Treatment of lifestyle risk factors such as and smoking is difficult. These risk factors play important roles in cardiovascular disease. Current therapeutic strategies only have minor impact on associated metabolic changes such as low HDL cholesterol and insulin resistance. Rimonabant is a novel compound of the class of the cannabinoid receptor antagonists blocking CB1 receptors. Clinical studies demonstrate its efficacy in the treatment of obese patients. Several clinical studies show a consistent weight reduction of around 7 kg with rimonabant which is maintained over the followup period of two years. Improved lipid profile (increased HDL cholesterol, +25%) and the reduction of inflammatory markers promise interesting results in upcoming morbidity and mortality trials. Besides, rimonabant is helpful in smoking cessation. The proportion of patients free from smoking was almost doubled by rimonabant (28% vs. 16% in the placebo group). Weight increase was avoided in rimonabant- treated patients whereas those in the placebo group slightly gained weight. Full article
