Visual Cortical Function Changes After Perceptual Learning with Dichoptic Attention Tasks in Adults with Amblyopia: A Case Study Evaluated Using fMRI

Background: Amblyopia is a neurodevelopmental disorder of vision, commonly caused by strabismus or anisometropia during early childhood. While studies demonstrated that perceptual learning improves visual acuity and stereopsis in adults with amblyopia, accompanying changes in visual cortical function remain unclear. Methods: We measured functional magnetic resonance imaging (fMRI) responses before and after perceptual learning in seven adults with amblyopia. Our learning tasks involved dichoptic high-attention-demand tasks that avoided V1 function-related tasks and required high-level cortical functions (e.g., intraparietal sulcus) to train the amblyopic eye. Results: Perceptual learning induced low-level visual cortical function changes, which were strongly associated with the etiology of amblyopia and visual function improvements. Anisometropic amblyopes showed functional improvements across all regions of interest (ROIs: V1, V2, V3, V3A, and hV4), along with improvements in visual acuity and stereoacuity. In contrast, strabismic amblyopes showed robust improvements in visual cortical functions only in individuals who experienced significant gains in visual acuity and stereoacuity. Notably, improvements in V1 functions were significantly correlated with the magnitude of visual acuity and stereoacuity improvements when combining both anisometropic and strabismic amblyopes. Conclusions: Our findings provide evidence that learning occurs in both high-level and low-level cortical processes. Our study suggests that early intervention to correct eye alignment (e.g., strabismus surgery) is critical for restoring both visual and cortical functions in strabismic amblyopia. Full article

Objectives: This study was aimed at analyzing the efficacy on the improvement of the visual function of a dichoptic online cloud-based platform for the treatment of amblyopia in anisometropic children. Methods: A quasi-experimental (pretest–post-test) study was conducted in 23 subjects with ages from 5 to 15 years old with anisometropic amblyopia combined with additional presence (2 subjects) or not (21 subjects) of microtropia. A total of 30 home-based training sessions of 30 min per session with Bynocs® platform were prescribed for 6 weeks. Results: Amblyopic eye logMAR visual acuity (VA) significantly improved from 0.28 ± 0.24 to 0.13 ± 0.20 after the 6-week treatment (p < 0.001). At baseline, 60.9% of participants had VA in amblyopic eye of 0.20 logMAR or worse, whereas this percentage decreased to 21.7% after treatment. Binocular function (BF) significantly improved from 2.82 ± 1.11 to 2.32 ± 0.94 (p < 0.001). Mean compliance was 92%, 87% and 93% at 2, 4 and 6 weeks of treatment, respectively. Conclusions: In conclusion, home-based dichoptic training with the digital platform evaluated is an effective method to improve amblyopic VA and stereoacuity in children with anisometropic amblyopia combined or not with microtropia. This study addresses an issue in attentional distribution in a binocular visual system using RSVP tasks under Attentional Blink (AB) experimental protocols. In Experiment 1, we employed dichoptic RSVP to verify whether, under interocular competition, attention may be captured by a monocular channel. Experiment 2 was a control experiment, where a monoptic RSVP assessed by both or only one eye determines whether Experiment 1 monocular condition results were due to an allocation of attention to one eye. Experiment 3 was also a control experiment designed to determine whether Experiment 1 results were due to the effect of interocular competition or to a diminished visual contrast. Results from Experiment 1 revealed that dichoptic presentations caused a delay in the type stage of the Wyble’s eSTST model, postponing the subsequent tokenization process. The delay in monocular conditions may be further explained by a visual attenuation, due to fusion of target and an empty frame. Experiment 2 evidenced the attentional allocation to monocular channels when forced by eye occlusion. Experiment 3 disclosed that monocular performance in Experiment 1 differs significantly from conditions with interocular competition. While both experiments revealed similar performance in monocular conditions, rivalry conditions exhibit lower detection rates, suggesting that competing stimuli was not responsible for Experiment 1 results. These findings highlight the differences between dichoptic and monoptic presentations of stimuli, particularly on the AB effect, which appears attenuated or absent in dichoptic settings. Furthermore, results suggest that monoptic presentation and binocular fusion stages were a necessary condition for the attentional allocation. Full article

Background: The ability to merge the two retinal images to perceive depth (stereopsis) plays an important role in human vision. Its proper development requires binocular alignment and good visual acuity in both eyes during childhood. Because treatments are more effective when applied early, early diagnosis is important. Unfortunately, assessing stereo deficiencies in infants and young children remains challenging. Recently, it has been shown that ocular-following responses (OFRs; reflexive, short-latency eye movements induced by the sudden motion of a large textured pattern) are sensitive to changes in interocular correlation, making them potentially useful for stereo deficiency assessments. To test this hypothesis, we measured OFRs elicited by dichoptic stimulation in children with normal and compromised stereopsis (due to amblyopia). Methods: Two groups of six children (age- and sex-matched: 3M/3F aged 7–12 yo), one with compromised stereopsis and one with normal stereopsis, were included. OFRs were recorded using a custom high-resolution video eye-tracking system. The relative differences between eye displacement induced by correlated stimuli (up-correlated–down-correlated) and anticorrelated (up-anticorrelated–down-anticorrelated) were compared. Results: We found significant differences between OFRs induced by two dichoptic conditions (correlated and anticorrelated stimuli) in most children with normal stereopsis, whereas no differences were observed in children with compromised stereopsis, indicating a lack of disparity detectors. Conclusions: OFRs might thus be exploited as a diagnostic tool for the objective identification of stereo deficiencies in children. This might lead to improved early diagnosis and treatment outcomes for conditions like amblyopia and strabismus. Full article

Background and Objectives: Amblyopia is a neurodevelopmental disorder caused by interocular suppression of visual input, affecting visual acuity, stereopsis, contrast sensitivity, and other visual functions. Conventional treatment comprises occlusion of the sound eye. In recent years, novel therapies that deploy perceptual learning (PL) principles have been introduced. The purpose of this study is to assess the latest scientific data on this topic. Materials and Methods: For this purpose, we conducted a literature search for relevant studies published during the previous 4 years (2020–2023). Results: A plethora of visual perceptual learning protocols have been recently developed. Dichoptic video games, contrast rebalanced movies, and online perceptual training platforms are the main formats. Perceptual learning activates neuroplasticity, overcomes interocular suppression, and improves the visual impairments induced by amblyopia. Conclusions: This novel treatment is effective in both children and adults, as well as in patients non-responding to patching. Full article

Binocular rivalry is the perceptual dominance of one visual stimulus over another. Conventionally, binocular rivalry is induced using a mirror-stereoscope—a setup involving mirrors oriented at an angle to a display. The respective mirror planes fuse competing visual stimuli in the observer’s visual field by projecting the stimuli through the stereoscope to the observed visual field. Since virtual-reality head-mounted displays fuse dichoptic vision in a similar way, and since virtual-reality head-mounted displays are more versatile and more readily available than mirror stereoscopes, this study investigated the efficacy of using a virtual-reality headset (Oculus Rift-S) as an alternative to using a mirror stereoscope to study binocular rivalry. To evaluate the validity of using virtual-reality headsets to induce visual dominance/suppression, two identical experimental sequences—one using a conventional mirror stereoscope and one using a virtual-reality headset—were compared and evaluated. The study used Gabor patches at different orientations to induce binocular rivalry and to evaluate the efficacy of the two experiments. Participants were asked to record all instances of perceptual dominance (complete suppression) and non-dominance (incomplete suppression). Independent sample t-tests confirmed that binocular rivalry with stable vergence was successfully induced for the mirror-stereoscope experiment (t = −4.86; p ≤ 0.0001) and the virtual-reality experiment (t = −9.41; p ≤ 0.0001). Using ANOVA to compare Gabor patch pairs of gratings at +45°/−45° orientations presented in both visual fields, gratings at 0°/90° orientations presented in both visual fields, and mixed gratings (i.e., unconventional grating pairs) presented in both visual fields, the performance of the two experiments was evaluated by comparing observation duration in seconds (F = 0.12; p = 0.91) and the alternation rate per trial (F = 8.1; p = 0.0005). The differences between the stimulus groups were not statistically significant for the observation duration but were significantly different based on the alternation rates per trial. Moreover, ANOVA also showed that the dominance durations (F = 114.1; p < 0.0001) and the alternation rates (F = 91.6; p < 0.0001) per trial were significantly different between the mirror-stereoscope and the virtual-reality experiments, with the virtual-reality experiment showing an increase in alternation rate and a decrease in observation duration. The study was able to show that a virtual-reality head-mounted display can be used as an effective and novel alternative to induce binocular rivalry, but there were some differences in visual bi-stability between the two methods. This paper discusses the experimental measures taken to minimise piecemeal rivalry and to evaluate perceptual dominance between the two experimental designs. Full article

The use of digital devices provides a wide range of possibilities for measuring and improving visual function, including concepts such as perceptual learning and dichoptic therapy. Different technologies can be used to apply these concepts, including, in recent years, the introduction of virtual reality (VR) systems. A preliminary experience in treating anisometropic amblyopia through an immersive VR device and using prototype software is described. A total of 4 children were treated by performing 18 office-based sessions. Results showed that distance VA in amblyopic eyes remained constant in two subjects, whereas the younger subjects improved after the training. Near VA improved in three subjects. All subjects showed an increase in the stereopsis of at least one step, with three subjects showing a final stereopsis of a 60 s arc. A total of three subjects showed an increase of approximately 0.5 CS units for the spatial frequency of 3 cpd after the training. Results from this pilot study suggest that visual training based on perceptual learning through an immersive VR environment could be a viable treatment for improving CS, VA, and stereopsis in some children with anisometropic amblyopia. Future studies should support these preliminary results. Full article

Background: In this study we aim to determines the effect of our vision therapy program for 7- to 10-year-old patients who exhibit bilateral amblyopia that is no longer responsive to conventional treatment. Methods: Children with bilateral amblyopia between the ages of 7 and 10 treated with vision therapy at the China Medical University Hospital between 2016 and 2019 were retrospectively reviewed. Age and visual acuity-matched bilateral amblyopes are included as a control group. The visual acuity for both groups showed no improvement for more than 3 months with part-time patching and full refraction correction. The initial and final visual acuity, stereopsis, and refractive status were analyzed. Results: Here, 15 cases were included as the treatment group and 16 cases as a control group. At the endpoint, the study group shows a significant improvement in BCVA, with a mean of 0.32 ± 0.15 logMAR (3 lines improvement) versus 0.003 ± 0.19 logMAR (nearly no improvement) for the control group (p < 0.001). The benefits of treatment are most obvious in the first 3 months after treatment (p < 0.001) and last until the end point. Stereoacuity also improves from 190.00 ± 163.34 to 85.00 ± 61.24 arc seconds, which is a 55.26% improvement. Conclusions: Vision therapy, comprising orthoptic therapy, perceptual learning and dichoptic training, is a successful program for increasing visual acuity and stereoacuity in 7- to 10-year-old children with bilateral amblyopia that is unresponsive to conventional treatment. Active vision therapy using perceptual learning and/or dichoptic or binocular environments has shown its potential effectiveness in amblyopia, but some doubts remain about the type of stimuli and the mode and sequence of presentation that should be used. A search was performed in PubMed, obtaining 143 articles with information related to the stimuli used in amblyopia rehabilitation, as well as to the neural mechanisms implied in such therapeutic process. Visual deficits in amblyopia and their neural mechanisms associated are revised, including visual acuity loss, contrast sensitivity reduction and stereopsis impairment. Likewise, the most appropriate stimuli according to the literature that should be used for an efficient rehabilitation of the amblyopic eye are described in detail, including optotypes, Gabor’s patches, random-dot stimuli and Vernier’s stimuli. Finally, the properties of these stimuli that can be modified during the visual training are discussed, as well as the psychophysical method of their presentation and the type of environment used (perceptual learning, dichoptic stimulation or virtual reality). Vision therapy using all these revised concepts can be an effective option for treating amblyopia or accelerating the treatment period when combining with patching. It is essential to adapt the stimuli to the patient’s individual features in both monocular and binocular training. Full article

Fixation disparity (FD) refers to a suboptimal condition of binocular vision. The oculomotor aspect of FD refers to a misadjustment in the vergence angle between the two visual axes that is measured in research with eye trackers (objective fixation disparity, oFD). The sensory aspect is psychophysically tested using dichoptic nonius lines (subjective fixation disparity, sFD). Some optometrists use nonius tests to determine the prisms for constant wear aiming to align the eyes. However, they do not (yet) use eye trackers. We investigate the effect of aligning prisms on oFD and sFD for 60 s exposure duration of prisms determined with the clinically established Cross test in far distance vision. Without prisms, both types of FD were correlated with the aligning prism, while with prisms the FD was close to zero (these analyses included all base-in and base-out cases). The effect of base-in prisms on oFD was proportional to the amount of the aligning prism for the present 60 s exposure, similar as for the 2–5 s exposure in Schmid et al. (2018). Thus, within 1 min of prism exposure, no substantial vergence adaptation seems to occur in the present test conditions. Further studies may investigate intra-individual responses to different exposure times of aligning prisms in both prism directions. Full article

Binocular rivalry is an important tool for measuring sensory eye dominance—the relative strength of sensory processing in an individual’s left and right eye. By dichoptically presenting images that lack corresponding visual features, one can induce perceptual alternations and measure the relative visibility of each eye’s image. Previous results indicate that observers demonstrate reliable preferences for several image features, and that these biases vary within the visual field. However, evidence about the persistence of these biases is mixed, with some suggesting they affect only the onset (i.e., first second) of rivalry, and others suggesting lasting effects during prolonged viewing. We directly investigated individuals’ rivalry biases for eye and color within the visual field and interestingly found results that mirrored the somewhat contradictory pattern in the literature. Each observer demonstrated idiosyncratic patterns of biases for both color and eye within the visual field, but consistent, prolonged biases only for the eye of presentation (sensory eye dominance, SED). Furthermore, the strength of eye biases predicted one’s performance on a stereoacuity task. This finding supports the idea that binocular rivalry and other binocular visual functions may rely on shared mechanisms, and emphasizes the importance of SED as a measure of binocular vision.
