Trajectories of Cancer Antigen 125 (CA125) Within 3 and 6 Months After the Initiation of Chemotherapy Treatment for Advanced Ovarian Cancer and Clinical Outcomes: A Secondary Analysis of Data from a Phase III Clinical Trial

Background: A single measurement or a summary of a limited number of measurements of CA125 was considered in the prediction of clinical outcomes for patients with ovarian cancer. We aimed to identify the classes of patients with advanced ovarian cancer based on their CA125 trajectory and to investigate the heterogeneity of clinical outcomes among the patients in the different classes. Methods: CA125 trajectory classes were identified by latent-class mixed models based on values collected within 3 and 6 months post-treatment for 819 women with advanced ovarian cancer enrolled in a randomized trial. Results: Based on their CA125 values during the first 6 months of treatment, the patients with low CA125 levels at baseline that remained low during treatment had the best clinical outcome (a median survival of 83 months and a progression-free survival of 34 months). In contrast, the patients with high CA125 values at baseline with a modest decrease during treatment had the highest risk of death and progression (hazard ratio [95% confidence interval]: 4.83 [3.56, 6.54] for overall survival and 5.15 [3.87, 6.87] for progression-free survival). Conclusions: Longitudinal trajectories of CA125 may provide more direct information for the prognoses of patients with advanced ovarian cancer undergoing chemotherapy treatment. Full article

The analysis of the vulnerability of vernacular buildings to climatic hazards is nowadays a subject of significant importance due to the consequences of climate change. This study assesses the vulnerability of vernacular buildings to three climatic hazards (heavy rains, strong winds and high heat) in the Kara region to identify the vulnerable parts of these constructions that require reinforcement. It is based on PTVA (Papathoma Tsunami Vulnerability Assessment), a multi-hazard analysis methodology, which uses vulnerability indicators. It focuses on the Kabiyè and Nawdeba peoples, who are the major ethnic groups in the region. Focus groups with the population, interviews with professionals and a series of surveys of 125 households in the visited territories enabled us to identify, firstly, the types of vernacular constructions in the region, the climatic hazards that occur there and the indicators that affect the vulnerability of the constructions. Secondly, we calculated the vulnerability index for each type of construction to the three climatic hazards. The vulnerability index of Kabiyè vernacular architecture (KVA) to heavy rain, high heat and strong wind is 0.379, 0.403 and 0.356, respectively. The Nawdéba vernacular architecture (NVA) vulnerability score is 0.359 for heavy rain, 0.375 for high heat, and 0.316 for strong wind. The index of vulnerability to heavy rain, high heat and strong wind for contemporary architecture (CA), as we term the current state of evolution of these two forms of architecture, is 0.499, 0.522 and 0.456, respectively. This study reveals that contemporary architecture (CA) in the Kara region, regardless of the type of hazard considered, is the most vulnerable construction model in the region. It also highlights the indicators that accentuate the vulnerability of vernacular constructions. Regardless of the type of construction, special attention must be paid to features such as roof style (roof slope, shape and material) and building style (form and state of maintenance of the building) to increase the resilience of buildings to climatic hazards. Background/Objectives: Carbohydrate antigen 125 (CA125) is a glycoprotein commonly overexpressed in epithelial ovarian cancer and widely recognized as a tumor marker. However, elevated CA125 levels are also observed in various non-malignant conditions, including diseases affecting mucosal surfaces, pleural or peritoneal effusions, cirrhosis (with or without ascites), endometriosis, uterine fibroids, adenomyosis, pelvic inflammatory disease, and pregnancy. This review aims to explore the role of CA125 in non-malignant serous effusions, highlighting its diagnostic and prognostic potential beyond the realm of oncology. Methods: A comprehensive literature search was conducted across multiple databases and clinical trial registries. Eligible studies included full-text original research articles, reviews, and case reports published in English over the past 10 years. Inclusion criteria were limited to studies involving human subjects and focused on the role of CA125 in non-malignant serous effusions. Results: CA125 is produced by coelomic epithelial cells lining the ovary, pleura, pericardium, and peritoneum. Its serum concentration is not significantly influenced by age, body weight, or renal function, even in the advanced stages of the disease. In peritoneal conditions, CA125 is synthesized by mesothelial cells and serves as a potential marker of peritoneal involvement. The prevailing pathophysiological mechanism suggests that mechanical stretching of mesothelial cells due to ascitic pressure stimulates CA125 release. Similarly, in heart failure, mesothelial cells of the pericardium produce CA125, which correlates with congestion severity, supports risk stratification, and may inform diuretic therapy. Conclusions: While a threshold of 35 U/mL is established for malignancy, no standardized cutoff exists for CA125 in non-malignant conditions. The utility of CA125 measurement in peritoneal, pleural, or pericardial effusions—and cardiovascular diseases such as acute heart failure—for purposes of differential diagnosis, treatment guidance, or prognostication warrants further investigation through prospective clinical trials. As part of the “2nd Training Conference on Heart Failure and Atrial Fibrillation for Residents”, held in Madrid in November 2024, a collaborative initiative was launched to address the most common practical challenges in the management of heart failure (HF) in daily practice. This document is the result of the joint efforts of residents from various hospitals nationwide, in collaboration with senior physicians with extensive HF expertise and members of the Working Group of the Spanish Society of Internal Medicine. Our aim is to provide a useful tool that promotes learning and collaboration among professionals interested in this field. The structure of this document is based on a compilation of the most interesting and challenging questions raised during the conference. Each question is addressed with a concise and practical response, supported by updated references to ensure scientific rigor and facilitate consultation. Full article

This study investigates the suppression effect of ultrafine CaCO3 powder on gas explosions through a series of experiments conducted in a medium-scale explosion tube (9.6 m in length) with varying concentrations of ultrafine CaCO3. The gas explosion suppression concentration was established at 9.5%. The results indicate that, at concentrations of 125 g/m3 and 300 g/m3, ultrafine CaCO3 powder significantly reduced the flame propagation speed of the gas explosion. Among the concentrations that did not fully suppress the gas explosion, 20 g/m3 effectively mitigated the flame propagation speed and overpressure throughout the entire process. The concentration of 75 g/m3 demonstrated a suppression–promotion–suppression–promotion pattern regarding the flame propagation rate, with significant fluctuations observed throughout the process. While the maximum suppression rate reached 60.9%, the maximum promotion rate was 131.12%. At 10 g/m3, the flame propagation rate initially increased before decreasing, with the most effective suppression observed within the first 6.6 m, where flame propagation suppression increased rapidly from 58.13% to 74.82%, and the peak explosion overpressure was reduced by up to 62.94%. These findings contribute valuable insights for the development of effective gas explosion suppression strategies in mining environments. Full article

This study presents a comprehensive evaluation of the biological properties of aqueous and aqueous–ethanolic leaf extracts from Malus domestica, Prunus armeniaca, and Prunus cerasus, which are plant waste materials. Phytochemical profiles were analyzed using HPLC, and antioxidant potential was assessed via DPPH, ABTS, FRAP, and superoxide dismutase (SOD) activity assays. Extracts showed concentration-, plant-, and extract type-dependent radical scavenging effects exceeding 80%, significant Fe3+ ion reduction, and up to 40% enhancement of SOD activity. In vitro studies on HDF and HaCaT cells revealed reduced intracellular ROS levels, indicating antioxidant potential. Cytotoxicity assays (Alamar Blue, Neutral Red) demonstrated increased skin cell viability by up to 35% at ≤50 or 125 µg/mL, while higher doses reduced viability by up to 70%, depending on the extract. Antibacterial activity varied among plant species and was effective against seven bacterial strains. ELISA assays showed anti-inflammatory effects, with IL-1β and IL-6 levels reduced by 48% and 40%, respectively, and IL-10 increased by up to 27%. These findings suggest that the tested leaf extracts support both enzymatic and non-enzymatic antioxidant defense mechanisms and may be valuable as functional ingredients in dermatological or cosmetic applications. Background: Serum biomarkers such as Carcinoma Antigen 125 (CA125) and Human Epididymis Protein 4 (HE4) are widely used in the diagnosis and prognosis of gynecological malignancies. Serum biomarkers such as CA125 and HE4 represent essential tools in improving early detection, risk stratification, and therapeutic decision-making for gynecological malignancies. However, their role in identifying uterine sarcomas remains debated. This systematic review and case series aims to examine the diagnostic and prognostic significance of CA125 and HE4 in uterine sarcomas. Methods: A systematic review was performed on studies investigating serum CA125 and HE4 levels in uterine sarcomas. A case series of all uterine sarcomas treated at the Campus Bio-Medico Gynecology Unit of Rome from 2010 to 2020 was analyzed. Results: The analysis of the 11 selected studies allowed us to investigate the role of CA125 in uterine sarcomas. No studies analyzing the role of HE4 in monitoring the disease were found. A total of 16 patients with confirmed uterine leiomyosarcoma were included in our case series. Conclusions: Neither CA125 nor HE4 can currently be considered definitive biomarkers for the diagnosis of uterine leiomyosarcomas. However, they may serve as useful adjuncts in the differential diagnosis between leiomyomas and leiomyosarcomas, particularly in reproductive-age patients. Full article

Background: Post-activation performance enhancement (PAPE) protocols are increasingly being used to acutely boost strength and power in resistance training. The objective of this systematic review was to determine how PAPE interventions influence barbell velocity in the bench press among trained individuals and address the following research question: which conditioning activities (CAs), rest intervals, and loading strategies most effectively enhance bench press velocity parameters under PAPE conditions? Methods: This systematic review, conducted according to PRISMA guidelines, searched PubMed, Scopus, SpringerNature, EBSCO, and Web of Science up to 31 January 2025 for studies involving healthy adults (18–50 years) that employed bench press or related conditioning activities intended to induce PAPE, and assessed barbell velocity (peak or mean). From pool of 111 records, 7 studies (total n = 125 males, 23–29 years old, ≥2-year training experience) met the inclusion criteria. The methodological quality of included studies was moderate (5/10 on the PEDro scale). Results: Submaximal loads (70–80% 1RM) applied for one to three short sets, with rest periods of around four to five minutes, frequently improved peak velocity (by up to 7%) and peak power (by up to 15.9%). Conversely, heavier loads (>85% 1RM) and insufficient rest tended to offset these benefits due to fatigue. Blood flow restriction or ischemic preconditioning strategies produced positive velocity outcomes mainly at lower loads (20–50% 1RM). Conclusions: These findings suggest that PAPE interventions can enhance bench press barbell velocity in trained individuals. Effectiveness, however, depends on training status, volume, load intensity, and recovery intervals. Future randomized controlled trials with larger samples, standardized reporting, and the inclusion of female athletes are recommended to refine and generalize PAPE applications for upper-body strength and power development. Background and Objectives: This study aimed to evaluate the diagnostic potential of systemic inflammatory indices such as Systemic Inflammation Response Index (SIRI) and Systemic Inflammatory Response (SIR). These were assessed in combination with CA 125 to distinguish ovarian carcinoma (OC) from borderline ovarian tumors (BOT) in patients with suspicious adnexal masses. Materials and Methods: A retrospective multicenter observational study including patients undergoing surgery for suspected ovarian neoplasms was conducted. Inclusion criteria required preoperative blood sampling for inflammatory markers and CA 125. SIR-125 and SIRI-125 were developed by combining SIR and SIRI with CA 125 levels. Diagnostic performance was assessed using ROC curve analysis and linear regression models. Results: A total of 63 patients (42 BOT, 21 OC) were analyzed. OC patients exhibited significantly higher SIR-125 and SIRI-125 values (p < 0.001). ROC analysis demonstrated good diagnostic accuracy, with AUCs of 0.83 (SIR-125) and 0.82 (SIRI-125). SIR-125 showed higher specificity (0.83), while SIRI-125 had superior sensitivity (0.86). Conclusions: SIR-125 and SIRI-125 enhance diagnostic differentiation between OC and BOT, providing a simple, cost-effective preoperative tool. Future prospective studies are needed to validate these findings in broader patient populations. Full article

The Role of Antigen Carbohydrate 125 in Modulating Soluble ST2: Prognostic-Related Effects in Acute Heart Failure

Background: Acute heart failure (AHF) is a complex syndrome associated with high mortality and hospital readmissions, characterized by volume overload and inflammation. Soluble ST2 (sST2) and antigen carbohydrate 125 (CA125) are emerging biomarkers that reflect these processes and may interact to influence long-term outcomes in AHF patients. This study aims to examine the prognostic relationship between sST2 and CA125 in predicting mortality and heart failure (HF)-related hospitalizations in patients with decompensated heart failure. Methods: In a cohort of 635 patients with AHF, we investigated whether the prognostic value of sST2 varies according to CA125 levels (≤35 vs. >35 U/mL). The endpoints were: (a) time to all-cause death, and (b) the combination of time to death or new HF admission. Results: This study of EAHFE registry data shows that the association between sST2 and long-term adverse outcomes (mortality and HF hospitalizations) in patients with AHF was differentially influenced by CA125 concentrations (p-value for interactions = 0.031 and 0.029, respectively). Higher sST2 was associated with the risk of death and the combined risk of death/HF readmission when CA125 was >35 U/mL [HR = 1.02 (CI 95%: 1.01–1.04), p = 0.006 and 1.02 (CI 95%: 1.01–1.03); p = 0.013 per increase in 10 ng/mL, respectively], but not when CA125 was ≤35 U/mL. Conclusions: This study highlights the prognostic interaction between sST2 and CA125 in AHF. Elevated sST2 predicts poor outcomes mainly in patients with high CA125 levels (>35 U/mL), suggesting CA125’s role in modulating inflammatory activity in HF. Further research is needed. Full article

Ovarian cancer (OC) is one of the leading causes of gynecological cancer-related death worldwide. Late diagnosis at advanced stages of OC is the reason for a higher mortality rate. Earlier diagnosis and proper treatment are important for improving the prognosis of OC patients. Biosensors offer accurate, low-cost, rapid, and user-friendly devices that can be employed for the detection of OC-specific biomarkers in the early stage. Therefore, it is important to consider the potential biomarkers in the biological fluids to confirm the OC prognosis. Out of many biomarkers, the most commonly tested clinically is cancer antigen 125 (CA-125). However, CA-125 is considered to be a poor biomarker for OC diagnosis. Several biosensing methods were developed for the sensitive and quantitative detection of each biomarker. In abnormal expression in OC patients, nucleic acids, enzymes, cells, and exosomes are used as target biomarkers for the construction of biosensors. This review focuses on the development for the detection of various biomarkers using multiple biosensing methods. Here, we describe the origin and the significance of OC-associated biomarkers, the working principle of biosensors, and the classification of biosensors based on their recognition elements and signal transducers. The modes of detection and sensitivity of the sensors are discussed. Finally, the challenges in the fabrication, obstacles in the clinical application, and future prospects are discussed. Large-Cell Neuroendocrine Carcinoma of the Cervix: Case Report and Literature Review

Background and clinical significance: Large-cell neuroendocrine carcinoma (LCNEC) of the cervix is considered a rare type of cancer: it represents <1% of invasive cervical cancers. The optimal treatment protocol is not fully established because of its rarity and diagnostic challenges. Case Presentation: A 72-year-old Asian female presented to our outpatient clinic with postmenopausal vaginal spotting for 1 month. Vaginal sonography revealed a cervical tumor of 2.7 cm in diameter with hypervascularity. Tumor markers such as CA 125, CA 19-9, carcinoembryonic antigen, and squamous cell carcinoma antigen all showed no abnormality. Due to high suspicion of cervical cancer, a pap smear and endocervical curettage were performed and confirmed the diagnosis of LCNEC. A positron emission tomography–computed tomography scan demonstrated a glucose hypermetabolic lesion in the mid-pelvic region, localized to the uterus, consistent with LCNEC. Surgery with radical hysterectomy, bilateral salpingo-oophorectomy, and bilateral pelvic lymph node dissection was performed. The patient was finally diagnosed with pT1b2N1mi, FIGO IIIC1. Immunohistochemical stain shows that the neoplastic cells were CK (+), p63 (−), p16 (−), CEA (−), vimentin (−), ER (−), WT-1 (−), p53 (−), and CD56 (+), with a high Ki67 index (75%). Concurrent chemotherapy with cisplatin and radiotherapy was performed. Four cycles of etoposide and cisplatin were planned. A 3-month follow-up of this patient revealed stable tumor marker levels. Conclusions: This case highlights the diagnostic challenges and aggressive nature of LCNEC of the cervix, emphasizing the need for a standardized treatment approach to improve patient outcomes. Predictive Factors of Non-Elevation of Carcinoembryonic Antigen 125 in Acute Heart Failure

This study aims to analyze the factors associated with the lack of carbohydrate antigen 125 (CA-125) elevation in cases of acute heart failure (HF) decompensation. This retrospective study was conducted on 3167 consecutive patients admitted for acute HF in the cardiology department of a referral hospital (June 2019 to June 2024). Admissions from outpatient clinics (n: 1018) and transfers from other hospitals (n: 752) were excluded. The variables of interest included clinical, echocardiographic, therapeutic, and analytical factors. Low CA-125 levels were defined as values ≤ 50 U/mL. A total of 1397 patients were included, of whom 515 had normal CA-125 levels and 882 had elevated levels. Clinically, independent predictors of low CA-125 were sinus rhythm on electrocardiogram (OR: 1.42, 95% CI: 1.12–1.64; p: 0.003) and sleep apnea–hyponpnea syndrome (OR: 1.76, 95% CI: 1.15–2.70; p: 0.009). Echocardiographically, inferior vena cava collapse greater than 50% with inspiration was associated with low CA-125 (OR: 1.78, 95% CI: 1.19–2.69; p = 0.005), as well as with non-severe right ventricular dysfunction. (OR: 2.42; IC95%: 1.39–4.20; p: 0.002). Analytically, elevated NT-proBNP levels were associated with elevated CA-125 levels (OR: 0.99; IC95%: 0.99–0.99; p: 0.006). Survival was higher in the group with CA-125 ≤ 50 U/mL (p: 0.019). Conversely, as CA-125 values increased, mortality also rose. In conclusion, the absence of CA-125 elevation in patients admitted for acute HF is associated with sinus rhythm, sleep apnea–hyponpnea syndrome, low NT-proBNP levels, and inferior vena cava collapse greater than 50% with inspiration. Evapotranspiration-Based Irrigation Management Effects on Yield and Water Productivity of Summer Cauliflower on the California Central Coast

Improvements in irrigation water productivity constitute an ongoing effort globally. In California, growers are under regulatory pressure to stabilize groundwater levels and reduce nitrate leaching, partially, by further improvements in irrigation optimization. Evapotranspiration (ET)-based methods can inform crop water requirements and boost irrigation efficiency, but in practice, they can be challenging for farmers to implement, especially in vegetable systems. Irrigation field trials were conducted near Salinas CA in 2018 and 2019 to evaluate the crop coefficient model employed by the CropManage ET-based irrigation decision support system (DSS) for summer cauliflower (Brassica oleracea var. botrytis cv. Symphony) and investigate potential water savings through improved irrigation scheduling. Overhead sprinklers were used for crop establishment, and surface drip was used subsequently. A randomized complete block design was used to administer treatments near 50, 75, 100, and 150% of crop evapotranspiration (ET) during the drip period with an added treatment at 125% in 2019. Water requirement for the 100% treatment was determined by the CropManage DSS model based on crop coefficients derived from fractional canopy cover. Deliveries to remaining treatments were scaled proportionally. The total yield and irrigation productivity were maximized by the 100% treatment both years with total applied water ranging from 275 to 300 mm. At present, the reported water application for summer cauliflower averages 465 mm in the region. Hence, implementing ET-based irrigation scheduling, administered through the CropManage DSS, could reduce water use in summer cauliflower by an average of 30% relative to current practices and serve to enhance groundwater management while maintaining crop returns. Cytosolic and Nucleosolic Calcium-Regulated Long Non-Coding RNAs and Their Target Protein-Coding Genes in Response to Hyperosmolarity and Salt Stresses in Arabidopsis thaliana

Long non-coding RNAs (lncRNAs) are involved in plant biotic and abiotic stress responses, in which Ca2+ also plays a significant role. There is diversity in the regulation of different gene expressions by cytosolic Ca2+ ([Ca2+]cyt) and nucleosolic Ca2+ ([Ca2+]nuc). However, no studies have yet explored the interrelationship between lncRNAs and calcium signaling, nor how calcium signaling regulates the expression of lncRNAs. Here, we use transgenic materials PV-NES and NLS-PV, which simulate [Ca2+]cyt- and [Ca2+]nuc-deficient mutants, respectively, and wild type (WT) materials in response to hyperosmolarity (250 mM sorbitol) or salt stresses (125 mM NaCl) at different time points to obtain RNA-seq data, respectively. Then, we proceed with the screening of lncRNAs, adding 688 new lncRNAs to the known Arabidopsis lncRNA database. Subsequently, through the analysis of differentially expressed lncRNA genes, it was found that cytosolic or nucleosolic calcium signals have distinct regulatory effects on differentially expressed lncRNAs (DElncRNAs) and differentially expressed protein-coding genes (DEPCGs) treated with high-concentration NaCl and sorbitol at different times. Furthermore, through weighted correlation network analysis (WGCNA), it is discovered that under hyperosmolarity and salt stresses, lncRNA-associated PCGs are related to the cell wall structure, the plasma membrane component, and osmotic substances through trans-regulation. In addition, by screening for cis-regulatory target PCGs of Ca2+-regulated lncRNAs related to osmotic stress, we obtain a series of lncRNA-PCG pairs related to water transport, cell wall components, and lateral root formation. Therefore, we expand the existing Arabidopsis lncRNA database and obtain a series of lncRNAs and PCGs regulated by [Ca2+]cyt or [Ca2+]nuc in response to salt and hyperosmolarity stress, providing a new perspective for subsequent research on lncRNAs. We also explore the trans- and cis-regulated target PCGs of lncRNAs regulated by calcium signaling, providing new insights for further studying salt stress and osmotic stress. Full article
