Background: Chronic lumbopelvic pain is often linked to sacroiliac joint dysfunction, where the joint’s complex structure and biomechanics complicate diagnosis and treatment. Variability in load distribution and ligament stabilization within the pelvic ring further contributes to challenges in managing this condition. This study aims to develop a finite element model of the “lumbar spine–sacrum–pelvis” system to analyze the effects of lumbar lordosis, pelvic tilt, and asymmetrical articular gaps on stress and strain in the sacroiliac joint. Methods: A three-dimensional model was constructed using CT and MRI data, including key stabilizing ligaments. Sacral slope angles of 30°, 60°, and 85° were used to simulate varying lordosis, while pelvic tilt was introduced through a 6° lateral rotation. Results: The analysis revealed that sacral slope, ligament integrity, and joint symmetry significantly influence stress distribution. Hyperlordosis led to critical stress levels in interosseous and iliolumbar ligaments, exceeding failure thresholds. Asymmetrical gaps and pelvic tilt further altered the sacral rotation axis, increasing stress on sacroiliac joint ligaments. Conclusions: These findings highlight the importance of maintaining sacroiliac joint symmetry and lumbar–pelvic alignment to minimize stress on stabilizing ligaments, suggesting that treatment should focus on restoring alignment and joint symmetry. Background/Objectives: Falls pose a significant health risk for older adults, often due to balance disorders and decreased mobility. Methods: The ability to perform sit-to-stand transfers, which involve squatting, is crucial for daily independence. Incorporating squats into exercise routines can enhance lower body strength, reduce fall risk, and improve overall quality of life. Results: While the back squat is beneficial, proper form is essential to avoid biomechanical errors, like lumbar hyperlordosis and knee valgus. Conclusions: Health and fitness professionals, such as physical therapists and/or clinical exercise physiologists, should carefully guide older adults in performing the back squat, addressing any functional deficits, and ensuring proper technique to minimize the risk of injury and maximize the benefits. Sarcoglycanopathies are among the most frequent and severe forms of autosomal recessive forms of limb-girdle muscular dystrophies (LGMDs) with childhood onset. Four subtypes are known: LGMDR3, LGMDR4, LGMDR5 and LGMDR6, which are caused, respectively, by mutations in the SGCA, SGCB, SGCG and SGCD genes. We present the clinical variability of LGMD 2C/R5 among a genetically homogeneous group of 57 patients, belonging to 35 pedigrees. Molecular genetic analysis showed that all 57 patients were homozygous for the C283Y variant. The muscles of the pelvic girdle and the trunk were affected early and were more severely affected, followed by the shoulder girdle. Macroglossia, hypertrophy of the calves, scapular winging and lumbar hyperlordosis were common in the ambulatory phase. A great intra and interfamilial variability in the clinical presentation of LGMD 2C/R5 was observed, despite having the same underlying molecular defect. Females demonstrated a relatively milder clinical course compared to males. Mean creatine phosphokinase (CK) CK levels were 20 times above normal values. Muscle computer tomography (CT) CT or MRIs showed earlier and more severe involvement of the flexor proximal limb muscles in comparison to extensor muscles. Full article

(1) Background: Lumbar spondylolisthesis affects ~20% of the US population and causes spine-related pain and disability. (2) Methods: This series reports on three patients (two females and one male) aged 68–71 years showing improvements in back pain, quality of life (QOL), and urinary dysfunction following correction of lumbar spondylolistheses using CBP® spinal rehabilitation. Pre-treatment radiographs showed lumbar hyperlordosis (−49.6°, ideal is −40°) and anterolisthesis (14.5 mm, ideal is 0 mm). Pre-treatment patient-reported outcome measures (PROMs) included a numeric rating scale (NRS) for back pain (7.3/10, ideal is 0), urinary urgency (8/10, ideal is 0), and SF-36 physical (PCS) and mental component score (MCS) (29.8 and 46.6, ideal is 46.8 and 52.8). Patients underwent 2–3 CBP® sessions per week to correct lumbar hyperlordosis and lumbar anterolistheses. (3) Results: Post-treatment radiographs showed improvements in lumbar curvature (−42.8°) and anterolisthesis (4.2 mm). Post-treatment PROMs showed improvements in NRS for back pain (1/10), urinary urgency (2.3/10), and SF-36 PCS and MCS (50.2 and 57.7). Long-term follow-up radiographs and PROMs showed maintained improvements. (4) Conclusions: This series documents the first-recorded long-term corrections of lumbar spondylolisthesis and concomitant improvements in back pain, urinary urgency, and QOL using CBP®. This series provides evidence for CBP® as a non-surgical approach to lumbar spinal rehabilitation and the possible impacts of spinal alignment on pain, urinary dysfunction, and QOL. Full article

Proper posture, characterized by the appropriate alignment of the cervical, thoracic, and lumbar segments of the spine, enables these regions to maintain their normal curvature. Body composition is recognized as one of the factors that can influence overall postural alignment of the spine. The objective of this study was to determine the influence of the parameters of body composition on the prevalence of postural disorders in the sagittal plane. The cross-sectional study was conducted on 152 children of both genders (78 boys), at a mean age of 11 years ± 6 months. In order to evaluate postural disorders, the Formetric 4D System, a tool manufactured by Diers, Schlangenbad, Germany was used. Based on its output data, the following variables were obtained: hyperkyphosis, hyperlordosis, kypholordosis, flatback, and normal alignment of the body in the sagittal plane. The evaluation of body composition parameters was conducted using the InBody 770 device. To determine how body composition influences the postural status of the spinal column, a discriminant analysis was employed. The results showed that approximately 65.8% of children exhibit various types of postural disorders when assessing the alignment of the spine in the sagittal plane. The most prevalent disorder observed was hyperkyphosis, affecting 34.2% of the subjects, followed by kypholordosis at 16.4%. Moreover, the results demonstrated that body composition significantly influences body posture (p = 0.004). An increase in fat mass corresponds to a deviation from normal body posture, whereas an increase in the percentage of skeletal muscle mass and fat-free mass is associated with a reduction in postural abnormalities in the sagittal plane. Considering the results, it is clear that body composition parameters serve as more reliable predictors of the influence on body posture compared to simply calculating the body mass index. Furthermore, it can be concluded that there are consistent patterns of influence by specific body composition parameters, including fat mass, percentage of skeletal muscle, and fat-free mass, on body posture among children from various climates. These results underscore the significance of implementing strength exercises in children, particularly during periods of rapid growth and development, as a means of preventing and correcting postural disorders. Background and Objectives: Abdominal muscle exercises with limb movements are more effective for trunk stabilization than traditional exercises involving trunk flexion alone. This study examined the effects of abdominal exercises incorporating sprinter pattern and crunch exercises on changes in the lordotic curve and abdominal muscle activation in individuals with low back pain caused by hyperlordosis resulting from weak abdominal muscles. Materials and Methods: In this single-blind, randomized controlled trial, a total of 40 participants with hyperlordosis were recruited and randomly assigned to perform either sprinter-pattern abdominal exercises or crunch exercises. The participants assigned to each group performed three sets of ten abdominal exercises. The lumbar lordotic angle (LLA) and sacrohorizontal angle (SHA) were assessed prior to and following the intervention, whereas abdominal muscle activity was gauged throughout the intervention period. Changes in the LLA and SHA were measured by radiography. Abdominal muscle activity was measured using electromyography. Results: The LLA and SHA decreased significantly in both groups (p < 0.001), while the sprinter-pattern exercise group showed a statistically significant decrease compared to the crunch exercise group (p < 0.001). In the activity of the abdominal muscles, there was no significant difference in the rectus abdominis muscle between the two groups (p > 0.005). However, a significant difference between the external and internal oblique muscles was observed, and the activities of both muscles were significantly higher in the sprinter-pattern exercise group than in the crunch exercise group (p < 0.005). Conclusions: Abdominal exercise using a sprinter pattern may be effective in reducing lumbar lordosis by strengthening the abdominal muscles in patients with hyperlordosis. Full article

Background: Femoroacetabular impingement is one possible cause for groin pain and can lead to long periods of absence for football players. In cam impingement, the end-grade position of the leg at kicking makes the hip particularly prone to faulty contact between the acetabulum and the femoral head. Studies suggest that the resting position of the pelvis in the sagittal plane may have an important role in the biomechanics of movement in the presence of cam impingement. Methods: A 19-year-old male competitive footballer complained of sudden groin pain during a period of low athletic load. Biomechanical tests (3D posture and isometric strength analyses) showed that unbalanced individual strength training had resulted in an increased forward tilt of the pelvis. At the same time, cam impingement was confirmed radiologically, which obviously contributed to the sudden onset of the symptoms. The kicking technique of the athlete showed increased hip and trunk flexion, which also indicated a muscular imbalance. Targeted strength and stretching exercises three times a week improved the pelvic position in terms of reduced anteversion. At the same time, the patient performed strength exercises to improve his kicking technique. Results: After 8 weeks, improvements in his pelvic position and global posture and increased muscle strength could be verified. At the same time, the athlete was free of complaints again. Conclusions: When groin pain occurs in football players with cam impingement, special attention should be paid to the resting position of the pelvis in the sagittal plane. Correcting increased pelvic anteversion can prevent unfavourable end-grade collisions of the acetabulum and femoral head during kicking with strong hip flexion and adduction. Possible changes in the pelvic position due to adverse individual strength training performed by young athletes should always be kept in mind. Full article

Spinal deformity refers to a range of disorders that are defined by anomalous curvature of the spine and may be classified as scoliosis, hypo/hyperlordosis, or hypo/hyperkyphosis. Among these, scoliosis stands out as the most common type of spinal deformity in human beings, and it can be distinguished by abnormal lateral spine curvature accompanied by axial rotation. Accurate identification of spinal deformity is crucial for a person’s diagnosis, and numerous assessment methods have been developed by researchers. Therefore, the present study aims to systematically review the recent works on spinal deformity assessment for scoliosis diagnosis utilizing image processing techniques. To gather relevant studies, a search strategy was conducted on three electronic databases (Scopus, ScienceDirect, and PubMed) between 2012 and 2022 using specific keywords and focusing on scoliosis cases. A total of 17 papers fully satisfied the established criteria and were extensively evaluated. Despite variations in methodological designs across the studies, all reviewed articles obtained quality ratings higher than satisfactory. Various diagnostic approaches have been employed, including artificial intelligence mechanisms, image processing, and scoliosis diagnosis systems. These approaches have the potential to save time and, more significantly, can reduce the incidence of human error. While all assessment methods have potential in scoliosis diagnosis, they possess several limitations that can be ameliorated in forthcoming studies. Therefore, the findings of this study may serve as guidelines for the development of a more accurate spinal deformity assessment method that can aid medical personnel in the real diagnosis of scoliosis. A considerable number of problems begin in childhood due to lifestyle changes, which include a transition from a previous period of extensive movement to prolonged hours of staying in a sitting position at school. The aim of this study was to examine the occurrence of back and side view postural defects in the study group of school-aged children and identify risk factors associated with the formation of postural abnormalities in the study group. Methods: This study was conducted on a group of 141 children aged 7–10, attending the first to third grades at a primary school in Białystok (northeastern Poland). This study involved measuring the children’s height and weight, assessing the children’s body posture based on the FITS method (Functional Individual Scoliosis Therapy) by Białek and M’hango (the authors of this study), and administering a diagnostic survey addressed to parents and guardians of the children (n = 104) using a self-designed questionnaire. Results: Almost all defects were more prevalent in boys, especially in the case of stature triangles (p = 0.0489) and knee alignment in the sagittal plane (p = 0.038). The age of the subjects differentiated the incidence of defects in the scapulae (p = 0.0037) and shoulder (p = 0.0129) alignment, correlating negatively with age. The risk of postural defects for knees (p = 0.0391) and abdominal arching (p = 0.0240) was significant with a higher BMI. The following lifestyle-related factors were significant: the seat for doing homework (stature triangles p = 0.0253), time spent in front of a computer (positioning of the scapulae in relation to each other p = 0.0233; vertical view of the intergluteal cleft p = 0.0324), and snacking between meals (feet p = 0.0003; shoulder positioning p = 0.0013; stature triangles p = 0.0186; positioning of the scapulae in relation to each other p = 0.0404). Conclusions: The body posture of the examined children was closed with the head pushed forward and drooped, rounded shoulders, hyperlordosis, and pelvic anteversion. Most exhibited various types of abnormalities related to the feet. The recognized risk factors for posture defects are overweight/obesity, the male gender, children who are older, lack of an adjustable work chair, 2 h a day or more spent using the computer, and snacking between meals. Psoriatic disease is a chronic, relapsing inflammatory disorder, characterized mostly by cutaneous erythematous scaly plaques sometimes associated with arthritis. Hidradenitis suppurativa (HS) is a chronic relapsing inflammatory disease of the apocrine glands, characterized clinically by painful abscesses, sinus tracts and scars. It typically occurs after puberty, affecting mainly intertriginous areas of the body. There is a strong association between HS and psoriasis since they share the same pathogenic inflammatory pathway. The patient presented: low birthweight, microcephaly, facial dysmorphisms, lumbar hyperlordosis, walking difficulties, global psychomotor developmental delay and learning disabilities. A genetic evaluation revealed a 2.5 Mb de novo microduplication in the 17q21.31 chromosomal region. Dermatological examination revealed HS (Hurley stage II-HS) distributed in the genital area and inguinal folds, psoriatic plaques on the retroauricolar folds, on the elbows bilaterally and on the lateral aspect of the right ankle and psoriatic arthritis. The patient was treated with adalimumab, with a marked improvement of both conditions. To our best knowledge, we report the first case of coexisting Psoriatic Arthritis Disease and Hidradenitis Suppurativa in a child with chromosome 17q21.31 microduplication syndrome. We hypothesize that gene CRHR1 duplication included in the 17q21.31 chromosomal region might be involved in the pathogenesis of both diseases. Full article

Postural deficits such as hyperlordosis (hollow back) or hyperkyphosis (hunchback) are relevant health issues. Diagnoses depend on the experience of the examiner and are, therefore, often subjective and prone to errors. Machine learning (ML) methods in combination with explainable artificial intelligence (XAI) tools have proven useful for providing an objective, data-based orientation. However, only a few works have considered posture parameters, leaving the potential for more human-friendly XAI interpretations still untouched. Therefore, the present work proposes an objective, data-driven ML system for medical decision support that enables especially human-friendly interpretations using counterfactual explanations (CFs). The posture data for 1151 subjects were recorded by means of stereophotogrammetry. An expert-based classification of the subjects regarding the presence of hyperlordosis or hyperkyphosis was initially performed. Using a Gaussian progress classifier, the models were trained and interpreted using CFs. The label errors were flagged and re-evaluated using confident learning. Very good classification performances for both hyperlordosis and hyperkyphosis were found, whereby the re-evaluation and correction of the test labels led to a significant improvement (MPRAUC = 0.97). A statistical evaluation showed that the CFs seemed to be plausible, in general. In the context of personalized medicine, the present study’s approach could be of importance for reducing diagnostic errors and thereby improving the individual adaptation of therapeutic measures. Musculoskeletal disorders characteristically induce pain and limitations in mobility, ability, and overall functioning. In athletes, including basketball players, disorders such as back pain, postural changes, and spinal injuries are common. This systematic review aimed to evaluate the prevalence of back pain and musculoskeletal disorders in basketball players and ascertain the associated factors. Methods: The Embase, PubMed, and Scopus databases were searched for studies published in English without a time limit. Using STATA, meta-analyses were performed to estimate the prevalence of pain and musculoskeletal disorders of the back and spine. Results: Of the 4135 articles identified, 33 studies were included in this review, with 27 studies included in the meta-analysis. Of these, 21 were used for the meta-analysis of back pain, 6 articles were used for the meta-analysis of spinal injury, and 2 studies were used for the meta-analysis of postural changes. The overall prevalence of back pain was 43% [95% CI, −1% to 88%]; of these, the prevalence of neck pain was 36% [95% CI, 22–50%], the prevalence of back pain was 16% [95% CI, 4–28%], the prevalence of low back pain was 26% [95% CI, 16–37%], the prevalence of thoracic spine pain was 6% [95% CI, 3–9%]. The combined prevalence of spinal injury and spondylolysis was 10% [95% CI, 4–15%], with a prevalence of spondylolysis of 14% [95% CI, 0.1–27%]. The combined prevalence of hyperkyphosis and hyperlordosis was 30% [95% CI, 9–51%]. In conclusion, we found a high prevalence of neck pain, followed by low back pain and back pain, in basketball players. Thus, prevention programs are important to improve health and sports performance. Full article

The changes in lumbar lordosis angle (LL) and sacral slope angle (SS) related to upper limb elevation and thoracic kyphosis angle (TK) in baseball players with spondylolysis remain unclear. Herein, we investigated baseball players with spondylolysis and those without low back pain, comparing LL and SS with upper limb elevation within and between groups and TK between groups. Baseball players with spondylolysis were enrolled as subjects, and baseball players without low back pain were enrolled as controls (n = 8 each). X-rays were obtained in the standing position and with maximal elevation position of the upper limb (elevation position). LL and SS were measured in the standing and elevated positions, and TK was measured in the standing position. LL was significantly larger in individuals with spondylolysis than controls. The SS of the control group was significantly larger in the elevated position than in the standing position, while the SS of the spondylolysis group was not significantly different between positions. SS was significantly larger in the spondylolysis group than in the control group, only in the standing position. Physical therapy for spondylolysis should focus on hyperlordosis alignment in the standing and maximal elevation positions of both upper limbs, sacral hyper-slope alignment in the standing position, and decreased sacral slope motion. Lumbar lordosis is one of the most important parts of the spine, which is of special importance due to its unique position and direct contact with the pelvis. The aim of this study was to combine the results of several studies and to evaluate the magnitude of the effect of different Lumbar lordotic angle correction programs through meta-analysis. This study has been developed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Statement. Four databases were searched for articles collection: PubMed, Cochrane Library, Web of Science, and Google Scholar. The key search terms were: “Lumbar Lordotic angle”, “Lordosis”, “Hyperlordosis”, “Corrective exercise”, and “Low back pain. “The articles included in our study were limited to original articles written only in English that met the following inclusion criteria: (1) participants with lumbar lordosis or hyperlordosis or low back pain; (2) different programs of corrective exercises were applied; (3) Lumbar lordotic angle used as outcome measures. Ten studies are included in our systematic review and meta-analysis. The effect size for the Lumbar lordotic angle outcome was (SMD = 0.550, p ˂ 0.001, moderate effect size). Subgroup analysis for Lumbar lordotic angle: Subgroup Younger group (SMD = 0.640, p ˂ 0.001), Subgroup Older group, (SMD = 0.520, p ˂ 0.001). Subgroup Treatment (SMD = 0.527, p ˂ 0.001), Subgroup No treatment (SMD = 0.577, p = 0.002). This was the only outcome assessed in our analysis. The current meta-analysis indicates that different correction methods have a positive effect on subjects with lumbar lordosis or hyperlordosis. In the following research, we should try to determine which corrective methods have the best effects. Full article

Study of the Association between Postural Misalignments in School Students

Nowadays, postural misalignments prevalence in students is increasingly evident and its early detection is the best way to prevent complications in adulthood. It is also known that postural problems appear to have association between them. The aim of this study was to characterize and study the association between spine, scapular and pelvic girdles postural misalignments in 213 school students (124 males and 89 females), aged between 10 and 20 years, from Escola Básica e Secundária de Santa Maria (EBSSM). To understand the relation between postural misalignments we compare the head positioning with the shoulder girdle, the shoulder girdle with the dorsal spine and the lumbar spine with the pelvic girdle. The postural assessment was made by visual scan analysis in a symmetrograph based on photogrammetry and the Adams. We found high percentages of postural misalignments in all subjects. High prevalence was observed in the posterior view of the frontal plane in the head lateral flexion, shoulders vertical asymmetry, scoliosis and in the presence of gibbosity, whereas, in the right view of the sagittal plane forward head posture and protracted shoulders were observed. The lumbar scoliosis was more prevalent in subjects with iliac crest’s asymmetry (p < 0.001) and forward head posture was significantly related to the subjects with protracted shoulders (p = 0.010). The protracted shoulders were more prevalent in subjects with dorsal hyperkyphosis (p = 0.048) and lumbar hyperlordosis was related to the subjects with pelvic anteversion (p < 0.001). The students’ postural assessment showed a wide variety of postural misalignments. It also seems evident that there is a relationship between their occurrences. It should be highlighted the importance of postural assessment in physical education (PE) classes to determine postural misalignments among school students. Full article
