The exploitation of hydrokinetic resources represents a sustainable and efficient alternative for renewable energy generation. This study presents the design and real-world implementation of a compact hydrokinetic system capable of converting rainwater runoff into electricity within smart homes. Unlike conventional large-scale hydrokinetic technologies, this system was specifically engineered for intermittent, low-flow conditions typical of residential rainwater collection networks. The turbine was manufactured using 3D-printed biodegradable materials to promote environmental sustainability and facilitate rapid prototyping. Through CFD simulations and laboratory testing, the system’s hydraulic behaviour and energy conversion efficiency were validated across different flow scenarios. The complete system, consisting of four turbines rated at 120 W each, was integrated into a real smart home without structural modifications. From an academic perspective, this study contributes a quantitatively validated hybrid hydrokinetic–low-head framework for residential rainwater energy recovery, addressing intermittent and low-flow urban conditions insufficiently explored in existing literature. Field tests demonstrated that the hydrokinetic system provides complementary energy during rainfall events, generating up to 6000 Wh per day and enhancing household energy resilience, particularly during periods of low solar availability. The results confirm the technical feasibility, sustainability, and practical viability of decentralized hydrokinetic energy generation for residential applications. Full article

Human Activity Recognition (HAR) has become crucial across various domains, including healthcare, smart homes, and security systems, owing to the proliferation of Internet of Things (IoT) devices. Several Machine Learning (ML) techniques, including Convolutional Neural Networks (CNNs) and Long Short-Term Memory (LSTM), have been proposed for HAR. However, they are still deficient in addressing the challenges of noisy features and insufficient data. This paper introduces a novel approach to tackle these two challenges, employing a Deep Learning (DL) Ensemble-Based Stacking Neural Network (SNN) combined with Generative Adversarial Networks (GANs) for HAR based on ambient sensors. Our proposed deep learning ensemble-based approach outperforms traditional ML techniques and enables robust and reliable recognition of activities in real-world scenarios. Comprehensive experiments conducted on six benchmark datasets from the CASAS smart home project demonstrate that the proposed stacking framework achieves superior accuracy on five out of six datasets when compared to literature-reported state-of-the-art baselines, with improvements ranging from 3.36 to 39.21 percentage points and an average gain of 13.28 percentage points. Although the baseline marginally outperforms the proposed models on one dataset (Aruba) in terms of accuracy, this exception does not alter the overall trend of consistent performance gains across diverse environments. Statistical significance of these improvements is further confirmed using the Wilcoxon signed-rank test. Moreover, the ASGAN-augmented models consistently improve macro-F1 performance over the corresponding baselines on five out of six datasets, while achieving comparable performance on the Milan dataset. The proposed GAN-based method further improves the activity recognition accuracy by a maximum of 4.77 percentage points, and an average of 1.28 percentage points compared to baseline models. By combining ensemble-based DL with GAN-generated synthetic data, a more robust and effective solution for ambient HAR addressing both accuracy and data imbalance challenges in real-world smart home settings is achieved. Background/Objectives: Limb salvage surgery is the preferred treatment for bone and soft tissue tumors. The Toronto Extremity Salvage Score (TESS) is a validated patient-reported outcome measure (PROM). However, its upper extremity section is outdated, lacking assessment of modern technology use. This study adapted TESS (creating TESS 2.0) by adding a question on touchscreen/keyboard use, based on an existing Italian version. Methods: Two independent translations of the new item were synthesized. Linguistic feedback was obtained from two German speakers from Germany and Switzerland. Pretesting with eight individuals refined the wording. Fourteen patients who underwent upper extremity surgery completed the TESS 2.0 twice (once in person, once at home) and the DASH questionnaire once. Reliability was assessed using Cronbach’s alpha and ICC, and validity was assessed using Spearman’s correlation between TESS 2.0 and DASH. Results: TESS 2.0 demonstrated excellent internal consistency (Cronbach’s alpha ≥ 0.98 at both time points). A strong, statistically significant inverse correlation existed between TESS 2.0 and DASH (r = −0.867, p < 0.001). Test–retest reliability was high (ICC = 0.98). Conclusions: German TESS 2.0 is a reliable and valid instrument for assessing upper extremity function in patients treated for bone and soft tissue tumors. Further research is needed to validate its use in postoperative follow-up and a larger, more diverse patient population. Full article

Introduction: The COVID-19 pandemic had significant effects on mental health and lifestyle behaviours, especially among university students who experienced academic disruptions, social isolation, and fewer social interactions. Alcohol consumption has long been part of student culture. Still, the influence of post-pandemic academic reintegration on drinking patterns and psychological distress remains relatively unexplored, particularly in countries like Portugal, where student traditions heavily shape consumption habits. This study aimed to describe the prevalence of alcohol consumption, depression, anxiety, and stress in a sample of Portuguese university students during the post-pandemic academic period, and to explore associations with sociodemographic variables. Methods: A cross-sectional study was conducted in November 2021 with 90 students from a private higher education institution in northern Portugal. Data were collected via an online questionnaire including sociodemographic information, the Alcohol Use Disorders Identification Test (AUDIT), and the Depression, Anxiety and Stress Scale (DASS-21). Result: The majority of the participants were not at risk of alcohol addiction (95.3%). In total, 15.1% of students reported anxiety symptoms ranging from severe to extremely severe. A binomial logistic regression was performed to ascertain the effects of being away from home and psychological distress (DASS-42 score), on the likelihood that participants were at risk of alcohol addiction (Level 3 and 4 in the AUDIT scale). The logistic regression model was statistically significant, χ2(2) = 9.20, p = 0.010. Living away from home was associated with a substantially lower likelihood of high-risk status (B = −2.79, p = 0.034), corresponding to an odds ratio of 0.06, indicating a strong protective effect. DASS-42 total score was positively associated with high-risk status (B = 0.04, p = 0.039), such that higher psychological distress increased the odds of being classified as high risk. Conclusions: The findings reveal a low prevalence of alcohol risk but heightened symptoms of anxiety, depression, and stress. Psychological distress notably increases the likelihood of hazardous alcohol use, emphasising the importance of targeted mental health and alcohol-use interventions among university students. New wearable technology opens new possibilities for low-cost, easily accessible home-based interventions as a supplement to typical clinical rehabilitation therapy. In this pilot study, we tested a new interactive adjustable Feedback training system on 14 infants at high risk of cerebral palsy between 2 and 12 months of age to facilitate increased movements. The system consists of four wireless motion sensors placed on the infant’s limbs. Inertial sensors track the infant’s movements which control auditory and visual stimuli that act as motivational feedback. A 15 min usage of the Feedback training system four days a week for approximately six months was aimed for. None of the participants reached the recommended amount of intervention, due to time limitations. Seven of the twelve participating infants (58%) achieved at least 50% of the recommended training amount. Parents found the Feedback training system easy to use with minimal need for technical assistance. Preliminary data suggest that infants engaged more actively during training sessions where their movements actively controlled the presentation of the stimuli. The Feedback training system is promising as a user-friendly add-on to the playful and interactive stimulation of motor and cognitive development in infants with neurodevelopmental disorders. The absence of a recognizable P wave in an electrocardiogram (ECG) is a critical indicator for the diagnosis of atrial fibrillation (AF). An algorithm capable of distinguishing between physiological and pathological states in a short period of time could serve as a valuable tool for timely and effective diagnosis, even in a home setting. To achieve this goal, a deterministic algorithm is proposed. The Fantasia Database and the AF Termination Challenge Database were used for training the model. Subsequently, for the test session, a one-minute recording was extracted from the Autonomic Aging Dataset and the Long-Term AF Database. After band-pass filtering, characteristic points such as R-peaks and P waves were extracted. The R-peak detection algorithm was compared with the gold standard Pan-Tompkins, obtaining a p-value > 0.05 on the Fantasia Database, which means that there is no statistical difference between them. Subsequently derived features such as duration, amplitude, subtended area, and P wave slope have been used to discriminate healthy subjects from AF patients. The P-wave slope emerged as the most effective feature, achieving a classification accuracy of 100% and 96% for the training and test sets, respectively. This algorithm thus represents a significant advancement as it achieves a performance comparable to other deterministic methods based on P wave analysis using only one-minute recordings, thereby enabling accurate diagnosis in a shorter time frame. Applications of smart cities, smart buildings, smart agriculture systems, smart grids, and other smart systems benefit from Internet of Things (IoT) protocols, networks, and architecture. Wireless Sensor Networks (WSNs) in smart systems that employ IoT use wireless communication technologies between sensors in the Things layer and the Fog layer hub. Such wireless protocols and networks include WiFi, Bluetooth, and Zigbee, among others. However, the payload formats of these protocols are heterogeneous, and thus, they lack a unified frame format that ensures interoperability. In this paper, a lightweight, interoperable frame format for low-rate, small-size Wireless Sensor Networks (WSNs) in IoT-based systems is designed, implemented, and tested. The practicality of this system is underscored by the development of a gateway that transfers collected data from sensors that use the unified frame to online servers via message queuing and telemetry transport (MQTT) secured with transport layer security (TLS), ensuring interoperability using the JavaScript Object Notation (JSON) format. The proposed frame is tested using market-available technologies such as Bluetooth and Zigbee, and then applied to smart home applications. The smart home scenario is chosen because it encompasses various smart subsystems, such as healthcare monitoring systems, energy monitoring systems, and entertainment systems, among others. The proposed system offers several advantages, including a low-cost architecture, ease of setup, improved interoperability, high flexibility, and a lightweight frame that can be applied to other wireless-based smart systems and applications. Objectives/Background: In this study, we aimed to examine the associations of participation in leisure dance activity with perceived health status, happiness level, and perceptions of leisure, and the relevance of demographic characteristics to these associations. Moreover, we sought to explore ways of revitalizing leisure dance activity. Methods: We used data from the 2022 Korea National Leisure Activity Survey organized by the Korean Ministry of Culture, Sports, and Tourism. The 2022 Korea National Leisure Activity Survey was conducted from September to November 2022. The survey was conducted among 10,046 Koreans aged ≥15 who lived in 17 cities and provinces across the country. The participants were informed about the survey schedule in advance, and interviews were conducted at the participants’ homes using tablet PCs. Random telephone verification was performed on the recovered questionnaires to ensure the accuracy of responses. Questionnaires that passed the first verification were subjected to secondary verification by a computerized program, and questionnaires that did not pass the verification were supplemented and re-examined. The collected data were entered electronically through an encoding process, and only the data that passed the final test were compiled in the multi-stage verification process. The data were analyzed using frequency analysis, chi-squared tests, and multivariate logistic regression analysis. Results: Of the 1004 participants, 655 (6.5%) participated in leisure dance activity. Women, individuals in their twenties and thirties, college graduates or those with lower-level educational qualifications, and unmarried individuals were more likely to participate in leisure dance activities. In addition, leisure dance activities were found to be likely to increase happiness levels and leisure life satisfaction, and leisure expenses were likely to be low. Conclusions: Schools should provide high-quality dance education and enhance the professionalism of physical education teachers in the management of dance classes. This is because dance-related experiences in childhood can increase the likelihood of engaging in dancing in adulthood. Efforts are also needed to increase men’s awareness of and participation in leisure dance activities and to lower barriers to entry. The convergence of dance, games, and technology can make this possible. Full article

Background/Objectives: Telerehabilitation is intended for remote treatment, but studies on group training after stroke performed in the patient’s home have not yet been conducted. The purpose of this study was to evaluate adherence rates, safety, usability and enjoyment and preliminary clinical effects of home-based remote group training for balance and mobility in chronic stage after stroke. Methods: Community-dwelling patients in chronic stage after stroke who walked independently and had mild balance deficits participated. Over a 6-week period, they completed 60-min sessions of balance and mobility training twice a week in a group from their home. Adherence rates, adverse events and technical problems were recorded. Participants’ satisfaction was assessed using Modified Physical Activity Enjoyment Scale. The primary outcomes were Mini-Balance Evaluation Systems Test (mini-BESTest), 5TSTS and 10MWT and secondary outcomes were limits of stability, weight-bearing symmetry and Activities-Specific Balance Confidence Scale (ABC Scale), measured before, immediately after and six weeks after remote group training. Results: Participants expressed a very high level of satisfaction with training. There were no adverse events and dropouts, but some minor technical challenges. Results showed significant improvements in primary outcomes (mini-BESTest, 10MWT fast walking speed, 5TSTS; all p < 0.001), however there were no significant improvements in secondary outcomes (weight-bearing symmetry, limits of stability and ABC Scale). All improvements persisted six weeks after training. Conclusions: Remote group training at home is feasible, safe and efficient to improve balance and mobility in patients in the chronic phase after stroke. Full article

REM sleep behavioral disorder (RBD) is of increasing interest in Parkinson’s disease (PD). Previous studies exploring the association between REM sleep without atonia (RWA) and clinical PD features or other objective sleep metrics are scarce and have used PSG findings. A mobile electroencephalography (EEG)/electrooculography (EOG) recording system with two channels can objectively measure sleep parameters, including RWA, during natural sleep at home. We investigated whether RWA measured on a portable recording device at home could be associated with clinical PD features or other sleep metrics using baseline data from the ZEAL study. Differences between patients with and without RWA was analyzed using ANCOVA test. REM sleep length was significantly longer in patients with RWA than in those without RWA. A multivariate comparison using ANCOVA showed a significant difference in log-transformed REM sleep duration of patients with RWA after adjustment for potential confounders (adjusted mean difference of 1.203; 95% confidence interval 0.468 to 1.937; p = 0.003). The strength of this study was that it evaluated the association between RWA during natural sleep at home and clinical variables as well as other sleep metrics. The major result was that patients with and without RWA did not differ in their clinical variables, and there was no relation between RWA and objective sleep metrics other than REM sleep. The duration of REM sleep may be associated with RWA during natural sleep at home. Full article

Toxoplasmosis is a parasitic disease associated with significant morbidity and mortality. This cross-sectional study aimed to determine the prevalence, associated risk factors, and ocular health outcomes related to Toxoplasma gondii seropositivity in 161 residents from four Quilombolas communities in the northern region of Tocantins, Brazilian Legal Amazon. Peripheral blood samples were collected and tested by Enzyme-Linked Immunosorbent Assay (ELISA) for Immunoglobulin G (IgG) and/or Immunoglobulin M (IgM) and Polymerase Chain Reaction (PCR), while a standardized form was used to collect sociodemographic, health, and behavioral data. Statistical analysis, conducted using Epi-Info 3.3.2, considered T. gondii seropositivity as the primary outcome, with a significance level less than 5% (p ≤ 0.05). An overall seroprevalence of 62.11% (100/161) was observed. Key risk factors significantly, as measured by the Odds Ratio (OR), associated with T. gondii seropositivity included being elderly (OR: 4.07, CI: 2.05–8.06, p < 0.01), having cats (OR: 5.56, CI: 2.74–22.27, p < 0.01), a low parental education level (OR: 2.97, CI: 1.46–6.02, p < 0.01), children playing on the ground (OR: 2.50, CI: 1.30–4.82, p < 0.01), and having a home vegetable garden (OR: 3.80, CI: 1.94–7.47, p < 0.01). Regarding ocular health, no conclusive direct association was established between T. gondii seropositivity and specific ocular manifestations when analyzed for children and the elderly separately. Observed ocular problems in the grouped population were primarily linked to age-related comorbidities rather than parasitic infection. High rates of T. gondii seropositivity, driven by specific environmental and socioeconomic factors, highlight the vulnerability of these communities, emphasizing the need for targeted preventive strategies. Background: Childhood undernutrition and overnutrition continue to be major public health challenges in South Africa. There is limited evidence on how socio-economic factors and dietary behaviors influence nutritional outcomes among school-aged children, particularly in rural areas such as Thulamela Municipality. Objective: This study aimed to examine the socio-demographic determinants, dietary patterns, and nutritional status among school-aged children in Thulamela Municipality, Limpopo Province, South Africa. Methods: A cross-sectional survey was conducted with 347 children aged 8–12 years. Simple random sampling was used to select eight villages from a total of 227 within the municipality. A snowball sampling method was used to recruit eligible children. Data on socio-demographic characteristics, including the child’s sex, parental education level, marital status, and employment status, were collected. Additionally, their dietary habits and meal frequency patterns were collected using structured questionnaires. Anthropometric measurements including height, weight, and BMI-for-age were obtained following WHO growth standards. Associations between variables were assessed using chi-square tests, with p-values < 0.05 considered statistically significant. Results: The prevalence of severe and moderate stunting was 20.5% and 21.0%, respectively. Overweight conditions and obesity affected 32.6% and 16.2% of participants, respectively. Parental education (p = 0.027), marital status (p = 0.001), and household income (p = 0.043) showed significant associations with height-for-age and BMI-for-age Z-scores. Additionally, regular breakfast consumption and the frequent intake of vegetables and dairy products were positively associated with improved nutritional outcomes (p < 0.05). Conclusions: The nutritional profile of school-aged children in Thulamela Municipality reflects a double burden of malnutrition, with concurrent high rates of stunting, overweight conditions, and obesity. Interventions that promote balanced diets and address socio-economic disparities are crucial for improving child growth and overall health. Socio-economic factors, including parental education, marital status, and household income, were significantly associated with children’s height-for-age and BMI-for-age. Furthermore, the regular consumption of breakfast, vegetables, and dairy products was associated with better nutritional outcomes, highlighting the influence of both dietary behaviors and socio-demographic determinants on child growth and health. Implementing nutrition education programs within schools that emphasize the value of balanced diets and highlighting the significance of eating breakfast regularly and incorporating vegetables and dairy products into daily meals is important. These programs should include both children and their caregivers to support regular healthy eating behaviors at home and in school. Additionally, schools should carry out regular growth monitoring and nutritional assessments to identify early indications of undernutrition or overnutrition, enabling prompt referrals and interventions for children who may be at risk. This paper presents the design, prototype, and experimental evaluation of the AlmatyExoElbow, a lightweight cable-driven robotic exoskeleton that is intended to support elbow joint rehabilitation. The device provides two active degrees of freedom for flexion/extension and pronation/supination. It also incorporates a sensor-based control system for accurate motion tracking. The mechanical structure is fabricated using 3D-printed PLA plastic, resulting in a compact, modular, and comfortable design suitable for prolonged use. The control architecture is based on an Arduino Nano microcontroller integrated with IMU sensors, enabling the real-time monitoring of elbow motion and the precise reproduction of physiologically relevant movement patterns. The results of experimental testing demonstrate smooth and stable operation, confirming reliable torque transmission through antagonistic cable mechanisms. Overall, the proposed design achieves a balanced combination of functionality, portability, and user comfort, highlighting its potential for upper-limb rehabilitation applications in both clinical and home-based settings. Background: Simulation-based laparoscopic training increasingly relies on portable, low-cost platforms that support home-based practice, but detailed descriptions of reproducible, do-it-yourself (DIY) trainers and their educational potential remain limited. Methods: We updated a low-budget laparoscopic simulator constructed from an inexpensive plastic container, wood components, a low-cost webcam, and plywood task pads modeled on Fundamentals of Laparoscopic Surgery (FLS) exercises. We then conducted informal qualitative usability testing in which 10 residents and 5 fellows from general surgery, gynecology, and urology used the simulator at home for one week and completed an eight-item feedback form plus free-text comments on assembly, ergonomics, realism, and educational value. Results: All participants successfully assembled and used the simulator; most described set-up as easy or intuitive, reported adequate image quality and lighting, and considered the platform useful for practicing depth perception, bimanual coordination, and cutting and suturing tasks. Feedback emphasized low cost, portability, and cross-specialty applicability, with only minor suggestions such as adjustable camera height or increased base weight. Conclusions: This DIY laparoscopic simulator could be assembled and used in a home-based setting, and trainees reported favorable usability and perceived educational value. More structured validation studies addressing face, content, and construct validity are needed to define its potential role within contemporary surgical curricula. Full article
