With respect to alternative devices like traditional wheelchairs, handbikes can offer advantages from biomechanical and physiological perspectives, to several kinds of users. Assuring high mechanical efficiency and homogeneous force distributions along cycles, and being suitable for indoor and outdoor activities, these systems are used for rehabilitation, sports, and daily applications. From a technical perspective, their main characteristics can vary with the device final purpose and operational context. This review aims to provide an overall outline of handbikes in the literature from a general and comprehensive point of view, up until 2022. The analysis is performed (i) with a systematic approach, without a priori limitations on document type and content focus, and (ii) to identify the areas of interest for the scientific development of these systems. A systematic evaluation method for the identification and analysis of the documents was designed and implemented and the selection criteria, as well as the rationale for the procedure, are described. A specific taxonomy was defined and applied for the subsequent analysis, and each category is specifically evaluated and described, detailing the main outcomes of the literature analysis and relative discussion. Particular attention is paid to actuation strategies and propulsion efficiency. Finally, the main results of the work and future developments for handbikes are briefly synthesized. Full article

This study examines four tracks (Ts) suitable for handbikes (HBs) in the urban area of Brescia, Italy. WHO physical activity (PA) recommendations challenge kinesiologists to find opportunities to promote exercise in the urban context using HBs. This study aims to characterize T features, assess the physical demands of HB cycling, and promote the use of HBs. Track length, elevation changes, slope, and ascent sections were analyzed. Participants’ physiological engagement during HB cycling was measured. The data were gathered using specialized equipment including a metabolimeter, a power meter, a heart rate monitor, and a cycle computer. Crucially, the findings demonstrate that all four Ts facilitate PA above the recommended threshold (>3 MET) regardless of the speed. The study identifies specific speeds required to achieve vigorous PA (>6 MET) on each T. In summary, this study’s analysis of various tracks reveals their potential to meet PA guidelines, even at a slow pace. Moreover, this research establishes speed thresholds for vigorous PA. This information is valuable for both able-bodied individuals and those with mobility challenges when planning effective exercise routines. Moreover, the findings support municipalities in promoting adapted PA in urban areas, enhancing path usability. Full article

Effect of Training for an Athletic Challenge on Illness Cognition in Individuals with Chronic Disability: A Prospective Cohort Study

Illness cognitions (IC) influence how a patient adapts to a chronic disease. The aim was (1) to determine if training for a handcycling mountain time trial (HandbikeBattle) improves IC and (2) to identify factors associated with IC change scores. Persons with a chronic disability (N = 220; including N = 151 with spinal cord disorder) trained 5 months and participated in the time trial. The IC Questionnaire measured helplessness, acceptance, perceived benefits and was assessed before training (T1), after training (T2), and four months after the event (T3). Age, sex, body mass index (BMI), time since injury (TSI), disability characteristics, self-efficacy, mental health (MH) and musculoskeletal pain were obtained at T1. Multilevel regression analyses showed that helplessness decreased (from 11.96 to 11.28, p < 0.01) and perceived benefits increased (from 16.91 to 17.58, p < 0.01) from T1 to T2. For helplessness this decrease persisted during follow-up (11.16 at T3). Changes in helplessness were associated with self-efficacy (p = 0.02), MH (p = 0.02) and lesion completeness (p = 0.02), and were independent of disability type (p = 0.66), lesion level (p = 0.30) and demographics such as sex (p = 0.29) and age (p = 0.67). Training with peers may improve helplessness and perceived benefits in individuals with a chronic disability. Especially individuals with MH problems might benefit from training for an athletic challenge with peers to improve illness cognitions, and ultimately, quality of life. Full article

Para-cycling is a sport including athletes with different disabilities competing on the track and on the roads using bicycles, tandems, tricycles, and handbikes. Scientific literature in this special population is scarce, especially in the field of sports nutrition. This review summarizes the physiological aspects and demands of para-cycling. This information together with the existing literature on nutritional interventions in this population, helps to discuss the nutritional considerations. To date, only a limited amount of recommendations are available for this population. In most para-cycling athletes, a reduction in active muscle mass and consequently a reduction in resting energy expenditure occurs, except for visually impaired athletes. Furthermore, carbohydrate and protein intake and hydration, supplementation, heat, and weight loss need to be tailored to the disability-specific adaptations such as the reduced active muscle mass, neurogenic bladder, and bowel, a reduced metabolic cost during exercise, and a higher risk of micronutrient deficiency. In the Republic of Korea, 90.5% of those living with spinal cord injury (SCI) are faced with medical complications that require chronic care. Some of the more common ones include urinary tract infections, pressure sores, and pain symptomatology. These and other morbidities have been recognized to deteriorate the individual’s health, eventually restricting their community participation. Telerehabilitation, using information and communication technology, has propelled a modern-day movement in providing comprehensive medical services to patients who have difficulty in mobilizing themselves to medical care facilities. This study aims to verify the effectiveness of health care and management in the SCI population by providing ICT-based health care services. We visited eight individuals living with chronic SCI in the community, and provided ICT-based health management services. After using respiratory and urinary care devices with the provision of home visit occupational therapy, data acquisition was achieved and subsequently entered into a smart device. The entered information was readily accessible to the necessary clinicians and researchers. The clients were notified if there were any concerning results from the acquired data. Subsequently, they were advised to follow up with their providers for any immediate medical care requirements. Digital hand-bike ergometers and specialized seating system cushions are currently in development. The ICT-based health care management service for individuals with SCI resulted in a favorable expected level of outcome. Background and Objectives: Playing competitive sports is associated with stress, especially during the starting season. Disabled athletes are additionally burdened with physical and/or emotional factors, resulting from the trauma they have experienced. The aim of the work was to assess the relationship between strategies of coping with stress and the level of education, category of disability and its duration of handcyclists before the competition. Materials and Methods: 44 handcyclists with a mean age of 41.8 ± 11.6, from European countries, were divided according to the severity of mobility impairments, education and duration of the disability. The participants were asked to fill in the Mini-COPE Inventory for Measuring Coping with Stress, which provided answers in writing to some sociodemographic questions regarding age, sex, education, type of mobility impairment and duration of the disability. Results: The subjects who had suffered spinal injury at the cervical section obtained the lowest scores regarding their subjective assessment of their active stress management in difficult situations (p = 0.007). They scored the lowest, 1.5 points, when asked about acceptance in difficult circumstances compared to those with university education (p = 0.02). A statistically significant correlation was found to exist between education levels and positive revaluation, acceptance and seeking instrumental support. A negative correlation was observed between education and sustained use of psychoactive substances and denial. Conclusions: Highly educated cyclists with short-lasting disability, damage to the lower spine section or amputations tend to cope better with stress than other study participants. Full article

Background: This study aimed at comparing bioenergetics and biomechanical parameters between athletes with tetraplegia and paraplegia riding race handbikes at submaximal speeds in ecological conditions. Methods: Five athletes with tetraplegia (C6-T1, 43 ± 6 yrs, 63 ± 14 kg) and 12 athletes with paraplegia (T4-S5, 44 ± 7 yrs, 72 ± 12 kg) rode their handbikes at submaximal speeds under metabolic measurements. A deceleration method (coasting down) was applied to calculate the rolling resistance and frontal picture of each participant was taken to calculate air resistance. The net overall Mechanical Efficiency (Eff) was calculated by dividing external mechanical work to the corresponding Metabolic Power. Results: Athletes with tetraplegia reached a lower aerobic speed (4.7 ± 0.6 m s−1 vs. 7.1 ± 0.9 m s−1, P = 0.001) and Mechanical Power (54 ± 15 W vs. 111 ± 25 W, P = 0.001) compared with athletes with paraplegia. The metabolic cost was around 1 J kg−1 m−1 for both groups. The Eff values (17 ± 2% vs. 19 ± 3%, P = 0.262) suggested that the handbike is an efficient assisted locomotion device. Conclusion: Handbikers with tetraplegia showed lower aerobic performances but a similar metabolic cost compared with handbikers with paraplegia at submaximal speeds in ecological conditions. In this paper, an innovative system of propulsion inspired by a rowing gesture for manual wheelchairs is shown. The innovative system of propulsion, named Handwheelchair.q, can be applied to wheelchairs employed in everyday life and to sports wheelchairs for speed races, such as Handbike and Wheelchair racing. The general features of the innovative system of propulsion and the functional designs of the different solutions are described in detail. In addition, the design of the mechanism for the transmission of motion, employed in a second prototype, Handwheelchair.q02, is presented and analysed. Finally, the dynamic model of the Handwheelchair.q has been developed in order to obtain important results for the executive design of Handwheelchair.q. Full article
