Intracranial vascular hypertension revealing a neuroblastoma of the adrenal gland in a young Togolese. The authors report a neuroblastoma of the left adrenal gland revealed by swelling of the skull and gross, superinfected exophthalmia of insidious onset in the context of intracranial hypertension, with violent headaches and permanent irritability. The significance of this observation lies in the histological diagnosis, the rarity of such cases in a subcontinent where, 50 years after independence, acute poverty restricts access to technology in the health services. Swiss and Indian professionals have joined forces to develop better strategies for improving care of people with intellectual disability (ID). The purpose of this research is to investigate the relationship between behavioural disorders, the feelings of satisfaction that educators have about the care process, and educators’ perceived effectiveness of the care for people with ID. In addition, we investigated the relationship between behavioural disorders and various life skills in people with ID. Subjects included 27 Indian adults with ID living in an institution in India. Professional educators of these adults with ID filled out visual analogue scales on their satisfaction with and perceptions of the effectiveness of care, and provided an assessment of the behavioural disorders of the subjects. The Psychoeducational Profile Revised (PEP-R; Schopler, 1990) was then used to assess subjects’ social, behaviour and cognitive skills. We observed significant negative correlations between motor stereotypies and PEP-R subscales “relationships/emotions” and “play / interest in materials”. A significant positive correlation was found between the “inappropriate language” behavioural disorder and educators’ satisfaction of the care. Possible implications of these results are discussed. ADHD adults’ perception of their self-esteem. Problems with self-esteem have proven to be a frequent and central issue for patients with ADHD. The responsible pathogenic mechanisms of ADHD are significantly different from other phenomenologically similar forms of affected self-esteem e.g. classic narcissistic disturbances. The takeover of external judgements and psychodynamic developments are involved as well as specific forms of seemingly logical but false conclusions by the ADHD-patient himself in respect to his worth. Furthermore, the nature and significance of negative hyper-focusing are discussed, including new conceptualisations from a clinical point of view. A number of specific ADHD-mechanisms were detected by integrating neurobiological factors and clinical observations. They are presented in combination with an adaptive model of ADHD, that contributes to the clinical understanding of ADHD-specific problems with self-esteem. Finally, corresponding general conclusions for the treatment of patients with ADHD are drawn. Full article

To fail in the postmodern society – active resignation in psychotherapy. In contrary to a crisis, failing is irreversible. To fail claims for renouncement. Active resignation accepts the borders of a human being but tries to seize what is possible and facilitates a restart. To fail often feels like being offended. A postmodern culture of individualism holds a human being responsible for success and failure but cannot release him from the fact that his self-esteem depends on the acceptance of his fellow men. All the more, an individual failing can be very offending. After a short introduction, this article examines the following three points: 1. Social background and its consequences for modern failing; 2. To be hurt and the defence of shame; 3. Therapeutic attitude towards failing. Full article
