Preprocessing of Physician Notes by LLMs Improves Clinical Concept Extraction Without Information Loss

Clinician notes are a rich source of patient information, but often contain inconsistencies due to varied writing styles, abbreviations, medical jargon, grammatical errors, and non-standard formatting. These inconsistencies hinder their direct use in patient care and degrade the performance of downstream computational applications that rely on these notes as input, such as quality improvement, population health analytics, precision medicine, clinical decision support, and research. We present a large-language-model (LLM) approach to the preprocessing of 1618 neurology notes. The LLM corrected spelling and grammatical errors, expanded acronyms, and standardized terminology and formatting, without altering clinical content. Expert review of randomly sampled notes confirmed that no significant information was lost. To evaluate downstream impact, we applied an ontology-based NLP pipeline (Doc2Hpo) to extract biomedical concepts from the notes before and after editing. F1 scores for Human Phenotype Ontology extraction improved from 0.40 to 0.61, confirming our hypothesis that better inputs yielded better outputs. We conclude that LLM-based preprocessing is an effective error correction strategy that improves data quality at the level of free text in clinical notes. This approach may enhance the performance of a broad class of downstream applications that derive their input from unstructured clinical documentation. Full article

Wearable technology can nowadays be used to improve para-swimming coaching; however, the extent to which individual anatomy affects features of swimming variability is unclear. Six paralympic swimmers were recruited, their upper-limb segment lengths were measured, and their absolute bilateral limb asymmetry indices (AbsLAIUL) were calculated. They were instrumented with a sacrum-worn inertial measurement unit and performed an in-water, fatiguing, freestyle aerobic test at incrementally faster paces. Stroke-to-stroke outcome and execution variability were calculated, respectively, using sample entropy (SampEn) and fractal dimension (FD) on forward and mediolateral linear acceleration signals. Significantly increased perceived exertion scores (F(4,28) = 154.1, p < 0.001) were observed. Execution and outcome variability increased in the forward (SampEn = F(4,25) = 11.86, p < 0.001; FD = F(4,24) = 6.17, p = 0.001) and mediolateral (SampEn = F(4,25) = 9.46, p < 0.001; FD = F(4,24) = 27.64, p < 0.001) directions. Modelling of FD (only) improved with AbsLAIUL as a covariate (forward = F(1,24) = 9.68, p = 0.005; mediolateral = F(1,24) = 8.57, p = 0.021), suggesting that AbsLAIUL affects only execution, but not outcome, variability. This information could help coaches determine which coordination indices should be personalized when monitoring variability during para-swimming training. Full article

To meet the demands of laser communication, quantum precision measurement, cold atom technology, and other fields for narrow linewidth and low-noise light sources, an external cavity diode laser (ECDL) operating in the wavelength range around 780 nm was set up with a Fabry–Pérot etalon (F–P) and an interference filter (IF) in the experiment. The interference filter type ECDL (IF–ECDL) with butterfly-style packaging configuration has continuous wavelength tuning within a specified range through precise temperature and current control and has excellent single-mode characteristics. Experimental results indicate that the output power of the IF–ECDL is 14 mW, with a side-mode suppression ratio (SMSR) of 54 dB, a temperature-controlled mode-hop-free tuning range of 527 GHz (1.068 nm), and an output linewidth of 570 Hz. Compared to traditional lasers operating at 780 nm, the IF–ECDL exhibits narrower linewidth, lower noise, and higher spectral purity, and its dimensions are merely 25 × 15 × 8.5 mm3 weighing only 19.8 g, showcasing remarkable miniaturization and lightweight advantages over similar products in current research fields. Rhinovirus C (RV-C) infection can trigger asthma exacerbations in children and adults, and RV-C-induced wheezing illnesses in preschool children correlate with the development of childhood asthma. Surfactant protein A (SP-A) plays a critical role in regulating pulmonary innate immunity by binding to numerous respiratory pathogens. Mature SP-A consists of multiple isoforms that form the hetero-oligomers of SP-A1 and SP-A2, organized in 18-mers. In this report, we examined the efficacy of SP-A to antagonize RV-C infection using the wild-type (RV-C15) and reporter-expressing (RV-C15-GFP) viruses in differentiated nasal epithelial cells (NECs) from asthmatic and non-asthmatic children. We also determined the antiviral mechanism of action of SP-A on RV-C15 infection. The native SP-A was purified from alveolar proteinosis patients. The recombinant (r) SP-A1 and SP-A2 variants were expressed in FreeStyle™ 293-F cells. SP-A reduced the fluorescent focus-forming units (FFUs) after RV-C15-GFP infection of NECs by 99%. Both simultaneous and 4 h post-infection treatment with SP-A inhibited RV-C15 and RV-C15-GFP viral RNA load by 97%. In addition, the antiviral genes and chemokines (IFN-λ, IRF-7, MDA-5, and CXLC11) were not induced in the infected NECs due to the inhibition of RV-C propagation by SP-A. Furthermore, SP-A bound strongly to RV-C15 in a dose- and Ca2+-dependent manner, and this interaction inhibited RV-C15 binding to NECs. In contrast, rSP-A1 did not bind to solid-phase RV-C15, whereas the rSP-A2 variants, [A91, K223] and [P91, Q223], had strong binding affinities to RV-C15, similar to native SP-A. This study demonstrates that SP-A might have potential as an antiviral for RV infection and RV-induced asthma exacerbations. Full article

This study aimed to identify the relationship between dryland tests and swimming performance in elite Paralympic swimmers. Fifteen competitive swimmers (age: 27.4 ± 5.4 years, height: 1.70 ± 6.8 m, body mass: 67.9 ± 9.2 kg; 9 males, 6 females) performed a lat pull-down and a bench press incremental load test to determine maximum power (Pmax), the strength corresponding to maximum power (F@Pmax), and the barbell velocity corresponding to maximum power (V@Pmax) from the force–velocity and power–velocity profiles. These outcomes were also normalized by the athlete’s body mass. Swimming performance was carried out from the best result in a 100 m freestyle race registered during an international competition. Lat pull-down F@Pmax was significantly associated with 100 m freestyle chronometric time (ρ = −0.56, p < 0.05), and lat pull-down V@Pmax presented a relationship with mean swimming velocity (ρ = 0.71, p < 0.01). Similarly, bench press F@Pmax and the normalized F@Pmax were significantly related to the mean swimming velocity (ρ = −0.51, ρ = −0.62, p < 0.05). Stepwise multiple regression showed that lat pull-down V@Pmax, bench press normF@Pmax, and V@Pmax accounted for 40.6%, 42.3%, and 65.8% (p < 0.05) of the mean swimming velocity variance. These preliminary results highlighted that simple dryland tests, although with a moderate relationship, are significantly associated with 100 m freestyle swimming performance in elite Paralympic swimmers. This study endeavors to ascertain alterations in the in-line registered milk fat-to-protein ratio as a potential indicator for evaluating the metabolic status of dairy cows. Over the study period, farm visits occurred biweekly on consistent days, during which milk composition (specifically fat and protein) was measured using a BROLIS HerdLine in-line milk analyzer (Brolis Sensor Technology, Vilnius, Lithuania). Clinical examinations were performed at the same time as the farm visits. Blood was drawn into anticoagulant-free evacuated tubes to measure the activities of GGT and AST and albumin concentrations. NEFA levels were assessed using a wet chemistry analyzer. Using the MediSense and FreeStyle Optium H systems, blood samples from the ear were used to measure the levels of BHBA and glucose in plasma. Daily blood samples were collected for BHBA concentration assessment. All samples were procured during the clinical evaluations. The cows were categorized into distinct groups: subclinical ketosis (SCK; n = 62), exhibiting elevated milk F/P ratios without concurrent clinical signs of other post-calving diseases; subclinical acidosis (SCA; n = 14), characterized by low F/P ratios (<1.2), severe diarrhea, and nondigestive food remnants in feces, while being free of other post-calving ailments; and a healthy group (H; n = 20), comprising cows with no clinical indications of illness and an average milk F/P ratio of 1.2. The milk fat-to-protein ratios were notably higher in SCK cows, averaging 1.66 (±0.29; p < 0.01), compared to SCA cows (0.93 ± 0.1; p < 0.01) and healthy cows (1.22). A 36% increase in milk fat-to-protein ratio was observed in SCK cows, while SCA cows displayed a 23.77% decrease. Significant differences emerged in AST activity, with SCA cows presenting a 26.66% elevation (p < 0.05) compared to healthy cows. Moreover, SCK cows exhibited a 40.38% higher NEFA concentration (p < 0.001). A positive correlation was identified between blood BHBA and NEFA levels (r = 0.321, p < 0.01), as well as a negative association between BHBA and glucose concentrations (r = −0.330, p < 0.01). Notably, AST displayed a robust positive correlation with GGT (r = 0.623, p < 0.01). In light of these findings, this study posits that milk fat-to-protein ratio comparisons could serve as a non-invasive indicator of metabolic health in cows. The connections between milk characteristics and blood biochemical markers of lipolysis and ketogenesis suggest that these markers can be used to check the metabolic status of dairy cows on a regular basis. Glycosylation of IgG regulates the effector function of this antibody in the immune response. Glycosylated IgG is a potent therapeutic used for both research and clinical purposes. While there is ample research on how different cell culture conditions affect IgG glycosylation, the data are missing on the stability of IgG glycome during long cell passaging, i.e., cell “aging”. To test this, we performed three independent time course experiments in FreeStyle 293-F cells, which secrete IgG with a human-like glycosylation pattern and are frequently used to generate defined IgG glycoforms. During long-term cell culturing, IgG glycome stayed fairly stable except for galactosylation, which appeared extremely variable. Cell transcriptome analysis revealed no correlation in galactosyltransferase B4GALT1 expression with galactosylation change, but with expression of EEF1A1 and SLC38A10, genes previously associated with IgG galactosylation through GWAS. The FreeStyle 293-F cell-based system for IgG production is a good model for studies of mechanisms underlying IgG glycosylation, but results from the present study point to the utmost importance of the need to control IgG galactosylation in both in vitro and in vivo systems. This is especially important for improving the production of precisely glycosylated IgG for therapeutic purposes, since IgG galactosylation affects the inflammatory potential of IgG. Full article

The name “Spergularia hanoverensis Simon” has been misapplied to an endemic taxon confined to inland semidesert ecosystems in central-western South Africa. It is commonly accepted as a small annual species occurring in saline habitats in a wide elevation range, but its identity still remains obscure. In the context of taxonomic and phylogenetic research on the African species of Spergularia, we found that the name was never validly published. After revision of herbarium material housed in South African herbaria, a voucher collected from Hanover was found at PRE bearing some labels handwritten by E. Simon that suggest it might be an intended type for the name. Additional herbarium material and wild populations from the Karoo region were identified that matched the samples in that voucher, and taxonomic research was conducted to clarify their identity. Among other characters, those Karoo plants show a woody dense compact habit, woody perennial at base; stems prostrate to ascendent; leaves entirely glabrous, somewhat glaucous; large white-hyaline conspicuous stipules; inflorescence glanduliferous, many-flowered subdichasial cyme, with minute bracts; flowers small, with white petals approximately equalling sepals in length, stamens 7–8, and styles free from base; capsule small, with seeds dimorphic, unwinged to broadly winged, with testa always densely tuberculate. Molecular analyses of plastid (trnL-trnF region) and nuclear ribosomal (5.8S-ITS2 region) DNA sequence data support morphological differentiation of the Karoo plants, for which the name S. hanoverensis is here effectively published. A full morphological description and data on ecology, habitat, distribution, and taxonomic and phylogenetic relationships of S. hanoverensis are compared to other members of the “South African group”, namely S. glandulosa, S. namaquensis, and S. quartzicola, from which the new species considerably differs. The adaptative significance of dimorphic seeds of S. hanoverensis is briefly commented on in the context of the species habitat preference. An identification key is presented for the South African related taxa. Full article

HEK293 is a widely used cell line in the fields of research and industry. It is assumed that these cells are sensitive to hydrodynamic stress. The aim of this research was to use particle image velocimetry validated computational fluid dynamics (CFD) to determine the hydrodynamic stress in both shake flasks, with and without baffles, and in stirred Minifors 2 bioreactors to evaluate its effect on the growth and aggregate size distribution of HEK293 suspension cells. The HEK FreeStyleTM 293-F cell line was cultivated in batch mode at different specific power inputs (from 63 W m3 to 451 W m3), whereby 60 W m3 corresponds to the upper limit, which is what has been typically described in published experiments. In addition to the specific growth rate and maximum viable cell density VCDmax, the cell size distribution over time and cluster size distribution were investigated. The VCDmax of (5.77±0.02)·106cellsmL1 was reached at a specific power input of 233 W m3 and was 23.8% higher than the value obtained at 63 W m3 and 7.2% higher than the value obtained at 451 W m3. No significant change in the cell size distribution could be measured in the investigated range. It was shown that the cell cluster size distribution follows a strict geometric distribution whose free parameter p is linearly dependent on the mean Kolmogorov length scale. Based on the performed experiments, it has been shown that by using CFD-characterised bioreactors, the VCDmax can be increased and the cell aggregate rate can be precisely controlled. Full article

Videogames and their business models have evolved significantly over time, with consumers preferring a shift towards free-to-play (F2P) without any initial purchase, as evidenced in the popularity of Fortnite, Warzone and others. The aim of this research is to establish the viability of a switch from a buy-to-play (B2P) to a F2P model for the game Overwatch and the impact on its associated esports. The relevant literature within the subject area was identified. A framework was then developed to determine whether a switch to F2P would be successful for Overwatch, based on the criteria seen as significant within the literature identified. These criteria represent a mix of quantitative and qualitative approaches, and a mix of styles, with some being more descriptive with biographical elements of the author’s experience, and others being more analytical. The main conclusion drawn from the analysis undertaken is that Overwatch would be well suited for a switch to F2P. The sequel to Overwatch, Overwatch 2, is due to release in the near future, which would have opened the possibility of Overwatch being free, while Overwatch 2 is paid. However, Overwatch 1 is confirmed to be shutting down completely. It is also concluded there would be a likely increase in player numbers, and that a switch to F2P is likely to improve the problematic esports scene associated with Overwatch. Full article

Simple hands-free input methods using ear accessories have been proposed to broaden the range of scenarios in which information devices can be operated without hands. Although many previous studies use canal-type earphones, few studies focused on the following two points: (1) A method applicable to ear accessories other than canal-type earphones. (2) A method enabling various ear accessories with different styles to have the same hands-free input function. To realize these two points, this study proposes a method to recognize the user’s facial gesture using an infrared distance sensor attached to the ear accessory. The proposed method detects skin movement around the ear and face, which differs for each facial expression gesture. We created a prototype system for three ear accessories for the root of the ear, earlobe, and tragus. The evaluation results for nine gestures and 10 subjects showed that the F-value of each device was 0.95 or more, and the F-value of the pattern combining multiple devices was 0.99 or more, which showed the feasibility of the proposed method. Although many ear accessories could not interact with information devices, our findings enable various ear accessories with different styles to have eye-free and hands-free input ability based on facial gestures. Pathology reports represent a primary source of information for cancer registries. University Malaya Medical Centre (UMMC) is a tertiary hospital responsible for training pathologists; thus narrative reporting becomes important. However, the unstructured free-text reports made the information extraction process tedious for clinical audits and data analysis-related research. This study aims to develop an automated natural language processing (NLP) algorithm to summarize the existing narrative breast pathology report from UMMC to a narrower structured synoptic pathology report with a checklist-style report template to ease the creation of pathology reports. The development of the rule-based NLP algorithm was based on the R programming language by using 593 pathology specimens from 174 patients provided by the Department of Pathology, UMMC. The pathologist provides specific keywords for data elements to define the semantic rules of the NLP. The system was evaluated by calculating the precision, recall, and F1-score. The proposed NLP algorithm achieved a micro-F1 score of 99.50% and a macro-F1 score of 98.97% on 178 specimens with 25 data elements. This achievement correlated to clinicians’ needs, which could improve communication between pathologists and clinicians. The study presented here is significant, as structured data is easily minable and could generate important insights. Full article

A cancer pathology report is a valuable medical document that provides information for clinical management of the patient and evaluation of health care. However, there are variations in the quality of reporting in free-text style formats, ranging from comprehensive to incomplete reporting. Moreover, the increasing incidence of cancer has generated a high throughput of pathology reports. Hence, manual extraction and classification of information from these reports can be intrinsically complex and resource-intensive. This study aimed to (i) evaluate the quality of over 80,000 breast, colorectal, and prostate cancer free-text pathology reports and (ii) assess the effectiveness of random forest (RF) and variants of support vector machine (SVM) in the classification of reports into benign and malignant classes. The study approach comprises data preprocessing, visualisation, feature selections, text classification, and evaluation of performance metrics. The performance of the classifiers was evaluated across various feature sizes, which were jointly selected by four filter feature selection methods. The feature selection methods identified established clinical terms, which are synonymous with each of the three cancers. Uni-gram tokenisation using the classifiers showed that the predictive power of RF model was consistent across various feature sizes, with overall F-scores of 95.2%, 94.0%, and 95.3% for breast, colorectal, and prostate cancer classification, respectively. The radial SVM achieved better classification performance compared with its linear variant for most of the feature sizes. The classifiers also achieved high precision, recall, and accuracy. This study supports a nationally agreed standard in pathology reporting and the use of text mining for encoding, classifying, and production of high-quality information abstractions for cancer prognosis and research. Using a mixed-methods design, we evaluated Farm Fresh Food Box (F3B) a market innovation designed to expand producer markets, stabilize rural retail businesses, and improve rural food access. In the F3B model, pre-ordered Community Supported Agriculture (CSA)-style produce boxes are sold through rural retail outlets. F3B was implemented from 2016 to 2018 as part of a United States Department of Agriculture (USDA)-funded multi-state extension and research collaboration project in 3 geographically diverse and rural areas: Vermont, Washington, and California. The F3B evaluation aimed to (1) assess market potential; (2) determine logistics for successful implementation; (3) describe the benefits and drawbacks for farmers and retailers; and (4) measure consumers’ attitudes and purchase behavior. A national market survey indicated consumers would be likely to purchase F3B if it was perceived to offer good value on fresh local produce, without need for a subscription. The model put a few additional labor burdens on farmers and retailers, but required time for relationship-building and more record-keeping time for farmers. Those who purchased a F3B were generally satisfied with the quality, quantity and variety of produce each week and a high proportion considered F3B to be a good value for the money. As a new business innovation, F3B showed only modest profit, but retailers and farmers felt it was worthwhile to expand their customer base, promote their brand and develop their partnership. F3B began a means to address flattened growth in direct to consumer produce sales, food deserts and dwindling retail options for fresh foods in rural areas. We discuss F3B as a potential solution to food system weaknesses exposed by the COIVD-19 pandemic because it offers touch-free, high-quality local produce ready for curbside pickup at a convenient location. In this paper, methods are presented for designing a quadrotor attitude control system with disturbance rejection ability, wherein only one parameter needs to be tuned for each axis. The core difference between quadrotor platforms are extracted as critical gain parameters (CGPs). Reinforcement learning (RL) technology is introduced in order to automatically optimize the controlling law for quadrotors with different CGPs, and the CGPs are used to extend the RL state list. A deterministic policy gradient (DPG) algorithm that is based on an actor-critic structure in a model-free style is used as the learning algorithm. Mirror sampling and reward shaping methods are designed in order to eliminate the steady-state errors of the RL controller and accelerate the training process. Active disturbance rejection control (ADRC) is applied to reject unknown external disturbances. A set of extended state observers (ESOs) is designed to estimate the total disturbance to the roll and pitch axes. The covariance matrix adaptation evolution strategy (CMA-ES) algorithm is used to automatically tune the ESO parameters and improve the final performance. The complete controller is tested on an F550 quadrotor in both simulation and real flight environments. The quadrotor can hover and move around stably and accurately in the air, even with a severe disturbance. Full article
