As most of models,related to health development policies, it is attractive but does not give confidence in the application of the findings.

The title should be changed and consistent with the key conclusion.

The introduction points out the key macroeconomic issues of health in poor countries. It could give few references on the sustainability challenge linked with  domestic public investment in health in countries withe small open economy.

The model proposed is innovative and interesting.I wonder wheter the determinants of health could be better considered to adjust the expectations of the sustainability of long term health investment.

The inclusion in the model of foreign aid is excellent but quite complex as foreign aid in health is usually tied with designated allocation of funds, sometimes constrained by conditionalities and with various effects on long term capacity building development. Foreign aid sustainability is ten unpredictable especially bilateral aid for health.

The discussions are quite focused on the results. They could highlight the limitations and suggest adaptations to be considered given the great diversitf geographic, demographic, economic and political situations. To which extend, the model could be tested in specific countries?

In conclusion, I recommand the author to be more critical on the use of a model for policy making in health.

This paper conducted a solution to the health problem in developing countries. They found that when the government makes public investment in health-related quality of life (HRQOL) temporarily, HRQOL deteriorates or does not improve at least. On the contrary, when public investment is enforced permanently, it improves in the long run.  I do have some comments as listed below in the order noted.

 

Comment 1:

The authors mentioned that HRQOL is a concept that shows the physical and psychological domains of health status. HRQOL value is a broad multidimensional concept that usually includes subjective evaluations of both positive and negative aspects of life. Please specifically identify and define HRQOL in the present study.

 

Comment 2:

I suggest the authors provide an overall conceptual framework in the section of Baseline Model.
