Little is known about functional eye movement disorders (FEMDs). We present the case of a patient who was brought to the emergency department after an episode of unexplained loss of consciousness followed by horizontal diplopia in any gaze direction, and who was finally diagnosed with functional convergence insufficiency after meticulous work-up. The purpose of this case report is to recall a rare presentation of a FEMD to neurologists, psychiatrists and ophthalmologists. This case illustrates the importance of knowledge about FEMDs in the neurological clinic and of close interdisciplinary collaboration, in order to address the patient’s problem as efficiently as possible without detours. Probably half of the multiple sclerosis (MS) patients worldwide have progressive MS (PMS) and seek therapy beyond prevention of inflammatory bouts. Therapy plans in PMS have to focus on improvement of abilities in individual problem constellations. Standardized assessment methods in MS beyond the expanded disability status scale (EDSS) have been developed. Recent studies show positive effects for activating therapies in immobile MS patients. Fair evidence exists for several nonpharmacological therapies. Some data indicate activation of neurotrophic factors and neuroplasticity during therapy. Few medication strategies including corticoid pulses are at hand for PMS. Combined pharmacological and multimodal symptomatic approaches promise increased efficacy. Complex inpatient therapy schemes may be of special value for severely affected patients. Full article

Children and adolescents with refugee experiences. The number of accompanied and unaccompanied minor asylum seekers in Switzerland has increased in recent years. Numerous studies, mostly from abroad, show that children and adolescents with refugee experiences are at high risk of mental illness. Events in their country of origin, the escape itself and the adjustment to the country of arrival can be associated with traumatic experiences and impede the mobilisation of resources. This article focuses on the housing of children and adolescents who have experienced displacement in the country of arrival and shows, using various examples and with regard to current international research, that the housing of children, adolescents and their families often does not take place in a way that is appropriate to their needs and age. Some stress factors in the country of arrival can represent a high psychosocial burden and a challenge for integration. Participation in social life is hindered for new arrivals in many regards. Accompanied and unaccompanied minor asylum seekers are evidently equally affected by stress and risk factors. Resources can be activated through suitable support measures and targeted integration measures. Full article

Open doors in the acute treatment of substance dependence. The concept of “open doors” in in-patient psychiatry has fortunately increased in relevance in German-speaking regions in recent years. Although there is only little evidence supporting treatment in closed wards, this approach is often pursued in the management of substance dependence as to avoid relapse or treatment discontinuation. Based on current literature, however, it cannot be deduced that this restrictiveapproach benefits patients or the course of treatment. There are even indications that closed treatment might be associated with negative effects. Although study results remain unsatisfactory and methodologically clean studies are urgently required, this context does not justify continuing the treatment of substance dependence in a closed setting.
