Background and Objectives: Hamstring strain injuries (HSIs) are frequent and recurrent in athletes who perform high-speed running. The long head of the biceps femoris (BFlh) is often affected by HSIs. While the Nordic hamstring exercise (NHE) is used for prevention, evidence shows it mainly activates the semitendinosus (ST) instead of the biceps femoris (BF). It was argued that hamstrings may contract isometrically during sprinting’s late swing phase; exercises like the single-leg Roman Chair-Hold (RCH) might better mimic sprinting. Limited electromyographic (EMG) data compare NHE and RCH. This study examined EMG activation of BF and ST during both exercises in athletes. Materials and Methods: Thirty-six professional handball players (17 females, 19 males) were randomly assigned to NHE (n = 18; mean age 22.1 ± 3.9 years) or RCH (n = 18; mean age 22.6 ± 4.9 years). A wireless EMG system recorded dominant leg BFlh and ST activity, normalised to maximal voluntary isometric contraction (MVIC%). NHE participants completed one set of ten repetitions; RCH participants performed three sets of ten repetitions with progressive loads (bodyweight, +10 kg, +20 kg). Results: RCH led to a significantly higher mean BFlh activation in the third set with +20 kg weight compared to NHE (72.9% versus 46.5%; p < 0.001, g = 1.52). BFlh activation steadily increased across RCH sets, coinciding with additional load increments (p < 0.001). Conversely, NHE produced greater ST activation than the RCH at the first set, where RCH was performed with only bodyweight (p < 0.001). Conclusions: NHE primarily activates the ST, while RCH gradually increases BFlh activation, particularly under load. Future research should investigate which exercises are more effective at reducing HSIs to draw more robust conclusions based on the study’s findings. Full article

Background: Biomechanical factors behind non-contact anterior cruciate ligament (ACL) injury in soccer and handball are still not fully understood. Unfortunately, ACL injuries more frequently appear in game situations. Aim: To describe a possible ACL injury mechanism in male professional handball players using MRI images and our own biomechanical model. Hypothesis: The friction parameters of the surface have extreme importance in the non-contact ACL injury mechanism. If the surface is more slippery, the horizontal component of the ground reaction force (GRF) will be smaller, consequently the torque originating from the GRF acting on the knee will be greater during the landing phase of a vertical jump, resulting in greater abduction effect on the knee. Consequently, the risk of knee injury increases. Methods: We have collected MRI images and anthropometric data of 15 healthy male individuals (age 19–23) to create a biomechanical model to calculate the torques in the knee to obtain more knowledge about ACL injury mechanism. Results: The lower extremity lean angle during the landing phase of a jump and friction parameters substantially affect abduction torques in the knee and consequently the risk of ACL injury occurrence. Conclusions: The landing posture when the knee is fully extended during landing is highly unfortunate for the ACL, compared to when the knee is partially flexed. If the knee is fully extended, greater hip abduction will increase the risk of an ACL injury, and if the surface is more slippery, e.g., the surface is wet, the possibility of ACL injury is even greater. In addition, we also applied a molecular working hypothesis through acquired Piezo2 channelopathy theory, as the proposed preceding neuromuscular disruptor prior to non-contact ACL injury. Handball is a sport that demands explosive movements and unique skills, and its popularity has been rising in recent years. This study evaluated elite handball players’ nutritional supplement (NS) use profiles and the differences in sex, competition level, and competition type based on the Australian Institute of Sport (AIS) criteria. The data collection form contains questions about participants’ sociodemographic characteristics, training details, use of supplements, and related factors. Supplements were classified into A, B, C, and D classes according to the scientific evidence level of the AIS. The study involved 92 elite handball athletes, comprising 48 professionals and 44 amateurs, and included 37 females and 55 males. The most frequently used supplements among participants were magnesium (37.0%) (Group C—AIS), vitamin C (20.7%) (Group B—AIS), whey protein (19.6%), sports bars (19.6%), and vitamin D (19.6%) (Group A—AIS). Regarding sex differences, a significant difference was observed only in Group C supplements, with male players using them more frequently than female players (p < 0.05). Professional athletes exhibited a significantly higher prevalence of supplement use, covering total, Group A, sports foods, performance supplements, and Groups B and C, relative to amateur players (p < 0.05). The results reveal that handball players have limited awareness of NS, emphasizing the need for training and consulting services. Full article

Backward running is common in handball defense and relies heavily on proprioceptive control when visual information is limited. Twenty-eight male handball players were allocated to three groups: experimental novice group with blindfold training (n = 7), control novice group with the same training without blindfold (n = 7), and target professional group (n = 14). Both novice groups completed a 6-week balance program (3 × 20 min/week). Lower-limb kinematics during backward running were captured with a 6-camera motion analysis system, and inter-joint coordination was quantified by Mean Absolute Relative Phase (MARP) and Deviation Phase (DP) for ankle–knee and knee–pelvic couplings. At baseline, professionals showed greater ankle–knee MARP than novices (ANOVA F(2,25) = 9.42, p < 0.001). Representative means (mean ± SD): ankle–knee MARP novices 1.62–1.79 vs. professionals 3.83. After training, ankle–knee MARP increased in both novice groups (experimental: t(6) = 4.72, p < 0.001; control: t(6) = 5.02, p < 0.001), approaching professional values (post-training novices ≈ 3.22–3.26). Post-training between-group differences were non-significant for ankle–knee MARP (ANOVA F(2,25) = 1.24, p = 0.30), while ankle–knee DP showed a group effect (F(2,25) = 5.12, p = 0.01; experimental vs. professional t(19) = 3.12, p = 0.01). A short-term balance program improved ankle–knee coordination during backward running in novice male players; additional blindfolding did not yield extra benefit over 6 weeks. These findings can inform short-term training and rehabilitation planning for handball, while long-term effects require future study. Full article

Background: Super High-Intensity Continuous Training (SHCT) is a type of aerobic training program that combines high intensity with continuous loads, such as running for 20 min at 75%, 80%, or even 95% of the velocity at maximal oxygen uptake. Recent studies show significant positive effects, but the consequences of prolonged use remain unknown. Purpose: This study aims to investigate and evaluate the effects of prolonged application of the SHCT model in elite team handball players. Method: For this purpose, a field-based quasi-experiment was organized using the SHCT training model on 14 professional female team handball players competing in the first national league who participated in 16 weeks of SHCT training during the competition season. Results: After the application of SHCT training, the increases in the parameters of the aerobic profile (distance run in Cooper’s 12 min run test, maximum rate of oxygen consumption, value of the maximum relative oxygen consumption, running speed for which maximum rate of oxygen consumption occurs) reached from 25.4% to 35.2%. The effect size of these changes was η2p > 0.90 and was significant at the p < 0.001 level. Conclusions: The investigated aerobic model is effective. Therefore, its use is recommended for designing aerobic training for elite teams and the general sports population. Full article

Professional training in elite team handball academies is key to developing top players, yet its direct impact on physical performance remains unclear. This study aimed to (1) provide professional training to elite adolescent players and (2) assess performance improvements using a team handball-specific test. Thirty elite male players (six goalkeepers, 24 field players) participated in an 11-week program, with nine under-19 (17.2 ± 1.3 years) and nine under-17 (15.6 ± 0.9 years) field players completing at least 80% of sessions. All underwent pre- and post-testing using the game-based performance test. A two-way ANOVA analyzed differences between tests and age groups as well as playing positions. Significant improvements (p < 0.05) were found in defense and offense time and body weight for both groups. Under-17 players also showed a significant increase in peak oxygen uptake (+9%), ball velocity (+7%), and jump height (+20%). Agility in defense and offense improved in under-19 (+3%) and under-17 (+6%) players, aligning with training goals. Positional differences were observed between backcourt players and wings (p < 0.01) in the ball velocity, while all players showed improvements in both defense and offense performance. We suggest that professional and targeted specific training at this age has a significant impact on the long-term development of adolescent team handball players and is the basis for a professional handball career. Background: Musculoskeletal injuries are frequent in handball players due to the high-impact nature of the sport. Accurate diagnosis and tailored treatment are essential for recovery. Magnetic resonance imaging (MRI) has become the gold standard for evaluating complex sports-related injuries. This case report aims to assess the role of MRI in diagnosing trauma in professional handball players and the effectiveness of individualized treatment approaches. Methods: Four male members of the “Politehnica” Timișoara first team who sustained match- or training-related injuries between January 2023 and December 2024 underwent an 1.5 T MRI. Individualized conservative protocols included rest, NSAIDs, physiotherapy, and graded kinesitherapy. Results: The first case involved a right back with a knee hematoma and a vastus lateralis tear. Conservative treatment led to recovery. The second case was a left back with peripheral neuropathy caused by hamstring avulsion at the ischial tuberosity. Conservative therapy alleviated symptoms. The third case involved a left winger with adductor muscle tears, which recovered with conservative management. The fourth case, a goalkeeper, had a type III navicular fracture misdiagnosed on radiography, correctly identified via MRI and treated conservatively. Conclusions: MRI is invaluable in diagnosing handball-related injuries, enabling accurate assessments and effective individualized treatment, resulting in early recovery. Full article

Adequate hydration is crucial to an athlete’s health and performance. There is some evidence that the different compositions of various mineral water types may improve exercise performance and affect different biomarkers. The aim was to investigate the consumption of mineral water from the “Topla voda” spring in terms of its safety profile and its effect on body composition and functional and biochemical parameters in professional athletes. During the preparation phase of their mesocycle, 14 male professional handball players underwent a complete sports medical screening exam with a cardiopulmonary exercise test (CPET), blood gas analysis, and oxidative stress marker dynamics taken at four points during the CPET. The athletes were then randomized into two equal groups; the first group consumed mineral water, and the second group consumed tap water. After four weeks, the biochemical analysis and CPET were repeated. Routine analyses showed that the “mineral water” group had increased their mean corpuscular hemoglobin (ANCOVA = 0.050) and mean corpuscular hemoglobin concentration (ANCOVA = 0.001) and had a greater metabolic equivalent of task (MET) value at the end of the test (ANCOVA = 0.049), with no significant changes in the other measured parameters. Consuming “mineral water” appears to be safe, with some potential positive effects compared with tap water, mostly in terms of hemoglobin parameters and exercise tolerance. Full article

The aim of this study was to evaluate center of pressure (CoP) changes in unilateral sports and examine how these changes affect the athlete’s feet, ankles, knees or posture. The study sample consisted of 40 professional male players (age: 19.4 ± 2.08 years; height: 165.78 ± 4.92 cm; weight: 59.04 ± 4.02 kg; BMI: 21.57 ± 2.22; foot size: 40.9 ± 1.6), divided by type of sport into group H—handball (n: 20) and group F—football (n: 20). To evaluate the monitored parameters, we used the instrumental diagnostic techniques: 3D laser footscan, baropodometric platform FreeStep and 2D Videography. We found no significant differences between the groups in the loading of the right and left foot (F: 8.3 ± 4.22; H: 7.7 ± 6.1) (p = 0.410). We found a significant difference in the load on the front and back of the left (p = 0.0079) and right foot (p = 0.0210) depending on the type of sport performed. Maximum and mean values of CoP (g/cm2) showed statistically significant differences depending on the sport performed (p < 0.0001). The shift in CoP (mm) from the norm depending on the sport performed was confirmed in the latero-lateral direction (p = 0.003), but not in the antero-posterior direction (p = 0.320). We found a difference in the angulation of the knees and heels depending on the sport played. Handball players showed higher knee varosity/valgosity (p = 0.015) and heel values than football players (p = 0.002). The handball players also confirmed a worse postural load and initial forward posture. The one-sided sports, handball and football, showed negative effects on the athlete’s movement system. These changes were more pronounced in handball players. Proper training programs should be applied in athletes’ daily routine to improve the negative effects of unilateral sports. Full article

Background: The Mediterranean Diet (MD) has gained attention for its potential benefits in enhancing athletic performance and overall health. This systematic review aims to evaluate the effects of the MD on athletic performance, strength, body composition, and metabolic markers in both athletes and non-professional athletes. Methods: The review included seven studies with a total of 116 participants, ranging from professional handball players to non-professional strength athletes. The studies assessed various aspects of athletic performance, including strength, power, endurance, and body composition. Results: The main key findings from the review showed that MD may improve muscle endurance and power, as well as anaerobic performance in CrossFit athletes, and MD was associated with enhanced strength performance, including increased vertical jump height, hand grip strength, and shuttle run performance. The results on the impact on body composition were mixed, with some studies showing improvements in fat-free mass and skeletal muscle mass, while others found no significant changes. The MD also demonstrated positive effects on several markers, such as increased plasma total antioxidant activity and decreased markers of oxidative stress and inflammation. Conclusions: In conclusion, while the MD seems to represent a viable dietary strategy for enhancing athletic performance and overall health, more rigorous studies are necessary to clarify its impact across diverse athletic populations. Handball is a team sport characterised by physical interaction with other opponents. This interaction produces a high load on the players that can manifest itself in various ways, from discomfort to prolonged injuries due to tears caused by excessive load. In order to establish correct protocols for application in women’s teams, context- and gender-specific reference data must be available. For this reason, the present research aims to find out how women’s teams in European competitions prepare for decisive matches during the match week, analysing the load in a segmented way and the level of specificity that should be achieved in training. Ex post facto research was used in which a total of 17 players belonging to a women’s first division handball team in Spain participated. The variables player load and impacts extracted from the use of Wimu ProTM inertial devices were analysed. The results showed a high neuromuscular load in players at this competitive level, especially in the variable impacts, reaching values per session of up to 1000 impacts. The individuality analyses show that the load varies significantly depending on the subject, which is why it is considered essential to establish protocols for strength work and load control in the most specific way possible. Anterior cruciate ligament (ACR) rupture is a frequent injury in professional sports players. We conducted a retrospective cohort study, including 41 professional female handball players, undergoing ACR reconstruction surgery, using a Ligament Advanced Reinforcement System (LARS) graft (n = 12) or a Soft Tissue (ST) graft (n = 29). After return-to-play, the patients were asked to take a survey, reporting subjective and objective performance indexes before the injury and after return-to-play. Time from surgery to first practice and to return-to-play were significantly shorter in the LARS group (3.92 ± 1.14 vs. 6.93 ± 2.19 months, p < 0.001 and 4.71 ± 1.2 vs. 8.81 ± 2.9, respectively). While there was no difference between postoperative mean time on court, number of goals/match, number of matches played at 6 months return-to-play and 50 m, 100 m and gate-to-gate sprint times, there was a significantly greater increase in preoperative times in the ST group than in the LARS group (1.45 ± 1.05 s vs. 0.21 ± 0.58 s slower than preoperatively, p < 0.001 for 50 m; 1.09 ± 0.95 s vs. 0.08 ± 1 s, p = 0.01 for 100 m; 1.66 ± 1 s vs. 0.21 ± 0.66 s for gate-to-gate). In conclusion, LARS grafts provide a faster recovery time and better functional outcome, significantly impacting the performance of professional handball players. Full article

The aim of the study was to determine the effect of step load in hypoxia on the effectiveness of preoperative rehabilitation (PR) and hormone levels based on a case study. Introduction: We assessed the impact of variables such as rate of movement and time under tension (TUT) in normobaric hypoxia on the levels of growth hormone (GH), insulin-like growth factor 1 (IGF-1), and erythropoietin (EPO). Additionally, the impact of step load on the hypertrophy and strength of knee extensors and flexors was assessed. Methods: The work uses a case study, the research subject of which was a 23-year-old female professional handball player. The tests included an isokinetic assessment of the peak torque of knee extensors and flexors as well as body composition analysis. Results: The results showed a more than (10.81-fold) increase in GH after the microcycle with time under tension (TUT). The deficit between the lower limbs was also reduced. Conclusions: Using a hypoxic environment based on an appropriate altitude, combined with changes such as a short rest break between sets and a controlled tempo of movement with an eccentric phase, TUT may offer an alternative to the PR process, especially among athletes who care about fast RTS. Ligament Injuries in Professional Male Handball Players: A 6-Year Longitudinal Study

Handball is a sport that involves high-intensity actions throughout the game, such as sprints, jumps, landings, and high-speed, repeated throws. This, along with competitive and tactical factors, congested schedules, and the need to maintain a high level of performance throughout the season, contributes to a high injury rate. This study aimed to analyse ligament injuries in a professional handball team over six consecutive seasons. A total of 68 elite male Spanish handball players participated, with 54 time-loss injuries (i.e., injuries involving at least one day of absence) observed during this study period. Ligament injury information was recorded following the International Olympic Committee consensus statement. The overall incidence was 0.89 ligament injuries per 1000 h of exposure. Additionally, a higher incidence and burden of ligament injuries was observed during match-play compared to training. Most ligament injuries were classified as minor or moderate (i.e., 79.63% of the total), and 46.29% were reinjuries. A significantly higher incidence of ligament injuries was suffered in the lower limbs compared to the upper limbs (0.81 vs. 0.08 ligament injuries per 1000 h; p < 0.001). Specifically, the highest incidence was observed in the anterior talofibular ligament of the ankle (0.57 injuries per 1000 h of exposure), while the greatest burden was related to the anterior cruciate ligament (24.08 absence days per 1000 h of exposure). This study provides an overview of ligament injuries among professional handball players, highlighting the need to implement strategies with positive effects during competition (e.g., specific activation strategies or training programmes based on strength and balance) and to reduce injury recurrences. The Paleo diet (PD) involves a restriction of carbohydrates and increased fat content (35% energy from carbohydrates, 35% energy from fats and 30% energy from protein). The aim of this study was to examine the effect of the PD on body composition, concentration of carbohydrates and lipids, as well as insulin, irisin, adiponectin and leptin in the blood. A total of 25 handball players were assigned to two groups: 14 in the experimental group (PD) and 11 in the control group (CD), using a PD and a rational diet, respectively. Analysis of body mass and body composition (body mass index, fat mass, lean body mass, fat-free mass, muscle mass, bone mineral content and bone mineral density), as well as blood concentration of metabolism markers (glucose, insulin, total cholesterol, HDL-cholesterol, non-HDL-cholesterol, LDL-cholesterol, triglycerides, free fatty acids, β-hydroxybutyrate, irisin, adiponectin and leptin), were determined at the beginning and after 4 and 8 weeks of nutritional intervention. Body mass was lower (p < 0.01), and adiponectin blood concentration was higher (p = 0.03) in the PD group at the end of the intervention. There were no changes (p ≥ 0.05) in body composition and blood levels of other biochemical markers in either group. Full article
