First, I congratulate the authors for the manuscript. It is well written, interesting and readable. Consider the suggestions below, just as a way to contribute and refine the presentation of the report.
Overall, the language needs revision to refine some terms.
Line 27: Replace "Salmonella sp." with "Salmonella spp."; Replace "Shigella sp." with "Shigella spp.".
Keywords are too long. Some are found in the title. Suggestion: AIDS, diarrhea, enteric pathogens, molecular diagnosis, opportunistic infections.
Line 43: Replace "well-trained personnel". Suggestion: specific skills.
Line 51: Cryptosporium and Enterocytozoon must be abbreviated. Also, species names should be in italics.
Line 55-56: Replace "severe headache, or weight loss" with "severe headache and weight loss".
Line 56-58: The phrase is confusing. It needs revision to understand the content properly.
The patient's history is too detailed, especially given the purpose of the study clearly described in the title. Much of the history can be summarized, for greater textual fluidity.
Line 95-109: In this paragraph abbreviate the genus of species mentioned earlier in the text. Also, replace "sp." with "spp." for standardization,

Reply: thanks for spotting this inconsistency. The sentence has been rephrased as “The patient had a history of poor adherence to the antiretroviral therapy (ART, 300 mg tenofovir, 200 mg emtricitabine, 300 mg atazanavir, and 100 mg ritonavir once daily) but declared adherence at time of admission, which was well tolerated”.

Reply: Genera abbreviations have been introduced in current lines 101 and 103. The species Cyclospora cayetanensis, Cystoisospora belli, Giardia duodenalis, Entamoeba histolytica, Entamoeba dispar, and Blastocystis sp. have been described in full as this is the first time that they are mentioned in the text. Finally, please note that the correct naming of Blastocystis (previously know and Blastocystis hominis) is Blastocystis sp., not Blastocystis spp. See Stensvold et al. Trends Parasitol. 2007;23(3):93-6. doi: 10.1016/j.pt.2007.01.004.
