Cross Sectional Study

The growing aging population presents unique challenges in maintaining their quality of life (QoL), which encompasses physical, psychological, and social wellbeing. This study aimed to assess the impact of multidisciplinary collaboration on QoL among older patients receiving primary care. A cross-sectional study was conducted involving a purposive sampling of 200 participants aged 60 years and above from Primary Healthcare Centers in Al-Ahsa, Saudi Arabia, between March and May 2024. Data were collected using standardized tools: SF-36 for health-related QoL, Multidisciplinary Collaboration Evaluation Tool (MCET), and Lubben Social Network Scale (LSNS). Multivariate regression analyses were performed to examine the associations between multidisciplinary collaboration, social support, and QoL outcomes. Multidisciplinary collaboration exhibited a significant positive association with improvements in physical functioning (β = 2.35, p < 0.001), mental health (β = 3.01, p < 0.001), and general health perceptions (β = 2.12, p < 0.001). Key drivers of effective collaboration included effective communication (β = 0.48, p < 0.001), coordination (β = 0.42, p < 0.001), and patient involvement (β = 0.40, p < 0.001). Social support, particularly from friends (β = 0.33, p < 0.001) and family (β = 0.28, p < 0.001), was also a robust predictor of better QoL. Younger age, female gender, married status, and absence of chronic conditions were associated with greater QoL improvements from multidisciplinary care. Multidisciplinary collaboration and social support networks significantly enhance QoL among older primary care patients. Healthcare systems should prioritize developing collaborative care models, fostering interdisciplinary teamwork, and integrating strategies to promote social connectedness for the aging population. Ikigai—a Japanese concept that includes elements such as life’s purpose and meaning—has been reported to be associated with various systemic health conditions, such as the risk of developing physical dysfunction or death in older adults. However, there are no reports that comprehensively examine the psychological and social aspects of Ikigai. We attempted to clarify the characteristics of Ikigai by examining it from a biopsychosocial model using physical, psychological, and social perspectives through a cross-sectional study on sarcopenia, frailty and healthy life expectancy in a hilly and mountainous area of Japan. Koyadaira in Mima City, which is located in a hilly and mountainous region on Shikoku Island in Japan, was targeted. This cross-sectional study included 105 outpatients aged 65 and over, with an average age of 79.02 ± 6.91 years. Ikigai (self-rating score on a scale of 0 (no Ikigai) to 5 (the highest Ikigai)) participants’ level of physical activity (the Physical Activity Scale for the Elderly, PASE), degree of depression (the Geriatric Depression-15 Scale, GDS-15), cognitive function (the Mini-Mental State Examination, MMSE) and social isolation (the abbreviated Lubben Social Network Scale, LSNS-6) was assessed. Significant positive correlations were found between PASE and MMSE. The LSNS-6 significantly correlated with the MMSE and GDS-15. In a path model, out of four paths from PASE, GDS-15, MMSE, and LSNS-6 to Ikigai, the path from the GDS-15 alone was significant (correlation coefficient −0.271, p < 0.01). The adaptability of this model was good. This study indicates that depressive status has a large impact on Ikigai, along with physical, cognitive, and social conditions; thus, it is appropriate to consider that an affective psychological status, such as depressive symptoms, is a fundamental condition for having Ikigai. This paper aims to investigate the response and local buckling of locally sharp-notched C2700 brass circular tubes (LSN C2700 brass circular tubes) under cyclic bending loads. The study considers four different notch orientations (0°, 30°, 60°, and 90°) and five distinct notch depths (0.2, 0.4, 0.6, 0.8, and 1.0 mm). The results reveal that notch orientation and depth exert minimal impact on the moment–curvature relationship, leading to the formation of stable loops. The ovalization–curvature graphs demonstrate a trend of symmetry, serration, and growth with an increasing number of bending cycles. Additionally, larger notch orientations or smaller notch depths result in reduced ovalization. Furthermore, the double logarithmic coordinates of controlled curvature–number of cycles necessary to induce local buckling reveal five non-parallel lines representing different notch depths when the notch orientation is fixed. Finally, by adopting the formulas for smooth tubes and for locally sharp-notched 304 stainless steel circular tubes (LSN SS304 circular tubes), this study adjusts the related material parameters accordingly. These modifications effectively describe the controlled curvature–number of cycles necessary to induce local buckling for LSN C2700 brass circular tubes with different notch orientations and depths under cyclic bending, demonstrating reasonable agreement with the experimental results. Controlled human infection models are important tools for the evaluation of vaccines against diseases where an appropriate correlate of protection has not been identified. Enterotoxigenic Escherichia coli (ETEC) strain LSN03-016011/A (LSN03) is an LT enterotoxin and CS17-expressing ETEC strain useful for evaluating vaccine candidates targeting LT-expressing strains. We sought to confirm the ability of the LSN03 strain to induce moderate-to-severe diarrhea in a healthy American adult population, as well as the impact of immunization with an investigational cholera/ETEC vaccine (VLA-1701) on disease outcomes. A randomized, double-blinded pilot study was conducted in which participants received two doses of VLA1701 or placebo orally, one week apart; eight days after the second vaccination, 30 participants (15 vaccinees and 15 placebo recipients) were challenged with approximately 5 × 109 colony-forming units of LSN03. The vaccine was well tolerated, with no significant adverse events. The vaccine also induced serum IgA and IgG responses to LT. After challenge, 11 of the placebo recipients (73.3%; 95%CI: 48.0–89.1) and 7 of the VLA1701 recipients (46.7%; 95%CI: 24.8–68.8) had moderate-to-severe diarrhea (p = 0.26), while 14 placebo recipients (93%) and 8 vaccine recipients (53.3%) experienced diarrhea of any severity, resulting in a protective efficacy of 42.9% (p = 0.035). In addition, the vaccine also appeared to provide protection against more severe diarrhea (p = 0.054). Vaccinees also tended to shed lower levels of the LSN03 challenge strain compared to placebo recipients (p = 0.056). In addition, the disease severity score was lower for the vaccinees than for the placebo recipients (p = 0.046). In summary, the LSN03 ETEC challenge strain induced moderate-to-severe diarrhea in 73.3% of placebo recipients. VLA1701 vaccination ameliorated disease severity, as observed by several parameters, including the percentage of participants experiencing diarrhea, as well as stool frequency and ETEC severity scores. These data highlight the potential value of LSN03 as a suitable ETEC challenge strain to evaluate LT-based vaccine targets (NCT03576183). Full article

Objectives: Social isolation is recognised as a risk factor in the inflammatory process. This study explored the association between social isolation and the Dietary Inflammatory Index (DII) in community-dwelling older persons. Methods: This cross-sectional pilot study recruited 107 community-dwelling people aged over 55 years living in the Australian Capital Territory. Participants completed an extensive food frequency questionnaire and provided anthropometric and sociodemographic data. Social isolation was evaluated using the Lubben Social Network Scale (LSNS). Diet quality was assessed using DII. Results: Average age was 70.1 (±8.61) years and 62.8% were female. The average DII score was −1.10 (±1.21), indicating an anti-inflammatory diet. Higher LSNS was associated with lower DII (b (95% CI) = −0.041 (−0.066, −0.17); p < 0.01) and was positively influenced by the number of people in household (b (95% CI) = 5.731 (2.336, 9.127); p = 0.001). Conclusion: Increased risk of social isolation was associated with an increased tendency towards a more inflammatory diet. Reducing social isolation may decrease the inflammatory component of dietary intake for older persons living independently in the community. Full article

In this paper, nitride coatings were prepared using three preparation processes, namely laser surface nitriding (LSN), physical vapor deposition (PVD), and plasma ion implantation (PII). Their microstructure, microhardness, tribological behavior, and high-temperature oxidation characteristics were compared. The experimental results revealed that nitrided coatings were successfully prepared using the three methods. However, a comparison of these data shows that the LSN coating exhibited superior comprehensive performance. It achieved the maximum thickness within the shortest preparation time: the thickness was about 280 μm and the deposition rate of the LSN method was 2250 and 90,000 times higher than those of the PVD and PII methods. Nitrides have high hardness, but the carrying capacity could be attributed to the thickness of the coatings: the PVD coating could withstand a force of 500 g, while the PII coating only withstood a force of less than 25 g. In addition, as hardness is the most important factor for excellent wear resistance, the average volumetric wear rate of the LSN and PVD coatings was about 9 × 10−6 mm3/m·N, and their relative wear resistance was 49.2 times that of Ti6Al4V. Meanwhile, the excellent bond between the LSN coating and the substrate was evidenced by a high-temperature oxidation test during a rapid heating–cooling cycle. Full article

Psychological Determinants of Vaccination Readiness against COVID-19 and Seasonal Influenza of the Chronically Ill in Primary Care in Germany—A Cross-Sectional Survey

Vaccines against COVID-19 and influenza are highly recommended for the chronically ill. They often suffer from co-morbid mental health issues. This cross-sectional observational study analyzes the associations between depression (PHQ-9) and anxiety (OASIS) with vaccination readiness (5C) against COVID-19 and influenza in chronically ill adults in primary care in Germany. Sociodemographic data, social activity (LSNS), patient activation measure (PAM), and the doctor/patient relationship (PRA) are examined as well. Descriptive statistics and linear mixed-effects regression models are calculated. We compare data from n = 795 study participants. The symptoms of depression are negatively associated with confidence in COVID-19 vaccines (p = 0.010) and positively associated with constraints to get vaccinated against COVID-19 (p = 0.041). There are no significant associations between symptoms of depression and vaccination readiness against influenza. Self-reported symptoms of a generalized anxiety disorder seem not to be associated with vaccination readiness. To address confidence in COVID-19 vaccines among the chronically ill, targeted educational interventions should be elaborated to consider mental health issues like depression. As general practitioners play a key role in the development of a good doctor/patient relationship, they should be trained in patient-centered communication. Furthermore, a standardized implementation of digital vaccination management systems might improve immunization rates in primary care. Full article

Large-scale low Earth orbit satellite networks (LSNs) have been attracting increasing attention in recent years. These systems offer advantages such as low latency, high bandwidth communication, and all terrain coverage. However, the main challenges faced by LSNs is the calculation and maintenance of routing strategies. This is primarily due to the large scale and dynamic network topology of LSN constellations. As the number of satellites in constellations continues to rise, the feasibility of the centralized routing strategy, which calculates all shortest routes between every satellite, becomes increasingly limited by space and time constraints. This approach is also not suitable for the Walker Delta formation, which is becoming more popular for giant constellations. In order to find an effective routing strategy, this paper defines the satellite routing problem as a mixed linear integer programming problem (MILP), proposes a routing strategy based on a genetic algorithm (GA), and comprehensively considers the efficiency of source or destination ground stations to access satellite constellations. The routing strategy integrates ground station ingress and exit policies and inter-satellite packet forwarding policies and reduces the cost of routing decisions. The experimental results show that, compared with the traditional satellite routing algorithm, the proposed routing strategy has better link capacity utilization, a lower round trip communication time, and an improved traffic reception rate. Cannabidiol (CBD) is a promising natural agent for treating psoriasis. CBD activity is attributed to inhibition of NF-kB, IL-1β, IL-6, and IL-17A. The present study evaluated the anti-psoriatic effect of cannabidiol in lipid-stabilized nanoparticles (LSNs) using an imiquimod (IMQ)-induced psoriasis model in mice. CBD-loaded LSNs were stabilized with three types of lipids, Cetyl alcohol (CA), Lauric acid (LA), and stearic-lauric acids (SALA), and were examined in-vitro using rat skin and in-vivo using the IMQ-model. LSNs loaded with coumarin-6 showed a localized penetration depth of about 100 µm into rat skin. The LSNs were assessed by the IMQ model accompanied by visual (psoriasis area severity index; PASI), histological, and pro-psoriatic IL-17A evaluations. Groups treated with CBD-loaded LSNs were compared to groups treated with CBD-containing emulsion, unloaded LSNs, and clobetasol propionate, and to an untreated group. CBD-loaded LSNs significantly reduced PASI scoring compared to the CBD emulsion, the unloaded LSNs, and the untreated group (negative controls). In addition, SALA- and CA-containing nanoparticles significantly inhibited IL-17A release, showing a differential response: SALA > CA > LA. The data confirms the effectiveness of CBD in psoriasis therapy and underscores LSNs as a promising platform for delivering CBD to the skin. Full article
