Effect of Rubber Cup Rotational Speeds during Tooth Polishing on Sound, Early Caries, and Cracked Tooth Surfaces

High-speed rubber cup polishing can exacerbate tooth surface damage, especially when preexisting conditions such as early caries or cracks exist. This study aimed to quantify the extent of damage to sound teeth based on rotating rubber cup speed and assess the damage in relation to the tooth surface condition. Using a rubber cup, 36 sound teeth were polished at 100, 3000, and 10,000 rpm, and 24 teeth with early carious lesions and 24 cracked teeth were polished at 3000 and 10,000 rpm. Polishing was performed using a rubber cup and prophylaxis paste, applying an on–off method (3.0 N force for 3 s). Damage depth was quantified using a surface profilometer and examined using scanning electron microscopy. Polishing at 10,000 rpm caused significantly more damage to sound teeth than polishing at lower speeds (depth increase: 71.45 ± 15.12 µm at 100 rpm; 61.91 ± 17.82 µm at 3000 rpm; p < 0.001). Teeth with early carious lesions or cracks demonstrated more damage after polishing than sound teeth (p < 0.05). Therefore, the rotational speed of the rubber cup has a critical impact on the extent of enamel damage. Higher speeds can increase the damage depth in both sound and damaged tooth surfaces. Tooth fractures are a common cause of tooth loss, frequently starting as enamel cracks. However, methods for the detection of enamel cracks are poorly investigated. The aim of the study was the validation of three clinical methods for the detection of enamel cracks: dental operating microscope (DOM), near-infrared transillumination (NIR), and fiber-optic transillumination (FOTI), with hard-tissue slices serving as controls. A total of 89 extracted teeth, set up as diagnostic models, were investigated, and the maximum crack depth was scored by two examiners. The actual crack depth was determined microscopically (25×) using horizontal sections. The accuracy of each method was analyzed using receiver operating characteristic (ROC) curves. Across all tooth surfaces, the area under the curve (AUC) amounted to 0.57 (DOM), 0.70 (FOTI), and 0.67 (NIR). For crack detection on vestibular/oral surfaces, the AUC was 0.61 (DOM), 0.78 (FOTI), and 0.74 (NIR); for proximal surfaces, it was 0.59 (DOM), 0.65 (FOTI), and 0.67 (NIR). However, the actual crack depth was underestimated with each method (p < 0.001). Under in vitro conditions, FOTI and NIR are suitable for detection of enamel cracks, especially on vestibular and oral tooth surfaces. However, an exact estimation of crack depth is not possible. Therefore, FOTI and NIR seem to be helpful for the clinical detection of enamel cracks. Full article

Enamel cracks generated in the anterior teeth not only affect the function but also the aesthetics of the teeth. Chair-side tooth enamel crack detection is essential for clinicians to formulate treatment plans and prevent related dental disease. This study aimed to develop a dental imaging system using a near-IR light source to detect enamel cracks and to investigate the relationship between anterior enamel cracks and age in vivo. A total of 68 subjects were divided into three groups according to their age: young, middle, and elderly. Near-infrared radiation of 850 nm was used to identify enamel cracks in anterior teeth. The results of the quantitative examination showed that the number of enamel cracks on the teeth increased considerably with age. For the qualitative examination, the results indicated that there was no significant relationship between the severity of the enamel cracks and age. So, it can be concluded that the prevalence of anterior cracked tooth increased significantly with age in the young and middle age. The length of the anterior enamel cracks tended to increase with age too; however, this result was not significant. The silicon charge-coupled device (CCD) with a wavelength of 850 nm has a good performance in the detection of enamel cracks and has very good clinical practicability. Full article

Oral and dental diseases are a major global burden, the most common non-communicable diseases (NCDs), and may even affect an individual’s general quality of life and health. The most prevalent dental and oral health conditions are tooth decay (otherwise referred to as dental caries/cavities), oral cancers, gingivitis, periodontitis, periodontal (gum) disease, Noma, oro-dental trauma, oral manifestations of HIV, sensitive teeth, cracked teeth, broken teeth, and congenital anomalies such as cleft lip and palate. Herbs have been utilized for hundreds of years in traditional Chinese, African and Indian medicine and even in some Western countries, for the treatment of oral and dental conditions including but not limited to dental caries, gingivitis and toothaches, dental pulpitis, halitosis (bad breath), mucositis, sore throat, oral wound infections, and periodontal abscesses. Herbs have also been used as plaque removers (chew sticks), antimicrobials, analgesics, anti-inflammatory agents, and antiseptics. Cannabis sativa L. in particular has been utilized in traditional Asian medicine for tooth-pain management, prevention of dental caries and reduction in gum inflammation. The distribution of cannabinoid (CB) receptors in the mouth suggest that the endocannabinoid system may be a target for the treatment of oral and dental diseases. Most recently, interest has been geared toward the use of Cannabidiol (CBD), one of several secondary metabolites produced by C. sativa L. CBD is a known anti-inflammatory, analgesic, anxiolytic, anti-microbial and anti-cancer agent, and as a result, may have therapeutic potential against conditions such burning mouth syndrome, dental anxiety, gingivitis, and possible oral cancer. Other major secondary metabolites of C. sativa L. such as terpenes and flavonoids also share anti-inflammatory, analgesic, anxiolytic and anti-microbial properties and may also have dental and oral applications. This review will investigate the potential of secondary metabolites of C. sativa L. in the treatment of dental and oral diseases. Full article

Dental enamel constitutes the outer layer of a crown of teeth and grows nearly parallel. This unique nanostructure makes enamel possess birefringence properties. Currently, there is still no appropriate clinical solution to examine dental hard tissue diseases. Therefore, we developed an optical polarization imaging system for diagnosing dental calculus, caries, and cracked tooth syndrome. By obtaining Stokes signals reflected from samples, Mueller images were constructed and analyzed using Lu-Chipman decomposition. The results showed that diattenuation and linear retardance images can distinguish abnormal tissues. Our result also aligns with previous studies assessed by other methods. Polarimetric imaging is promising for real-time diagnosing. Full article
