The authors present a well-planned and successful endoscopic therapy of infected pseudocysts of the pancreas after pancreatitis with additional presence of a fistula into the duodenum. By means of a transgastric endoscopic drainage of the pseudocyst (using a "lumen-apposing metal stent/LAMS") the fistula to the duodenum was brought to healing 

The case report describes an innovative endoscopic procedure for the treatment of pancreatic pseudocysts. Specific reference is made to the treatment of situations complicated by fistulas to the duodenum. The discussion includes the findings of the current literature on the topic. The authors also describe the advantages of the stent used here (LAMS). In addition, the strength of the case report is the excellent illustrations (CT, endoscopy), so that the performed diagnostics and therapy can be followed very well. 

Minor revision: I recommend linguistic revision of the manuscript before publication.

The case report is interesting, dealing with predictable complications in acute pancreatic processes. It can be of help to internists and surgeons, if They have such pazients, for possible therapeutic choices. The integrated path with collegial evaluation is excellent. Very good the choise  of references.
