Foam Rolling or Percussive Massage for Muscle Recovery: Insights into Delayed-Onset Muscle Soreness (DOMS)

Background: Pain manifestations as well as increased muscle tone and stiffness noted in the course of delayed-onset muscle soreness (DOMS) are reflected in altered values of the biomechanical and visco-elastic parameters of muscles. This study aimed to compare the effects of soft tissue mobilization with foam rolling and percussive massage on symptoms of DOMS induced by a standardized muscle fatigue protocol. Methods: Healthy volunteers (n = 60) were divided into three groups: FR group—foam rolling (n = 20), PM group—percussive massage (n = 20) and CON group—control/passive rest (n = 20). The fatigue protocol for the gastrocnemius muscle was carried out for development of DOMS in subsequent days. Therapeutic procedures were applied to participants for 3 consecutive days. The results of therapy were assessed by means of myotonometry, performed five times (before, three times during the treatment procedure, and after the end of the procedure). Results: Foam rolling significantly reduced the onset and duration of increased muscle tone (p = 0.006) and stiffness (p < 0.001), unlike percussive massage. The control group exhibited higher tone and stiffness after 48 h, at the peak of DOMS-related pain symptoms. Only foam rolling improved elasticity (decrement, p < 0.001), while visco-elastic properties (relaxation, creep) varied inversely with tone and stiffness. Foam rolling led to significantly lower stiffness (day 2) and reduced decrement and relaxation (day 4) compared to the control. Neither therapy was more effective than passive rest for pain relief during the observation period. Conclusions: Foam rolling and percussive massage accelerate recovery of muscle tone, stiffness, and elasticity after DOMS as compared to passive rest but offer no added benefit for pain relief. Endermologie is a non-invasive mechanical massage technique that combines suction, mechanized rollers and/or flaps, and mechanotransduction principles to stimulate the skin and subcutaneous tissues. This review assessed endermologie’s therapeutic indications and physiological effects. A comprehensive literature search was conducted using EDS DB (Med Univ) and PubMed to identify relevant studies published between 2000 and February 2025. Two authors independently screened studies, resulting in 24 articles included in the qualitative synthesis. Key applications identified included for burns, scars, muscle regeneration, lymphedema, cellulite, panniculitis/lipoatrophy, skin elasticity improvement, fat reduction, morphea, fibromyalgia, pre- and post-liposuction care, peri-oncology rehabilitation, orthopedics, and postoperative recovery. Research highlights the importance of treatment frequency, duration, and mechanostimulation parameters in determining therapeutic outcomes. Studies indicate that endermologie induces extracellular matrix remodeling, fibroblast activation, adipocyte fat release sensitivity, and enhanced venolymphatic circulation. Documented effects include improved microcirculation, anti-fibrotic properties, enhanced skin elasticity, fluid drainage, and pain relief, contributing to scar management, tissue softening, and post-surgical rehabilitation. Despite its potential, methodological heterogeneity across studies limits direct comparability, emphasizing the need for future research on standardization and long-term efficacy validation. Massage is one of the oldest forms of therapy practiced since ancient times. Nowadays, it is used in sports practice, recovery from injury, or supportive therapy for various conditions. The practice of massage uses a variety of instruments that facilitate massaging while relieving the stress on the masseur. One of them is a foam roller. Although roller massage is widely used, there are still no scientific studies describing the biological mechanisms of its effects on the body. The purpose of our study was to analyze the effect of roller massage on BDNF levels in men undergoing self-massage 4x/week/7 weeks. The control group consisted of men who did not perform self-massage. Before the test and after the first, third, fifth, and seventh weeks of self-massage, the study participants’ blood was drawn, the serum BDNF was determined, and the results were subjected to analysis of variance by ANOVA test. After the first week of self-massage, an increase in BDNF concentration was observed in the self-massage group compared to the control group (p = 0.023). Similarly, changes were observed in week five (p = 0.044) and week seven (p = 0.046). In the massaged group, BDNF concentrations were significantly higher after the first week of self-massage compared to baseline. In the third week of the study, BDNF decreased to a value comparable to the baseline study, then increased significantly in the fifth and seventh weeks compared to the value recorded in the third week (p = 0.049 and p = 0.029). It was significantly higher in week seven compared to week five (p = 0.03). Higher concentrations of BDNF in subjects undergoing roller self-massage may be one of the biological mechanisms justifying the therapeutic effects of massage in both sports and clinical practice. Studies analyzing the stimulation of BDNF synthesis through various massage techniques should be performed on a larger group of healthy individuals, patients after trauma of multiple origins, and sick people with indications for therapeutic massage. Therapists and strength and conditioning specialists use self-myofascial release (SMR) as an intervention tool through foam rollers or massage rollers for soft tissue massage, with the purpose of improving mobility in the muscular fascia. Moreover, the use of SMR by professional and amateur athletes during warm-ups, cool downs, and workouts can have significant effects on their physical performance attributes, such as range of motion (ROM) and strength. The purpose of this study was to analyse the literature pertaining to these types of interventions and their effects found in different physical performance attributes for athletes. A systematic search was carried out using the following databases: PUBMED, ISI Web of Science, ScienceDirect, and Cochrane, including articles up to September 2023. A total of 25 articles with 517 athletes were studied in depth. SMR seems to have acute positive effects on flexibility and range of motion, without affecting muscle performance during maximal strength and power actions, but favouring recovery perception and decreasing delayed-onset muscle soreness. Some positive effects on agility and very short-range high-speed actions were identified, as well. In conclusion, although there is little evidence of its method of application due to the heterogeneity in that regard, according to our findings, SMR could be used as an intervention to improve athletes’ perceptual recovery parameters, in addition to flexibility and range of motion, without negatively affecting muscle performance. Roller massage has been recognized as an effective intervention for managing various conditions. However, data on the effects of roller massage on the dynamic mechanisms of the myofascial and soft tissues of the lower back are limited. This study aimed to examine the effect of the self-myofascial release of the lower back on myofascial gliding, lumbar flexibility, and abdominal trunk muscle strength using a roller massager. This crossover study included 24 college athletes who underwent three interventions—roller massage, static stretching, and control (rest). Before and after the intervention, lumbar and fascial gliding were evaluated using ultrasonography. Long-seat anteflexion (lumbar flexibility) and abdominal trunk muscle strength were assessed. The movement velocities of the subcutaneous tissue and the multifidus muscle over time were calculated using echo video analysis software, and gliding was estimated using the cross-correlation coefficient between the velocities. Gliding, lumbar flexibility, and abdominal trunk muscle strength showed significant intervention-by-time interactions. Roller massage significantly improved gliding, lumbar flexibility, and abdominal trunk muscle strength. The self-myofascial release of the lower back using a roller massager improved the lumbar/fascia gliding, lumbar flexibility, and abdominal trunk muscle strength compared to static stretching. Paraplegic patients have always been ideal candidates for physiotherapy due to their body’s inability to recover on its own. Regardless of the cause that led to the onset of paraplegia (traumatic or degenerative), physiotherapy helps these patients with devices and methods designed to restore the proper functioning of their motility, as well as their quality of life. A total of 60 paraplegic dogs without deep pain in the hindlimbs caused by intervertebral disc extrusion or thoracolumbar fractures underwent physiotherapy sessions: manual therapy (massage), electrostimulation (10–20 min with possible repetition on the same day), ultrasound therapy, laser therapy, hydrotherapy, and assisted gait in supportive devices or on treadmills to stimulate and relearn walking, which was the main focus of the study. To maintain the standing position over time, we developed different devices adapted for each patient depending on the degree of damage and the possible associated pathologies: harnesses, trolleys, straps, exercise rollers, balancing platforms and mattresses, physio balls and rollers for recovery of proprioception. The main objective of our study was to demonstrate that physiotherapy and assisted gait in supportive devices to maintain the standing position may help paraplegic dogs to develop spinal walking. Concurrent pathologies (skin wounds, urinary infections, etc.) were managed concomitantly. Recovery of SW was evaluated by progression in regaining the reflectivity, nociception, gait score, and quality of life. After 125 to 320 physiotherapy sessions (25 to 64 weeks), 35 dogs (58.33%) developed spinal walking and were able to walk without falling or falling only sometimes in the case of a quick look (gait score 11.6 ± 1.57, with 14 considered normal), with a lack of coordination between the thoracic and pelvic limbs or difficulties in turning, especially when changing direction, but with the recovery of the quadrupedal position in less than 30 s. The majority of dogs recovering SW were of small size, with a median weight of 6.83 kg (range: 1.5–15.7), mixed breed (n = 9; 25.71%), Teckel (n = 4; 11.43%), Bichon (n = 5; 14.28%), Pekingese (n = 4; 11.43%), and Caniche (n = 2; 5.71%), while those who did not recover SW were larger in size, 15.59 kg (range: 5.5–45.2), and mixed breed (n = 16; 64%). Foam rolling (FR) is a new and popular technique for increasing range of motion. While there are a few studies that demonstrate increased performance measures after an acute bout of FR, the overall evidence indicates trivial performance benefits. As there have been no meta-analyses on the effects of chronic FR on performance, the objective of this systematic meta-analytical review was to quantify the effects of FR training on performance. We searched PubMed, Scopus, the Cochrane library, and Web of Science for FR training studies with a duration greater than two weeks, and found eight relevant studies. We used a random effect meta-analysis that employed a mixed-effect model to identify subgroup analyses. GRADE analysis was used to gauge the quality of the evidence obtained from this meta-analysis. Egger’s regression intercept test (intercept 1.79; p = 0.62) and an average PEDro score of 6.25 (±0.89) indicated no or low risk of reporting bias, respectively. GRADE analysis indicated that we can be moderately confident in the effect estimates. The meta-analysis found no significant difference between FR and control conditions (ES = −0.294; p = 0.281; I2 = 73.68). Analyses of the moderating variables showed no significant differences between randomized control vs. controlled trials (Q = 0.183; p = 0.67) and no relationship between ages (R2 = 0.10; p = 0.37), weeks of intervention (R2 = 0.17; p = 0.35), and total load of FR (R2 = 0.24; p = 0.11). In conclusion, there were no significant performance changes with FR training and no specific circumstances leading to performance changes following FR training exceeding two weeks. Full article

Although the effectiveness of static self-stretching exercises (SSSEs) and foam roller self-massaging (FRSM) in joint range of motion and muscle strength of the lower limbs has been extensively investigated, little is known about their effectiveness on the posterior trunk muscles. The present study aimed to investigate the acute effects of two 7-min SSSEs and FRSM intervention protocols on the range of trunk movements and the strength of the trunk extensors. Twenty-five healthy active males (n = 14) and females (n = 11) performed each intervention separately, one week apart. The range of motion (ROM) of the trunk-hip flexion (T-HF), the ROM of the trunk side-flexion (TSF) and rotation (TR) bilaterally, as well as the isometric maximum strength (TESmax) and endurance (TESend) of the trunk extensors were measured before and after each intervention. The ROMs of T-HF, TSF, and TR were significantly increased following both SSSEs and FRSM. The TESmax and TESend were also significantly increased after FRSM, but decreased following SSSEs. While both interventions were effective in increasing the range of motion of the trunk, a single 7-min session of FRSM presented more advantages over a similar duration SSSEs protocol due to the increase in the strength of the trunk extensors it induced. Expert Consensus on the Contraindications and Cautions of Foam Rolling—An International Delphi Study

Background: Foam rolling is a type of self-massage using tools such as foam or roller sticks. However, to date, there is no consensus on contraindications and cautions of foam rolling. A methodological approach to narrow that research gap is to obtain reliable opinions of expert groups. The aim of the study was to develop experts’ consensus on contraindications and cautions of foam rolling by means of a Delphi process. Methods: An international three-round Delphi study was conducted. Academic experts, defined as having (co-) authored at least one PubMed-listed paper on foam rolling, were invited to participate. Rounds 1 and 2 involved generation and rating of a list of possible contraindications and cautions of foam rolling. In round 3, participants indicated their agreement on contraindications and cautions for a final set of conditions. Consensus was evaluated using a priori defined criteria. Consensus on contraindications and cautions was considered as reached if more than 70% of participating experts labeled the respective item as contraindication and contraindication or caution, respectively, in round 3. Results: In the final Delphi process round, responses were received from 37 participants. Panel participants were predominantly sports scientists (n = 21), physiotherapists (n = 6), and medical professionals (n = 5). Consensus on contraindications was reached for open wounds (73% agreement) and bone fractures (84%). Consensus on cautions was achieved for local tissue inflammation (97%), deep vein thrombosis (97%), osteomyelitis (94%), and myositis ossificans (92%). The highest impact/severity of an adverse event caused by contraindication/cautions was estimated for bone fractures, deep vein thrombosis, and osteomyelitis. Discussion: The mechanical forces applied through foam rolling can be considered as potential threats leading to adverse events in the context of the identified contraindications and cautions. Further evaluations by medical professionals as well as the collection of clinical data are needed to assess the risks of foam rolling and to generate guidance for different applications and professional backgrounds. The purpose of this study was to examine the acute effect of foam rolling and vibration foam rolling on drop jump performance. The optimal time interval between warm-up using foam rolling or vibration foam rolling and drop jump performance was identified. This study included 16 male NCAA Division I college volleyball athletes. Three interventions were performed in a randomized order: the foam rolling exercise (FRE), vibration foam rolling exercise (VFRE), and static rest (control). The drop jump was performed before interventions, as well as 2 and 5 min after interventions. The FRE exhibited higher values for drop jump height (DJH) (p = 0.001; η2 = 0.382; statistical power = 0.964) and mean power generation at the hip joint (p = 0.006; η2 = 0.277; statistical power = 0.857) at 2 min compared with before intervention but not at 5 min (p > 0.05). However, the VFRE showed no significant changes in DJH (p > 0.05), and found that hippower was decreased at 5 min (p = 0.027; η2 = 0.214; statistical power = 0.680). The FRE completed in 2 min before rapid single action competition (sprint, long jump, triple jump, etc.) could increase sports performance. Full article

Objectives: The main purpose of the present study was to investigate the acute effects of myofascial tissue rolling on endurance performance and recovery using a novel designed mechanical self-induced multi-bar roller-massager. Methods: a randomized crossover, repeated measure design was used. Eight national levelled, junior and neo-senior, speed skaters underwent a 10 min myofascial quadriceps rolling pre- and fifteen minutes post- a stepwise incremental cycling-test to exhaustion followed by a Wingate performance-test. The myofascial quadriceps rolling was used in one out of two laboratory testing-days. Time to exhaustion, peak oxygen uptake (VO2peak), blood lactate concentration during 30 min of recovery, and peak- and mean- power during the consecutive Wingate test were recorded. Results: Myofascial quadriceps rolling using roller-massager resulted in higher blood lactate concentration at exhaustion and a larger blood lactate clearance after 10 min to post exhaustion test (both p < 0.05), a tendency for a positive effect on Wingate peak-power (p = 0.084; d = 0.71), whereas no marked differences were observed on VO2peak, time to exhaustion and Wingate mean-power. Conclusion: Despite indications for potential benefits of the quadriceps myofascial tissue release using the mechanical self-induced multi-bar roller-massager on blood lactate concentration and Wingate peak-power, the myofascial tissue release gave no marked performance improvements nor indications of negative effects. Future studies could examine the long-term effects of myofascial tissue release on performance and recovery. Furthermore, integrating a measure of the participants’ subjective experience pre- and post the myofascial tissue release would be of great interest. Full article
