The Review of Genetic Modifying Factors of Cystic Fibrosis Phenotype: A Challenge for Modern Medicine is a precious work for those who want to understand the genetic changes that underlie the pathophysiological changes in Cystic Fibrosis. The authors addressed all the genetic facets of this disease with a vital prognosis from a young age. From my point of view, this review required intense research work to capture the correlation between genotypes and phenotypes, the identification of gene modifiers, and the environmental factors involved in this multifactorial disease with multiple phenotypes. Genetic analysis will be of great interest in changing the vital prognosis of patients receiving early treatment.

The main aim of the paper is to make a review of literature data about the factors that may 86 influence the phenotype of patients with cystic fibrosis, providing a perspective on the role of modifier genes intervention (epistasis), without minimizing the role of environmental factors. 

I think this is an interesting and well-organized paper.

The topic is quite original.

The paper is well written and the text is clear and easy to read.

The conclusions are consistent with the evidence and arguments presented, and they address the main question posed.

I only suggest to modify in figure 1 the word "asociated" into "associated".
