Canine demodicosis is a contagious skin disease caused by the over-proliferation of Demodex mites in the host’s hair follicles. This study examines the prevalence, clinical signs, and hematological changes associated with demodicosis in street dogs of Rupandehi, Nepal. Between August 2023 and January 2024, 100 skin scrapings were collected from each street dog presenting dermatological symptoms. The samples, treated with 10% KOH and microscopically examined, revealed a 21% positivity rate for demodicosis, with all cases involving Demodex canis. The infection predominantly affected young puppies (37.5%), females (21.6%), mixed breeds (33.3%), and dogs with above-ideal body conditions (25%). There was no significant association between infection and variables such as age, gender, breed, or nutritional status. Clinically, all affected dogs exhibited alopecia, primarily on the legs. Hematological assessments indicated significant increases in neutrophils and eosinophils and a notable decrease in mean corpuscular hemoglobin concentration and lymphocytes among infected dogs (p < 0.05). This study underscores the importance of vigilant monitoring and comprehensive diagnostic practices in effectively managing and treating canine demodicosis, especially in street dogs. Full article

COVID-19 Recovery Time and Its Predictors among Hospitalized Patients in Designated Hospitals in the Madhesh Province of Nepal: A Multicentric Study

This study aimed to determine COVID-19 recovery time and identify predictors among hospitalized patients in the Dhanusha District of Madhesh Province, Nepal. This hospital-based longitudinal study involved 507 COVID-19 patients admitted to three distinct medical facilities for therapeutic intervention between April and October 2021. Data were collected for patient demography, symptoms, vital signs, oxygen saturation levels, temperatures, heart rates, respiratory rates, blood pressure measurements, and other health-related conditions. Kaplan–Meier survival curves estimated the recovery time, and a Cox proportional hazard model was used to identify the predictors of recovery time. For the total participants, mean age was 51.1 (SD = 14.9) years, 68.0% were males. Of the total patients, 49.5% recovered, and 16.8% died. The median for patient recovery was 26 days (95% CI: 25.1–26.7). Patients with severe or critical conditions were less likely to recover compared to those with milder conditions (hazard ratio (HR) = 0.34, 95% CI: 0.15–0.79; p = 0.012). In addition, an increase in oxygen saturation was associated with an elevated likelihood of recovery (HR = 1.09, 95% CI = 1.01–1.17, p = 0.018). This study underscores the need for early admission to hospital and emphasizes the targeted interventions in severe cases. Additionally, the results highlight the importance of optimizing oxygen levels in COVID-19 patient care. Full article

Escherichia coli in untreated milk leads to economic losses from subclinical mastitis and reduced milk production, while also posing a public health risk due to the emergence of antimicrobial resistant strains, particularly associated with consuming unpasteurized milk and dairy products. This study aimed to determine the prevalence and antimicrobial resistance (AMR) of E. coli isolated from buffalo milk in Siddarthanagar Municipality of Rupandehi district, Nepal. A total of 102 milk samples were collected from lactating buffaloes. The isolation and identification of E. coli were carried out using enrichment media, selective media, and biochemical tests. Antimicrobial susceptibility testing was carried out using the Kirby–Bauer disk diffusion method on Mueller–Hinton agar (Merck), according to the Clinical and Laboratory Standards Institute (CLSI) recommendations. Resistance was tested against gentamicin, amikacin, ciprofloxacin, enrofloxacin, ceftriaxone, ceftazidime, cotrimoxazole, and chloramphenicol. In addition to this, farmers were administered a questionnaire consisting of both open- and close-ended questions to identify various animal-related and management-related risk factors associated with the prevalence of E. coli. The prevalence of E. coli in our study was 29.4% (n = 30/102). Ceftriaxone and ceftazidime showed 100% resistance, while cotrimoxazole and amikacin showed 86.7% and 80% resistance, respectively. Furthermore, 86.7% of E. coli isolates were multidrug resistant (MDR). Despite suggestive trends, associations between E. coli prevalence and risk factors lacked statistical significance, necessitating further research. While some antibiotics exhibited effectiveness, many faced resistance, highlighting the need for prudent antimicrobial usage and increased awareness among farmers. Raising awareness about milk pasteurization and implementing food safety practices is essential for ensuring farmers and public health. Full article

Rabies is a vaccine-preventable, zoonotic, viral disease and a major public health concern for developing countries such as Nepal. A study was conducted from October–December 2021 among 308 household heads from three districts in Nepal (Siraha, Parsa, and Nawalparasi West) through an in-person interview to examine the rural people’s practices towards rabies. Of 70 respondents owning pet animals, 82.9% vaccinated them against rabies but 87.9% (51/58) of them kept a vaccination record. Nearly all respondents (99.7%, 307/308) said they would visit hospitals after being bitten by rabid or rabies suspected animals, and 18.2% (56/308) of them said they would also opt to visit traditional healers seeking treatment against rabies. Seven in ten respondents knew that they should wash bitten body area with soap and water. Around 60% (184/308) of respondents said they would not bother to notify or report to the local authorities if they saw someone bitten by a presumed rabid dog or observed animal behavior suggestive of rabies. The Chi-square test showed a significant association between the socio-demographic characteristics of respondents with practices (good practice and poor practice) towards rabies. The study findings suggest that rural people in Nepal need to be educated with applied rabies control and prevention practices and made aware of health seeking behavior and the role that a community members have to play to control, prevent, and eradicate rabies. Vaccination is the most effective preventive measure of COVID-19 available at present, but its success depends on the global accessibility of vaccines and the willingness of people to be vaccinated. As the vaccination rollouts are increasing worldwide, it is important to assess public perception and willingness towards vaccination, so that the aim of mass vaccination will be successful. This study aimed to understand public perception towards COVID-19 vaccines and their willingness to get vaccinated in Nepal. This cross-sectional online survey was conducted among 1196 residents of Nepal in August 2021; most of the participants of this online survey were young adults (18–47 years) with university-level education. A total of 64.5% (771/1196) of the participants perceived COVID-19 vaccines to be safe and risk-free, while 68.6% (820/1196) agreed that vaccination would be efficient in the fight against this pandemic. Most of the participants (841/1196, 70.3%) disagreed that people are getting COVID-19 vaccines easily in Nepal, while they agree with the prioritization of older adults and healthcare workers for vaccination. A total of 61.1% (731/1196) of the participants had received at least one dose of the vaccine. Among the unvaccinated, 93.3% (434/465) were willing to get vaccinated when their turn came. The higher confidence of younger adults in vaccines and the vaccination process is encouraging, as that can help educate others who are hesitant or are not positive towards the idea of receiving vaccines. Dissemination of correct and current information, acquisition of enough doses of vaccines, and equitable distribution of vaccines will be required to achieve successful completion of the COVID-19 vaccination campaign in Nepal. Do leaders who build a sense of shared social identity in their teams thereby protect them from the adverse effects of workplace stress? This is a question that the present paper explores by testing the hypothesis that identity leadership contributes to stronger team identification among employees and, through this, is associated with reduced burnout. We tested this model with unique datasets from the Global Identity Leadership Development (GILD) project with participants from all inhabited continents. We compared two datasets from 2016/2017 (n = 5290; 20 countries) and 2020/2021 (n = 7294; 28 countries) and found very similar levels of identity leadership, team identification and burnout across the five years. An inspection of the 2020/2021 data at the onset of and later in the COVID-19 pandemic showed stable identity leadership levels and slightly higher levels of both burnout and team identification. Supporting our hypotheses, we found almost identical indirect effects (2016/2017, b = −0.132; 2020/2021, b = −0.133) across the five-year span in both datasets. Using a subset of n = 111 German participants surveyed over two waves, we found the indirect effect confirmed over time with identity leadership (at T1) predicting team identification and, in turn, burnout, three months later. Finally, we explored whether there could be a “too-much-of-a-good-thing” effect for identity leadership. Speaking against this, we found a u-shaped quadratic effect whereby ratings of identity leadership at the upper end of the distribution were related to even stronger team identification and a stronger indirect effect on reduced burnout. Land cover (LC) is a crucial parameter for studying environmental phenomena. Cutting-edge technology such as remote sensing (RS) and cloud computing have made LC change mapping efficient. In this study, the LC of Rupandehi District of Nepal were mapped using Landsat imagery and Random Forest (RF) classifier from 2005 to 2020 using Google Earth Engine (GEE) platform. GEE eases the way in extracting, analyzing, and performing different operations for the earth’s observed data. Land cover classification, Centre of gravity (CoG), and their trajectories for all LC classes: agriculture, built-up, water, forest, and barren area were extracted with five-year intervals, along with their Ecosystem service values (ESV) to understand the load on the ecosystem. We also discussed the aspects and problems of the spatiotemporal analysis of developing regions. It was observed that the built-up areas had been increasing over the years and more centered in between the two major cities. Other agriculture, water, and forest classes had been subjected to fluctuations with barren land in the decreasing trend. This alteration in the area of the LC classes also resulted in varying ESVs for individual land cover and total values for the years. The accuracy for the RF classifier was under substantial agreement for such fragmented LCs. Using LC, CoG, and ESV, the paper discusses the need for spatiotemporal analysis studies in Nepal to overcome the current limitations and later expansion to other regions. Studies such as these help in implementing proper plans and strategies by district administration offices and local governmental bodies to stop the exploitation of resources. Full article

Despite being vaccine preventable, the global burden of dog rabies remains significant, and historically it is the rural and marginalized communities in developing countries of Africa and Asia that are most threatened by the disease. In recent years, the developing world has been experiencing unprecedented increases in urbanization, with a correspondingly massive increase in municipal solid waste generation, among other things. Inefficient and inadequate waste collection and management, due to lack of resources and planning, led to significant increases in the volumes of waste on the streets and in open dumps, where it serves as food sources for free-roaming dogs. In this commentary, we discuss examples of poor waste management and the likely impact on rabies control efforts through the sustenance of free-roaming dogs in some dog rabies-endemic countries. We aim to stress the importance of implementing strategies that effectively address this particular issue as an important component of humane dog population management, as it relates to aspirations for the control and elimination of dog rabies per se. Besides a proper agronomic management followed by Nepalese farmers, wheat (Triticum aestivum L.) production has been severely affected by changing climate. There are many interventions, including climate-smart practices, to cope with this situation and possibly enhance crop and soil productivity. Field experiments were set up in a randomized complete block design with six treatments (TRT) with four replications in three locations (LOC) during wheat-growing seasons in Nepal from 2014 to 2016. Treatments included (i) Controlled Practice (CP), (ii) Improved Low (IL), (iii) Improved High (IH), (iv) Climate Smart Agriculture Low (CSAL), (v) Climate Smart Agriculture Medium (CSAM), and (vi) Climate Smart Agriculture High (CSAH), whereas those LOC were Banke, Rupandehi and Morang districts. There was a significant main effect of TRT and LOC on grain yield and a significant interactionn effect of TRT × LOC on biomass yield in 2014–2015. About 55.5% additional grain yield was produced from CSAM treatment compared to CP in 2014–2015. Among locations, grain yield was the highest in Banke (3772.35 kg ha−1) followed by Rupandehi (2504.47 kg ha−1) and Morang districts (2504.47 kg ha−1). In 2015–2016, there was a significant interaction effect of TRT × LOC on grain and biomass yields. The highest grain yield was produced from CSAH treatment in Banke district in 2015–2016. Overall, grain yield and other parameters showed a better response with either of the climate-smart interventions (mostly CSAH or CSAM) despite variability in geography, climate, and other environmental factors indicating the potential of climate-smart practices to improve wheat production in southern plains of Nepal. Coronavirus Disease 2019 (COVID-19) has rapidly spread to all corners of the globe. Different diagnostic tools, such as Chest X-ray (CXR), lung ultrasound (LUS), and computerized tomography (CT), have been used to detect active pneumonic lesions associated with COVID-19 with their varying degrees of sensitivity and specificity. This study was undertaken to investigate the clinical characteristics of COVID-19 patients with a pneumonic lung lesion detected by CT that is not detected by CXR. A total of 156 COVID-19 patients hospitalized at three nationally designated South Korean hospitals with no active lesion detected by CXR but on clinical suspicion of pneumonia underwent the CT examination and were enrolled. Medical records, which included demographic and clinical features, including comorbidity, symptoms, radiological, and laboratory findings on admission, were reviewed and analyzed. The risk factors of pneumonia detected by CT for patients without an active lesion detected by CXR were investigated. Of the 156 patients without an active lesion detected by CXR, 35 (22.44%) patients were found to have pneumonia by CT. The patients with pneumonia defined by CT were older than those without (64.1 years vs. 41.2 years). Comorbidities such as hypertension, diabetes, cardiovascular disease, preexisting stroke, and dementia were more common among patients with pneumonia defined by CT than those without. Serum albumin level, C-reactive protein (CRP), stroke, and age ≥ 70 years were significantly associated with pneumonia defined by CT after adjustment for age. In multivariable regression analysis, serum albumin level (adjusted odds ratio (AOR) = 0.123, 95% CI = (0.035–0.429)) and preexisting stroke (AOR = 11.447, 95% CI = (1.168–112.220)) significantly and independently predicted pneumonia detection by CT. Our results suggest that CT scans should be performed on COVID-19 patients negative for a pneumonic lung lesion by CXR who are suspected to be pneumonic on clinical grounds. In addition, older patients with a lower albumin level and a preexisting stroke should be checked for the presence of pneumonia despite a negative CXR finding for an active lesion. Full article

In Nepal and elsewhere in the Indo-Gangetic plains where rice-wheat is a major crop rotation, interest in conservation practices such as direct-seeding of rice on zero-tilled soil and retention of crop residue is increasing. However, the use of herbicide is increasing in the region due to a shortage of labor and its ease of operation compared to manual weeding. This field experiment was conducted to identify the response of rice to tillage and planting systems, level of wheat residue retention and weed managements under rice-wheat cropping system. This study was conducted during three growing seasons of rice (June through November) in 2014, 2015 and 2016 at the National Wheat Research Program (NWRP), Rupandehi, Nepal. The experiment was conducted in a split-split plot design. Tillage and planting systems were the main plots where rice was either transplanted on puddled field managed with conventional tillage (CT) or direct seeded on zero till (ZT) soil. The level of residue retention was the sub-plot which included three levels of residue retention as whole (WR), partial (PR) or no (NR) retention. Forms of weed management were the sub-sub plots with manual weeding (MW) compared with chemical weeding (CW) through the application of bispyribac-sodium. Each treatment combination consisted of three replicated units. Averaged across the years, grain yield of rice under the CT system (4.8 t ha−1) was significantly higher than ZT (4.4 t ha−1). Increased level of wheat residue retention increased grain yield consistently in all three years. Grain yield was not influenced by systems of weed management. The following conclusions were drawn from the results: (i) rice grain yield might decrease under a direct-seeded ZT system more than the conventional system, (ii) wheat residue retained in the field can increase rice grain yield significantly, and (iii) application of bispyribac-sodium could be equally effective as manual weeding for weed control in both tillage/planting systems of rice. Studies have confirmed COVID-19 patients with diabetes are at higher risk of mortality than their non-diabetic counterparts. However, data-driven evidence of factors associated with increased mortality risk among hospitalized COVID-19 patients with diabetes is scarce in South Korea. This study was conducted to determine the mortality rate and identify risk factors of mortality among hospitalized COVID-19 patients with type 2 diabetes in Gyeongsangbuk-do province, South Korea. In this hospital-based, cross-sectional study, we enrolled a total of 324 patients with confirmed COVID-19, hospitalized at two of the tertiary level healthcare facilitates of Gyeongsangbuk-do, South Korea from 18 February to 30 June 2020. Demographic and clinical data and laboratory profiles were analyzed and multivariate logistic regression analysis was used to identify risk factors of mortality among diabetic patients with COVID-19. Of the 324 patients, 55 (16.97%) had diabetes mellitus. The mean age of all study subjects was 55 years, and the mean age of those with diabetes was greater than that of those without (69.8 years vs. 51.9 years). Remarkably, the mortality rate was much higher among those with diabetes (20.0% vs. 4.8%). Multivariate logistic regression analysis revealed that an older age (≥70 years) and a high serum lactate dehydrogenase (LDH) levels significantly predicted mortality among hospitalized COVID-19 patients with diabetes. Our study cautions more attention to be paid to patients with diabetes mellitus hospitalized for COVID-19, especially those aged ≥ 70 years and those with a high serum LDH level, to reduce the risk of mortality. The livestock and poultry sectors are an integral part of Nepalese economy and lifestyle. Livestock and poultry populations have continuously been increasing in the last decade in Nepal and are likely to follow that trend as the interests in this field is growing. Infectious diseases such as Foot and Mouth Disease (FMD), Peste des Petits Ruminants (PPR), hemorrhagic septicemia (HS), black quarter (BQ), swine fever, avian influenza, and Newcastle disease (ND) constitute one of the major health challenges to the Nepalese livestock and poultry industry. Vaccinations are an efficient means of preventing the occurrence and spread of several diseases in animals and birds. Considering this fact, the government of Nepal began the production of veterinary vaccines in the 1960s. Nepal is self-reliant in producing several vaccines for cattle and buffaloes, sheep and goats, pigs, and poultry. Despite these efforts, the demand for vaccines is not met, especially in the commercial poultry sector, as Nepal spends billions of rupees in vaccine imports each year. There is a need of strengthening laboratory facilities for the isolation and characterization of field strains of pathogens and capacity building for the production of different types of vaccines using the latest technologies to be self-reliant in veterinary vaccine production in the future in Nepal. (1) Background: In response to the notification made by an elementary school authority that reported a number of elementary school children being absent in three schools as a result of gastroenteritis symptoms on 4 July 2018, in Gyeongsangbuk-Do Province, South Korea, an epidemic investigation was carried out to determine the extent, cause, and source of the outbreak in order to prevent secondary cases and make recommendations to prevent future recurrences. (2) Methods: In this epidemiologic study, a total of 106 human subjects (school children, staff members, and cooks) who had consumed the possibly contaminated foodstuffs were enrolled retrospectively. Human specimens from clinically defined cases, food and drinks, supply and storage of them, and environmental and sanitary conditions were also assessed by observation, laboratory tests, and survey questionnaires—where and whatever applicable. The attack rate and positive rate for human specimens were first presented followed by the calculation of the relative risk ratio (RR) with 95% CI (confidence intervals) in order to identify the exposure and outcome relationships. (3) Results: The attack rate was 12.26% (13/106) for those who had ingested the food items at the three schools and the positive rate of enteropathogenic Escherichia coli (EPEC) was 15.38% (2/13). The relative risk (RR) of developing food poisoning of those who consumed the cucumber chili with ssamjang and seasoned cucumber and chives were 4.55 (95% CI 1.05–19.54) and 9.20 (95% CI 1.24–68.22), respectively. In addition, within the human specimens as well as the water and environmental samples different strains of diarrhoeagenic enteropathogenic Escherichia coli (EPEC) were detected. (4) Conclusions: Provision of safe and wholesome water access to all elementary schools by concerned authorities, especially during the likely seasons of water source contamination, as well as health education promotion about foodborne outbreaks to all school stakeholders is therefore recommended. Brachial-ankle pulse wave velocity (baPWV) provides a useful means of assessing cardiovascular events and diabetic complications. However, the nature of associations between baPWV and microalbuminuria (MAU) and its presence in Type 2 diabetes mellitus (Type 2 DM) have rarely been investigated. This study aimed to examine the association between baPWV and MAU coupled with prediction of MAU using baPWV measurement among Type 2 DM patients. In this cross-sectional study, we enrolled 424 Type 2 DM patients who visited the cardiology and endocrinology department at a tertiary level health care facility, Republic of Korea between 1 January 2006 to 31 December 2008. Clinical and laboratory data were collected, and risk factors associated with MAU and prediction of risk for the development of MAU using baPWV measurement. The association between MAU and baPWV was examined using multivariable logistic regression analysis and predicted MAU by using receiver operating characteristic (ROC) curve analysis. Of the 424 Type 2 DM patients, 93 (21.9%) had MAU (20–200 μg/min). baPWV (cm/sec) was found to be significantly correlated with MAU levels (ug/min) (r = 0.791, p < 0.001). Further, baPWV was significantly associated MAU with higher odds ratio (adjusted odds ratio (AOR) 10.899; 95% confidence interval (CI) (4.518–26.292)). Similarly, smoking (AOR 5.736; 95% CI (1.036–31.755)), and low-density lipoprotein (LDL)-cholesterol (mg/dL) (AOR 1.017; 95% CI (1.001–1.033)) were also significantly associated with MAU. The appropriate cut-off value for baPWV to predict MAU 20 μg/min in our study was 1700 cm/sec (area under ROC curve = 0.976). This study shows that baPWV, cigarette smoking, and LDL-cholesterol are associated with MAU in Type 2 DM patients and suggests that a baPWV cut-off of 1700 cm/sec could be used to predict the presence of MAU (20 μg/min) in Type 2 DM patients in the Korean community.
