Total Coronary Revascularization via left Anterior Thoracotomy (TCRAT) represents a modern evolution of sternum-sparing, on-pump multivessel coronary artery bypass grafting. In this review, we will summarize the historical development, detail the surgical principles, and provide a comprehensive overview of the clinical outcomes of TCRAT. The technique combines cardiopulmonary bypass using peripheral arterial as well as venous cannulation and cardioplegic cardiac arrest using transthoracic aortic cross-clamping with surgical access through a left anterior minithoracotomy. By applying special slinging and rotational maneuvers, both a stable exposition of all coronary territories—in particular those of the right and the circumflex coronary artery—and a quiet, bloodless operating field enable complete anatomical revascularization and complex coronary surgery procedures, including all variations in multiarterial grafting in unselected patients. Data from all published clinical series were integrated, and a weighted analysis of a total of 2282 patients was performed. TCRAT proved to be very effective with regard to complete anatomical revascularization and modern grafting strategies, and it showed excellent perioperative safety in an all-comers population. Both the 30-day mortality and perioperative stroke incidence were distinctly below 1.0%. Data from mid-term follow-up, although rare so far, are promising and compare well to those of the important RCTs. The TCRAT approach eliminates sternal complications completely and accelerates recovery. As an on-pump arrested-heart surgery, TCRAT inherently permits the combination of minimally invasive multivessel CABG with a variety of other cardiac operations, mainly the combination with valve procedures. The integration of robotic and endoscopic assistance represents the next evolutionary step. With its reproducibility and broad applicability, TCRAT holds strong potential to become a standard routine technique in the field of minimally invasive cardiac surgery. Full article

This study evaluated the effects of a passive upper-limb exoskeleton in agricultural work using a repeated-measures design with 24 adult males across three exoskeleton conditions (No Exo, Prototype, and Airframe), three work directions (left, front, and right), and two work distances (near and far). Outcomes included muscle activity, center of pressure travel, task completion time, perceived exertion, body part discomfort, and usability. Airframe use significantly reduced shoulder and upper-arm muscle activity by about 25–35 percent, consistent with the mechanical sharing of shoulder elevation torque. Erector spinae activity showed a compensatory increase trend, suggesting heightened trunk stabilization demands. Center of pressure varied by direction and distance, with greater excursion for leftward and far tasks, but did not differ among exoskeleton conditions, indicating preserved postural stability. Task time was unaffected by exoskeleton and distance but was longest for leftward tasks. Perceived exertion and discomfort varied by direction and distance and tended to be lower on average with Airframe. Usability differed only in the effectiveness factor, which favored Airframe. An integrated evaluation emphasizing EMG, center of pressure, and perceived fatigue, with usability as needed, is recommended for field validation. Full article

Against the backdrop of intensifying global climate change and human activities, the increasing frequency and evolution of major wildfire events pose severe challenges to global disaster prevention and mitigation systems. Systematically understanding their disaster characteristics, spatiotemporal patterns, and societal response efficacy is an urgent scientific requirement for formulating effective coping strategies. This study constructed a comprehensive database covering 137 major global wildfire events from 2018 to 2024, with data sourced from GFED, EM-DAT, and official national reports. Utilizing a synthesis of methods including descriptive statistics, spatiotemporal clustering analysis, K-means pattern recognition, and non-parametric tests, a multi-dimensional quantitative analysis was conducted on disaster characteristics, evolutionary trends, casualty patterns, and policy effectiveness. Despite potential reporting biases and heterogeneous data standards across countries, the analysis reveals the following: (1) All key wildfire metrics (e.g., burned area, casualties, evacuation scale) exhibited extreme right-skewed distributions, indicating that a minority of catastrophic events dominate the overall risk profile; (2) Global wildfire hotspots demonstrated dynamic expansion, spreading from traditional regions in North America and Australia to emerging areas such as Mediterranean Europe, Chile, and the Russian Far East, forming three significant spatiotemporal clusters; (3) Four distinct casualty patterns were identified: “High-Lethality”, “Large-Scale Evacuation”, “Routine-Control”, and “Ecological-Destruction”, revealing the differentiated formation mechanisms under various disaster scenarios; (4) A substantial gap of nearly 65 times in emergency evacuation efficiency—defined as the ratio of evacuated individuals to total casualties—was observed between developed and developing countries, highlighting a significant “development gap” in emergency management capabilities. This study finds evidence of increasing extremization, expansion, and polarization in global wildfire risk within the 2018–2024 event sample. The conclusions emphasize that future risk management must shift from addressing “normal” events to prioritizing preparedness for “catastrophic” scenarios and adopt refined strategies based on casualty patterns. Simultaneously, the international community needs to focus on bridging the emergency response capability gap between nations to collectively build a more resilient global wildfire governance system. This study analyzes the political speeches of Brazilian federal representatives from the Liberal Party (PL), the primary platform for Bolsonarism, to identify patterns and features of conspiracy theories. Two core concepts are used: conspiracy theories as a worldview that addresses unpredictability and complexity of life in contemporary society, and Bolsonarism as a fundamentally conspiracist worldview grounded in reactionary authoritarianism and populism. Analyzing speeches delivered between 2019 and 2024, our inductive methodology identified both epistemological (logic) and narrative (rhetoric) elements. These individual elements organically integrated to form a pervasive, overarching conspiracy theory that we term “The Grand Collusion”. This theory was strategically deployed to support electoral campaigns and structure political opposition to the Lula government. “The Grand Collusion” alleges a vast alliance between the top echelons of the Judiciary (led by STF Minister Alexandre de Moraes) and the Brazilian Left (led by Lula), with assistance from major media and multilateral organizations. Its alleged objectives include rigging the 2022 elections and orchestrating the systematic persecution and censorship of Right-wing politicians and conservative citizens. Full article

Climate change is one of the most pressing environmental challenges of our time, with consequences that extend far beyond temperature rise. Its impacts include extreme weather events, sea level rise, biodiversity loss, and disruptions to food and water systems, all of which threaten ecosystems and human well-being. Addressing this crisis requires a broad understanding and engagement from society. However, climate change denial persists, often amplified through online platforms, slowing down effective action. Universities can play a critical role in this context, not only as spaces where scientific knowledge is produced and shared, but also as institutions that train future leaders to respond to environmental crises. In this study, we examined the prevalence of climate change denial among members of a Portuguese public university and explored its relationship with gender, religion, and political orientation. We collected 89 responses and analyzed the data. The findings indicate that individuals with right-leaning political views, certain religious affiliations, and male respondents were more likely to deny climate change. These results highlight the need for targeted educational approaches that address specific audiences, fostering a better understanding of the scientific and environmental realities of climate change, and ultimately promoting informed action toward sustainability. The conjuncture of our present time, as Stuart Hall would argue, calls for a critical scrutiny of socio-political forces that aim to destabilize epistemologies and praxis of inclusion, diversity and equity. Such forces use education as a strategic site to perpetuate far-right ideologies and the idea of superiority of white, Western, middle-class nation-states. This article explores more recent manifestations of fortress Europe through the co-optation of inclusive education for migrant and refugee students in Italy and Tunisia. As critical scholars from opposite sides of the Mediterranean, we draw on anti-Blackness to engage in an analysis of the use of education policies to reproduce white supremacy in Italian society, while investing in humanitarian education in Tunisia to contain the movement of African migrants towards Europe. Lastly, the article intends to center the voices of Afro-descendant activists, who have increasingly gained a platform to speak back against such policies, and advocate for a more equitable society, with a more inclusive citizenship law. Cross-national public opinion surveys show that a significant majority of young people are frequently exposed to hateful content on social media, which suggest the need to better understand its political implications. This systematic narrative literature review addresses three key questions: (1) Which factors have been explored in political science as the main drivers of hate speech on social media? (2) What do empirical studies in political science suggest about the political consequences of online hate speech? (3) What strategies have been proposed within the political science literature to address and counteract these dynamics? Based on an analysis of 79 research articles published in the field of political science and international relations retrieved from the Web of Science Core Collection, this review found that online hate is linked to social media platform policies, national and international regulatory frameworks, perceived threats to in-group identity, far-right and populist rhetoric, politically significant events such as elections, the narratives of traditional media, the post-truth environment, and historical animosities. The literature shows that hate speech normalizes discriminatory behavior, silences opposing voices, and mobilizes organized hate. In response, political science research underscores the importance of online deterrence mechanisms, counter-speech, allyship, and digital literacy as strategies to combat hate during the social media era. Pre-trial detention is intended to be a measure of last resort, yet it is excessively applied across jurisdictions worldwide. This paper examines its use, with particular emphasis on its application to women and its incompatibility with international human rights law, standards, and norms. We demonstrate that the inappropriate and widespread use of custodial remand violates fundamental human rights, while exposing the gendered and intersectional barriers that impede women’s access to bail. We further underscore the far-reaching social, economic, and emotional consequences of women’s incarceration. Drawing on a limited but expanding body of research, we argue that pre-trial detention operates as a form of gendered punishment that reflects and reinforces structural inequalities, producing enduring harms for women, their families, and communities. The paper concludes by calling for investment in gender-sensitive, non-custodial, and community-based alternatives that advance women’s decarceration. These measures must be underpinned by reforms that give practical effect to human rights law, standards, and norms, while also addressing the structural conditions that lead to women’s involvement in the criminal-legal system, and ending the unnecessary imprisonment of those who are legally innocent. This article examines how populist radical right parties (PRR) in three contrasting European contexts—Slovenia, France, and Poland—strategically instrumentalize Christianity within their anti-immigration agendas. Rather than using religion as a matter of faith, these parties recast Christianity as a cornerstone of national and European identity, positioning it in opposition to Islam and non-European migration. The study argues that such instrumentalization serves not only to construct a religiously defined national identity, but also to legitimize exclusionary policies. By analyzing selected political speeches, party manifestos, and media discourse, we explore how far-right actors frame Islam as incompatible with European values, reinforcing the division between “Christian Europe” and “foreign non-Christian migrants.” Drawing on recent scholarship on civilizational populism and religious boundary-making, we further assess how processes of globalization and European integration have been interpreted by populist parties to fuel anti-immigrant sentiment. Methodologically, we employ qualitative content analysis to identify recurring themes and rhetorical strategies, with a focus on the intersection of religion, nationalism, and migration. The findings contribute to debates on religious pluralism in contemporary Europe, shedding light on how far-right populism reframes pluralism and challenges secular principles across different political and cultural settings. Extremist narratives combine two relational dynamics: the in-group is portrayed as both socially superior and simultaneously victimized by an antagonistic out-group, which legitimizes hostility or defensive solutions. Despite their relevance, such narratives remain comparatively understudied. To date, little research has examined how extremist narratives are represented through memes, and particularly how humour operates within memetic forms. This article develops and tests a three-layered analytical framework for examining humour in extremist digital cultures. The framework integrates insights from narrative studies, multimodal discourse analysis, and humour theory to capture how memes condense antagonisms, stabilize symbolic contrasts, and calibrate affective positioning. The Virgin vs. Chad meme format is used as a case study due to its binary archetypal structure and recurrent circulation in Spanish far-right meme ecologies. The study draws on 1225 posts on X (May–August 2024), from which 17 memes employing the format were selected for in-depth qualitative analysis. The findings show that the format performs symbolic compression by staging binary oppositions between in-group and out-group identities, typically valorizing figures associated with nationalism, masculinity, and epistemic certainty while delegitimizing those linked to progressivism, pluralism, or emotional expressiveness. These meanings are stabilized through repeated visual and typographic conventions, including character archetypes, split-panel layouts, and indexical stylization. Humour arises through devices such as irony, reversal, exaggeration, and incongruity, which render these oppositions as recognizable contrasts. By integrating insights from humour theory, narrative framing, and multimodal discourse analysis, the article contributes a methodological model for examining how memes condense and circulate antagonistic distinctions in online political ecologies. The indoor microclimate conditions of historical libraries play a pivotal role in ensuring the long-term preservation of their valuable collections, while also influencing the comfort and well-being of staff and visitors. These two objectives may be in contrast, as proved in extensive literature. Microclimate monitoring is essential to evaluate which factors could expose the content of the library to a risk of damage and to design prevention measures. This paper presents the monitoring project, the systems and methodology, and the initial results of an experimental study on the indoor microclimate conditions of the University Library of Bologna (BUL), a very relevant cultural heritage building in the historic city center. The overall objective of the monitoring project is to gain knowledge of the specific microclimate conditions and the historical climate of the three main rooms of the BUL to define the right balance between the needs of conservation and the thermal comfort of staff, users, and visitors. The paper focuses on the short-term indoor monitoring assessment, carried out in the initial phase of the monitoring campaign. This phase, rarely addressed in the literature, is crucial because it enables the collection of results that can guide and orient the entire long-term monitoring campaign. The research results produced so far demonstrate the validity of the methodological approach and the monitoring framework, as well as the reliability of the related data. Moreover, they offer insights that can support the forthcoming inclusion of the BUL in a broader museum system. Full article

This article examines the rhetorical affordances of political claims to victimhood by US president Donald Trump during his first and second terms in office. By applying Critical Discourse Analysis to victimhood claims tactically deployed in the discursive performances of the US president, this analysis demonstrates the versatility and multi-functionality of victimhood claims as a political communication strategy in different contexts, which may account for, at least partly, the appeal of far-right populist leaders to national electorates. The analysis calls attention to a novel argumentative pattern, attested in Trump’s empowered victimhood rhetoric upon his inauguration as second-term president of the USA. This pattern consists of constructing prolonged economic injury inflicted on the nation and announcing retribution against the constructed victimizer(s). This emancipated performative style of claiming victimized nationhood is used to justify and forewarn the implementation of illiberal and coercive politics, in this case, the waging of a global trade war by the US president. Focusing on Trump as an original case study of the construction of ‘economic victimhood’ to justify aggressive economic policy, this paper aims to advance our understanding of the rhetorically complex and continuously evolving victimhood rhetoric of authoritarian populists, as well as the leverages accrued thereof, and adds to a growing body of the literature on the discursive–ideological shifts triggered by authoritarian populism. Background and Clinical Significance: Cervical ectopic pregnancy (CEP) is a rare and potentially serious condition, in which the embryo implants within the cervical canal rather than the uterine cavity and is present in less than 1% of all ectopic pregnancies. There are different treatment options depending on the particular situation and the woman’s reproductive desire but conservative approaches as the first line of treatment is preferred in all cases and hysteroscopic resection of the fetus is one of these options. Several types of laser systems are available for use in hysteroscopic surgery, including neodymium:YAG (Nd:YAG) lasers, KTP and Argon lasers, as well as diode lasers. The holmium:YAG (Ho:YAG) laser, although more commonly used in urology due to its ability to cut, coagulate, and vaporize tissue, has gained interest in gynecologic procedures because of its precision and favorable safety profile. Case Presentation: We present the case of a 32-year-old woman, pregnant for the first time, who was diagnosed with CEP and successfully treated using a Ho:YAG laser during an outpatient hysteroscopic procedure. As far as we know, this is the first published case using this approach. Conclusions: The Ho:YAG laser is a proven tool for outpatient hysteroscopic procedures like septum and adhesion removal. Its ability to both cut and coagulate offers a minimally invasive, fertility-sparing option for managing cervical ectopic pregnancy. The aim was to explore using machine learning (including cluster mapping and k-means methods) to classify types of mead based on sensory analysis and aromatic compounds. Machine learning is a modern tool that helps with detailed analysis, especially because verifying aromatic compounds is challenging. In the first stage, a cluster map analysis was conducted, allowing for the exploratory identification of the most characteristic features of mead. Based on this, k-means clustering was performed to evaluate how well the identified sensory features align with logically consistent groups of observations. In the next stage, experiments were carried out to classify the type of mead using algorithms such as Random Forest (RF), adaptive boosting (AdaBoost), Bootstrap aggregation (Bagging), K-Nearest Neighbors (KNN), and Decision Tree (DT). The analysis revealed that the RF and KNN algorithms were the most effective in classifying mead based on sensory characteristics, achieving the highest accuracy. In contrast, the AdaBoost algorithm consistently produced the lowest accuracy results. However, the Decision Tree algorithm achieved the highest accuracy value (0.909), demonstrating its potential for precise classification based on aroma characteristics. The error matrix analysis also indicated that acacia mead was easier for the algorithms to identify than tilia or buckwheat mead. The results show the potential of combining an exploratory approach (cluster map with the k-means method) with machine learning. It is also important to focus on selecting and optimizing classification models used in practice because, as the results so far indicate, choosing the right algorithm greatly affects the success of mead identification. This article identifies the structural factors underlying the rise of the Populist Radical Right (PRR) in Europe and evaluates its implications for liberal democracies. Our research finds that the emergence of the PRR is driven by the decay of democratic institutions resulting from the neoliberal globalization that was catalyzed by the collapse of the Soviet Union. We argue that the electoral success of the PRR lies in its capacity to use emotionally charged, single-issue narratives that resonate with the political demands of orphaned electorates, who engage in protest voting to express their frustrated expectations. Far from being an ephemeral phenomenon, we show that the PRR reflects structural transformations of the liberal political architecture, and is capable to further eroding democratic institutions by procedurally adopting liberal norms as a means to undermine them. A critical reassessment of the liberal perspective is necessary to address the dysfunctions of democratic institutions. Full article
