Background/Objectives: In recent years, Narrative Medicine (NM) has gained prominence in the context of neurodegenerative diseases, such as dementia, offering tools to understand the subjective experience of illness and to improve the care relationship. Methods: This scoping review, conducted following the PRISMA guidelines, analyzed the scientific literature from PubMed, PsycInfo, Web of Science, and Medline, encompassing 10 contributions focused on NM and patients with dementia or Alzheimer’s disease. Results: The analysis identified three main themes: 1) narrative, memory, and personal identity, highlighting the role of narrative in preserving a sense of self; 2) personalization of care, oriented towards person-centeredness; 3) the use of narrative in a formative and reflective function as a tool to promote empathy, clinical awareness, and observation skills in the training of health professionals. Conclusions: NM confirms itself as a relational and reflexive paradigm, capable of humanizing care and promoting therapeutic pathways that are more inclusive and sensitive to the patient’s subjectivity. Background: Multimodal artificial intelligence (MMAI) is transforming urological oncology by enabling the seamless integration of diverse data sources, including imaging, clinical records and robotic telemetry to facilitate patient-specific decision-making. Methods: This narrative review summarizes the current developments, applications, opportunities and risks of multimodal AI systems throughout the entire perioperative process in uro-oncologic surgery. Results: MMAI demonstrates quantifiable benefits across the entire perioperative pathway. Preoperatively, it improves diagnostics and surgical planning via multimodal data fusion. Intraoperatively, AI-assisted systems provide real-time context-based decision support, risk prediction and skill assessment within the operating theater. Postoperatively, MMAI facilitates automated documentation, early complication detection and personalized follow-up. Generative AI further revolutionizes surgical training through adaptive feedback and simulations. However, critical limitations must be addressed, including data bias, the barrier of closed robotic platforms, insufficient model validation, data security issues, hallucinations and ethical concerns regarding liability and transparency. Conclusions: MMAI significantly enhances the precision, efficiency and patient-centeredness of uro-oncological care. To ensure safe and widespread implementation, resolving the technical and regulatory barriers to real-time integration into robotic platforms is paramount. This must be coupled with standardized quality controls, transparent decision-making processes and responsible integration that fully preserves physician autonomy. Full article

This study examines Richard of Saint Victor’s conception of wisdom and love, understood as the ultimate ends of human life and deeply connected with the notion of care. For Richard, authentic care requires discerning the true object of concern: the human being as a rational creature created for happiness through knowledge and love of God. His anthropology highlights the dignity of man, composed of body, reason, and affection, and called to participate in divine happiness. Richard develops a spiritual pedagogy in which the ordering and moderation of affections—fear, sorrow, hope, love, joy, hatred, and modesty—are indispensable for the path toward contemplation. Through an allegorical reading of Jacob, his wives, and their children, Richard presents a symbolic itinerary where the progression of affectivity and reason leads ultimately to contemplation, embodied in Benjamin. This contemplative fulfillment transcends both fear and greed, liberating the human being from self-centeredness and opening him to love and divine wisdom. The work demonstrates Richard’s synthesis of Platonic, Augustinian, and Victorine traditions, proposing a transformative vision of the human person: happiness is inseparable from love, and wisdom is achieved not through rational argument but through the lived experience of love that surpasses reason. Digital adherence technologies (DATs) could improve the person-centeredness of tuberculosis (TB) treatment. DATs are found to be acceptable, though evidence of their effectiveness is varied. Our objective was to understand the fidelity of DAT interventions within five cluster-randomized trials. Two DATs (smart pillbox, medication labels) were assessed, with real-time adherence data available to healthcare providers (HCPs) on a digital platform in Ethiopia, the Philippines, South Africa, Tanzania, and Ukraine. A framework assessed four components of implementation: inputs (training, support, mobile access), processes (SMS, home visits, platform usage), outputs (DAT engagement, manual dosing), and outcomes (people with TB (PwTB)–HCP relationship). Fidelity was evaluated by quantitative indicators, and content analysis of qualitative sub-studies supplemented some indicators. Engagement with DATs was high among PwTB. Pillbox users showed high levels of sustained engagement (box opening), with digitally recorded doses ranging from 82% to 91%. Differences were observed in login frequency by HCPs to the adherence platform. In Ethiopia, Tanzania, and Ukraine, there was at least one login to the platform on 71% of weekdays per facility compared with the Philippines and South Africa at 42% and 52%, respectively. Intervention fidelity varied among countries, suggesting a need for future work on optimizing implementation. Full article

This study assessed suicidal risk in patients suffering from non-psychotic depressive disorders within various clinical and nosological forms (F31–F34 mood disorders and F60.31—emotionally unstable personality disorder). Clinical and psychological features were presented, as well as predictors of suicidal risk in patients of these groups. We performed a comparative analysis of the anxiety and depression level, the level of mental pain, fear of death and the severity of anti-suicidal motives in patients with affective disorders and borderline personality disorder (BPD). Based on the results, 100% of patients in these clinical nosological groups were found to have a high level of suicidal risk. Patients with affective disorders have weak anti-suicidal motives and are not fully aware of the consequences of their own death. Patients with BPD have a higher suicidal risk than patients with affective disorders; they are characterized by less pronounced social orientation, demonstrativeness, self-centeredness, less pronounced levels of anxiety and fear of death. This study assessed attitudes toward patient-centred care, empathy, assertiveness, and subjective perception of communication skills and technical knowledge among Portuguese undergraduate students in healthcare. These students may develop rehabilitation activities with patients in their person-oriented or technique-oriented professions. Portuguese nursing and allied health students from two public higher education schools completed questionnaires in the first and third academic years: Patient-Practitioner Orientation Scale, Jefferson Scale of Physician Empathy, Scale for Interpersonal Behaviour, and a subjective perception of technical knowledge and communication skills. A total of 183 students completed the surveys. In the first year, students showed moderate to high scores on patient-centredness attitudes, empathy, and assertiveness and perceived themselves as having good communication skills. Students from person-oriented programmes significantly improved their Total and shared patient-centred attitudes in the third year compared with students attending technique-oriented professions. Significant differences in empathy were found between groups in the third year. Distress associated with assertive behaviours increased significantly across time in students from technique-oriented programmes compared with their peers in person-oriented programmes. The results suggest that the health profession’s orientation and the programmes’ specific curriculum might have a role in how some dimensions evolved in the two groups of students. The increasing assertiveness-related discomfort highlighted the importance of assessing and monitoring students’ emotional wellbeing during their initial interactions with patients. Full article

Japanese society’s traditional gender norms are reflected by sentence-final particles (SFPs) in daily conversation. However, recently, Japanese young people have started to use gendered SFPs in “unclassical” ways. This study mainly examines the usage of the so-called “female” SFPs wa and no by male speakers. In total, 68 cases of wa (43 by male speakers and 25 by female speakers) and 84 cases of no (47 by male speakers and 37 by female speakers) usage were collected from casual conversations of Japanese college students in TalkBank, a public linguistic database. This study demonstrates that the “female” SFPs wa and no are used more frequently by male speakers than by female speakers. Different from the female speakers’ usage to soften the utterances and enhance conversational rapport, wa and no used by male speakers perform other functions. In particular, wa directly indexes self-centeredness, serving the speaker to express emotion, share personal ideas, or perform speech acts such as teasing or amae in a self-focused way, while no directly indexes truthfulness, which allows the speaker to share a story in a vivid tone, reconfirm the speaker’s prior statement, or provide the speaker’s explanation/reasoning in an assertive tone. This study suggests that the new, unconventional gender-based usages of SFPs reveal the social changes in gender dynamics in modern Japanese society, which should not be overlooked in language education. A model of recovery and recovery-oriented practice has been developed based on three previously published meta-syntheses of experiences and processes of mental health and substance use recovery. The model integrates the findings of these three meta-syntheses into three components: experiences of recovery, processes of recovery-oriented practice, and social and material capital. The experiences of recovery involve being, doing, and accessing and are viewed as embedded in the processes of recovery. The processes of recovery-oriented practice aim to mobilize and apply various forms of capital to support the recovery journey. Social and material capital, in turn, constitute the context in which recovery occurs and requires mobilization for the individual and the service system. The model is grounded in the principles of well-being, person-centeredness, embedding, self-determination, and the interdependency of human living. The model is both descriptive and explanatory, as it depicts the experiential and processual aspects of recovery and recovery-oriented practice and their interrelationships. The model as a framework needs to be elaborated further through application in practice and research, especially for understanding how experiences, processes and practices interact over time, and how they are affected by access to material and social capital. Truly personalized precision medicine combines pharmacogenomics (PGx), a person’s lived medication experiences and ethics; person-centeredness lies at the confluence of these considerations. A person-centered perspective can help inform PGx-related treatment guidelines, shared decision-making for PGx-related therapeutics and PGx-related healthcare policy. This article examines the interplay between these components of person-centered PGx-related care. Ethics concepts addressed include privacy, confidentiality, autonomy, informed consent, fiduciary responsibility, respect, the burden of pharmacogenomics knowledge for both the patient and healthcare provider and the pharmacist’s ethical role in PGx-testing. Incorporating the patient’s lived medication experience and ethics principles into PGx-based discussions of treatment can optimize the ethical, person-centered application of PGx testing to patient care. We are preoccupied with early childhood education policies at the international level in general, but in particular with early childhood education in Romania. Therefore, the research direction of our study focuses on how early education teachers can integrate the principles of slow education into the development of child-centered competence. The formation and development of the child’s early ontogeny personality occur at the natural pace of development and learning rather than in an accelerated manner. While preparing for the research, we reviewed relevant articles circumscribed to the same topic: slow, child-centered education, deep learning, constructivism, and the enthusiasm of children and teachers. Most of them conclude that the use of child-centered competence in early childhood significantly contributes to slow education principles and characteristics. At the same time, the research aims to provide quantitatively and qualitatively validated arguments for verifying the incidence of child-centered competence from the perspective of sustainable education, in connection with the constructivist orientation of the teachers. Our research included a group of 376 teachers selected from early childhood education institutions in the North-West of Romania, all of them with initial preparation in the field. Using the focus group method, we identified differences in the perception of the two paradigms (child-centeredness and slow pedagogy), the link between these two, and the connection with the constructivist orientation. At the same time, we intended to identify the degree of continuous professional development training according to these paradigms that are not studied and practiced together in early education in Romania, because only the child-centeredness principle is mentioned in the initial teacher training. Therefore, in-service training activities were conducted and they were focused on facilitating and raising awareness of the value of using child-centered competence and the principles of slow education among training participants. At the end of the program, through the quantitative method—the survey—we concluded that the activities included in the training program led to a significant increase in the teachers’ capacity to appreciate the natural pace of development and learning of children to such an extent that the participants themselves adopted the promotion of sustainable slow education in a dynamic society. The methods used helped us deduce that all teachers with a constructivist orientation improved their child-centered competence, and teachers who gave children the freedom to choose activities from the proposed educational offers, according to their needs, interests, and potential, identified the development of the ability to consider children’s natural pace of development and learning as a result of the training. In conclusion, our research complements the literature, proposing a new and useful perspective to approach early childhood education practices by respecting the child’s natural pace of development and learning, which provides a suitable context for promoting international sustainable educational policies. We encourage teachers around the world to reflect on an important issue in the holistic development of the child—if faster means better—in the race to performance. Full article

Background: Transformation toward digital services offers unique opportunities to meet the challenges of responding to changing public healthcare needs and health workforce shortages. There is a knowledge gap regarding digital health and person or patient-centered care. Aim: The aim of the current scoping review was to obtain an overview of existing research on person or patient centeredness in digital primary healthcare services. Design: A scoping review following the five stages by Arksey and O’Malley. Methods: Literature searches were conducted in the databases PubMed, Scopus (Elsevier), APA PsychInfo (Ovid), Embase (Ovid), Cinahl (Ovid) and Cochrane Library in June 2022. The Preferred Reporting Items for Systematic reviews and Meta-Analyzes extension for Scoping Reviews (PRISMA-ScR) Checklist was followed. Results: The electronic database searches identified 782 references. A total of 116 references were assessed in full text against the inclusion and exclusion criteria. Finally, a total of 12 references were included. The included papers represent research from 2015 to 2021 and were conducted in eight different countries, within a variety of populations, settings and digital solutions. Four themes providing knowledge about current research on patient or person centeredness and digital primary health care were identified: ‘Accessibility’, ‘Self-management’, ‘Digitalization at odds with patient centeredness’ and ‘Situation awareness’. The review underlines the need for further research on these issues. Full article

The characteristics of health professionals and their understanding of person-centeredness may have important implications for the development of person-centered practice in specific care settings. In this study, we characterized the perceptions of the person-centered practice of a multidisciplinary team of health professionals working in the internal medicine inpatient unit of a Portuguese hospital. Data were collected using a brief sociodemographic and professional questionnaire and the person-centered practice inventory-staff (PCPI-S), and the effect of different sociodemographic and professional variables on each PCPI-S domain was determined using an analysis of variance (ANOVA). The results showed that a person-centered practice was positively perceived in the major constructs of prerequisites (M = 4.12; SD = 0.36), the practice environment (M = 3.50; SD = 0.48), and person-centered process (M = 4.08; SD = 0.62) domains. The highest scored construct was developed interpersonal skills (M = 4.35; SD = 0.47), and the lowest was supportive organization systems (M = 3.08; SD = 0.80). Gender was found to influence the perceptions of knowing self (F(2,75) = 3.67, p = 0.03, partial η2 = 0.089) and the physical environment (F(2,75) = 3.63, p = 0.03, partial η2 = 0.088), as was profession on shared decision-making systems (F(2,75) = 5.38, p < 0.01, partial η2 = 0.125) and commitment to the job (F(2,75) = 5.27, p < 0.01, partial η2 = 0.123), and the educational level on being professionally competent (F(1,75) = 4.99, p = 0.03, partial η2 = 0.062) and having commitment to the job (F(2,75) = 4.49, p = 0.04, partial η2 = 0.056). In addition, the PCPI-S proved to be a reliable instrument for characterizing healthcare professionals’ perceptions of the person-centeredness of care in this context. Identifying personal and professional variables that influence these perceptions could provide a starting point for defining strategies to move practice toward person-centeredness and for monitoring changes in healthcare practice. The 862-bed “Hospital of Hope” was established at the Cape Town International Convention Centre specifically to cope with the effects of the first wave of the COVID-19 pandemic in Cape Town. Methods: We aimed to systematically describe and assess the effects on staff and the local health system. A cross-sectional descriptive study design was employed using mixed methods including record reviews and interviews with key informants. Results: Quantitative results confirmed high job satisfaction and low staff infection rates. The emerging themes from the qualitative data are grouped around a “bull’s eye” of the common purpose of person-centeredness, from both patient and staff perspectives, and include staff safety and support, rapid communication, continuous learning and adaptability, underpinned by excellent teamwork. The explanations for the positive feedback included good disaster planning, adequate resources, and an extraordinary responsiveness to the need. Conclusions: The “Hospital of Hope” staff experience produced valuable lessons for designing and managing routine health services outside of a disaster. The adaptability and responsiveness of the facility and its staff were largely a product of the unprecedented nature of the pandemic, but such approaches could benefit routine health services enormously, as individual hospitals and health facilities realize their place in a system that is “more than the sum of its parts”. Background: Adopting a person-centered care approach has transformed different healthcare settings worldwide. However, this concept has gained little attention in many Middle Eastern countries, including Saudi Arabia and other Arab countries. This study aimed to evaluate the psychometric characteristics of the Arabic person-centered climate questionnaire—patient version, measuring to what extent the climate of health care settings is perceived as being person-centered. Method: This is a psychometric analysis study. The original validated version of the English Person-Centered Climate Questionnaire—Patient version (PCQ-P) was translated into Arabic and tested among a sample of hospital patients (n = 300) in Saudi Arabia using translation and back translation procedures. For psychometric evaluation, statistical analyses of validity and reliability were used, including exploratory factor analysis as well as conformity analysis. Results: The Arabic version of the person-centered climate questionnaire—patient version—showed good reliability as the Cronbach’s alpha value of the total of 17 items was 0.84, and the Cronbach’s alpha values of the three sub-scales (safety, everydayness, and hospitality) were 0.83, 0.56, and 0.68, respectively. Internal consistency results were high in terms cof orrelation coefficient for all 17 items. The exploratory factor analysis identified the three factors (safety, everydayness, and hospitality) responsible for 47.174% of the total variance. Conclusion: The Arabic version of the PCQ-P showed satisfactory reliability and validity for measuring patients’ perceptions of person-centeredness in Arab healthcare settings. This Arabic version will be accessible to those interested in generating and using empirical evidence to promote a patient-centered care approach in Arab healthcare settings. The results of this study can be used as a starting point for assessing and developing a person-centered care culture in Saudi hospitals and other Arab countries in the Middle East. Full article

Introduction: Patient opinion surveys have become a widely used method for assessing key aspects of the functioning of medical facilities and, thus, of the functioning of the entire health care system. They are a prerequisite for developing patient-centered care and an essential component of quality improvement programs. In many countries, including Poland, patient opinion surveys are written into the accreditation standards of medical institutions. Patient’s readiness to recommend a hospital is a recognized indicator of the quality of patient-centered care. In a report on strategies for improving the quality of health care in Europe published in 2019 by WHO and the OECD (Organisation for Economic Cooperation and Development), patient’s readiness to recommend a hospital was cited as one of the basic indicators of ‘patient centeredness’ along with patient satisfaction. Therefore, as well consideration of the quality of medical care, a patient recommendation index was also used in the study presented in this paper. The index was based on the answers to questions about the patient’s readiness to recommend a hospital ward to family and friends. Aim: The aim of the study was to investigate patients’ opinions on the quality of services in particular hospital wards. A patient opinion survey can be used to improve the quality of services and monitor the effects of health-related activities, identify areas that need improvement, motivate medical staff and prevent their burnout, build a trusting relationship with patients, and compare the quality of health care in various facilities. Material and methods: The study was carried out in March 2022. The patient opinion survey was conducted using the CAWI (Computer-Assisted Web Interview). The sample selection was purposive. The respondents were patients with a history of hospitalization. The sample selection used an algorithm for the random selection of patients who met the criteria for the sample. The inclusion criterion was hospitalization in the 12 months prior to the study. A standardized questionnaire was used that was aimed at the assessment of the quality of medical care and the patient’s rights to information. Additionally, the survey contained questions about the demographic characteristics of the respondents. Results: A total of 38% of patients with a history of hospitalization expressed criticisms. The majority of statistically significant differences were observed when differentiating respondents according to age. Elderly persons significantly more often declared having been treated with respect and interest. They also rated more highly the meals served in the hospital, effective pain treatment, and respect for the patient’s dignity and intimacy during diagnosis and treatment. Younger persons assessed all these aspects of hospitalization less favorably. Conclusions: Variables including age and the level of income had a statistically significant influence on the opinion of the respondents. Elderly persons assessed most aspects of the quality of care in a hospital ward more favorably. There were a similar number of “promoters” (36%) and “detractors” (38%) of the quality of hospital services. Detractors mainly pointed to long waiting times for hospital admission, the poor quality of medical and nursing care, and unappealing meals. The promoters emphasized the high quality of medical and nursing care and the favorable conditions of the accommodation. Regular patient satisfaction surveys are helpful in identifying areas in which the functioning of a medical entity requires changes. Full article
