Purpose: Speech-language pathologists (SLPs) are essential in evaluating tongue structure and function. Due to limited psychometrically validated assessment tools, evidence-based practitioners often rely on clinical expertise to inform their assessment and clinical decision-making. This study aimed to explore how SLPs assess tongue structure and function in children aged 1 to 10 years suspected of having a tongue-tie by examining global practice patterns. Methods: A total of 194 practicing, English-speaking SLPs participated in a global online survey. The survey gathered information on participant demographics, classification tools used, and methods for assessing tongue structure and function, oral motor function and speech production in children with suspected tongue-tie. Results: Participants reported using various measures, including case history, oral examination, and clinical assessment. These measures encompassed evaluation of tongue structure, oral motor tasks and functional measures, including observation of speech, feeding, and swallowing. Notably, 40% of participants indicated they did not use any published assessment tool. While over 90% of participants evaluated feeding skills through parent questionnaires, only 55% observed feeding during mealtimes. Additionally, SLPs in the United States reported using different classification tools for tongue-tie compared to their counterparts in Australia, the United Kingdom and other countries. Conclusion: There is a global trend of limited use of published tongue-tie assessment tools in clinical practice. Most clinicians rely on various measures to evaluate tongue structure and function in children with suspected tongue-tie. These findings highlight the need for a specialized assessment tool that is designed and validated for evaluating tongue structure and function in children beyond infancy. Full article

We used published data consisting of 263 treatment mean observations from beef cattle and dairy steers and heifers, in which CH4 was measured via chambers or head boxes, to evaluate relationships between enteric CH4 production and dry matter intake (DMI) and dietary components. Daily DMI was positively related (slope = 15.371, p < 0.001) to total daily production (g/d) of CH4 (r2 = 0.821). Among chemical components, dietary neutral detergent fiber (NDF) concentration was the most highly related (r2 = 0.696; slope = 0.2001; p < 0.001) to CH4 yield (g/kg of DMI), with strong relationships also noted for dietary starch:NDF ratio (r2 = 0.662; slope = −2.4587; p < 0.001), starch (r2 = 0.495; slope = −0.106; p < 0.001), and the proportion of metabolizable energy relative to gross energy (r2 = 0.561; slope = −23.663; p < 0.001). The slope (−0.5871) and intercept (22.2295) for the dietary ether extract vs. CH4 yield were significant (p < 0.001), but the relationship was highly variable (r2 = 0.150). For dietary crude protein concentration, the slope for CH4 yield was not significant (−0.0344; p < 0.381) with an r2 value near zero. Decreasing DMI by programming body weight gain or restricting feed intake could decrease CH4 production in confined cattle, but these approaches might negatively affect growth performance and product quality, potentially negating positive effects on CH4 production. Feeding higher-quality forages or using grazing management systems that decrease dietary NDF concentrations or substituting grain (starch) for forage should decrease both CH4 yield from enteric production and manure CH4 production via increased digestibility. Effects of feeding management and diet formulation strategies should be additive with other mitigation approaches such as feed additives, allowing the cattle industry to achieve maximal decreases in enteric CH4 production, while concurrently maintaining optimal beef production. Background: Superficial venous thrombosis (SVT) is a common clinical condition caused by inflammation and the presence of a thrombus inside a superficial vein. It has traditionally been considered a benign and banal disorder, although it can progress or can be associated with thromboembolic disease of deep territories in up to 20%, asymptomatic or symptomatic pulmonary embolism (PE), especially if it affects the main trunk of the internal saphenous vein. The impact of deep vein thrombosis on the quality of life and its sequelae have long been described in the literature; however, they have not been studied in superficial vein thrombosis. Objectives: We aimed to evaluate the risk factors, management, and complications of SVT and its impact on the quality of life of our patients. Methods: Observational, prospective, single-center study to evaluate the management of SVT. The ultrasound (US) was performed initially on symptomatic patients, during treatment with low-molecular-weight heparin (LMWH), at a follow-up, and at the end of 45 days of treatment. A quality-of-life questionnaire was administered to determine the risk factors, management, and complications of SVT at the moment of diagnosis and at the end of treatment. We included patients referred from the emergency department to a monographic consultation for thromboembolic disease, over 18 years of age with a diagnosis of acute SVT symptomatic, without contraindication to initiate anticoagulation. Results: In total, 63 patients were evaluated between October 2020 and April 2022. The mean age was 65.8 years (SD 13.5), of which 35 were women (55.6%), 39 presented cardiovascular risk factors (61.9%), 25 had a history of previous personal venous thromboembolism (VTE) (39.7%), and 10 had obesity (15.9%), 47 had chronic venous insufficiency or varicose veins (74.9%). During follow-up with ultrasound, 39.7% had partial revascularization, and at discharge, 63.5% had permeabilized the thrombosis against 19% who had residual thrombosis or progression of thrombosis. There was a positive correlation between mobility parameters and improvement in the performance of daily activities (rho = 0.35; p = 0.012) and with improvement in pain/discomfort (rho = 0.37; p = 0.007). An improvement in pain parameters was statistically significantly related to a global assessment health perception (rho = 0.48; p < 0.001). Anxiety and depression parameters were related to a global assessment health perception (rho = 0.462; p = 0.001) and to an overall improvement at 12 months (rho = 0.45; p = 0.001). CONCLUSIONS: Superficial venous thrombosis (SVT) is a highly prevalent disease, which is traditionally considered banal and has good evolution, with heterogeneous management in clinical practice and limited information on patient selection for therapies, current treatment routes, and drug use, as well as outcomes. In recent years, the importance of this entity has become evident due to its frequency in clinical practice, its risk of complications, and the impact it has on the quality of life. This study’s results emphasize the importance of the diagnosis, treatment, and follow-up of superficial venous thrombosis. Full article

The accurate calculation of reactor core heating is vital for the design and safety analysis of reactor physics. However, negative KERMA factors may be produced when processing and evaluating libraries of the nuclear data files ENDF/B-VII.1 and ENDF/B-VIII.0 with the NJOY2016 code, and the continuous-energy neutron cross-section library ENDF71x with MCNP also has the same problem. Negative KERMA factors may lead to an unreasonable reactor heating rate. Therefore, it is important to investigate the influence of negative KERMA factors on the calculation of the heating rate. It was also found that negative KERMA factors can be avoided with the CENDL-3.2 library for some nuclides. Many negative KERMA nuclides are found for structural materials; there are many non-fuel regions in fast reactors, and these negative KERMA factors may have a more important impact on the power distribution in non-fuel regions. In this study, the impact of negative KERMA factors on power calculation was analyzed by using the RBEC-M benchmark and replacing the neutron cross-section library containing negative KERMA factors with one containing normal KERMA factors that were generated based on CENDL-3.2. For the RBEC-M benchmark, the deviation in the maximum neutron heating rate between the negative KERMA library and the normal library was 6.46%, and this appeared in the reflector region. In the core region, negative KERMA factors had little influence on the heating rate, and the deviations in the heating rate in most assemblies were within 1% because the heating was mainly caused by fission. However, in the reflector zone, where gamma heating was dominant, the total heating rate varied on account of the gamma heating rate. Therefore, negative KERMA factors for neutrons have little influence on the calculation of fast reactor heating according to the RBEC-M benchmark. Full article

The body of a high-speed train is a composite structure composed of different materials and structures. This makes the design of a noise-reduction scheme for a car body very complex. Therefore, it is important to clarify the key factors influencing sound insulation in the composite structure of a car body. This study uses machine learning to evaluate the key factors influencing the sound insulation performance of the composite floor of a high-speed train. First, a comprehensive feature database is constructed using sound insulation test results from a large number of samples obtained from laboratory acoustic measurements. Subsequently, a machine learning model for predicting the sound insulation of a composite floor is developed based on the random forest method. The model is used to analyze the sound insulation contributions of different materials and structures to the composite floor. Finally, the key factors influencing the sound insulation performance of composite floors are identified. The results indicate that, when all material characteristics are considered, the sound insulation and surface density of the aluminum profiles and the sound insulation of the interior panels are the three most important factors affecting the sound insulation of the composite floor. Their contributions are 8.5%, 7.3%, and 6.9%, respectively. If only the influence of the core material is considered, the sound insulation contribution of layer 1 exceeds 15% in most frequency bands, particularly at 250 and 500 Hz. The damping slurry contributed to 20% of the total sound insulation above 1000 Hz. The results of this study can provide a reference for the acoustic design of composite structures. Full article

Pinus oocarpa is the most important conifer for resin production in Mexico, so superior resin trees were selected in the Southern Mountains of Oaxaca, Mexico. The objective was to determine the variation and differences among provenances and among trees according to the parameters of seedling emergence and the number of cotyledons, and their relationship with elevation and climatic variables. The seedling emergence of four replicates of 20 seeds from 80 trees was counted daily. For the emergence parameters, provenance contributed 42.02% to the total variance, tree 29.19% and error 28.79%. Only tree (11.71%) and error (88.29%) contributed to the total variance of the cotyledon number. The effect of provenance (p ≤ 0.0006) and tree (p ≤ 0.0001) was significant for all variables evaluated. Higher-elevation provenances and trees had higher emergence values. The emergence parameters were positively associated with tree elevation. Climatic variables related to precipitation and temperature were negatively related to the emergence parameters. The results allow for the selection of phenotypes without emergence problems to establish seed orchards. Full article

Endometriosis is a common cause of infertility among reproductive-age women. A low ovarian reserve is associated with the presence of endometriotic cysts, and this is accentuated even more after surgery. Patients with a history of endometrioma are a special category of poor ovarian reserve requiring in vitro fertilization (IVF). The aim of this retrospective study was to evaluate the characteristics and outcome of ovarian stimulation and embryo transfer in women with a history of ovarian surgery for endometrioma compared with a control group with tubal factor infertility. A total of 146 patients had previous laparoscopic cystectomy for endometrioma (group A) and their IVF results were compared with 136 patients with documented tubal obstruction (group B). In both groups, the most frequently used ovarian stimulation protocol was the short antagonist in 84.24% versus 80.88%. The number of stimulation days was between 6 and 15 days in the two groups with a mean value of 12.76 days in group A and 9.47 days in group B. The clinical pregnancy rate was 26.77% in the endometrioma group and 39.68% in the tubal obstruction group. Patients with a history of endometrioma are less likely to conceive than those with tubal obstruction despite having similar ovarian reserve and stimulation results. Full article

The world is currently grappling with the two critical issues of global warming and climate change, which are primarily caused by the emission of greenhouse gases. The construction industry and buildings significantly contribute to these emissions, accounting for roughly 40% of the total greenhouse gas emissions. In response to this pressing issue, environmental organizations and governments have pushed the construction industry to adopt environmentally friendly practices to reduce their carbon footprint. This has led to a greater emphasis on designing and planning sustainable buildings that are in line with the principles of sustainable development. Hence, it is imperative to evaluate buildings in terms of their greenhouse gas emissions and explore ways to reduce them. This research examines the impact of material selection on the carbon footprint of reinforced concrete buildings, aiming to reduce embodied carbon. For this purpose, two reinforced concrete buildings are designed for their embodied carbon to quantify their environmental impact. The first building employs commonly used materials such as ceramics, clay bricks, stone, and plaster. In contrast, the second building incorporates sustainable materials such as cork, plywood, and rockwool. According to the findings, using sustainable materials in the second building leads to a 41.0% reduction in the carbon footprint of the construction process. Additionally, using sustainable materials can mitigate pollution levels in the three categories of endangerment to human health, ecosystem pollution, and resource consumption by 31.4%, 23.7%, and 33.3%, respectively. Introduction: Frailty is a complex pathobiological process characterized by diminished physiological reserve and increased vulnerability to stressors, which has been associated with unfavorable outcomes before and after lung transplantation. Methods: We undertook an extensive narrative review, encompassing a thorough exploration of original papers, observational studies, case reports, and meta-analyses published between 1990 and July 2023, in various databases, including PubMed, Embase, Cochrane Library, Wiley Online Library databases, and Google Scholar. The search terms [frailty] AND [lung transplant] were utilized. Additionally, the reference lists of retrieved articles were examined. Inclusion criteria comprised studies written in English and involving human subjects. The identified studies were categorized into pre-transplant and post-transplant populations, and the measurement tools used to assess frailty were analyzed, along with the clinical implications reported in the studies. Results: From 1 January 1990 to 1 July 2023, a total of 10 studies on frailty and lung transplantation were identified through online sources and bibliographic searches, involving a total of 2759 patients. Among these studies, six focused on the pre-transplant population, while four examined the post-transplant population. The Fried Frailty Phenotype (FFP) and the Short Physical Performance Battery (SPPB) were the most employed tools for measuring frailty. A table presents additional frailty assessment instruments and the clinical implications described in the studies. Conclusions: Frailty is prevalent both in patients with end-stage respiratory diseases awaiting lung transplantation and in postoperative lung transplant recipients. Most transplant centers recognize the value of assessing frailty in the evaluation of potential candidates for lung transplantation. Frailty has been shown to impact mortality on the waitlist and in the post-transplant period. However, the most effective methods for measuring frailty in lung transplant candidates and recipients have yet to be determined. Strategies to reverse frailty are available and show promising results on outcomes. Full article

Introduction: The role of EBUS-TBNA in the diagnosis and staging of lung cancer is well established. EBUS-TBNA can be performed using different aspiration techniques. The most common aspiration technique is known as “suction”. One alternative to the suction technique is the slow-pull capillary aspiration. To the best of our knowledge, no studies have assessed the diagnostic yield of slow-pull capillary EBUS-TBNA in PD-L1 amplification assessment in NSCLC. Herein, we conducted a single-centre retrospective study to establish the diagnostic yield of slow-pull capillary EBUS-TBNA in terms of PD-L1 in patients with NSCLC and hilar/mediastinal lymphadenopathies subsequent to NSCLC. Materials and Methods: Patients with hilar and/or mediastinal lymph node (LN) NSCLC metastasis, diagnosed by EBUS-TBNA between January 2021 and April 2022 at Pulmonology Unit of “Ospedali Riuniti di Ancona” (Ancona, Italy) were enrolled. We evaluated patient characteristics, including demographic information, CT scan/ FDG-PET features and final histological diagnoses, including PD-L1 assessment. Results: A total of 174 patients underwent EBUS-TBNA for diagnosis of hilar/mediastinal lymphadenopathies between January 2021 and April 2022 in the Interventional Pulmonology Unit of the “Ospedali Riuniti di Ancona”. Slow-pull capillary aspiration was adopted in 60 patients (34.5%), and in 30/60 patients (50.0%) NSCLC was diagnosed. EBUS-TBNA with slow-pull capillary aspiration provided adequate sampling for molecular biology and PD-L1 testing in 96.7% of patients (29/30); in 15/29 (51.7%) samples with more than 1000 viable cells/HPF were identified, whereas in 14/29 (48.3%) samples contained 101–1000 viable cells/HPF. Conclusion: These retrospective study shows that slow-pull capillary aspiration carries an excellent diagnostic accuracy, almost equal to that one reported in literature, supporting its use in EBUS-TBNA for PD-L1 testing in NSCLC. This study aimed to evaluate the effects of grafting using different rootstocks on plant, fruit and seed quality in cantaloupe melons. The double haploid (DH) female (H27) and male (H4) parental lines of Solmaz F1 (Cucumis melo L. var. cantalupensis) variety developed by Cukurova University Faculty of Agriculture were used as scion and three commercial interspecific hybrid Cucurbita (Cucurbita maxima Duchesne × Cucurbita moschata Duchesne) varieties, Nun-9075 F1 (Nunhems), Ares F1 (ITU) and TZ-148 (Clause) were used as rootstocks. The parental lines were also self grafted and ungrafted parents were used as the control group. The grafted and ungrafted parents were crossed during pollination. Graft combinations and control were compared for performances in measured parameters such as the main stem length (cm), main stem diameter (mm), node number, harvest time (day), fruit weight (g), fruit length (cm), fruit diameter (cm), fruit cavity length (cm), fruit cavity diameter (cm), fruit flesh thickness (cm), fruit rind thickness (mm), total soluble solids (TSS, %), fruit flesh productivity (%), number of full and empty seeds (number/fruit), 1000 seeds weight (g), seed germination rate (%) and time (day), seed emergence rate (%), and time (day). Most of the plant, fruit and seed parameters have been positively affected by grafting. The use of commercial interspesific Cucurbita hybrid rootstocks resulted in high values compared to the control group however, harvest time, fruit cavity length and diameter, fruit rind thickness and fuit flesh productivity parameters were not statistically significant between rootstocks. Considering seed emergence and germination rate Cucurbita hybrid rootstocks performed lower values than self grafted and ungrafted control. The current study concludes that grafting plays a crucial role in plant growth, fruit and seed characters in cantaloupe melons. Statistically significant differences were mostly observed based on evaluated parameters. Full article

Stroke is one of the main causes of long-term disabilities, increasing the cost of national healthcare systems due to the elevated costs of rigorous treatment that is required, as well as personal cost because of the decreased ability of the patient to work. Traditional rehabilitation strategies rely heavily on individual clinical data and the caregiver’s experience to evaluate the patient and not in data extracted from population data. The use of machine learning (ML) algorithms can offer evaluation tools that will lead to new personalized interventions. The aim of this scoping review is to introduce the reader to key directions of ML techniques for the prediction of functional outcomes in stroke rehabilitation and identify future scientific research directions. The search of the relevant literature was performed using PubMed and Semantic Scholar online databases. Full-text articles were included if they focused on ML in predicting the functional outcome of stroke rehabilitation. A total of 26 out of the 265 articles met our inclusion criteria. The selected studies included ML approaches and were directly related to the inclusion criteria. ML can play a key role in supporting decision making during pre- and post-treatment interventions for post-stroke survivors, by utilizing multidisciplinary data sources. Full article

When subjected to cold environmental temperatures, cheloniid sea turtles can experience debilitating lethargy, anorexia, and potential mortality in a phenomenon known as cold-stunning. Every year, hundreds to thousands of cold-stunned sea turtles are transported to rehabilitation centers for medical and nutritional care. The objective of this study was to investigate one aspect of nutritional status in cold-stunned sea turtles: fatty acid profiles. Blood was collected from eleven green (Chelonia mydas), twelve Kemp’s ridley (Lepidochelys kempii), and three loggerhead (Caretta caretta) juvenile sea turtles found cold-stunned along the coast of North Carolina, USA. Whole blood (~160 µL) was dried onto specialized paper spot cards, frozen, and subsequently analyzed via gas chromatography to quantify fatty acid percentages. Significant differences among species were identified for 19 out of 36 individual fatty acids analyzed and six out of seven fatty acid groups evaluated (P < 0.5). The whole blood fatty acid profiles of cold-stunned green and Kemp’s ridley sea turtles were similar to prior published profiles of healthy conspecifics. Marginal numerical differences noted upon visual comparison included that cold-stunned sea turtles had lower proportions of total polyunsaturated fatty acids (PUFA) and monoenes and higher proportions of total saturated fatty acids relative to healthy conspecifics. These differences may reflect acute impacts of cold-stunning on circulating plasma fatty acids or may be the result of natural seasonal variations. These data provide practical information to aid in the diet design of sea turtles in rehabilitation settings. Full article

Floodplains are critically important ecosystems that provide a whole suite of ecosystem services, including nutrient and carbon sequestration, flood mitigation, water storage, and critical wildlife habitat. However, human modification of rivers and floodplains through channelization, artificial levee construction, reductions in the active floodplain area, and water management can significantly reduce the ecosystem function of river–floodplain systems. In this study, we evaluated the changes in the nutrient loading of the Kissimmee River floodplain during the restoration of the river–floodplain system. In addition to time-series loading analysis, we also evaluated soil nutrient concentrations across the lower portion of the Kissimmee River floodplain. During the 44-year nutrient loading time-series, the floodplain remained a nutrient exporter with changes in nutrient loading generally corresponding to both water quality (i.e., point source reductions) and hydrologic restoration activities in the watershed and Kissimmee River floodplain. During the study period, inputs of total phosphorus and total nitrogen loads from upstream either significantly increased or remained the same. In addition to external sources of nutrients, internal sources of nutrients from floodplain soils can also contribute to the total nutrient export from the system. These internal sources could be organic via the decomposition of organic matter or geologic from the original excavation of the canal and/or restoration backfilling. Soil nutrient concentrations vary between vegetative communities and landscape position and could be a significant source of phosphorus to the downstream system, which is plagued by eutrophic conditions. Therefore, as floodplain function in the Kissimmee River continues to be restored and managed, additional effort may be needed to address nutrient inputs and internal legacy nutrients. Full article

Purpose: Nasal disuse and mouth breathing are associated with negative structural, functional, postural, occlusal, and behavioural changes. While there is some research to suggest that nasal breathing exercises can reduce mouth breathing, clinical protocols have not been extensively investigated. The purpose of this research was to determine the feasibility and effectiveness of a nasal breathing rehabilitation protocol based on Integrative Breathing Therapy principles called Functional Nasal Breathing Rehabilitation (FNBR). Methods: Twenty-three participants with symptoms of nasal obstruction and self-reported mouth breathing completed the 4-week online FNBR training. Outcome measures included the Nasal Obstruction Symptom Evaluation (NOSE) scale, a numeric rating scale (NRS) for nasal breathing difficulty and obstruction, allergy symptoms, self-reported daytime and nighttime mouth breathing, and a composite questionnaire called the Self-reported Nasal Breathing Difficulty Questionnaire (SRNBQ) to compare symptoms of nasal obstruction, allergy and reported mouth breathing pre- and post-trial. A content analysis was performed on qualitative data collected during weekly online interviews. Results: There were statistically significant improvements in the SRNBQ total score (p = 0.002), NOSE scale (p = 0.006), NRS score (p = 0.008), and mouth breathing daytime and night-time (MBDS) score (p = 0.024), but not in allergy symptoms. Participants were highly adherent with the techniques, with 91% of the participants completing formal breathing practice more than four times a week and 96% reporting that they used the practices informally in daily life mostly or all of the time. Conclusion: Functional Nasal Breathing Rehabilitation appears to be a feasible and effective modality for reducing mouth breathing and improving nasal obstruction symptoms in patients with subjective signs of nasal obstruction.
