Understanding groundwater and surface water interaction is critical for managing water resources, particularly in water-stressed and rapidly urbanizing areas, such as many parts of Africa. A survey was conducted of borehole, spring, seep and river water radon, δ2H, δ18O and field parameters in the Jukskei River catchment, Johannesburg. Average values of electrical conductivity (EC) were 274 and 411 μS·cm−1 for groundwater and surface water, and similarly for radon, 37,000 and 1100 Bq·m−3, with a groundwater high of 196,000 Bq·m−3 associated with a structural lineament. High radon was a good indicator of baseflow, highest at the end of the rainy season (March) and lowest at the end of the dry season (September), with the FINIFLUX model computing groundwater inflow as 2.5–4.7 L·m−1s−1. High EC was a poorer indicator of baseflow, also considering the possibility of wastewater with high EC, typical in urban areas. Groundwater δ2H and δ18O values are spread widely, suggesting recharge from both normal and unusual rainfall periods. A slight shift from the local meteoric water line indicates light evaporation during recharge. Surface water δ2H and δ18O is clustered, pointing to regular groundwater input along the stream, supporting the findings from radon. Given the importance of groundwater, further study using the same parameters or additional analytes is advisable in the urban area of Johannesburg or other cities. Full article

Effective water infrastructure planning and management is key to sustainable water supply globally. This research assesses water infrastructure planning and management in Gauteng, South Africa, amid growing challenges from rapid urbanisation, high water demand, climate change, and resource scarcity. These challenges threaten the achievement of Sustainable Development Goals 6 and 11; hence, an integrated approach is required for water sustainability. The study responds to a gap in the literature, which often treats planning and management separately, by adopting an integrated, multi-institutional approach across the water value chain. A mixed-methods triangulation strategy was employed for data collection whereby surveys provided quantitative data, while two sets of structured interviews were conducted: the first round to determine causal relationships among the critical success factors and the second round to validate the proposed framework. The findings reveal a misalignment between infrastructure planning and implementation, contributing to infrastructure backlogs and a short- to medium-term focus. Infrastructure management is further constrained by inadequate system redundancy, leading to ineffective maintenance. External factors such as delayed adoption of 4IR technologies, lack of climate resilient strategies, and fragmented institutional coordination exacerbate these issues. Using Decision-Making Trial and Evaluation Laboratory (DEMATEL) analysis, the study identified Strategic Alignment and a Value-Driven Approach as the most influential critical success factors in water asset management. The research concludes by proposing an integrated water infrastructure and planning framework that supports sustainable water supply. Full article

Pharmaceuticals are increasingly being detected in coastal ecosystems globally, but contamination and bioaccumulation levels are understudied in temporarily closed estuaries. In these systems, limited freshwater inputs and periodic closure may predispose them to pharmaceutical accumulation. We quantified in situ water column pharmaceutical levels at five sites in a temporarily closed model urban estuary (Zandvlei Estuary) in Cape Town, South Africa, that has been heavily anthropogenically modified. The results indicate an almost 100-fold greater concentration of pharmaceuticals in the estuary relative to False Bay, into which the estuary discharges, with acetaminophen (max: 2.531 µg/L) and sulfamethoxazole (max: 0.138 µg/L) being the primary pollutants. Acetaminophen was potentially bioaccumulative, while nevirapine, carbamazepine and sulfamethoxazole were bioaccumulated (BAF > 5000 L/kg) by sandprawns (Kraussillichirus kraussi), which are key coastal endobenthic ecosystem engineers in southern Africa. The assimilative capacity of temporarily closed estuarine environments may be adversely impacted by wastewater discharges that contain diverse pharmaceuticals, based upon the high bioaccumulation detected in key benthic engineers. Over the last decade, the scientific community has increasingly focused on forest-based practices for human well-being (FBPW), a term that includes all forest activities (e.g., forest bathing, forest therapy, social outdoor initiatives) important for improving people’s health and emotional status. This paper aims to develop a quantitative literature review on FBPW based on big data analysis (text mining on Scopus title and abstract) and PRISMA evaluation. The two techniques facilitate investigations across different geographic areas (major areas and geographical regions) and allow a focus on various topics. The results of text mining highlight the prominence of publications on FBPW for the improvement of human health in East Asia (e.g., Japan and South Korea). Furthermore, some specific themes developed by the literature for each geographical area emerge: urban green areas, cities, and parks in Africa; sustainable forest management and planning in the Americas; empirical studies on physiological and psychological effects of FBPW in Asia; and forest management and FBPW in Europe. PRISMA indicates a gap in studies focused on the reciprocal influences of forest variables and well-being responses. An investigation of the main physiological indicators applied in the scientific literature for the theme is also developed. The main strengths and weaknesses of the method are discussed, with suggestions for potential future lines of research. Full article

Africa’s rapid urbanization often exceeds the capacity of governments to provide essential services and infrastructure, exacerbating structural inequalities and exposing vulnerable populations to serious health risks. This paper examines the case of Douala, Cameroon, to demonstrate that health inequities in African cities are not simply the result of urban growth but are shaped by spatial inequities, historical legacies, and systemic exclusion. Disadvantaged neighborhoods are particularly impacted, becoming epicenters of health crises. Using a mixed-methods approach combining spatial analysis, household surveys and interviews, the study identifies three key findings: (1) Healthcare services in Douala are unevenly distributed and dominated by private providers, which limits access for low-income residents. (2) Inadequate infrastructure and environmental risks in informal settlements lead to a higher disease burden and an overflow of demand into better-equipped districts, which overwhelms public health centers across the city. (3) This structural mismatch fuels widespread reliance on informal and unregulated care practices. This study positions Douala as a microcosm of broader public health challenges in rapidly urbanizing African cities. It highlights the need for integrated urban planning and health system reforms that address spatial inequalities, strengthen public health infrastructure, and prioritize equity—key principles for achieving the third Sustainable Development Goal (ensuring good health and well-being for all residents) in sub-Saharan Africa. The Sustainable Development Goals seek to achieve universal, adequate, and equitable access to drinking water and sanitation by the year 2030. Yet, significant and persistent disparities in water and sanitation access exist, with rural and low-income households in Sub-Saharan Africa exhibiting some of the lowest levels. This paper uses household surveys from rural villages in Zambia’s Western Province to identify, highlight, and examine spatial barriers to improved water and sanitation. Most households included in the study area drink unimproved water, including surface water, and either use unimproved sanitation facilities or practice open defecation. Access to improved water sources and improved sanitation in the study area lags behind the rest of rural Zambia. Beyond the distance to urban areas that makes piped water and sanitation expensive, the location of these villages in the Barotse Floodplain necessitates seasonal migration, which creates barriers to universal access to improved water and sanitation. The modern world has brought extensive socioeconomic and ecological changes. Urbanization in developing nations has significantly increased municipal solid waste, necessitating in-depth understanding of waste composition particularly in developing nations for sustainable management practices. This study aimed to classify and characterize waste while evaluating potential waste management methods. Mixed methods were used to examine landfilled waste from Soshanguve and Hatherley sites in Tshwane Metropolitan, South Africa, using techniques such as Fourier transform infrared spectroscopy, X-ray fluorescence, proximate, and ultimate analysis. Seasonal variations in waste components were analysed over two seasons. The study identified that both sites are predominantly composed of organic waste, accounting for over 42 wt.%, with moisture content of ~50 wt.%, and minimal recyclables (<5 wt.%). Seasonal variations in MSW were significant for glass (<4% increase), organic waste (<5% increase), while plastic decreased by ~7% during spring. The biodegradable waste showed high carbon (>50%) and oxygen (>40%) levels, low ash content (<18%), and calorific values of 15–19 MJ/kg. Biodegradables mainly contained oxides of calcium, silicon, iron (III), and potassium with chemical composition indicating functional groups that emphasize composting and energy recovery benefits. The research provides insights into sustainable waste management, revealing waste composition at Tshwane landfills, aiding informed decision-making for resource usage and environmental conservation. With rising energy costs and the need for sustainable power solutions in urban South African settings, grid-tied renewable energy systems have become viable alternatives for reducing dependence on traditional grid supply. This study investigates the techno-economic feasibility of a grid-connected hybrid photovoltaic (PV) and battery storage system designed for a commercial facility located in Johannesburg, South Africa—an area characterized by a subtropical highland climate. We conducted the analysis using the HOMER Grid software and evaluated the performance of the proposed PV/battery system against the baseline grid-only configuration. Simulation results indicate that the optimal systems, comprising 337 kW of flat-plate PV and 901 kWh of lithium-ion battery storage, offers a significant reduction in electricity expenditure, lowering the annual utility cost from $39,229 to $897. The system demonstrates a simple payback period of less than two years and achieves a net present value (NPV) of approximately $449,491 over a 25-year project lifespan. In addition to delivering substantial cost savings, the proposed configuration also enhances energy resilience. Sensitivity analyses were conducted to assess the impact of variables such as inflation rate, discount rate, and load profile fluctuations on system performance and economic returns. The results affirm the suitability of hybrid grid-tied PV/battery systems for cost-effective, sustainable urban energy solutions in climates with high solar potential. Full article

Amid growing environmental concerns and an increasing push for sustainable development, countries in the Middle East and North Africa (MENA) region have taken proactive steps toward green growth, energy transition, and technological innovation. As a result, this study examines the effects of green growth, energy transition, technological innovation, financial inclusion, and urbanization on environmental sustainability in the Middle East and North Africa (MENA) region. Moreover, this study breaks new ground by exposing the hidden environmental costs of financial inclusion, urbanization, and technological innovation in the MENA region’s development trajectory, thereby providing compelling evidence for rethinking sustainability through an integrated approach that aligns economic ambition with ecological responsibility. Data for the studied variables were sourced from the World Bank database covering the period 1990 to 2021. The results show that green growth and energy transition significantly reduce CO2 emissions, supporting the idea that economic expansion aligned with environmental priorities can contribute to ecological improvement. However, the impact of technological innovation is statistically insignificant, indicating that innovation in the region has not yet translated into meaningful environmental gains, possibly due to the dominance of non-green or industrial-focused innovation. Financial inclusion is found to increase CO2 emissions, likely by facilitating greater access to credit and financial services that fuel energy-intensive consumption and production activities. Similarly, urbanization also contributes to rising emissions, reflecting the unsustainable nature of urban growth in many MENA region. Based on this study, we advocate for a coordinated regional approach to climate and energy policy, underpinned by shared governance and collective action. Full article

Background: Liver trauma is prevalent following blunt and penetrating injuries. This study examined liver trauma in a South African urban trauma centre, focusing on the unique prevalence of penetrating injuries and surgical interventions. Methods: A retrospective analysis was conducted of 512 priority-one patients with liver trauma from January 2017 to December 2023 at Charlotte Maxeke Johannesburg Academic Hospital. The data collected included demographics, injury mechanisms, liver injury grades, associated injuries, injury severity scores (ISS and NISS), surgical interventions, and mortality rates. Statistical analyses were performed using Stata (V.18) and R software (version 4.3.2). Results: The median age of the patients was 31 years, with a predominance of male patients (91%) and patients of African ethnicity (95%). Penetrating trauma accounted for 73% of the cases. Most liver injuries were minor (grades I–III). There was a 5% overall mortality rate, with a higher rate observed in patients requiring emergency surgery (10% vs. 1% for non-operative management, p < 0.001). Just over half of the patients required emergency laparotomy, and the majority of these patients sustained penetrating liver trauma. Complications occurred in 6.6% of the patients, predominantly biliary in nature. Conclusions: This study highlights the high incidence of penetrating liver trauma in South Africa, which reflects the context of interpersonal violence. The mortality rate aligns with international standards and demonstrates the need for effective management strategies. These findings emphasise the need for tailored approaches to liver trauma based on injury patterns and demographics, and further research is needed to explore the associated mortality and complications. Consanguinity is the marriage of two related persons. This type of marriage is one of the main pillars when it comes to recessive hereditary diseases, birth defects, infertility, miscarriages, abortion, and infant deaths. Intermarriage continues to be a common practice in various communities in North Africa, the Middle East, and West and South Asia, as well as among migrants from Europe and North America, even though in more and more countries it has become illegal. Even if security and stability are some of the motivations for consanguineous marriage, studies show that women often suffer physical and verbal abuse from their husbands. However, because of the blood bond, tolerance for these habits is much higher. In addition, it seems that the divorce rate is much lower because separation would affect the entire state of the family. The choice of partner is significantly influenced by variables such as limited access to education and financial resources. Illiterate people coming from poor rural areas are much more likely to choose consanguineous marriage to maintain wealth in the family. The lack of medical knowledge about the negative effects of consanguinity leads to an increased rate of abortions, infant deaths, and births of children with congenital birth defects. Today, because of the process of urbanization and increased levels of knowledge, the younger generation is becoming increasingly less receptive to this particular form of marriage. In addition, as education has become more accessible to women, they have become more independent and eager to fulfill their own goals and not the wishes of the family. In conclusion, contrary to the many apparent advantages of consanguineous marriage, partners should put genetic risks first, as medical problems bring with them increased costs in the medical system and also within the family, leading to even lower economic status and consequently perpetuation of this type of marriage. Urbanization is rapidly transforming cities, especially in the Global South, with Sub-Saharan Africa expected to see the fastest growth in the next 30 years. In South Africa’s Mopani District, this urban expansion has led to the growth of informal settlements, increasing disaster risks related to water, health, and fire. This study focuses on Giyani Local Municipality, examining disaster risks in its informal settlements and the factors influencing the implementation of spatial planning tools. Using a SWOT analysis combined with the Analytical Hierarchy Process (AHP), the study found that while the municipality has strong land use and disaster management policies, poor enforcement, lack of integration, and weak governance limit their effectiveness. Opportunities for improvement include securing grants from the National Government and Disaster Management Centre to support disaster risk reduction (DRR) initiatives. However, challenges such as land shortages and ecological degradation threaten sustainable planning. The findings provide important insights for policymakers, urban planners, and disaster management professionals. The SWOT-AHP approach helps in prioritizing resource allocation, identifying risk trends, and focusing on key areas for mitigation. Overall, the study supports efforts to enhance resilience and promote sustainable urban development in informal settlements through better spatial planning. Full article

Neonatal jaundice (NNJ) is a leading cause of death in the early neonatal period, disproportionately affecting newborns in sub-Saharan Africa. In a setting without access to rapid assessment via transcutaneous bilirubin meter, we sought to determine how closely the diagnosis and treatment of neonatal jaundice at an urban district hospital aligned with retrospective assessment and treatment recommendations using the BiliApp (based on the UK NICE Guideline CG98). This retrospective chart review study aimed to identify: (1) What percent of admissions within 8 days of birth to the Mother and Baby Unit (MBU) at our study site were admitted for a primary diagnosis of neonatal jaundice, and what characterized those admissions? (2) How did treatment provided compare to the recommendations of the United Kingdom NICE Guideline CG98 via the “BiliApp”? and (3) Among those with jaundice, what factors were associated with an increased likelihood of severity indicative of the need for blood exchange therapy? The charts of all neonates admitted to the MBU at Suntreso Government Hospital (SGH), in Kumasi, Ghana, in 2020 were reviewed by trained research assistants. Data were collected regarding demographics, reason for admission, diagnostic markers (e.g., serum bilirubin level), treatments performed in the hospital, and outcome. Data were analyzed using Stata 18.0. There were 1059 newborns admitted to the MBU in 2020 at less than 8 days of age. A total of 179 (16.9%) were admitted with a primary diagnosis of neonatal jaundice. According to the BiliApp, 29.4% (n = 50) of newborns admitted for jaundice had bilirubin levels that were normal or below the phototherapy threshold for their gestational age on admission; 25.3% (n = 43) were at or near the threshold for phototherapy; 21.2% (n = 36) were above the phototherapy threshold; and 24.1% (n = 41) were above the blood exchange therapy threshold. The BiliApp recommended no treatment for 21.2% (n = 36) of newborns, repeated assessment for 33.6% (n = 57), phototherapy for 21.2% (n = 36), and exchange therapy for 24.1% (n = 41). By comparison, 8.2% (n = 14) of neonates admitted for jaundice received no treatment, 77.8% (n = 133) received phototherapy only, and 14.0% (n = 24) received both phototherapy and exchange therapy. Without sufficient data on G6PD status and parent/newborn blood type to include in the analysis, the biggest risk factors for a BiliApp recommendation of exchange therapy included serum bilirubin level (OR 1.01, p < 0.001) and gestational age (OR 0.51, p < 0.001), even after controlling for breastfeeding and male sex of the newborn. Without access to rapid assessment tools, many providers in low-resource settings are put in a position to presumptively treat newborns suspected of having jaundice, rather than waiting for serum lab tests to return. Given the cost of transcutaneous bilirubin meters, additional options for rapid diagnostic testing are warranted. Full article

Flood Exposure Patterns Induced by Sea Level Rise in Coastal Urban Areas of Europe and North Africa

Coastal cities and low-lying areas are increasingly vulnerable, and accurate data is needed to identify where interventions are most required. We compared 53 cities affected by a 1 m increase in land levels and a 2 m rise in sea levels. The geographical scope of this study covered selected coastal cities in Europe and northern Africa. Data were sourced from the European Environment Agency (EEA) in the form of prepared datasets, which were further processed for analysis. Statistical methods were applied to compare the extent of urban flooding under two sea level rise scenarios—1 m and 2 m—by calculating the percentage of affected urban areas. To assess social vulnerability, the analysis included several variables: MAPF65 (Mean Area Potentially Flooded for people aged 65 and older, indicating elderly exposure), Age (the percentage of the population aged 65+ in each city), MAPF (Mean Area Potentially Flooded, representing the average share of urban area at risk of flooding), and Unemployment Ratio (the percentage of unemployed individuals living in the areas potentially affected by sea level rise). We utilized t-tests to analyze the means of two datasets, yielding a mean difference of 2.9536. Both parametric and bootstrap confidence intervals included zero, and the p-values from the t-tests (0.289 and 0.289) indicated no statistically significant difference between the means. The Bayes factor (0.178) provided substantial evidence supporting equal means, while Cohen’s D (0.099) indicated a very small effect size. Ceuta’s flooding value (502.8) was identified as a significant outlier (p < 0.05), indicating high flood risk. A Grubbs’ test confirmed Ceuta as a significant outlier. A Wilcoxon test highlighted significant deviations between the medians, with a p << 0.001, demonstrating systematic discrepancies tied to flood frequency and sea level anomalies. These findings illuminated critical disparities in flooding trends across specific locations, offering essential insights for urban planning and mitigation strategies in cities vulnerable to rising sea levels and extreme weather patterns. Information on coastal flooding provides awareness of how rising sea levels affect at-risk areas. Examining factors such as MAPF and population data enables the detection of the most threatened zones and supports targeted action. These perceptions are essential for strengthening climate resilience, improving emergency planning, and directing resources where they are needed most. Full article

This study quantitatively evaluates the effects of human activities (HAs) and climate change (CC) on the terrestrial ecosystem carbon cycle, providing a scientific basis for ecosystem management and the formulation of sustainable development policies in urban agglomerations located in arid and ecotone regions. Using the LanXi urban agglomeration in China as a case study, we simulated the spatiotemporal variation of vegetation net primary productivity (NPP) from 2000 to 2023 based on MODIS remote sensing data and the CASA model. Trend analysis and the Hurst index were employed to identify the dynamic trends and persistence of NPP. Furthermore, the Geographical Detector model with optimized parameters, along with nonlinear residual analysis, was employed to investigate the driving mechanisms and relative contributions of HAs and CC to NPP variation. The results indicate that NPP in the LanXi urban agglomeration exhibited a fluctuating upward trend, with an average annual increase of 4.26 gC/m2 per year. Spatially, this trend followed a pattern of “higher in the center, lower in the east and west,” with more than 95% of the region showing an increase in NPP. Precipitation, mean annual temperature, evapotranspiration, and land use types were identified as the primary driving factors of NPP change. The interaction among these factors demonstrated a stronger explanatory power through factor coupling. Compared with linear residual analysis, the nonlinear model showed clear advantages, indicating that vegetation NPP in the LanXi urban agglomeration was jointly influenced by HAs and CC. These findings can further act as a basis for resource and environmental research in similar ecotone regions globally, such as Central Asia, the Mediterranean Basin, the southwestern United States, and North Africa.
