Round 1

Reviewer 1 Report

This article showed the safety of Brazilian green propolis in prostate cancer patients after radical prostatectomy. The result of oncological outcome was disappointing one, however, the study is well designed and is well presented. The present study could support the clinical benefit and feasibility.

Major point: none

Please add the new data and make discusssion why the expression level of AKR1C3 was not correlate the efficacy in the view point of PSA slope rate. Is there any different regulatory mechanism in growth of prostate cancer in propolis?

Reviewer 2 Report

In this manuscript the authors sought out to clarify the efficacy and safety of Brazilian green propolis in biochemically re- 2

current PCa after RP prostatectomy within a a single-arm

phase II study.

Overall interesting results.

Please consider the following comments for major revision.

Any information about variant histology at time of RP pathological examination? Humphrey eyt al. (doi: 10.1016/j.eururo.2016.02.028) described such variants as associated with worse pathological and oncological outcomes. Please consider to add this as a limitation in case of non availability of such information.

Please consider to enrich the Discussion section with the novel emerging tools able to personalize the radical treatment in order to target metastatic lymph nodes with a potential benefit in term of oncological outcomes and BCR-free survival: doi: 10.1111/bju.15881; doi: 10.1016/j.urolonc.2022.08.005; doi: 10.1111/iju.14513; doi: 10.1097/RLU.0000000000004301.
