In Jordan, where aging at home reflects Islamic cultural values, elderly populations face housing environments that fail to meet their needs, challenging sustainable aging-in-place objectives. This study explores how space personalization enhances autonomy and control among Jordan’s elderly using a mixed-methods approach combining surveys and interviews across multiple governorates. Findings reveal that space personalization strengthens elders’ sense of control over daily activities and household decisions, with private room allocation emerging as particularly significant. While conflicts between generations present barriers to control, cherished objects, such as heirlooms and religious artifacts, play a vital role in maintaining emotional connections and territorial identity. The study highlights the need for culturally sensitive housing modifications that respect cultural traditions while addressing practical needs, recommending community-based care programs and intergenerational dialogue initiatives. These insights contribute to sustainable urban development strategies that support dignified aging while preserving socio-cultural heritage, offering lessons for similar communities facing rapid demographic transitions. As the populations of many countries are aging, institutions providing support for the elderly increasingly often use nature-based interventions (NBIs) as part of their therapeutic activities. This study aimed to show the impact of 8-week active and passive NBI programs on self-esteem, well-being, and relaxation in the elderly. The NBI programs were conducted in two groups of elderly citizens of Poland: independent senior students and seniors requiring 24 h care. The Rosenberg’s Self-Esteem Scale and the World Health Organization Well-Being Index were used to assess self-esteem and well-being before and after the programs. To assess relaxation, pre–post session measurements of pulse rate (PR) and blood oxygen saturation (SpO2) were used. Our study showed an improvement in the self-esteem, well-being, and relaxation of the participants of both active and passive NBI programs. In all study groups, self-esteem and well-being improved after the programs, regardless of the type of intervention, and after each NBI session, the elderly showed a decrease in PR and an increase in SpO2, which indicated a deeper state of relaxation. Our research showed a greater improvement in the tested parameters in the seniors participating in the passive programs in a garden. This confirms a key role of green spaces in residential areas for the elderly and in nursing homes. Full article

China has been successful in getting her people to live longer. But, merely adding years to life and not life to years poses immense socio-economic challenges. However, not much is known about the effects of government policy and program interventions on changes in how well the elderly live over the years. Accordingly, we cross-temporally meta-analyzed 45 research reports (N = 36,352) that utilized the health-related quality of life (HRQoL) scale (SF-36) from 2000 to 2020. We found that: (1) the bodily pain, general health, vitality, and mental health of the elderly deteriorated over time; however, their physical and emotional roles, as well as their social functioning, improved with time; (2) the rising dependency ratio impoverished the HRQoL of the elderly; (3) the HRQoL indicators of the elderly revealed positive gains under the home-based care model whilst they showed a downward trend under the institutional pension model; (4) the HRQoL indicators of the elderly in economically developed areas produced mixed results; but, they all worsened over the years in economically underdeveloped areas. Thus, more investment efforts from the government and private entities are needed to reduce the dependency ratio and to improve the lives of the elderly under institutional care and/or in economically underdeveloped areas. Cognitive impairment is frequent in elderly subjects. It is associated with motor impairment, a limitation in quality of life and frequently, institutionalization. The aim of this work is to test the efficacy of a therapeutic group program based on action-observation learning. Methods: a non-randomized controlled trial study was conducted. We included 40 patients with cognitive impairment from a nursing home who were categorized into mild and moderate cognitive impairment and divided separately into a control and experimental group. Experimental group performed a 4-week group work, in which each patient with mild cognitive impairment was paired with a patient with moderate cognitive impairment. Thus, patients with mild cognitive impairment observed a series of functional exercises performed by their peers and replicated them. Simultaneously, the patients with moderate cognitive impairment replicated the movement after observing it performed by a patient with mild cognitive impairment. The control group continued to receive their usual care at the center. The upper limb function, cognitive level and function in basic activities of human daily life were measured before and after the intervention and compared with the control group. Results: statistically significant differences were found in the functionality of basic activities of daily living, in the functionality of the upper limb and in the cognitive level in all patients in the experimental group regardless of the initial cognitive level. No statistically significant differences were found in the control group. Conclusions: the implementation of a group, peer-based, action-observation learning therapeutic program is effective in improving the basic activities of human daily life, cognitive level and upper limb functionality in patients with mild and moderate dementia. Total hip (THA) and total knee (TKA) arthroplasty procedures have steadily increased over the past few decades, and their use is expected to grow further, mainly due to an increasing number of elderly patients. Cost-containment strategies, supporting a rapid recovery with a positive functional outcomes, high patient satisfaction, and enhanced patient reported outcomes, are needed. A Fast Track surgical procedure (FT) is a coordinated perioperative approach aimed at expediting early mobilization and recovery following surgery and, accordingly, shortening the length of hospital stay (LOS), convalescence and costs. In this view, rapid rehabilitation surgery optimizes traditional rehabilitation methods by integrating evidence-based practices into the procedure. The aim of the present study was to compare the effectiveness of Fast Track versus Care-as-Usual surgical procedures and pathways (including rehabilitation) on a mid-term patient-reported outcome (PROs), the SF12 (with regard both to Physical and Mental Scores), 3 months after hip or knee replacement surgery, with the use of Propensity score-matching (PSM) analysis to address the issue of the comparability of the groups in a non-randomized study. We were interested in the evaluation of the entire pathways, including the postoperative rehabilitation stage, therefore, we only used early home discharge as a surrogate to differentiate between the Fast Track and Care-as-Usual rehabilitation pathways. Our study shows that the entire Fast Track pathway, which includes the post-operative rehabilitation stage, has a significantly positive impact on physical health-related status (SF12 Physical Scores), as perceived by patients 3 months after hip or knee replacement surgery, as opposed to the standardized program, both in terms of the PROs score and the relative improvements observed, as compared with the minimum clinically important difference. This result encourages additional research into the effects of Fast Track rehabilitation on the entire process of care for patients undergoing hip or knee arthroplasty, focusing only on patient-reported outcomes. In China, the home-based elderly care program system plays an important role in meeting the needs of elderly individuals. The routing scheduling of home-based elderly care (RSHEC) programs is closely related to the quality of the home-based elderly care programs. The structural supply problem faced by home-based elderly care programs is a prominent problem, and the RSHEC programs are an important aspect that has rarely been studied. This paper explores RSHEC programs under uncertainty by comprehensively considering the costs of home-based elderly care, such as the fixed costs, time, and transportation costs. First, a deterministic mixed integer programming (MIP) model was constructed to solve the general routing scheduling problem. In addition, to effectively cope with the uncertainty and risk of the modern market, the robust optimization theory and algorithm model are introduced, namely, the mixed integer box set robust optimization (MIBRO) model and the mixed integer ellipsoid set robust optimization (MIERO) model. Finally, MATLAB and the Gurobi package are applied to obtain the solutions of the models. The case verification shows that the MIP model has the lowest total cost under deterministic conditions. However, the MIERO and MIBRO models can achieve more robust RSHEC programs under uncertain conditions. The results prove the effectiveness and feasibility of the optimization model and algorithm, which provides reference value for management decisions regarding home-based elderly care programs. The growing share of the population over the age of 65 is putting pressure on the social health insurance system, especially on institutions that provide long-term care services for the elderly or to people who suffer from chronic diseases or mental disabilities. This pressure can be reduced through the assisted living of the patients, based on an intelligent system for monitoring vital signs and home automation. In this regard, since 2008, the European Commission has financed the development of medical products and services through the ambient assisted living (AAL) program—Ageing Well in the Digital World. The SmartCare Project, which integrates the proposed Computer Vision solution, follows the European strategy on AAL. This paper presents an indoor human activity recognition (HAR) system based on scene understanding. The system consists of a ZED 2 stereo camera and a NVIDIA Jetson AGX processing unit. The recognition of human activity is carried out in two stages: all humans and objects in the frame are detected using a neural network, then the results are fed to a second network for the detection of interactions between humans and objects. The activity score is determined based on the human–object interaction (HOI) detections. Full article

Population aging has increased the demand for elderly care worldwide. The home-based elderly care system plays an important role in meeting this demand in developing countries. The quality of home-based elderly care is associated with the job satisfaction of caregivers in home-based elderly care programs, which has rarely been studied. This paper explores the factors that affect the job satisfaction of these elderly caregivers, including personal characteristics, working conditions, employment status, training, caregiver–client relationships, welfare, work experience, and burnout. It utilizes data from the Shanghai Domestic-work Professionalization Survey (SDPS), which was conducted among four types of in-home caregivers (n = 1000) in Shanghai over the period from May to September 2021. This paper selected a sample of elderly caregivers (n = 285) to examine their job satisfaction. The results show that gender, age, marital status, how they earned the job, relation with clients, social insurance, and work experience are significantly associated with the job satisfaction of in-home elderly caregivers, and their job satisfaction is negatively associated with their burnout levels. However, training and working conditions have no significant effect on the job satisfaction of in-home elderly caregivers, which is different from previous studies on formal care workers, such as nurses, in the institutional care system. Background: The Taiwanese government implemented a stay-at-home order that restricted all community-based health promotion activities for the elderly by shutting down all community care centers from May 2021 to August 2021 to control the spread of COVID-19. Community-based dementia care centers were barely able to provide dementia care services during that period. Methods: The data used in this study were collected from a community-based dementia care center that was able to continue their dementia care services through a Tele-Health intervention program. The difference-in-differences methodology was applied to evaluate the effects of the Tele-Health intervention program on home-dwelling persons with dementia or mild cognitive impairment and on their primary caregivers during the COVID-19 pandemic. Results: The Tele-Health intervention program significantly increased the well-being of the participants and their primary caregivers, but the negative correlations between the Tele-Health intervention program and family functioning were also found to be significant. Conclusions: The significant substitution (negative) effects between the Tele-Health intervention program and family functioning raises the concern that promotion of the Tele-Health intervention program comes at the potential cost of a loss of family functioning. Policymakers should be cautious when considering the Tele-Health intervention program in response to pandemics and demographic transitions. Full article

Population ageing becomes a perplexing conundrum with social and economic development. Many senior citizens are now empty nesters because the younger generation prefer to stay in metropolises for a better life. Therefore, living in a nursing home is a popular choice for the aged. This objective-oriented paper proposes a sustainable elderly healthcare system for nursing homes. The main work is the design and implementation of a new rapid and interactive assistance service. Based on cost-effective fingerprint indoor-positioning technology, the alert message that a person is at risk will be immediately sent to nearby people before professionals arrive. Warning messages are available when nearing marked areas (e.g., slippery floors). The parallel path-finding algorithm plays a significant role in finding nearby people and alerting people who approach specific areas. Furthermore, this system provides application programming interfaces to connect to health devices, such as smart bracelets, watches, and glasses. In general, the system is designed to ensure the safety of the elderly and improve management efficiency, which corresponds to present smart elderly care proposals from governments. Full article

Embedding the program of elderly care into community-based service system seems to imply that China is reorganising capacities of neighbourhood governance. The program, created by transformation of neighbourhood governance, represented the state government’s frustration with the institutional embodiment of neoliberalism. However, stimulating neighbourhood organisations in elderly care service through involvement of market instruments demonstrated the neoliberal approach. In this study, we provided a research framework in the context of embedded neoliberalism to explore the dilemma of neighbourhood governance in China. By interviewing 100 elderly people in five neighbourhoods in Nanjing, China, we examined the home-based elderly care (HEC) model to analyse the changes in socio-spatial relationships of neighbourhoods. We argued that the state-organised system of market instruments as a form of neighbourhood system weaken the spontaneity of elderly residents in developing social capitals. Moreover, the emerging program is struggling to operate because the devolution of conservative governance capacity from the state to the neighbourhood does not provide resources, leading to the restrained market provision. Thus, this transformation of neighbourhood governance can only be effective if there is a clear complementarity relationship between the role of state and market instruments. The attention of further studies on neighbourhood governance needs to re-examine the reciprocal relationships in the context of declining neoliberalism. Full article

Korea is currently executing a pilot program for community care of its aging population and aims to implement community care systems on a national scale by 2025. This study examines the traditional Korean medicine (TKM) service to be provided within community care by understanding the current status of TKM services. The Ministry of Health and Welfare (MoHW) sent official letters to 242 local governments (cities, districts, and counties) from October to November 2019 to survey the status of the public TKM services provided in 2018. The items of the survey included basic demographic information as well as information that could reveal how the program was implemented. In 112 local government jurisdictions (response rate 46.3%), a total of 867 TKM service programs were in place. As a result of the survey, it was revealed that they did not have any service manuals or evaluation results. To provide home-care-based TKM service for the elderly as an integrated part of a community care system, it is necessary to develop, distribute, and evaluate a standard service manual including an evaluation index by the central government. This study identified the trends in end-of-life care and nursing through text network analysis. About 18,935 articles published until September 2019 were selected through searches on PubMed, Embase, Cochrane, Web of Science, and Cumulative Index to Nursing and Allied Health Literature. For topic modeling, Latent Dirichlet Allocation (K = 8) was applied. Most of the top ranked topic words for the degree and betweenness centralities were consistent with the top 1% through the semantic network diagram. Among the important keywords examined every five years, “care” was unrivaled. When analyzing the two- and three-word combinations, there were many themes representing places, roles, and actions. As a result of performing topic modeling, eight topics were derived as ethical issues of decision-making for treatment withdrawal, symptom management to improve the quality of life, development of end-of-life knowledge education programs, life-sustaining care plan for elderly patients, home-based hospice, communication experience, patient symptom investigation, and an analysis of considering patient preferences. This study is meaningful as it analyzed a large amount of existing literature and considered the main trends of end-of-life care and nursing research based on the core subject control and semantic structure. Population ageing is an important global issue. The Taiwanese government has used various Internet of Things (IoT) applications in the “10-year long-term care program 2.0”. It is expected that the efficiency and effectiveness of long-term care services will be improved through IoT support. Home-delivered meal services for the elderly are important for home-based long-term care services. To ensure that the right meals are delivered to the right recipient at the right time, the runners need to take a picture of the meal recipient when the meal is delivered. This study uses the IoT-based image recognition system to design an integrated service to improve the management of image recognition. The core technology of this IoT-based image recognition system is statistical histogram-based k-means clustering for image segmentation. However, this method is time-consuming. Therefore, we proposed using the statistical histogram to obtain a probability density function of pixels of a figure and segmenting these with weighting for the same intensity. This aims to increase the computational performance and achieve the same results as k-means clustering. We combined histogram and k-means clustering in order to overcome the high computational cost for k-means clustering. The results indicate that the proposed method is significantly faster than k-means clustering by more than 10 times. Full article

The deployment of the Ambient Intelligence (AmI) paradigm requires designing and integrating user-centered smart environments to assist people in their daily life activities. This research paper details an integration and validation of multiple heterogeneous sensors with hybrid reasoners that support decision making in order to monitor personal and environmental data at a smart home in a private way. The results innovate on knowledge-based platforms, distributed sensors, connected objects, accessibility and authentication methods to promote independent living for elderly people. TALISMAN+, the AmI framework deployed, integrates four subsystems in the smart home: (i) a mobile biomedical telemonitoring platform to provide elderly patients with continuous disease management; (ii) an integration middleware that allows context capture from heterogeneous sensors to program environment´s reaction; (iii) a vision system for intelligent monitoring of daily activities in the home; and (iv) an ontologies-based integrated reasoning platform to trigger local actions and manage private information in the smart home. The framework was integrated in two real running environments, the UPM Accessible Digital Home and MetalTIC house, and successfully validated by five experts in home care, elderly people and personal autonomy.
