Foods as Modulators of GLP-1 Secretion

Background/Objectives: Glucagon-like peptide-1 (GLP-1), which is released by enteroendocrine L-cells, is the primary incretin hormone and plays a pivotal role in glucose regulation, appetite control, and metabolic health. While GLP-1 receptor agonists offer significant therapeutic benefits, there is a growing interest in identifying foods that can increase endogenous GLP-1 production through physiological processes. However, the physiological impact of modulating GLP-1 through diet is modest compared with activating GLP-1 receptors pharmacologically. This narrative review summarizes the latest reports on how specific food matrices and gut microbiota-dependent pathways influence GLP-1 release, emphasizing their importance in precision nutrition. Methods: Literature published between 2001 and 2026 was reviewed, including experimental, clinical, and translational studies examining the effects of foods on GLP-1 secretion, L-cell activation, nutrient-sensing receptors, microbial metabolites, and metabolic outcomes. Evidence was synthesized for food categories such as fermented dairy products, fermented vegetables, whole grains, legumes, fiber-rich root vegetables, berries, grapes, green tea, turmeric and oily fish. Results: Foods modulate endogenous GLP-1 secretion through complementary pathways involving L-cell stimulation and the activation of receptors, including FFAR2, FFAR3, GPR119, TGR5 and FFAR4 (GPR120), as well as the production of microbial metabolites. Fermented dairy products, kimchi, kefir, Jerusalem artichokes, chicory root, oats, garlic, onions, berries, grapes, green tea and oily fish support incretin regulation through microbial fermentation, the generation of short-chain fatty acids (SCFAs), bile acid signalling and the release of bioactive compounds. These food matrices shape metabolite profiles more effectively than isolated nutrients, highlighting the importance of the interaction between whole foods and the gut microbiota. Conclusions: Foods rich in fermentable substrates, polyphenols, bioactive peptides and omega-3 fatty acids are promising dietary modulators of physiological GLP-1 activity. Taking into account microbiome characteristics, metabolic phenotype and individual postprandial responses could facilitate the development of personalized nutrition strategies to help prevent and manage obesity, type 2 diabetes and related cardiometabolic disorders. Obesity is a multifactorial metabolic disorder characterized by dysregulated energy homeostasis, impaired lipid metabolism, and altered hypothalamic appetite signaling. This study aimed to evaluate the anti-obesity effects and underlying mechanisms of a 95% ethanol extract of Artocarpus integer stem bark in a high-fat diet (HFD)-induced obese rat model. Obesity was induced in male Wistar rats using HFD feeding, followed by treatment with the extract (200–800 mg/kg/day, oral). Semaglutide and resveratrol were used as reference compounds. Body weight, food intake, adiposity, serum lipid profile, adipokines, histological changes, and gene expression related to lipid metabolism (Fas, Pparγ, Cpt-1, AdipoR1) and hypothalamic appetite regulation (Pomc, Mc4r, Npy) were evaluated. Phytochemical profiling was performed using LC–HRMS. The extract markedly reduced body weight gain, visceral adiposity, and food intake compared with the HFD control (p < 0.05), with the high-dose extract demonstrating a substantial weight-reducing effect that approached the magnitude of semaglutide. Treatment improved dyslipidemia by decreasing total cholesterol and triglycerides while increasing HDL levels. Adipokine imbalance was restored, as evidenced by reduced leptin and amylin and increased adiponectin levels. At the molecular level, the extract downregulated lipogenic genes (Fas, Pparγ), partially restored Cpt-1 expression, and normalized AdipoR1. In the hypothalamus, Pomc and Mc4r were upregulated, whereas Npy was suppressed. LC–HRMS analysis revealed the presence of stilbene, chalcone, and flavonoid derivatives. In conclusion, A. integer stem bark extract exerts anti-obesity effects through coordinated modulation of appetite signaling and lipid metabolism, supporting its potential as a multi-target phytotherapeutic agent for obesity management. Introduction: Selective serotonin reuptake inhibitors (SSRIs) remain the first-line pharmacological treatment for depressive disorders. However, long-term treatment is often accompanied by weight gain, which negatively impacts medication adherence and cardiometabolic health. Emerging evidence suggests that, in addition to their effects on the central nervous system (CNS), SSRIs may alter the composition and metabolic activity of the gut microbiome, potentially contributing to metabolic disorders. However, the mechanisms linking SSRI-induced microbial changes to weight regulation remain poorly understood. Methods: This narrative review summarizes the current evidence regarding the interactions between SSRIs, the gut microbiome, host metabolism, and weight regulation. Experimental, translational, and clinical studies were critically assessed, with particular emphasis on recent systematic reviews, meta-analyses, randomized controlled trials, and mechanistic studies. A conceptual mechanistic framework was developed based on the available evidence. Results: Experimental and clinical studies indicate that SSRIs can modify both the taxonomic composition and functional activity of the gut microbiota. These changes may influence the production of microbiota-derived metabolites, including short-chain fatty acids, bile acids, and tryptophan metabolites, thereby influencing gut barrier integrity, low-grade inflammation, gut hormone secretion, appetite regulation, and energy homeostasis. Current evidence also suggests that dietary interventions—including increased dietary fiber intake, adherence to a Mediterranean diet, and microbiota-targeted strategies using prebiotics, probiotics, synbiotics, and postbiotics—can beneficially modulate these pathways. However, no randomized, controlled trials have specifically assessed whether such interventions prevent SSRI-associated weight gain. Conclusions: Taken together, the available evidence raises the possibility that gut microbiota alterations may contribute to SSRI-associated weight gain, although this hypothesis has not yet been directly tested in studies evaluating the complete mechanistic pathway. Objectives: To evaluate the efficacy of a fixed-dose combination of berberine, white mulberry leaf extract, and chromium picolinate on body weight, body composition, and insulin sensitivity in Greek adults with obesity. Research Methodology/Design: This was a 12-week, randomized, double-blind, placebo-controlled trial. Adults with obesity and no diagnosed chronic disease were randomly assigned to receive either berberine (1000 milligrams), white mulberry leaf extract (400 milligrams standardized to 5% 1-deoxynojirimycin), and chromium picolinate (0.644 milligrams) or a matching placebo, administered in two daily doses before the main meals. Setting: The study was conducted in Greece among community-dwelling adults with obesity. Main Outcome Measures: The primary outcome was insulin sensitivity, assessed using the Matsuda Composite Insulin Sensitivity Index derived from a 75-g oral glucose tolerance test. Secondary outcomes included body mass index, total body fat mass, and visceral fat rating. Results: Ninety-three participants were randomized, with 46 assigned to the active intervention and 47 to placebo. Compared with placebo, the active intervention produced significantly greater reductions in body mass index (adjusted mean difference −1.24 kg per square meter; 95% confidence interval −2.15 to −0.33), total body fat mass (−2.35 kg; 95% confidence interval −4.12 to −0.58), and visceral fat rating (−0.92 units; 95% confidence interval −1.74 to −0.10). Insulin sensitivity improved significantly, with a 28% greater improvement in the active group than in the placebo group (adjusted geometric mean ratio 1.28; 95% confidence interval 1.14 to 1.44). Conclusions: The fixed-dose combination improved BIA-derived body-composition measures and OGTT-derived insulin sensitivity over 12 weeks compared with placebo. Because no single-agent arms were included, individual component contributions and synergy remain undetermined; confirmatory trials using direct metabolic and imaging methods are warranted. Background/Objectives: Undernutrition remains a public health concern among preschool children in Malaysia. Although oral nutritional supplementation (ONS) globally has been recommended to improve nutritional status, evidence from community-based randomised controlled trials among underweight Malaysian preschool children remains limited. This study evaluated the effectiveness of ONS on growth, dietary intake, appetite, and common illnesses among underweight preschool children in Kuala Lumpur, the capital city of Malaysia. Methods: A multi-centre, community-based, cluster-randomised controlled trial was conducted among underweight preschool children aged 4–6 years from 16 government-subsidised preschools. Participants were assigned to receive ONS plus nutrition education (ONS + NE) or nutrition education alone (NE only) for 90 days. Anthropometric measurements, dietary intake, appetite, attainment of catch-up growth, and parent-reported common illnesses were assessed. Between-group differences were analysed using analysis of covariance adjusted for baseline values. Results: Ninety-six underweight preschool children were randomised to ONS plus nutrition education (ONS + NE, n = 56) or nutrition education alone (NE only, n = 40). Compared with the NE group, children receiving ONS + NE demonstrated significantly greater improvements in weight-for-age z-scores at day 60 and day 90 (p < 0.05). The ONS + NE group also achieved significantly higher catch-up growth attainment at day 60 (116.6 ± 38.5% vs. 99.1 ± 17.4%; p = 0.029) and day 90 (114.6 ± 37.2% vs. 95.5 ± 23.8%; p = 0.021). In addition, significantly higher energy, macronutrients, selected micronutrient intakes, and appetite scores were observed in the ONS + NE group (all p < 0.05). Conclusions: ONS combined with nutrition education improved weight-related growth, dietary adequacy, catch-up growth energy achievement, and appetite among underweight preschool children. ONS may serve as an effective community-based strategy for nutritional rehabilitation in this population. Trial registration: ISRCTN15093638. Full article

Background: Aggression and violence are complex behavioral phenomena with substantial biological underpinnings involving immune activation and lipid metabolism. Previous evidence has linked inflammatory signaling and altered cholesterol homeostasis to impulsive and antisocial behaviors. The present exploratory study investigated differential gene expression patterns associated with severe violent offending and their relationship with aggression-related phenotypes. Methods: Whole-blood samples from 19 male severe violent offenders and 11 healthy controls were analyzed using Clariom S gene expression arrays followed by qPCR validation. Participants additionally completed psychometric assessments including the State Trait Anger Expression Inventory-II (STAXI-II), the Psychopathic Personality Inventory-Revised (PPI-R), and the Appetitive and Facilitative Aggression Scale (AFAS). Results: Differential expression analysis identified 833 significantly dysregulated genes (FDR < 0.1; fold change ≥ 1.5) in violent offenders relative to controls. Among the most strongly altered genes were IL1B, TLR4, PTGS2 (COX-2), ABCA1, IL6R, PRF1, and MMP9. qPCR validation confirmed significant upregulation of MMP9, PTGS2, IL6R, and PRF1. Gene x behavior analyses demonstrated significant associations between PTGS2 expression and reactive as well as outwardly directed aggression, while MMP9 expression correlated with psychopathy-related traits. Furthermore, several lipid metabolism-related genes, including ABCA1, ABCG1, NPC1, DGAT2, and TLR4, showed expression patterns suggestive of altered intracellular lipid trafficking and lysosomal lipid accumulation. ABCA1 expression correlated positively with outward anger expression (STAXI-II), whereas ABCG1 and TLR4 were negatively associated with inwardly expressed anger (STAXI-II). Conclusions: These findings support a model in which innate immune activation and dysregulated lipid metabolism contribute to aggression-related phenotypes and severe violent behavior. Although exploratory and limited by sample size, the results highlight potential molecular pathways underlying pathological aggression and warrant further investigation in larger cohorts. Background/Objectives: This study aimed to examine the acute effects of a dual-task intervention combining moderate-intensity cycling with a food-related Go/No-Go task on inhibitory control, subjective appetite, and frontal electroencephalogram (EEG) activity in healthy adults. Methods: Forty healthy adults were randomly assigned to an experimental group or a control group in a single-blind design. The experimental group performed cycling combined with a food-related Go/No-Go task, whereas the control group performed only the cognitive task. Outcomes were assessed before and after the intervention, and Group × Time effects were analyzed using repeated-measures ANOVA with correction for multiple comparisons. Results: No significant Group × Time interactions were observed for the primary outcomes, including EEG alpha and theta power (ηp2 = 0.004 and 0.040, respectively) and Stroop reaction time and accuracy (ηp2 = 0.076 and 0.062, respectively). Subjective appetite decreased in the experimental group and increased in the control group, showing a significant interaction at the uncorrected level (p = 0.017, ηp2 = 0.142); however, this interaction did not remain significant after correction for multiple comparisons. The exploratory DEBQ outcomes showed no significant Group × Time interactions. Conclusions: In this single-session study, the intervention did not demonstrate clear intervention-specific benefits across the cognitive, EEG, or eating-related outcomes. Nevertheless, the contrasting descriptive pattern in subjective appetite warrants further investigation in larger studies involving repeated interventions and clinical populations. Full article

Background: In phenylketonuria (PKU), the lifelong phenylalanine (Phe)-restricted diet may contribute to altered feeding behaviours and disordered eating. The true prevalence of disordered eating and eating disorders remains uncertain, partly because validated PKU-specific assessment tools are lacking. This systematic review aimed to identify, appraise, and synthesise the evidence on disordered eating and eating disorders in PKU. Methods: Four electronic databases (PubMed, Scopus, Web of Science, and Cochrane) were searched from inception to 24 February 2026. English-language studies reporting disordered eating or eating disorders in early-diagnosed individuals with PKU managed with a Phe-restricted diet, with or without drug therapy, were included. Studies of late- or never-treated patients, pregnancy, lactation, reviews, and preclinical studies were excluded. Two independent reviewers conducted study selection and data extraction. Outcomes included the prevalence of eating disorders, disordered eating behaviours and attitudes, associated factors, and the influence of drug treatment. Findings were synthesised qualitatively, and risk of bias was assessed using the NIH Study Quality Assessment Tools. Results: Twenty-four studies (2928 individuals with PKU) met the inclusion criteria. Most were observational or survey-based. Eating disorders were reported more frequently in PKU (3–13%) than in the general population (n = 4 studies). Common disordered eating features included food neophobia (n = 5 studies), limited food variety (n = 9), food aversion or refusal (n = 5), poor appetite (n = 5), prolonged mealtimes (n = 3), negative parent–child mealtime interactions (n = 6), and specific taste preferences (n = 5). Delayed feeding skill development (n = 6 studies), reduced social eating (n = 6), gastrointestinal symptoms (n = 8), and psychological or neurodevelopmental difficulties (n = 7) were also associated with disordered eating. Early feeding difficulties frequently persisted into adulthood, and longstanding eating behaviours did not consistently improve with pharmaceutical treatment (n = 6 studies). Most studies were rated as fair quality (19/24), with common limitations including small sample sizes, lack of sample-size justification, non-validated assessment tools, and absence of randomised controlled trials. Conclusions: Current evidence suggests that people with PKU experience a greater burden of eating-related difficulties than the general population, although robust estimates of eating disorder prevalence remain lacking. Existing screening tools may misclassify treatment-related dietary behaviours as pathological or fail to identify PKU-specific eating concerns. Development of PKU-specific screening tools could support routine clinical discussions about eating behaviours and the psychosocial impact of dietary treatment, facilitating earlier identification of individuals requiring specialist eating disorder assessment. Proximal gastrectomy preserves gastric function; however, reconstruction choice influences reflux control and quality of life (QOL), and the comparative efficacy of available techniques remains unclear. A PRISMA-guided search of PubMed and Web of Science Core Collection (January 2000–April 2025) identified studies reporting reflux or QOL outcomes after proximal gastrectomy. Eligible studies included adult gastric cancer cohorts with original clinical data. Two reviewers independently screened articles, extracted predefined variables, and performed a narrative synthesis. The protocol was archived on the OSF. Thirty-two studies (2958 patients) met inclusion criteria. Simple esophagogastrostomy (EG) consistently demonstrated the poorest reflux control and widest range of stricture rates. Double-tract reconstruction (DTR) showed a generally favorable balance among reflux control, low leakage rates, and acceptable stricture rates, suggesting that it may represent a practical and broadly applicable reconstruction option. Jejunal interposition and pouch variants were reported in small, heterogeneous series, limiting their generalizability. Among valve-forming methods, the double-flap technique was associated with very low rates of endoscopic reflux in two Los Angeles–graded cohorts, though a 4–6% stricture rate remained a concern. Evidence for the side overlap with fundoplication by Yamashita, both original and modified, was sparse and inconsistent. Global QOL, assessed using PGSAS-45/37 and EORTC instruments, did not differ consistently between reconstruction types, although specific domains—such as appetite loss, nausea, and weight maintenance—varied sporadically. Available evidence suggests that DTR may be considered a practical and broadly applicable reconstruction option following proximal gastrectomy, whereas the double-flap technique appears to provide strong reflux control but may be associated with a risk of anastomotic stricture. Simple EG may was associated with higher reflux rates across studies and may be less favorable for reflux control. Larger multicenter studies using standardized, nutrition-sensitive QOL measures and physiological reflux testing are needed to optimize reconstruction choice. Quinoa (Chenopodium quinoa Willd.) is a nutrient-dense pseudocereal with a favorable nutritional profile that may improve postprandial glycemic control and satiety. This randomized crossover pilot study compared the acute effects of cooked quinoa and white bread on postprandial blood glucose and subjective appetite responses in healthy young adults. Participants consumed four test meals on separate occasions: cooked quinoa or white bread providing either 25 g or 35 g available carbohydrate. Capillary blood glucose and appetite ratings were assessed over 120 min. Quinoa produced significantly lower postprandial glucose responses and incremental area under the curve (iAUC) than white bread at both carbohydrate doses (p < 0.05). Appetite responses were also improved following quinoa consumption compared to the control. Blood glucose iAUC was positively correlated with appetite net area under the curve (r = 0.57; p < 0.001). Palatability and gastrointestinal tolerance were comparable between treatments. These findings indicate that cooked quinoa elicits more favorable postprandial glycemic and satiety responses than white bread and may serve as a functional carbohydrate alternative to support metabolic health. Background and Aims: Insufficient dietary intake is common among hospitalized older (≥65 years) adults with or at risk of malnutrition. Identifying barriers and facilitators is essential for improving nutritional care in this population. This study aimed to identify the most frequently reported and observed barriers and facilitators linked to the achievement of estimated dietary requirements during a nutritional intervention. Methods: This descriptive, inductive qualitative content analysis used data from the intervention group (n = 65) of the OptiNAM trial (NCT03741283), a randomized controlled trial evaluating a multidisciplinary nutritional intervention among acutely admitted older adults at Copenhagen University Hospital, Hvidovre. A total of 23 of the 65 participants were hospitalized for ≥24 h, allowing dietary intake to be assessed by a dietitian using validated 24 h dietary records, supplemented by dietitian-conducted 24 h recalls when records were unavailable. Individual energy and protein requirements were estimated, and intake was validated and recorded. Meaning units were derived from free-text data from patient interviews or observations during patient interviews that were linked to dietary intake and categorized as barriers or facilitators depending on whether dietary intake was below or above the estimated requirements. Results: In acutely admitted older adults with malnutrition or malnutrition risk receiving a nutritional intervention during hospitalization, poor appetite was the most frequently reported barrier to sufficient dietary intake (73.9%), followed by disliking the food (39.1%). In contrast, intrinsic motivation (69.6%), good appetite (34.8%), and support from relatives (30.4%) were the most commonly reported facilitators. Conclusions: Appetite, hospital meals, motivation, and support from relatives play central roles in achieving sufficient dietary intake among acutely admitted older adults with or at risk of malnutrition receiving a nutritional intervention. Background/Objectives: Non-caloric sweeteners are increasingly used as alternatives to sugar to reduce energy intake, yet their metabolic effects remain controversial. This study aimed to evaluate the effects of sucrose, saccharin, and steviol glycosides on glucose regulation and cardiometabolic risk markers in healthy adults. Methods: In a randomized, double-blind, crossover trial, 39 healthy, normal-weight adults consumed beverages containing sucrose (66 g/day), saccharin (220 mg/day), or steviol glycosides (220 mg/day) for 14 days, with washout periods between interventions. Metabolic outcomes were assessed at baseline and after each intervention under fasting conditions and during a 2 h postprandial test. Primary outcomes were glucose and insulin responses; secondary outcomes included gut hormones, lipids, inflammatory markers, and subjective appetite. Results: Compared with baseline, fasting glucose increased after sucrose and saccharin, and fasting insulin increased after stevia (p < 0.01). All interventions increased postprandial insulin responses and reduced indices of insulin sensitivity (p < 0.05). Fasting PYY and GLP-2 increased following all treatments (p < 0.001), without differences between sweeteners. Triglycerides were higher after sucrose than saccharin (p < 0.05), while no differences were seen for cholesterol, apolipoproteins, or CRP. Appetite ratings were unchanged, with a trend (p = 0.053) towards a reduced desire to eat after stevia in the late postprandial phase. Conclusions: Short-term intake of sucrose and non-caloric sweeteners resulted in broadly similar metabolic responses, with no significant differences in glucose regulation. However, triglyceride concentrations were higher following sucrose than saccharin, whereas no consistent differences were observed across the remaining outcomes. Observed deviations from baseline should be interpreted with caution. Further long-term studies in diverse populations are warranted. Full article

Background: Poor appetite, a major contributor to malnutrition, is highly prevalent among older adults and associated with adverse clinical outcomes. Nevertheless, there are currently no widely accepted or consistently effective pharmacological treatments targeting appetite that are routinely recommended alongside nutritional interventions. Medical cannabis has been proposed as a potential appetite stimulant in older adults with poor appetite through modulation of appetite-regulating hormones such as glucagon-like peptide-1 (GLP-1) and total ghrelin, but the evidence remains limited. Aims: To investigate the effects of Sativex® versus placebo on postprandial GLP-1 and total ghrelin concentrations following a single dose (8.1 mg Δ9-tetrahydrocannabinol (THC) and 7.5 mg cannabidiol (CBD)), and on subjective appetite following two separated doses, in older adults with poor appetite, and to investigate the associations between plasma concentrations of THC and its metabolites on these outcomes. Methods: This protocolized secondary analysis included 17 participants (≥65 years) with poor appetite from a double-blind, randomized, placebo-controlled crossover trial. Participants received Sativex® or placebo on two separate study days. GLP-1 and total ghrelin were measured following one dose of Sativex®/placebo and a standardized breakfast, and subjective appetite was assessed repeatedly using visual analog scales throughout the administration of two separate doses of Sativex®/placebo, the consumption of a standardized breakfast, and an ad libitum lunch meal. Effects/associations were analyzed using linear mixed-effects models. Results: Compared with placebo, Sativex® produced an estimated 2.38 pmol/L (95% confidence intervals (CI): −0.71 to 5.46; Bonferroni-corrected p = 0.274) reduction in mean GLP-1 and time-dependent changes in mean total ghrelin (Bonferroni-corrected p = 0.054). Compared with placebo, Sativex® produced minimal changes in separate subjective appetite sensations (all Bonferroni-corrected p = 1.00) and a slightly lower combined appetite score (−5.2 mm; 95% CI: −24.4 to 4.0; Bonferroni-corrected p = 1.00). Increased plasma concentrations of THC and metabolites were associated with reduced estimates of mean GLP-1 concentrations and subjective appetite, as well as increased mean total ghrelin concentrations. However, no statistically significant differences were detected for any analyses, and estimates had wide CIs, warranting cautious interpretation. Conclusions: In conclusion, no statistically significant differences were detected between Sativex® and placebo in postprandial GLP-1, total ghrelin, or subjective appetite over time in older adults with poor appetite. Similarly, no statistically significant associations were observed between plasma concentrations of THC and its metabolites and GLP-1, total ghrelin, or subjective appetite. Background: Obesity has reached epidemic proportions, affecting over one billion adults worldwide. While incretin-based pharmacotherapies—GLP-1 receptor agonists (semaglutide) and dual GIP/GLP-1 agonists (tirzepatide)—have transformed obesity treatment, their use remains limited by high costs, adverse effects, restricted eligibility, and rapid weight regain following discontinuation. Plant-derived bioactive compounds represent a promising complementary approach to address these gaps. Objective: This narrative review synthesizes clinical trial evidence on plant-based interventions for obesity management and examines their potential role as adjunctive strategies before, during, and after incretin-based pharmacotherapy. Methods: A literature search was conducted in PubMed, Scopus, and Web of Science (2012–2026), prioritizing randomized controlled trials in adults with overweight or obesity reporting at least one obesity-related metabolic outcome. Sixty-one studies were selected and classified by efficacy tier based on the magnitude and breadth of observed clinical effects. Results: The strongest evidence supports polyphenol-rich dietary patterns, particularly the green-Mediterranean diet, producing significant reductions in body weight, visceral fat, and cardiometabolic risk markers. Specific extracts—including curcumin, bergamot polyphenols, and Lippia citriodora/Hibiscus sabdariffa combinations—demonstrated clinically meaningful metabolic improvements. Isolated high-dose resveratrol and several single-compound interventions showed limited benefit, largely attributable to poor bioavailability. The most effective compounds acted through multiple pathways, including AMPK activation, gut microbiota modulation, and appetite hormone regulation. Conclusions: Plant-derived bioactive compounds offer a safe, accessible adjunctive strategy for obesity management, particularly relevant for patients ineligible for or discontinuing pharmacotherapy. Future trials should directly evaluate plant-polyphenol combinations alongside GLP-1 receptor agonists. Full article

The increase in the prevalence of non-communicable diseases globally has been attributed in part to poor lifestyle choices, including unhealthy dietary habits. Dietary polysaccharides, including resistant starch and non-starch polysaccharides, have gained increasing attention due to their potential role in the regulation of glucose and lipid metabolism. Therefore, the aim of this review was to evaluate the role of dietary polysaccharides in the regulation of blood glucose and lipid parameters. Method: A narrative review approach was adopted for this review. Searches were conducted through EBSCOHost and involved the following databases: Medline, APA PsycInfo, CINAHL Plus with Full Text, Psychology and Behavioural Sciences collection, Academic Search Premier and APA PsycArticles. Searches were conducted on 14 April 2026 and covered all records available from database inception to the search date. Search terms were combined using Boolean operators (AND/OR). The reference list of articles was also searched for more articles. Results: Twenty-one studies from thirteen different countries were included in this review. Based on narrative synthesis, five themes were identified: the effects of dietary polysaccharides on glycaemia, insulin, lipids, energy intake and satiety/appetite. The findings demonstrated considerable heterogeneity across studies. While several studies reported improvements in fasting glucose, postprandial glucose, glycated haemoglobin and insulin responses following resistant starch and non-starch polysaccharide interventions, other studies found no significant effects on glycaemic control or insulin levels. Lipid outcomes were similarly inconsistent, although some studies reported reductions in total cholesterol and low density lipoprotein cholesterol. Effects on energy intake and satiety varied according to the type and physicochemical characteristics of the polysaccharide investigated. Conclusion: The findings of this review suggest that dietary polysaccharides may contribute to improvements in glucose control and lipid metabolism, although the magnitude and consistency of these effects vary across populations, intervention types and study designs. The most frequently reported beneficial findings related to blood glucose parameters, although substantial heterogeneity remained across studies. Further, well-designed studies, including randomised controlled trials with longer durations, are needed to fully establish the role of dietary polysaccharides in the control of blood glucose and lipid parameters.
