Background: Interstitial lung disease (ILD) is a prominent complication in the course of rheumatoid arthritis (RA), with a prevalence ranging from 5% to 60% and several phenotypes. The existing knowledge on the impact of different pharmacological interventions in individuals with rheumatoid arthritis-related interstitial lung disease (RA-ILD) is inconclusive, and this variable response to treatment highlights the need for a personalized approach to the management of RA-associated ILD. Therefore, we aimed to evaluate the therapeutic effect and safety of different pharmacological agents, including conventional synthetic DMARDs (Cs DMARDs), biologic DMARDs (bDMARDs), targeted synthetic DMARDs (Ts DMARDs), and antifibrotic agents, in patients with RA-ILD. Method: This systematic review and meta-analysis searched for available randomized controlled trials (RCTs) and prospective cohort studies. A search was performed in the PubMed, Google Scholar, and Cochrane Central Register of Controlled Trials (CENTRAL) databases. Eligible studies comprised those involving hospitalized patients diagnosed with RA-ILD, regardless of concomitant medications, who were of adult age (≥18 years); the studies measured the effect of pharmacological interventions, including methotrexate, leflunomide, tumor necrosis factor inhibitors (anti-TNF), abatacept, rituximab, JAK inhibitors, and antifibrotic agents, compared to placebo or other therapies for RA. Results: Out of 446 studies from 2002 to 2024, only 16 were included in this systematic review, including 14 prospective cohort studies and 2 placebo-controlled studies. Unfortunately, no RCTs were found that address our research question. The most relevant studies (n = 4) were performed in different countries (mainly Spain and the UK), with sample sizes varying from 23 to 381 patients (total: 2199 patients). The current study reveals that non-anti-TNF biologics were associated with a decreased risk of radiologic progression, while advanced therapies improved disease-related outcomes in patients requiring oxygen therapy. Methotrexate and other DMARDs were found to have inconsistent effects on ILD progression and mortality. Conclusions: Our review supports the integration of personalized medicine into the management of RA-ILD. By considering patient-specific factors and therapeutic responses, clinicians can better tailor interventions. We confirmed the high methodological quality of the trials, yielding solid evidence for the clinical management of RA-ILD. This review adds to the existing literature by identifying nintedanib as a potential disease-modifying therapy with the potential to slow the progression of lung disease. Full article

Introduction: Longevity is increasingly recognized as the result of modifiable lifestyle, environmental, and social factors rather than genetics alone. While global interest in healthy aging is growing, public awareness of these determinants remains understudied in the Middle East. This study aimed to assess public awareness and perceptions of longevity-related factors among adults in Saudi Arabia. Methods: A cross-sectional online survey was conducted between November 2024 and January 2025 targeting residents of Saudi Arabia aged 18 and above. A researcher-validated questionnaire explored awareness across various domains, including lifestyle, environment, genetics, sleep, and religious beliefs. Perceptions were assessed using a series of items measuring beliefs about the influence of lifestyle, environmental, technological, and cultural factors on health and longevity. Descriptive and inferential statistics were used to analyze responses from 395 participants. Results: The sample included 395 participants, with 67.8% females and more than half (51.4%) aged 18–25 years. The participants exhibited high awareness of key lifestyle factors; greater than 88% acknowledged the role of sleep, and more than 90% recognized the importance of exercise and nutrition. However, fewer were aware of environmental (72.2%) or social determinants, and nearly half believed that genetic factors exert a primary influence on longevity. Higher awareness was significantly associated with behaviors like regular exercise (p = 0.004), dietary supplement use (p = 0.002), and recognition of the importance of sleep (p < 0.001). Younger adults showed higher awareness than older adults, while there were no significant differences in regards to gender, education, and income. Support for incorporating religious themes into health campaigns was also linked to greater awareness. Insurance status and chronic illness were not associated with awareness levels. Conclusions: While awareness of lifestyle-related longevity factors is strong, gaps remain in understanding broader influences such as environmental and social determinants. Culturally grounded public health strategies that integrate religious framing and emphasize actionable, evidence-based behaviors may enhance community engagement and support healthy aging across Saudi Arabia. Background: Previous research has linked high coffee consumption to an increased risk of metabolic syndrome (MetS). This study aimed to assess the relationship between coffee consumption and MetS components among Saudi adults. Methods: A cross-sectional study was performed on adults who met at least three criteria for a MetS diagnosis. Data concerning demographics, lifestyle, sleeping patterns, medical health, anthropometric measurements, habitual coffee drinking, and lab levels for HDL-C, LDL-C, TC, TGs, HbA1c, and FBG were collected. Results: Of the 95 participants, 51% were women, 75.8% were >50 years old, 75.8% were obese, 62% were used to practicing physical activity, 74.5% never smoked, 56.4% slept < 7 h/day, and 89.5% were coffee consumers. Of these, 94.7% had high waist circumference, 63.2% had high BP, 47.4% had high FBG, 41.1% had low HDL, and 23.2% had high TGs. For coffee consumers, 37.6% drank a small cup, 34.5% drank coffee once daily, 89.4% drank Arabic coffee, and 75.3% added no additives. Conclusions: No significant association was found between coffee consumption patterns and any MetS component, with the exception of elevated TGs, which was strongly associated with coffee cup size and number of daily cups. Waist circumference and BMI had a strong positive correlation with coffee cup size, and there was a significant relationship between the number of daily cups, BMI, and TC. Further prospective studies are needed to establish a causal relationship. Full article

Background/Objectives: Cancer is a chronic and serious disease that has a wide range of effects on patients, some of which extend to family members and primary caregivers (PCs), thereby affecting their quality of life (QOL). The aim of this study was to evaluate the QOL of PCs of patients with cancer and to investigate the sociodemographic and other factors that impact PCs’ QOL. Methods: This cross-sectional study was conducted at the Princess Noura Oncology Center, King Abdulaziz Medical City, Jeddah, and included 235 PCs. A short-form health survey—the SF-36, which includes eight domains—was used to measure the QOL of patients’ PCs. Results: The relationship between the QOL of PCs and the characteristics of patients and PCs was examined. The QOL of PCs was associated with several variables. Multiple regression analysis showed that older age, female sex, and caring for patients with hematological malignancies were independent, significant variables associated with lower PCs’ QOL, whereas PCs caring for female patients experienced a better QOL. Conclusions: These findings highlight the essential aspects of caregivers’ QOL and their influencing factors. To better understand the implications of these factors, future studies are required to demonstrate the effects of patient- and disease-related factors on PCs’ QOL. Full article

Coronary artery bypass grafting (CABG) is an essential operation for patients who have severe coronary artery disease (CAD). Both open and minimally invasive CABG methods are used to treat CAD. This in-depth review looks at the latest research on the effectiveness of open versus minimally invasive CABG. The goal is to develop evidence-based guidelines that will improve surgical outcomes. This systematic review used databases such as PubMed, MEDLINE, and Web of Science for a full electronic search. We adhered to the PRISMA guidelines and registered the results in the PROSPERO. The search method used MeSH phrases and many different study types to find papers. After removing duplicate publications and conducting a screening process, we collaboratively evaluated the full texts to determine their inclusion. We then extracted data, including diagnosis, the total number of patients in the study, clinical recommendations from the studies, surgical complications, angina recurrence, hospital stay duration, and mortality rates. Many studies that investigate open and minimally invasive CABG methods have shown that the type of surgery can have a large effect on how well the patient recovers and how well the surgery works overall. While there are limited data on the possible advantages of minimally invasive CABG, a conclusive comparison with open CABG is still dubious. Additional clinical trials are required to examine a wider spectrum of patient results. Full article

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common liver disease worldwide and simultaneously the most common indication for liver transplants in Western countries This study aims to evaluate the effectiveness of laparoscopic sleeve gastrectomy (LSG) or Roux-en-Y gastric bypass (RYGB) on MDASLD histologically and biochemically. 14 studies met our criteria with a total population of 1942 who underwent LSG or RYGB 1–14. The Newcastle-Ottawa Scale (NOS) was used for quality evaluation of the included studies. Results: Both surgeries were effective in decreasing laboratory biomarkers like ALP, GGT, AST, and ALT with non-significant superiority of LSG over RYGB which did not significantly improve the AST level after one year. LSG showed more decrease in ALT levels (MD = −17.56, 95% CI = (−23.04, −12.089), p 0.001) and LSG was associated with increased change in NAS score with slight superiority. Both LSG and RYBG improve NAD and NASH outcomes after one and 10 years of surgery. However, randomized clinical trials with large samples are needed to confirm these results. Full article

To constructively ameliorate and enhance traffic safety measures in Saudi Arabia, a prolific number of AI (Artificial Intelligence) traffic surveillance technologies have emerged, including Saher, throughout the past years. However, rapidly detecting a vehicle incident can play a cardinal role in ameliorating the response speed of incident management, which in turn minimizes road injuries that have been induced by the accident’s occurrence. To attain a permeating effect in increasing the entailed demand for road traffic security and safety, this paper presents a real-time traffic incident detection and alert system that is based on a computer vision approach. The proposed framework consists of three models, each of which is integrated within a prototype interface to fully visualize the system’s overall architecture. To begin, the vehicle detection and tracking model utilized the YOLOv5 object detector with the DeepSORT tracker to detect and track the vehicles’ movements by allocating a unique identification number (ID) to each vehicle. This model attained a mean average precision (mAP) of 99.2%. Second, a traffic accident and severity classification model attained a mAP of 83.3% while utilizing the YOLOv5 algorithm to accurately detect and classify an accident’s severity level, sending an immediate alert message to the nearest hospital if a severe accident has taken place. Finally, the ResNet152 algorithm was utilized to detect the ignition of a fire following the accident’s occurrence; this model achieved an accuracy rate of 98.9%, with an automated alert being sent to the fire station if this perilous event occurred. This study employed an innovative parallel computing technique for reducing the overall complexity and inference time of the AI-based system to run the proposed system in a concurrent and parallel manner. Background: Early detection and screening of breast cancer (BC) might help improve the prognosis of BC patients. This study evaluated the use of serum microRNAs (miRs) as non-invasive biomarkers in BC patients. Methods: Using quantitative real-time polymerase chain reaction, we evaluated the serum expression of four candidate miRs (miR-155, miR-373, miR-10b, and miR-34a) in 99 Egyptian BC patients and 40 healthy subjects (as a control). The miRs expression was correlated with clinicopathological data. In addition, the sensitivity and specificity of the miRs were determined using receiver operating characteristic (ROC) curve analysis. Results: Serum miR-155, miR-373, and miR-10b expression were significantly upregulated (p < 0.001), while serum miR-34a was downregulated (p < 0.00) in nonmetastatic (M0) BC patients compared to the control group. In addition, serum miR-155 and miR-10b were upregulated in BC patients with large tumor sizes and extensive nodal involvement (p < 0.001). ROC curve analysis showed high diagnostic accuracy (area under the curve = 1.0) when the four miRs were combined. Serum miR-373 was significantly upregulated in the human epidermal growth factor 2–negative (p < 0.001), estrogen receptor–positive (p < 0.005), and progesterone receptor (PR)-positive (p < 0.024) in BC patients, and serum miR-155 was significantly upregulated in PR-negative (p < 0.001) BC patients while both serum miR-155 and miR-373 were positively correlated with the tumor grade. Conclusions: Circulating serum miR-155, miR-373, miR-10b, and miR-34a are potential biomarkers for early BC detection in Egyptian patients and their combination shows high sensitivity and specificity. Intermediate filament (IF) proteins make up the largest family of cytoskeletal proteins in metazoans, and are traditionally known for their roles in fostering structural integrity in cells and tissues. Remarkably, individual IF genes are tightly regulated in a fashion that reflects the type of tissue, its developmental and differentiation stages, and biological context. In cancer, IF proteins serve as diagnostic markers, as tumor cells partially retain their original signature expression of IF proteins. However, there are also characteristic alterations in IF gene expression and protein regulation. The use of high throughput analytics suggests that tumor-associated alterations in IF gene expression have prognostic value. Parallel research is also showing that IF proteins directly and significantly impact several key cellular properties, including proliferation, death, migration, and invasiveness, with a demonstrated impact on the development, progression, and characteristics of various tumors. In this review, we draw from recent studies focused on three IF proteins most associated with cancer (keratins, vimentin, and nestin) to highlight how several “hallmarks of cancer” described by Hanahan and Weinberg are impacted by IF proteins. The evidence already in hand establishes that IF proteins function beyond their classical roles as markers and serve as effectors of tumorigenesis. Full article
